Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soyangin Disease of Sasang Constitutional Medicine(SCM): Yin-Depletion (Mangeum) Symptomatology. The CPG was developed by the national-wide experts committee consisting of SCM professors. Methods The CPG was developed by the national-wide experts committee considering of the society of Sasang Constitutional Medicine. It was performed by search and collection of literature related SCM, opinion of SCM experts and journal search and it was followed by CPG's guideline. Results & Conclusions The CPG of Yin-Depletion (Mangeum) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Yin-Depletion (Mangeum) Symptomatology is classified into severe and critical pattern by severity. The severe pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum) and the advanced pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum). The critical pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Cold-related diarrhea accompanied by the abdominal pain (Sinhan-bocktong Mangeum) and the advanced pattern of Cold-related diarrhea accompanied by abdominal pain (Sinhan-bocktong Mangeum).

CSL을 통한 음향특성과 사상체질간의 상관성 연구

1. 연구배경 사상인은 생리 및 병리 현상에 차이를 나타낼 뿐만 아니라 섭생법 및 치료법에서도 각기 다른 방법을 적용하고 있다. 그러므로 사상의학에 있어서 체질변증분야가 매우 중요한 과제라 하겠다. 동무는 사상인변증의 진단지표를 외형 심성 병증 등을 제시하였고, 많은 의가들에 의해 여러 가지 새로운 체질변증의 방법이 모색되어 임상에 시도 활용되고 있는 추세이나 체질변증의 객관성유지에 어려운 점이 많아서 학문의 발전에 적지 않은 난관이 따랐다. 최근에는 음성과학의 발달에 힘입어 음성의 특성을 가시적 수치적으로 객관화 할 수 있는 가능성이 높아졌다. 본 연구는 이러한 음성과학을 바탕으로 여러 의가들에 의해 제시되어 온 사상인의 음성적 특정을 각 개인의 사상체질과 직접적으로 연관성이 있다고 판단되는 공명주파수와 기본주파수 등 여러 음성학적인 pattern자료들을 모집단으로부터 획득하여 체질별로 객관적인 수치로 비교 분석함으로써 음성과 체질과의 관계에 대하여 살펴보고 개인의 성문에 의한 사상체질 판단의 기본자료로 활용하고자 하였다. 2. 연구방법 상지대한의과대학 남학생 132명과 기타8명을 대상으로 하였다. 음향특성과 체질과의 상관성에 관하여 문헌조사를 하였다. 설문지와 체질전문가에 의하여 연구대상자를 체질 판별하였다. 연구대상자의 음성특성에 대한 설문조사를 하였고 CSL로 Pitch. Formant Frequency. Energy, Time of reading composition 등을 분석하고 통계 처리하여 체질 판별을 시도하였다. 3. 연구결과 1) 설문조사결과 소음인은 음성이 낮고 완만하며 조용한 편으로, 소양인은 음성이 높고 밝으며 급하고 함부로 말을 하는 것으로, 태음인은 음성이 낮고 무거우며 완만하고 굵고 성량이 풍부한 것으로 나타났다. 2) 문장의 평균 발음 시간에서 태양인은 다른 체질에 비하여 유의성 있게 길었다. 몸무게에서 태음인은 다른 체질에 비하여 유의성 있게 높았다. Formant frequency 1에서는 태양인이, Bandwidth 1에서는 소음인과 태양인이 다른 체질에 비하여 유의성 있게 낮았으며, Bandwidth 2와 Formant frequency 5에서는 태양인이 유의성 있게 높았다. Pitch Maximum과 Pitch Maximum-Pitch Minimum에서는 소음인과 태양인이 유의성 있는 차이를 나타내었다. Energy Mean에서는 태양인과 태음인에서 유의성 있는 차이를 나타내었다. 3) Multi-dimensional 4-class minimum-distance classifier 분석결과 모든 항목으로 분석한 것 보다 체질별로 특정 항목을 갖고 분석한 것이 높은 일치를 나타내었다. SPSS/PC+프로그램에서 일원분산분석과 판별분석결과 네 체질사이에서의 정판별력보다 소양인을 제외한 3체질 사이에서의 정판별력이 높았다. CART모형에서 앞에서 제시된 다른 방법들보다 높은 예측율을 보이고 있다. 연구수행과정 중에 표준화, 녹음기술, 적적한 문장의 선택, 음성분석 항목선택, 통계기법, 적절한 알고리즘의 개발 등의 많은 문제점이 나타나고 있으며 관련분야의 보완과 연구진행이 지속적으로 있어야 할 것으로 사려 된다. 다각도로 진행되고 있는 사상의학에서의 객관화 노력과 통계적인 방법론들이 적용되고 체질진단에 있어 가장 최적의 알고리즘을 찾아낼 수 있다면, 정확한 체질 진단과 더불어 그에 따른 환자들의 치료와 약물처방에 있어서도 많은 도움을 줄 수 있으리라 생각된다. 성문과 음성특성의 분석을 통한 사상체질 분류검사방법은 사상체질의 객관화를 위한 하나의 보조적인 방법이 The purpose of this study is to help classifying Sasang Constitution through correlation with sound characteristic. This study was done it under the suppose that Sasang Constitution has correlation with sound spectrogram. The following result were obtained about correlation between sound spectrogram and Sasang Constitution by comparison and analysis 1. Soeumin answered his voice low tone, smooth and quiet in the survey. Soyangin answered his voice high, clear, fast and speaking random. Taeumin answered his voice low, thick and muddy. 2. Taeyangin was significantly slow compared with the others in the time of reading composition. Taeyangin was significantly slow compared with the others in Formant frequency 1. Taeyangin was significantly discriminated from Soeumin in Formant frequency 5. Taeyangin was significantly low compared with the others in Bandwidth 2. Soeumln was significantly low compared with Taeyangin in Pitch Maximum and Pitch Maximum-Pitch Minimum. Taeyangin was significantly high compared with the others in Energy mean. 3. In list of specification, the discrimination rate was higher than that by lists of 13 in the results of Multi-dimensional 4-class minimum-distance. The discrimination rate of three disposition except Soyangin was higher than that of four disposition in the results of One way ANOVA and Analysis of dis crimination in SPSS/PC+. In CART, the estimate rate of Sasang Constitution discrimination was higher than any other method. It is considered that there is a correlation between sound spectrogram and Sasang constitution according to the results. And method of Sasang constitution classification through sound spectrogram analysis can be one method as assistant for the objectification of Sasang constitution classification.

캠프 형식의 입원 치료를 받은 소아 아토피피부염 환자 6례

Objectives This study investigated the effect of camping style Hospitalization program for atopic dermatitis children patients. Methods 6 patients were admitted for camping style hospitalization program. The program includes acupuncture, herbal medicine, examination, education, psychotherapy, dressing, exercise. The patients were analyzed by Severity Scoring Atopic Dermatitis (SCORAD) index and measured skin pH value (pH). Also, patients evaluated themselves by writing an atopic dermatitis diary which consists of diet, emotion, severity of pruritus, and sleep loss. Checked questionnaires which consists of atopic dermatitis, self treatment methods and each program satisfaction. Results After 7 days of hospitalization program, the patient's SCORAD index scores were decreased and most of the patient's symptoms were improved. Conclusions The hospitalization program could be helpful for atopic dermatitis children patients.

코골이 대상자에게 비강사혈흡인요법의 안전성과 유효성을 평가하기 위한 다기관 임상시험

Objectives : The study is to investigate clinical trial for efficacy and safety with acupuncture & suction in nasal cavity on snoring patients. Methods : 24 subjects were recruited from the patients received a hospital visit to the Semyung University Chungju oriental medical center and Jaechon oriental medical center from 1 October 2014 to 31 December 2014. According to the clinical protocol, we treated the patients with acupuncture and suction in the nasal cavity 8 times for 4 weeks. We compared their symptoms between before and after treatment. Results and Conclusions : 1. There was no significant decrease in the mechanical measurement of the reduction of snoring in the effectiveness of snoring patients, but there was a significant effect on the improvement of sleep quality and it was effective in improving the symptoms such as eye fatigue. 2. The safety of nasal treatment for snoring patients was normal in pre- and post-treatment Lab tests, and there was no adverse reaction before and after treatment. Acupuncture & suction in nasal cavity on snoring patients is efficient and safe.

한국성인(韓國成人)의 사상체질음성분석기(絲狀體質音聲分析機)를 이용한 체질별(體質別) 음향특성(音響特性) 연구(硏究)

1. Objectives The purpose of this study was to objectively diagnose American male and female's production of two vowels /a, i/ by Sasang Constitution. 2. Methods It was analyzed the constitutional characteristics of the American adults voices with PSSC-2004. of 134 cases of vowels /a, i/ with a duration of $2.5{\sim}3$ seconds were inputted in PSSC-2004 and analyzed into 40 factors. 3. Results and Conclusions 1) APQ In the male group's production of vowel /a/, the Soyangin's APQ(l), APQ(3) and APQ(4) were significantly high compared with those of Taeumin and Soeumin. 2) Shimmer In the male group's production of vowel /a/, Soeumin's Octave1 Shimmer was significantly low compared with that of Taeumin and Soeumin. In the male group's production of vowel /i/, Soeumin's D-Shimmer was significantly low compared with that of Taeumin and Soeumin. In the female group's production of vowel /a/, the Soyangin's C-Shimmer was significantly high compared with that of Taeumin and Soeumin. 3) Octave In the male group's production of vowel /a/, the Soyangin's Octave3, Octave4, Octave5, Octave6 and Octave1 Ratio were significantly high compared with those of Taeumin and Soeumin. In the male group's production of vowels /a, i/, the Soyangin's Octave4 was significantly high compared with that of Taeumin and Soeumin. 4) Energy In the male group's production of vowel /a/, the Soyangin's Time Domain Total Sum /Time Domain Count, Freq Domain Total Sum /cnt(0), 0k-4k Total Sum, Dev., A(A#, C, E, D#, E, F#) tot E, and A(C,, D#, F#) Dev. were significantly high compared with those of Taeumin and Soeumin. In the male group's production of vowel /i/, the Soyangin's Time Domain Total Sum /Time Domain Count, Freq Domain Total Sum /cnt(0) and 0k-4k Total Sum, Dev. were significantly high compared with those of Taeumin and Soeumin. 5) Peak In the male group's production of vowels /a/ and /i/,, the Soyangin's Peak1 Ratio was significantly low compared with that of Taeumin and Soeumin. In the male group's production of vowels /a/ and /i/,, the Soyangin's Peak10 Ratio, Time Domain Peak Total/Total Energy Sum, Time Domain Peak Dev. and Total/Total Dev. Sum were significantly high compared with those of Taeumin and Soeumin. 6) It is necessary to expand the research of the acoustic analysis of American and Korean to other countries in the diagnosis of the Sasang Constitution by using the voice characteristics.

소음인(少陰人) 전신부종(全身浮腫)에 대한 증례(證例)

뇌졸중을 일으키는 환자들에서 있어서 심폐의 기능적 및 기질적 이상이 선행요인으로 작용하거나 질병을 악화시키는 요인 및 합병증으로 병발되는 경우가 많다. 심폐질환이 동반된 경우 병의 진행과정이 급격히 악화될 가능성이 크며 예후가 불량한 경향을 띠므로 심폐질환에 주안점을 두어 급히 치료해야 한다. 상지대학교 한방병원 입원환자 중 뇌경색과 함께 울혈성 심부전으로 인하여 폐부종을 동반한 소음인 부종환자를 사상의학적으로 변증하고 치료하는 과정 중에 좋은 결과를 얻었기에 이에 증례보고를 하고자 한다. 68세 여환으로 우반신불수(右半身不遂) 연부장애(嚥不障碍) 어순(語純)의 중풍으로 내원하였으며 병의 진행 과정에 울혈성 심부전으로 진단되었고 폐부종의 소견과 함께 전신부종이 심하였다. 이를 소음인 부종이 태음증(太陰證)에 속한다고 하였으나 식(食) 변(便) 면(眠) 맥(脈) 설태(舌苔) 증상(症狀) 등을 종합하여 소음증(少陰證)으로 변증하고 건비이강음(建脾而降陰)의 치법(治法)에 준하여 궁귀총소이중탕(芎歸蔥蘇理中湯)을 투여하여 증상의 호전을 확인하였다. Occasionally, apoplexy is caused by functional or physiologic cardiopulmonary abnormality. In addition, theses may become the factor of aggravating apoplexy and involve complication. If patient has cardiopulmonary disease, progressing of apoplexy is commonly used to be accelerate and prognosis is deteriorative. There by the patient who has cardiopulmonary disease, should be particularly treated for cardiopulmonary disease. We got good result, during treat Soeumin cerebral vascular infarction patient who has pulmonary edema leaded from congestive heart failure, with Sasang Constitutional treatment. Therefore I reported them here. She was 68 years old, female patient. She visited our hospital for apoplexy with right side hemiparesis, dysphagia, dysphasia. After her admission into our hospital, we knew that she has congestive heart failure. The diagnosis is established. She presented with severe general edema and pulmonary edema. Although Soeumin edema usually classified as Taiyin Disease, we classified this as Shaoyin Disease, through diet, discharge, sleep, tongue and symptoms. We has prescribed Gungguichongsoyijung-tang(芎歸蔥蘇理中湯) in accordance with the principle of invigorating the Spleen and lower Yin(建碑而降陰). And her symptoms has improved.

Laryngograph와 EGG를 이용한 음향특성(音響特性)과 사상체질간(四象體質間)의 상관성(相關性) 연구(硏究)

1. 연구목적 한의학에서 음성을 듣고 환자의 상태를 알아내는 것은 문진에 해당되며 이는 내경시대부터 응용되어 왔다. "동의수세보원"에서도 말하는 기운이 각 체질별로 차이가 있는 것으로 언급하고 있다. 각 체질별 장국의 차이에 의해서 호산지기(呼散之氣) 흡취지기(吸聚之氣) 납적지기(納積之氣) 출방지기(出放之氣)등을 설명하고 있는데, 이는 또한 음성과 성대진동의 기본이 되는 공기의 흐름과도 연관이 있다. 한의학에서 성음을 통하여 사람에 관한 많은 것을 알 수 있다고 했는데, 여기서 논하는 성음의 개념은 매우 추상적이라서 객관적인 자료를 제시하기도 어렵고, 또한 임상연구에 응용이 매우 어려운 실정이다. 그러나 현대과학기술의 발전으로 음성을 수치, 그래프, 스펙트럼을 통하여 객관적으로 기록, 재생, 분석하는 것이 가능해졌다. 여기서는 음성에 직접적인 영향을 주는 요소 중의하나인 성대의 진동이 사상체질 진단의 객관적인 자료로 활용될 수 있는가를 알아보고자 했다. 2. 연구방법 (1) 연구대상자들을 사상체질분류검사지(QSCCII)와 사상의학 전문가가 체질을 분류한다. (2) 체질별로 CSL(Cmputerized Speech Lab)과 Laryngograph, Micrphone을 사용하여 음성과 성대진동양상을 녹취한다. (3) EEG processing option으로 녹취한 파형을 분석한다. (4) 분석한 항목을 통계 처리하여 항목별로 유의성을 살펴본다. 3. 연구결과 성대진동을 보여주는 EGG Waveform의 분석결과와 체질간의 상관성은 아래와 같다. 1. 남자/a/(0.5초)에서는 태음인보다 소양인의 Open Std Deviation, Contact Std Deviation이 더 높은 것으로 나타났다. 2. 남자/a/(2.5초)에서는 소음인과 태음인보다 소양인의 Pitch range가 더 높은 것으로 나타났다. 3. 여자/e/(0.5초)에서는 소음인보다 태음인의 Pitch range가 더 높은 것으로, 소양인보다 태음인의 Pitch Maximum이 더 높은 것으로, 소음인 소양인보다 태음인의 Pitch Std Deviation이 더 높은 것으로 나타났다. 4. 여자/a/(2.5초)에서는 소음인보다 태음인의 Contact Maximum이 더 높은 것으로, 태음인 보다 소음인의 Open mean, Open Miximum가 더 높은 것으로 나타났다. 5. 남자/e/(0.5초), 남자/e/(2.5초), 여자/a/(0.5초), 여자/e/(2.5초)에서는 체질간의 유의한 차이가 나타나지 않았다. 6. CART Algolism에서는 전체적으로 소음인과 태음인의 정판별력이 높았고, 소양인은 비교적 오차율이 높게 나타났다. Purpose of this study is to help classifying Sasang Constitution through correlation with Larynx waveform. This study was done it under the suppose that Sasang Constitution would be correlation with Larynx waveform. The following result were obtained about correlation between Erectroglottograph waveform and Sasang Constitution by analysis EGG program. 1. Taeumin was lower than Soyangin in Open Std Deviation, Contact Std Deviation of male/a/(0.5sec) 2. Soeyangin was high compared with the others in Pitch range of maie/a/(2.5sec) 3. Taeumin was higher than Soeumin in Pitch range, Soeyangin in pitch Maximum, and the others in Pitch Std Deviation of female/e/(0.5sec) 4. Taeumin was higher than Soeumin in Contact Maximum and lower than Soeumin in Contact Maximum of female/a/(2.5sec) 5. There was no significantly difference in male/e/(0.5sec), male/e/(2.5sce), female/a/(0.5sec), female/e/(2.5sec) 6. The percent of correctly classified in Soeoumin and Taeumin was high in CART Algolism. The risk estimate of Soyangin was relatively high. The study may be use on of the method to make objective diagnosis in Sasang constitution.
