        교차소뇌해리현상 : SPECT와 MRI 소견의 비교와 임상징후

        Objectives : The purpose of our study was to compare findings of brain SPECT representing crossed cerebellar diaschisis(CCD) with brain MRI, to evaluate relation between CCD and location of lesions on MRI and to elucidate clinically apparent cerebellar sign in patients with CCD. Methods : The study population was 20 patients representing CCD on SPECT. Percentage differences(${\triangle}%$) of activity on each cerebellar hemisphere were obtained from ipsilateral and contralateral cerebellum[${\triangle}%cbll=(IL-CL)/IL{\times}100$] and from cerebrum [${\triangle}%cbr=(CL-IL)/CL{\times}100$]. From MR studies, the percentage differences of signal intensity were also calculated as the same method. We compared the degree of percentage differences with location of cerebral lesions and with clinical cerebellar signs of the patients. Results : Among those representing CCD, the parietal lesions were the most common. There was significant correlation of the percentage differences in cerebellum between SPECT($18.8{\pm}7.22$) and MRI($4.4{\pm}3.38$) (p<0.05) and in cerebrum between SPECT($28.7{\pm}15.35$) and MRI($42.8{\pm}10.94$) (p<0.05). Cerebellar signs were observed in 3 of the 20 patients. However, there was no statistically significance between degree of percentage differences of each cerebellar hemisphere on SPECT and clinical cerebellar sign(p>0.05). Conclusion : Using the percentage differences in the cerebellum, the CCD evaluation can be easily done. On MRI, the signal changes of cerebellum were not as definite as SPECT. Despite of our assumption, there was no significant correlation between clinical cerebellar signs and CCD on SPECT.

        정상인과 파킨슨병 환자에서 [I-123]IPT SPECT를 이용한 도파민 재섭취부위의 영상화

        Objective : Dopamine transporter concentrations have been known to decrease in Parkinson's disease(PD). The aim of the present study was to evaluate the correlation between SPECT measurements of [I-123]N-(3-iodopropene-2-yl)-$2{\beta}$-carbomethoxy-$3{\beta}$-(4-chlorophenyl) tropane(IPT) as an imaging agent for measuring changes in transporter concentrations with PD. Patients and Methods : IPT labelled with $4.87{\pm}1.29mCi$($180.19{\pm}47.73MBq$) of [I-123] was intravenously injected into 23 patients(age : $58{\pm}12$) with PD and three normal controls(NC)(age : $37{\pm}7$) as bolus. Brain SPECT were then performed at 1 hour and 2 hours after injection on a double headed camera. The statistical parameters were the contrast ratio of left basal ganglia(BG) and right basal ganglia to occipital cortex(OCC) per milli curies of injected radiotracer at 1 hour and 2 hours. The correlations were evaluated between these parameters and Hoehn-Yahr classification of the patients. Results : The(BG - OCC)/OCC/mCi ratios at 1 hour and 2 hours for PD and NC were $0.14{\pm}0.07$ and $0.27{\pm}0.07$(1 hour) and $0.12{\pm}0.07$ and $0.34{\pm}0.04$(2 hour), respectively. The(BG - OCC)/OCC/mCi ratios of Parkinson's disease were decreased with higher grade of Hoehn-Yahr classification of the patients. The ratio between BG and OCC for PD were clearly separated from NC and may be useful outcome measures for clinical diagnosis. Conclusion : The findings suggest that IPT may be a very useful tracer for early diagnosis and treatment of PD and study of dopamine re-uptake site.

        한국인 태아 조기질의 미세구조에 관한 연구

        The differentiation of nail matrix and fine structure of matrix cells were studied with light and electron microscope using specimens from nails of thumb finger in Korean fetuses 14 to 24 weeks old. Fetal nail matrix consisted of two horizontal layers, thicker ventral and thinner dorsal matrices, originating from invagination of epidermis in proximal nail field. Matrix being generally thicker in its distal region than the apex became gradually thickened with increase of the fetal age. Each matrix consisted of single layer of basal cells and multiple layers of squamous cells which are arranged close to and parallel to the central axis of the nail mairix. The process of keratinization of fetal nail matrix was noted to be occured concurrently in the ventral and dorsal matrices along the central axis of matrix toward distal and dorsal direction. Squamous cells became matured with accumulation of tonofilaments, increase of keratohyalin granules, discharge of membrane coating granules, and narrowing of intercellular spaces, thickening of plasma membrane and finally being transformed into horny cells of nail plate. Horny cells of nail plate filled with fibrous elements in the electron dense amorphous substance. These findings of keratinization process of fetal nail matrix appeared to be similar to those of keratinization in epidermis and inner root sheath of the hair. In the nail matrix, however, corresponding region to the keratogenous zone of growing hair follicle was not observed. Vacuolated squamous cells of nail matrix seen on light microscopy was considered to be artefactual product, but squamous cells with condensed small nuclei rarely found adjacent nail plate was considered to be one of the squamous cells with unknown function. Proximal end of nail plate was observed on dorsal surface of nail field distal to the proximal nail fold at 14 and 16 weeks old human embryos. Proximal prolongation of the proximal end of nail plate was occured with advancing fetal age and afterward 21 weeks nail plate invaded into nail matrix. Melanin granule containing cells and Merkel cells were present only on the basal layer of dorsal nail matirx.

        전이성 척추 악성 종양의 진단 - 골스캔과 자기공명영상의 비교

        Authors retrospectively compared the 99mTc MDP bone scans and corresponding MR imagings in 20 patients with histologically proven malignancy. Mean interval of the two studies was 16.6 days. Cancer diagnosis included 8 lung, 2 each of colon, breast, stomach, 1 each of prostate, thyroid, malignant lymphoma and 3 adenocarcinoma of unknown primary site. Of the 105 regions compared, 46 regions were positive for metast,ases in bone scans ar MR imagings. 30 regions(65.2%) were positive by bone scan and 44 regions(95. 7%) by MR imaging. 87 regions(82.9%) were concordantly positive or negative by bone scan and MR imaging, but 18 regions(17.1%) were discordant. In the discordant regions, 16 regions positive in MR imaging were negative in bone scan. The greatest number of discordant findings occurred in the cervica.l region and in the patient with stomach cancer. Our results suggest t.hat the sensitivity of MR imaging is great,er than that, of bone scan in det.ecting spinal metastases. And bone scan is useful screening test of metastasis for evaluting entire skeleton including spine.

        태전모반 (太田母斑) 에 대한 유전학적 ( 빈도를 중심으로 ) 및 임상적 연구

        The clinical observation was done on 126 patients of Ota's nevus, 113 of whom were seen at Department of Dermatology, Chonnam National University Hospital from January 1973 through July 1983, and 13 of whom were found among 45, 87l male and female students of high school in Kwangju City, Korea. The results obtained were as follows: 1. The incidence of male and female students of Ota's nevus were respectively 0. 013% and 0.043%, with the incidence,as a whole being 0.03%, whereas the frequency of Ota's nevus among the outpatients was 0. 24% with the ratio of male to female being 1: 2. 67. 2. There was only one family of familial occurrence, in which 2 sisters were involved among 5 siblings. 3. Thirty three percent of Ota's nevus showed the onset at birth. 4. According to Tanino's classification, our patients showed that Type II was 41.7% and then Type Ia, III, Ib, and IV being respectively 33. 3%, 16. 7, 4.2, and 4. 2%. 5. Seleral involvement was 54. 2%, especially in type III and Type II. 6. The color was increased in 54%, especially Type II by internal and external factors such as fatigue, warm temperature, menstruation and cold temperature. 7. The color intensification was prominent at the age of 10 to 14.

        외상후 뇌손상 환자에서 Tc-99m-HMPAO 국소뇌혈류 SPECT 와 자기공명영상의 비교

        The purpose of this study was to compare Tc 99m HMPAO SPECT with MRI aft,er acute and subacute closed head injury. There were thirty two focal lesions in all cases of these, Fifteen lesions(47%) were seen on both MRI and SPECT. Fourteen lesions(44%) were seen only on MRI. Three lesions(9%) were seen only on SPECT. Of the 14 lesions seen only on MRl, one was epidural hematoma, two were subdural hematoma, three were subdural hygroma, one was intracerebral hematoma, four were contusion, and three were diffuse axonal injuries. SPECT detected 52% of the focal lesions found on MRI. For the detection of lesions, MRl was superior to SPECT in fourteen cases, while SPKCT was superior to MRI in three cases. In conclusion, there was a tendency that detection rate af the traumat,ic lesions was higher on MRI, but the SPECT could delineate more wide extent, of lesion.

        정상인에서의 Tc-99m-DTPA 폐제거율

        Pulmonary clearance of Tc99m DTPA(PCD) has been used for the measurement of pul-monary epithelial permeability. It, has been reported to be increased not only in variety of pul-monary diseases including ARDS, interstitial fibrosis, and smokers, but also in normal subject,s on posit.ive end expiratory pre,ssure respirator, or after exereise. It was also noted that decrease of pulmonary blood flow due to pulmonary arterial obstruction results in delayed PCD. Normal range of PCD varies with institutes. We prospectively rneasured PCD in 17 normals (5 males and 12 females) consisted of staffs and trainees in the department of radiology of Kangnam St, Marys hospital using original Bark Nebulizer (India). Age ranged from 32 to 43 years. 370 MBq of Tc99m DTPA was inhaled in supine position and supine posterior images were subsequently obtained with 1 min/frame, 64 x 64 matrix and word mode for 30min. Regions of in 1,erest. Were set on each lung, whole lungs, and upper, middle and lower thirds of right lung, respectively. Best fit regression curve was obtained by least square method from inilial 7min after peak act,ivitu on each curve and time for half clearance of maximum activity (tl/2) was calculat,ed. Mean tl/2 was 51 +- 11.2min for whole lung, There was no significant difference between tl/ 2 of right and left lungs, Initial uptake was higher in the lower third and tl/2 was shorter in the lower third than in the upper third(P<0.05). We reviewed several reports on PCD and compared our data with the others. In t,his study, faster clearance in the lower third may be due to the position imaged with or the environment the subject.s belong to, and further invest,igation is under way.

        호산구성 봉소염

        We report a 44-year-old female patient with Eosinophilic cellulitis who had developed pruritic erythematous plaque on the right lower abdomen since two months ago. The blood eosinophil count was 2,066/mm at first visit. This lesion responded well to cortieosteroid therapy. However new erythematous swelling reappeared on the left lower abdomen and left inner thigh 3 months after the first treatment. A biopsy specimen taken from an erythematous plaque showed many flame figures and diffuse infiltration of eosinophils and histiocytes in the lower dermis and subcutaneous fat tissue.
