        20대 성인남녀의 인지체력과 측정체력과의 관계: U-healthcare 적용을 위한 접근

        Background: Physical fitness test as an index for monitoring health status has often been not feasible due to cost and time limitation. Self-perceived fitness level was thought to be useful in estimating fitness levels of a target population. This study examined whether perceived fitness level reflects actually measured fitness level and whether it has possible application to prepare U-exercare program. Methods: Two hundred young adults were recruited at one university. Body composition, aerobic capacity, muscular strength, flexibility, agility, power, balance and overall fitness level were measured using a self-questionnaire. Pearson correlation coefficients were used to analyze relationships among parameters. Results: Perceived and measured body fat contents were correlated in both males and females. Perceived and measured aerobic capacities were only correlated in females. Perceived and measured muscular strength,flexibility and power were correlated in both genders. No correlation was noticed in agility (reaction time) and balance in both genders. Conclusions: The relationship between perceived and actually measured physical fitness parameters among young adults could be used as essential information for building a U-healthcare exercise program. 연구배경: 운동관리에서 건강 상태를 진단하는 주요한 지표인 체력은 비용과 시간 등의 제한점 때문에 잘 활용되지 못하고 있다. 따라서 신뢰도가 높으면서 즉각적이고 간편한 자가인지(self-perception)체력 설문이 실제 체력을 얼마나 잘 대변하는지 살펴보고 이를 유헬스운동케어(U-exercare)에 적용 가능한지에 대해 논의하고자 하였다. 방법: 일개 대학교의 체력측정실에서 20대의 남·여 대학생을 분석 대상으로 하였다. 신체조성, 유산소성능력, 근력, 유연성, 민첩성, 순발력, 평형성, 종합체력에 대한 인지체력 설문을 실시한 뒤, 실제체력 수준이 측정되었다. 측정된 체력은 신체구성, 유산소성능력, 근력 및 근지구력, 유연성, 민첩성, 순발력, 평형성의 하위항목이었으며 측정을 마친 후 피어슨 상관분석을 실시하였다. 결과: 인지신체구성과 실제신체구성에서는 남·여 모두 체지방률과 상관이 있었으며, 유산소성 능력에 대한 인지와 실체측정값은 여성에게서만 약한 상관을 보였다. 근력 및 근지구력, 유연성, 순발력의 항목에서는 남·여 모두에서 높은 상관을 나타냈으며, 민첩성 항목의 반응 시간과 평형성에서는 남·여 모두 상관성이 없는 것으로나타났다. 결론: 본 연구에서 진행된 실제체력과 인지체력과의 상관관계 비교·분석결과는 유헬스케어에서 운동서비스 프로그램의 기본 자료로 사용될 수 있을 것이며, 보다 안전한 신체활동을 유도하고 운동 실천 문화 확산에 기여할 것이다.

        T-REX 삼축 가속도계의 다양한 부착위치에 따른 신체활동량 추정 타당도 검증

        이 연구의 목적은 새로 개발된 삼축 가속도계인 T-REX의 신체활동량 추정에 대한 타당도를 준거 도구(가스호흡분석기)와 참고 도구(가속도계)와의 비교를 통해 검증하는 것이다. 건강한 20대 남녀 50명은 운동자각인지도 12-14 수준으로 훌라후프, 스텝박스, 줄넘기와 제자리 걷기를 수행하였다. 대상자들은 운동을 수행하는 동안 T-REX(발목, 상완, 가슴, 허리, 손목 위치)와, ActiGraph(허리 위치), Actical(허리, 가슴 위치)을 착용하였으며, 산소섭취량이 동시에 측정 되었다. 타당성은 피어슨 상관계수로 분석하였다. T-REX는 발목, 가슴, 허리 위치에서 스텝박스 운동과(r=.83-.91) 훌라후프 운동 시 상완과 손목 위치에서(r=.82-.84) 산소섭취량과 매우 강한 상관관계를 보였으며, 제자리 걷기 운동 시에는 상완과 손목에서 보통이상의 상관관계(r=.63-.71)를 나타내었다. T-REX는 두 종류의 참고도구와 비교했을 때 산소섭취량과 더 높은 상관관계를 보여, 신체활동 검사 도구로 충분히 상용화될 수 있을 것으로 판단된다. This study examined the validity of the T-REX accelerometer to measure physical activity in men and women comparing with Actigraph and Actical. Fifty healthy men and women completed four different types of exercise (step box, jump rope, Hula hoop, and on-site walking) at a perceived exertion intensity rated 12-14. Oxygen uptake (VO<sub>2</sub>) were assessed from all participants while assessing physical activity using T-REX (attached at the ankle, arm, chest, waist, and wrist) and, simultaneously, Actigraph (attached at waist) and Actical (attached at the waist and chest), respectively. VO<sub>2</sub> was measured using the Cosmed K4b2 portablesystem. There were very strong to strong associations between T-REX and VO<sub>2</sub> measured during Step box activity (T-REXs were attached at the ankle (r=.91), chest (r=.83), and waist (r=.86). There were also strong associations between T-REX and VO<sub>2</sub> measured during Hula hoop activity (T-REXs were attached to the arm (r=.82) and wrist (r=.84). Also, there were significant associations between T-REX and VO<sub>2</sub> measured during on-site walking at five locations (r=.63-.71). When compared with ActiGraph and Actical, T-REX showed higher associations with VO<sub>2</sub>, which indicates a superior instrument to measure physical activity. The T-REX has high validity with VO<sub>2</sub> to measure physical activity including Step box activity in three locations (ankle, chest, and waist), Hula hoop activity in two locations (arm, wrist), and on-site walking in one location (ankle).

        Feasibility and Effects of a Postoperative Recovery Exercise Program Developed Specifically for Gastric Cancer Patients (PREP-GC) Undergoing Minimally Invasive Gastrectomy

        Purpose: Exercise intervention after surgery has been found to improve physical fitness and quality of life (QOL). The purpose of this study was to investigate the feasibility and effects of a postoperative recovery exercise program developed specifically for gastric cancer patients (PREP-GC) undergoing minimally invasive gastrectomy. Materials and Methods: Twenty-four patients treated surgically for early gastric cancer were enrolled in the PREP-GC. The exercise program comprised sessions of In-hospital Exercise (1 week), Home Exercise (1 week), and Fitness Improvement Exercise (8 weeks). Adherence and compliance to PREP-GC were evaluated. In addition, body composition, physical fitness, and QOL were assessed during the preoperative period, after the postoperative recovery (2 weeks after surgery), and upon completing the PREP-GC (10 weeks after surgery). Results: Of the 24 enrolled patients, 20 completed the study without any adverse events related to the PREP-GC. Adherence and compliance rates to the Fitness Improvement Exercise were 79.4% and 99.4%, respectively. Upon completing the PREP-GC, patients also exhibited restored cardiopulmonary function and muscular strength, with improved muscular endurance and flexibility (P<0.05). Compared to those in the preoperative period, no differences were found in symptom scale scores measured using the European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life Questionnaire (QLQ-C30) and Quality of Life Questionnaire-Stomach Cancer-Specific Module (QLQ-STO22); however, higher scores for global health status and emotional functioning were observed after completing the PREP-GC (P<0.05). Conclusions: In gastric cancer patients undergoing minimally invasive gastrectomy, PREP-GC was found to be feasible and safe, with high adherence and compliance. Although randomized studies evaluating the benefits of exercise intervention during postoperative recovery are needed, surgeons should encourage patients to participate in systematic exercise intervention programs in the early postoperative period (Registered at the ClinicalTrials.gov, NCT01751880).

        임신과 출산 후 걷기 강도에 따른 운동 및 회복 반응

        PURPOSE: To examine the influence of the walking exercise intensity during pregnancy and postpartum on physiological and perceived exertion responses. METHODS: Nine participants walked on a treadmill at 3, 5, and 6 km/h for 10 minutes followed by resting for 10 minutes after each speed. During exercise and resting, heart rate (HR), oxygen uptake (VO2), and perceived exertion (RPE) were measured. RESULTS: During pregnancy, HR at 5 and 6 km/h walking and the recovery HR from walking was higher at 3, 5 and 6 km/h than postpartum (p<.05). VO2 during pregnancy was higher than postpartum at 3 and 5 km/h walking (p<.05, p<.01). During pregnancy, RPE at 5 and 6 km/h walking was higher than postpartum (p<.001). Three pregnant women, due to abdominal cramp during walking at 6 km/h, no statistical comparisons in VO2 and RPE were adequate. During pregnancy, HR was higher as the exercise intensity increased, and the recovery was delayed. VO2 of pregnant women was higher during exercise than postpartum. CONCLUSIONS: Since pregnancy changes many aspects of physiological and physical status, long term and systemic longitudinal studies providing objective index are warranted.

        지리정보시스템(GIS)에 의한 여가공간 및 체육시설과 중장년 지역주민의 건강평가지표 변인과의 관계

        Although various aspects of the built environment are known to affect a status of community health, direct comparisons between the infrastructures of a community and health status has not been elucidated. In this study, the relationships between public and private leisure and recreational facilities as well as sport centers and directly measured health variables were examined. Health related variables from 5,497 (2,584 men and 2,913 women) residents aged over 40 of the district of Gangbuk, Seoul have been analyzed. Data based on four legal Dongs were compared and Pearson correlations were employed for relationships between variables. No definite relationships between the two variables were noticed, suggesting the availability of activity facilities may be not solely explaining the health measure outcomes. It is thought that both objective and perceptional measures for analyzing health outcomes should be included. Sociodemographic status also seemed to be a candidate for inclusion factors.

        자연과학편 : 임신부의 걷기 강도에 따른 심박수, 산소섭취량, 운동자각의 반응

        이 연구는 임신부와 비임신부의 운동 및 회복 시의 심박수, 산소섭취량, 운동자각도를 비교하여 임신부들에게 추천되는 걷기 운동의 강도를 확인하는데 목적을 두었다. 주치의에게 허가를 받은 중·후기 임신부 11명과 비임신부 15명이 연구에 참여하였다. 트레드밀에서 10분씩 세 가지 속도(3 km/h, 5 km/h, 6 km/h)에서 걷기를 수행하였으며, 각 속도에서 걷기 후 회복기를 가졌다. 운동 중과 회복 초기 10분 동안 심박수, 산소섭취량, 운동자각도, 배뭉침 정도를 측정하였다. 모든 운동 강도 및 회복기에 임신부들의 심박수가 비임신부들보다 높았다. 심박수회복과 운동자각의 변화는 3 km/h의 낮은 강도에서는 차이가 없었지만 5 km/h, 6 km/h의 속도에서는 유의한 차이가 있었다. 운동 강도가 높아질수록 회복기에 임신부는 비임신부에 비하여 안정 시 수준으로 심박수의 회복이 느리며 힘들게 느꼈다. 5km/h 수준의 걷기 운동 속도는 건강한 중·후기 임신부들에게 추천될 수 있는 걷기 운동 강도라고 여기진다. 추후 임신부의 주수를 세분화한 종단적 연구가 필요해 보인다. This study was designed to compare the heart rate (HR), oxygen uptake (VO2), and the ratings of perceived exertion (RPE) during exercise and recovery in pregnant and non-pregnant women and to verify the moderate walking exercise intensity recommended for pregnant women. Fifteen medically cleared mid- and late-stage pregnant women and eleven non-pregnant women participated in this study. They walked on a treadmill at 3 speeds (3, 5, and 6 kph) and recovered after each speed. During walking and 10-min recovery, their HR, VO2, RPE, and the pregnancy related abdominal cramp (AC) were measured. HR during exercise and recovery was higher in pregnant than non-pregnant women. Changes of HR recovery and RPE at 3 kph was not different, but was different at 5 and 6 kph. As exercise intensity increased, HR recovered slower in pregnant women than non-pregnant women. Pregnant women felt harder to the exercise intensity as the intensity increased. The walking of 5 kph level can be considered moderate walking intensity recommended for healthy mid- and late-stage pregnant women. Exercise responses to subdivided pregnancy stage and longitudinal studies are warranted.
