Mitochondrial DNA disorder is a heterogenous group of diseases that is related to disorders of mitochondrial (mt) energy metabolism. MELAS (mitochondrial encephalopathy with lactic acidosis and stroke-like episode) is a major maternally inherited multi-systemic disorder, in which 80% of cases are associated with mt DNA A3243G point mutation. The clinical course of MELAS is highly variable, ranging from asymptomatic to progressive muscle weakness, seizures, stroke-like episodes, encephalopathy with lactic acidosis and premature death. However, gastrointestinal symptoms in patients with MELAS are seldom documented. We report a 17-year-male with MELAS presenting chronic gastrointestinal pseudo-obstruction. We confirmed that he had the classical A3243G mt DNA point mutation.
프로트롬빈시간 모니터링을 위한 CoaguChek XS의 유용성

연구배경: 와파린의 치료 효과를 유지하고 부작용을 방지하기 위해서는 수시로 프로트롬빈 시간을 모니터링 할 필요가 있다. 이러한 목적의 현장검사기기는 편리하지만 그 정확성에 있어서 문제가 될 수 있으므로 평가가 필요하다. 방법: 2007년 8월부터 2008년 2월까지 순천향대학교 부천병원에 내원한 환자 중에서 경구용 항응고제(와파린)를 복용하는 85명의 환자들을 대상으로 하여 검사실 표준검사법과 CoaguChek-XS를 사용하여 각각 INR값을 측정하고 비교 분석하였다. 결과: 전체 85명 환자 중 2번 이상 INR을 측정한 사람은 모두 25명으로 이들의 INR 측정간격은 평균 8.6주이었고, CoaguChek-XS와 표준검사법에서 INR 2.0 미만의 측정값들이 각각 39%, 38%를 차지하였다. 와파린을 복용하는 환자에서 표준검사법으로 측정한 INR값이 높고, 낮은 두 명의 환자에서 각각 5번씩 측정한 결과 CoaguChek-XS의 변이계수는 각각 4.50, 2.45, CA-7000은 각각 1.67, 0.66이었다. 두 검사 기기간의 상관성은 R2이 0.966으로서 우수하였으며 Bland-Altman 분석을 통해서 두 기기간 차이의 평균이 0.13이었고 일치범위의 한계(95% 신뢰구간)는 -0.47∼+0.72이었다. 또한 INR 값이 증가할수록 CoaguChek-XS의 측정값은 표준방법에 비해서 높게 측정되는 경향을 보였다. 결론: CoaguChek-XS는 정밀도가 좋고 기존의 검사실 표준검사법과의 상관성이 우수하였다. 따라서 CoguChek XS는 경구 항응고제 치료환자의 INR을 모니터링 하는데 도움이 될 것으로 생각된다. Background: In order to achieve a maintenance level and to prevent hemorrhagic complications, regular monitoring of the INR is mandatory for patients on oral anticoagulation therapy (OAT). A point-of-care instrument for INR monitoring is convenient for users, but the accuracy of the results has been controversial, and so this calls for exact evaluation of the point-of-care instrument that is used for INR monitoring. Methods: From Aug 2007 through Feb 2008, 85 patients on OAT among the all the patients who were admitted to Soonchunhyang University Bucheon Hospital were involved in this study. Parallel measurements of the PT INR were performed using a CoaguChek-XS and, a CA-7000 laboratory reference instrument and the results were analyzed. In addition, the patients` clinical data, including the diagnosis and the frequency and interval of the INR measurements, were also analyzed. Results: Of the 85 patients, 25 were admitted more than once to undergo INR testing and the mean interval between testing was 8.6 weeks with 39% and 38% of the tests being less than INR 2 units with using the CoaguChek-XS and the reference method, respectively. The coefficients of variation of CoaguChek-XS were 4.50 and 2.45 for the high and low INR patients, respectively. An excellent correlation was found between the two methods with a R2 of 0.966 (p<0.001). Through Bland-Altman analysis, the mean INR difference between the two methods was 0.13 with the limit of agreement being -0.47∼+0.72 with a 95% confidence interval. CoaguChek-XS was shown to overestimate the INR value for patients with an increasing INR, as compared to the reference method. Conclusion: CoaguChek-XS demonstrated great precision and accuracy for patients on OAT when compared to the laboratory INR results. Accordingly, the instrument should help to monitor the INR in the patients on OAT. (Tuberc Respir Dis 2008;65:471-475)

The rotation of a spindle unit must be accurate for high-quality machining and to improve the quality of the machine tools. Therefore, the proper measurement of the rotation accuracy and ensuring a proper analysis are very important. Separate processes are necessary because spindle errors and roundness errors associated with the test balls can both factor into the measured rotation error values. We used three methods to discern test ball errors and analyzed which could be deemed as the most proper technique in a test of the rotation accuracy of the main spindle of a machine tool.

Wind power is a type of clean energy source which does not produce carbon dioxide. The wind turbine industry is considered as a major growth industry in many countries. The main cause of wind turbine failure arises in the wind turbine gearbox, and the main type of damage occurs in the bearings and gears. Therefore, predictions ofgear and bearing damage are very important to ensurethe reliability of the wind turbine reducers used in these systems. In this research, in order to optimize the wind turbine reducer, a series of simulations and redesigns was done using the tool RomaxDesigner. The RomaxDesigner model was used to analyze the bearing life of the duty cycle for a 5 MW wind-turbine pitch drive and to calculate the load in operating states. The reducer was designed to satisfy the life requirement by analyzingbearing damage and calculating the stress values of the main parts of the reducer.
Tooth Modification for Spur Gear for Articulated Hauler's Final Drive

Construction equipment is heavily loaded during normal operation. In recent years, there is a trend that lower gear noise levels are demanded for drivers to avoid annoyance and fatigue during operation. For articulated hauler"s final drive, meshing transmission error(T.E.) is the excitation that leads the tonal noise known as gear whine, and radiated gear whine is also the dominant source of noise in the whole gearbox. This paper presents a method for the analysis of the tooth profile modification, and the prediction of transmission error under the loaded torques for the spur gear pair of the articulated hauler’s final drive. And the transmission error, transmission error harmonics and contact stress are also calculated and compared before and after tooth modification under one torque. The simulation result shows that the transmission error and contact stress under the loads can be minimized by the appropriate tooth profile modification.

This study dealt with effect of the grinding in SCM420H planetary gears. The hardness, tooth profile, lead and roughness of the gear surface can be improved by grinding process. Therein, the grinding techniques are widely used especially as one of the physical surface improvement methods. As the results, the gear grinding process reduced vibration and noise, and increased the life of the gear. And the gear grinding improved the surface of gear tooth in a short time and low cost, thus, it is a very important process in manufacturing industry. But nowadays, it is hard to find the detail information of gear grinding process. The test piece is a planetary gear which is used in the industrial machine. The manufacture process of the test piece contains hobbing, shaving, carburizing and grinding. This study investigated the effect of the grinding process to the physiognomy and the fatigue strength of test piece which was made by SCM420H.

Objectives : Entrapment of the suprascapular nerve is frequently overlooked in the differential diagnosis of shoulder pain. Methods : Suprascapular entrapment neuropathy is a well-defined clinical entity and EMG and NCV is used to confirm a diagnosis. But the diagnosis typically not considered until patients develop severe weakness secondary to atrophy of the spinous musculature that the nerve supplies. Results : A narrow suprascapular notch has rarely been reported as a work-related factor of this entrapment neuropathy. Diagnosis of suprascapular entrapment neuropathy is based on the patients' clinical course, neurologic, radiologic, and electrophysiologic findings. One of the most helpful evaluations was anteriorposterior projection with the X-ray tube angled 15-30 degree caudally. The suprascapular entrapment neuropathy is relatively uncommon entity of shoulder discomfort(pain, weakness, and atrophy). Conclusions : If the worker who used his shoulder joint repetitiously having the shoulder pain and muscle weakness, we must rule out the suprascapular entrapment neuropathy. And it is needed to evaluate the motions which cause suprascapular entrapment neuropathy as the ergonomic factor.
고나트륨혈증후 발생한 중심성 뇌교및 외뇌교 수초 용해증 1예

중심성 뇌교 수초 용해증은 일반적으로 심한 기저질환을 지닌 환자에서 심한 저나트륨혈증이나 저나트륨혈증을 급속 교정후 오는 것으로 알려져 있다. 저자들은 최근 만성 주정중독환자에서 심한 고나트륨혈증을 서서히 교정후 발생한 중심성 뇌교 및 외뇌교 수초 용해증을 뇌 자기공명 전산화 단층촬영으로 확진하였다. 심한 기저 질환을 가진 환자에서 장기간의 고나트륨혈증의 경우도 중심성 뇌교 수초 용해증을 일으킬 수 있기 때문에 이런 환자에서 저나트륨혈증뿐 아니라 고나트륨혈증도 피해야 될 것으로 사료된다.
