척추관절경을 이용한 경피적 수핵제거술의 실패원인 및 합병증

Form December 1992 to December 1994, 207 patients who had a contained lumbar disc herniation refractory to non-operative modalities were treated by Arthroscopic Microdiscectomy at Lee Chun Tek Hospital in Suwon Korea. The purpose of this study was to reduce the incidence of complications and failures following Arthroscopic disc surgery. The results were obtained as follows; 1) Evaluation was made with St. Luke system's criteria. 31 patients were considered to have failed; Fair 11(5.3%) Poor 20(9.7%). Outcome analysis demonstrated failure rate of 15% 2) No serious complication was encountered. 3) The common causes of the failure were inadequate decompression of nuclear material, non-con-tained disc herniation and lateral recess stenosis. Although laminectomy remains the conerstone of spinal surgery, the low incidence of failure and complication in this group of patients suggest that AMD offers an alternative method of treatment for symptom producing lumbar disc herniation.

만성 불안정 족관절 증상을 가진 27명의 환자(31례)에 대하여 14명의 환자는 Evans술식으로, 13명의 환자에 대해서는 Modified Watson-Jones 술식으로 재건술을 시행하였으며 평균 3년간 추시 관찰한 결과 다음과 같은 결론을 얻었다. 1. 연령은 대부분 21세에서 30세 사이로 스포츠손상이 가장 많았다. 2. 족관절 동통과 불안정이 주증상이었다. 3. Evans 술식 및 Modified Watson Jones 술식으로 치료한 31례의 족관절에서 거골 경능식 평가검사로 환자의 87%에서 양호 및 우수의 결과를 얻었다. 4. Evans 술식 및 Modified Watson-Jones 술식은 비교적 다른 술식보다 간단하고 그 수술결과는 우수하였다. Twenty-seven patients(31 ankles) with chronic lateral instability of the ankle were treated by reconstructive operation. 14 patients were treated with Evans procedure, 13 patients with Modified Watson-Jones procedure. The average follow-up period was three years(1-6yrs). The results are as follows: 1. Most cases occurred between age 21 and 30, and sports injury was most common. 2. Pain and instability were main symptoms. 3. 31 ankles operated by Evans and modified Watson-Jones procedures showed following improvements. Talar tilt decreased from average 9.5 degrees to 2 degrees and and anterior translation from 4.3mm to 1.8mm. The 87% of cases were rated as Good or Excellent after functional evaluation. 4. Evans and Modified Watson-Jones techniques were relatively simple and the surgical results of them were excellent.
