자궁경부종양에서 중합효소 연쇄반응기법을 이용한 인유두종바이러스 type 16 및 type 18의 검출률과 예후인자와의 상관관계

Human papillomavirus(HPV) has been implicated in the development of uterine cervical cancer. Detection of the small amounts of HPV DNA in cervical cells has been very difficult. The polymerase chain reaction(PCR) is a new technique that can specifically amplify target DNA to facilitate its detection. PCR technique wes used to detect HPV types 16 and 18 in cervical specimens obtained from normal cervix(20 cases), dysplasia(25 cases), carcinoma in situ(21 cases), microinvasive cancer(ll cases), and invasive cancer(46 cases). And then, case of invasive carcinoma of the uterine cervix were analyzed to determine that the presence of specific human papillomavirus DNA in the neoplasm was a contributing factor to their outcome. The detection rate of HPV 16 DNA in normal cervix, dysplasia, ClS, microinvasive cancer, and invasive squamous cell carcinoma were 50.0%, 36.0%, 81.0%, 45.5%, and 58.7%, respectively. The detection rate of HPV 18 DNA in normal cervix, dysplasia, CIS, microinvasive cancer, and invasive squamous cell carcinoma were 0.0%, 8,0/, 4.8%, 0.0%, and 19.6%, respectively. Of the factors evaluated in invasive cervical cancer, adenocarcinomatous component(p= 0.004) and tumor grade(p=0.015) were found to be correlated with HPV l8 infection. 5 of 8 women whose tumors contained glandular elements had HPV 18 DNA, whereas only 4 of 38 women whose tumors contained only squamous elements showed this infection. 6 of 9 women of HPV l8 infected tumors were grade 3 tumors as compared to only 7 of 28 of HPV 16 infected tumors. Age at diagnosis and nodal status in relation to HPV type 18 exhibited a trend but were not statisitically significant. These observations suggest that HPV type 18 may be associated with a more aggressive form of cervical cancer than HPV type 16.

수술시 동결절편검사상 Krukenberg종양으로 진단되어 이후 원발병소에 대한 조사를 하였으나 원발병소를 확인할 수 없었던 원발성 난소 Krukenberg종양이라고 생각되어지는 1예를 경험하였기에 간단한 문헌고찰과 함께 그 증례를 보고하는 바이다. Krukenberg tumor is one of the malignant ovarian cancers, which has characteristic pathologic findings. We have experienced a case of primary Krukenberg tumor that we could not find the primary focus at any other organs. So we report this case with a brief review of concerned literature.
정상산모와 임신성 고혈압산모에서의 혈장내 Atrial Natriuretic Peptide (ANP)

정상산모와 임신성 고혈압산모(PIH)의 혈장내 ANP농도의 변화를 규명하기 위하여 1989년 3월 1일부터 5개월간 본원 산실에서 분만한 정상산모 20명과 PIH산모 18명을 관찰대상으로 하고 비임신 정상여성 17명을 대조군으로 하여 조사한 바 그 성적을 다음과 같이 요약한다. 정산산모의 혈장 ANP농도는 30.8±18.7pg/ml로서 정상 비임신 대조군의 19.7±8.2pg/ml보다 높았으나 유의차는 없었고, PIH산모의 혈장 ANP농도는 71.3±37.1pg/ml로서 정상산모나 정상 비임신 대조군보다 유의하게 높았다(p$lt;0.05). 중증 PIH산모의 혈장 ANP농도는 95.2±33.1pg/ml이고 경증 PIH산모의 농도는 47.4±23.4pg/ml로서 그 차이는 유의하였다(p$lt;0.05). 정상산모와 경증 PIH산모에서 분만 1주후의 혈장 ANP농도는 정상 비임신 대조군의 수치와 거의 비슷하였으며, 중증 PIH 산모는 분만 2주후에 혈압이 정상화되면서 ANP치가 정상 비임신 대조군의 수치와 거의 일치하였다. PIH산모에서 혈장 ANP농도는 수축기혈압과 유의한(p$lt;0.05) 상관관계(R=0.52633)가 있었으며 이완기혈압과는 무관하였다. Atrial natriuretic peptide (ANP) with potent natriuretic, diuretic, and smooth muscle relaxant activities is synthesized and secreted from atrial myocytes. Although the precise mechanism involved in the mediation of ANP secretion is unclear, atrial pressure and atrial stretch consequent to volume expansion appears to be one of the most important stimuli for ANP release. A striking hemodynamic changes occur in women during pregnancy that may alter plasma ANP concentration. We measured plasma ANP concentrations by specific radio-immunoassay method in 20 normal pregnant women and 18 pregnancy induced hypertensice (PIH) women. The mean plasma ANP concentration in normal pregnant women was higher (30.8 ± 18.7 pg/ml) than in young normal nonpregnant controls (19.7 ± 8.2 pg/ml), but not statistically significant. In PIH women, the mean concentration of plasma ANP was significantly higher (p $lt; 0.05) than in both normal pregnant women and normal nonpregnant controls. The mean plasma concentrations of ANP in severe and mild PIH women were 95.2 ± 33.1 pg/ml and 47.4 ± 23.4 pg/ml, respectively, and there was statistical (p $lt; 0.05) difference between two groups. The mean plasma ANP concentration in normal pregnant and mild PIH women fell to nonpregnant levels at postpartum one week (13.1 ± 6.0 and 22.8 ± 13.1 pg/ml). In contrast, the mean concentration of plasma ANP of postpartum one week in severe PIH women was still higher (34.9 ± 12.0 pg/ml) than that of nonpregnant controls, but fell to nonpregnant level as blood pressure returned to normal. When plasma ANP concentrations in PIH women were compared with the simultaneously measured blood pressure, a positive correlation (p $lt; 0.05) was found between plasma ANP concentrations and the systolic blood pressure, but no positive correlation was found plasma ANP concentrations and the diastolic blood pressure.
