동일인에의 내반변형슬과 동반된 외반변형슬에 대한 슬관절 전치환술

목적: 동일인에서 동반된 내반 및 외반 변형에 대해 인공관절 치환술을 시행하였고 이러한 변형을 가진 환자의 임상적 특징을 조사하고 술전 변형양상에 따라서 임상결과에 차이가 있는가를 규명하고자 하였다. 대상 및 방법: 2002년 3월부터 2005년 5월까지 한쪽 슬관절 내반변형과 반대편 슬관절 외반변형에 대해 인공 슬관절 전치환술을 시행한 환자 중 1년 이상 추시가 가능하였던 14명을 대상으로 하였다. 평균연령은 64.6세였고 평균 추시기간은 18.4개월이었다. 최종추시상의 방사선결과를 측정하였고 HSS 수치로 임상적 결과를 분석하였다. 결과: 술전 9명에서는 내반슬에, 2명에서는 외반슬에 통증이 심하였으며 8명에서 척추 측만증이 관찰되었고, 이 중 7명에서 외반슬 부위와 연관되었다. 2명에서 외반슬과 동측부위의 고관절 변형이 관찰되었다. 평균연령은 64.6세였고, 평균 추시기간은 18.4개월이었다. 술전 평균 외반변형각도는 10.5도였고 내반변형각도는 7.8도였다. 술후 평균 외반 및 내반각도가 각각 6.8도와 6.0도로 교정되었고, 술전 외반슬의 HSS 점수는 평균 술전 64.7점에서, 추시상 86.0점으로 향상되었으며, 내반슬은 술전 61.5점에서 추시상 86.9점으로 향상되었다. 수술 후 1예의 외반변형슬쪽에 patellar clunk 증상이 있어 관절경적 절제술 시행 후 완전 회복되었다. 결론: 동일인에서의 내반슬과 동반된 외반슬에 대한 인공관절 전치환술은 환자의 통증 감소, 기능향상 및 방사선적 결과에서 만족스러운 결과를 보였다. 변형의 원인으로 추정되는 척추 측만증과 동측의 고관절 변형이 관찰되었고 술전 변형양상과 동반변형으로 인한 임상적 결과는 차이를 보이지 않았다. 향후 더 많은 증례와 장기간의 추시가 필요할 것이다. Purpose: We have performed total knee arthroplasty for treating valgus and varus in the knees of one person. We investigated the clinical characteristics of these patients and the relationship between the kind of deformity and the postoperative result. Materials and Methods: From March 2002 to May 2005, fourteen patients who had simultaneous varus and valgus knee deformities underwent total knee arthroplasties and they were followed for more than 12 months, and these were the subjects of our study. The average age was 64.6.years and the average follow-up period was 18.4 months. Follow-up imaging assessments were done and the clinical outcomes were evaluated using the HSS score at the last follow-up. Results: Nine cases had more pain in the varus knee preoperatively and two cases had more pain in the valgus knee preoperatively. In 8 cases, degenerative scoliosises were associated with the knee deformity and among these cases, seven cases had vlagus deformities in the right knees. In two cases, hip deformities were noted in the ipsilateral side of the valgus deformity. The preoperative mean valgus angle and varus angle was 10.5 degrees and 7.8 degrees, respectively. The postoperative valgus and varus angle improved to 6.8 and 6.0 degree, respectively. The HSS score improved from 64.7 to 86.0 points for the valgus deformities and from 61.5 to 86.9 points for the varus deformities. Postoperative patellar clunk syndrome was identified in one valgus knee and arthroscopic resection was then performed. The knee had completely recovered on the last follow-up. Conclusion: Simultaneous or staged total knee arthroplasties in patients with simultaneous varus and contralateral valgus knee deformities achieved satisfactory outcomes with regard to the objective orthopedic criteria as well as the overall patient satisfaction in terms of pain relief and function. We found that scoliosis and ipsilateral hip deformities concurred with knee deformities, and this indicated that hip deformity and scoliosis are related with knee deformities. We found no significant difference between the kind of deformity and the postoperative result, yet a prospective study with more cases and longer follow-up is recommended in the future to reaffirm our findings.

The treatment of acute shoulder instability is different from chronic or recurrent instability. The acute shoulder instability usually accompanies bone defect of glenoid cavity and shows severe instability of double disruption of superior shoulder suspensory complex such as coracoid, acromion, clavicle and complex ligament injuries. We report 3 patients who underwent operations for double disruption of superior shoulder suspensory complex and regained shoulder stability.

인공 슬관절 전 치환술에서 확인된 내측 반월상 연골판 후방 골기시부 파열의 관련 요인에 대한 분석

목적: 인공 슬관절 전 치환술 도중 내측 반월상 연골판 후방 골기시부 파열에 대한 빈도를 조사하였고, 파열에 영향을 미칠 수 있는 요인에 대하여 분석하였다. 대상 및 방법: 2008년 1월부터 2009년 6월까지 슬관절염으로 인공 슬관절 전 치환술을 시행 받고 내측 반월상 연골판 후방 골기시부의 파열 유무를 조사받은 173명 258예를 대상으로 하였다. 평균 연령은 68.8세(50∼87세)였고 남자가 23명(28예), 여자가 150명(230예)이었다. 연령, 성별, 체질량 지수(BMI), Kellgren-Lawrence에 따른 골관절염의 정도, 외상의 유무, 좌우 빈도 및 내반 변형의 정도 등을 조사하여 통계적으로 분석하였다. 결과: 내측 연골판 후방 골기시부 파열은 258예 중 182예(70.5%)에서 관찰되었고, 남자가 28예 중 18예 (64.2%), 여자가 230예 중 164예(71.3%)였으며 통계적 유의성은 없었다(p=0.102). 골관절염의 정도 (p=0.040) 및 내반 변형의 정도와 파열의 비율은 통계적으로 유의한 값(p=0.030)을 보였고, 체질량 지수, 외상의 유무 및 좌우의 관계와 파열은 유의성이 없었다(p>0.05). 결론: 인공 슬관절 전 치환술시 관찰된 내측 연골판 후방 골기시부 파열은 골관절염의 정도 및 내반 변형과 연관성이 있는 것으로 보이나, 퇴행성 관절염의 선행 원인인가에 대한 더 많은 연구가 필요할 것이다. Purpose: We examined the incidence of root tear of the medial meniscus posterior horn among patients who had undergone total knee replacement and we analyzed the factors that cause this tear. Materials and Methods: We enrolled 258 knees of 173 patients who had undergone total knee replacement from January 2008 to June 2009. The mean age was 68.8 (range: 50∼87). There were 23 men (28 cases) and 150 women (230 cases). We performed statistical correlation analysis between the assumed causes of meniscal tears (age, gender, grade of osteoarthritis, the predisposition site, BMI and varus deformity) and root tears. Results: Meniscal tears were observed in 182 cases (70.5%), and there were 18 cases (64.2%) of 28 cases and 164 cases (71.3%) of 230 cases in the men and women, respectively. Root tear had correlation with the severity of osteoarthritis (p=0.040) and varus deformity (p=0.030), but other underlying factors didn`t show significant correlation (p>0.05). Conclusion: Root tears of the medial meniscus posterior horn in total knee arthroplasty were related to the severity of osteoarthritis and varus deformity, but active interventions are required whether or not there are predisposing factors that cause osteoarthritis.

전방 불안정성과 동반된 슬관절 내측 구획 진행성 관절염환자에서 전방십자인대 재건술 및 인공 관절 부분 치환술의 결과 - 3예 보고 -

비교적 젊은 연령의 환자에서 슬관절 내측 구획의 퇴행성 관절염(Kellegrene-Laurence 제 3단계 및 Outerbridge 제 4단계)과 전방십자인대의 파열이 동반되어 전방 불안정성이 있는 3예에서 2예는 전방십자인대 재건술의 6개월 이후 단계적으로 인공 관절 부분 치환술을 시행하고 나머지 1예는 동시에 전방십자인대 재건술 및 부분 치환술을 시행하였으며 수술 전, 후의 International Knee Documentation Committee (IKDC), Lysholm 점수와 최종 추시 상 Hospital for special surgery (HSS)와 knee society score (KSS) 점수를 측정하였다. 저자들의 슬관절의 전방 불안정성과 동반된 내측 구획의 진행성 관절염에서 전방십자인대 재건술과 함께 동시에 또는 단계적으로 시행한 내측 구획 인공 관절 부분 치환술은 슬관절 불안정성과 진행성 관절염에 의한 통증을 동시에 또한 만족스럽게 해결할 수 있는 좋은 선택이라고 사료된다. Three cases who had medial compartment osteoarthritis of the knee (Kellgrene-Laurence grade 3 and Outerbridge grade 4) and anterior instability of the knee due to rupture of the anterior cruciate ligament in relative young ages underwent staged anterior cruciate ligament reconstruction followed by medial unicondylar arthroplasty in 2 cases and simultaneous anterior cruciate ligament reconstruction and unicondylar arthroplasty. We evaluated clinical results some kinds of preoperative and postoperative International Knee Documentation Committee (IKDC), Lysholm score and last follow-up hospital for special surgery (HSS), knee society score (KSS). We consider that medial unicondylar arthroplasty with staged or simultaneous anterior cruciate ligament reconstruction is very good option of the treatment for the anterior instability and pain from advanced arthritis.

Pigmented villonodular synovitis (PVNS) is a slowly, progressive, proliferative disorder of synovial tissue characterized by villous or nodular changes of synovial-lined joints, bursae, and tendon sheaths and most frequently affects the large joints, with the knee and hip. A few studies have been reported that occurred PVNS in small joint, but mainly in hands. It is a very rare condition that occurs in the small joints of the forefoot. We have experienced the case, which developed in small joint of the forefoot, and performed total synovectomy. After the operation, there was no recurrence. We report a case of PVNS in forefoot with a review of the literature.

흡수성 반월상 연골나사를 사용한 얇은 골연골 골절의 치료 - 증례 보고 -

관절내 골연골 골절은 적절한 치료가 이루어 지지 않은 경우 골관절염을 초래하여 통증과 기능장애를 초래할 수 있다. 골 연골 골절의 크기가 큰 경우에는 관혈적 정복 및 금속고정 수술등으로 치료를 할 수 있으나, 골편의 두께가 2mm 이하로 얇은 경우 튼튼한 고정력을 얻기 어려워 제거 하는 경우가 많았다. 저자들은 대퇴 내과에 발생한 박리성 골연골염 환자와 슬개골 골절로 골편의 두께가 2mm이하인 환자에서 흡수성 반월상 연골나사를 이용하여 좋은 결과를 얻었기에 보고하고자 한다. Inappropriate treatment of osteochondral fracture can cause osteoarthritis, pain, functional disorder. With large osteochondral fracture, reduction and fixation of the fragment using metal implant. However, when the bone fragment had less than 2mm, the fragment extracted because of difficulty of fragment fixation. Authors treated patients with fracture fragment thickness less than 2mm of osteochondral dissecans in medial femoral condyle and patella fracture using biodegradable meniscus screw, and then we obtained good result.
