        임신란관조직의 조직화학적 연구

        A histochemical study was performed in order to observe the changes of fallopian tubes during pregnancy. Forty-six cases of normal fallopian tubes were obtained at minilaparotomy for tubal sterilization during pregnancy and cesarean section. Two pieces of the tube, one from the isthmic and the other from the ampullar portion, were used in this study in order to study the differences between them. The results obtained were summarized as follows: 1. The average age and parity of these sterilization cases were 34.3 years and 3.3 respectively. 2. Fat was noted mildly in the cytoplasm of tubal epithelium, but was not present in the stroma and muscle. 3. Glycogen was seen moderately, as granules in the perinuclear portion of the tubal epithelium, but midly in the stroma and muscle. 4. Diastase resistant periodic acid Schiff reactive material markedly scattered over the basement membrane, stroma and muscle tissue. But its amount in the apical parts of the secretory cells in the tubal epithelium was only scanty. 5. Mucin content in the secretory cells of the tubal epithelium was slight in amount, but it was not noted in any other cells of the salpingeal tissue. 6. Alkaline phosphatase activity was identified along the surface of the secretory cells of tubal epithelium in mild degree, but moderately in the capillary and endothelium of the arteriole. 7. Acid phosphatase activity was observed moderately in the whole cytoplasm of the tubal epithelial cells with diffuse and granule forms. But its activity was not identified in any other sites of the tube. 8. Beta-glucuronidase activity was moderately present in the tubal epithelium, but it was mild in the stroma, muscle and blood vessel endothelium. 9. Esterase activity observed only slightly in the apical parts of the tubal epithelium and stroma, and it was not noted in any other sites of the tubal tissue. 10. Any differences in the contents of the above materials and enzymes were not identified between the isthmus and ampulla, and between the early and term

        간문맥 종양혈전을 동반한 간세포암종의 예후 인자

        Background/Aims: Hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) exhibits poor prognosis. The aim of this study is to evaluate factors associated with survival of HCC patients with PVTT to suggest better therapeutic options. Methods: Patients with HCC which were newly diagnosed at three tertiary hospitals between January 2004 and December 2012, were reviewed retrospectively. Among them, Barcelona Clinic of Liver Cancer stage C patients with PVTT were identified. Factors affecting overall survival (OS) were analyzed and efficacies of the treatment modalities were compared. Results: Four hundred sixty five patients with HCC and PVTT were included. Liver function, tumor burden, presence of extrahepatic tumor, alfa fetoprotein, and treatment modalities were significant factors associated with OS. Treatment outcomes were different according to the initial modalities. OS of the patients who received hepatic resection, radiofrequency ablation (RFA), transarterial chemoembolization (TACE), hepatic arterial infusion chemotherapy (HAIC), sorafenib, systemic cytotoxic chemotherapy, radiation therapy (without combination), and supportive care were 27.8, 7.1, 6.7, 5.3, 2.5, 3.0, 1.8, and 0.9 months, respectively (P<0.001). Curative-intent treatments such as hepatic resection or RFA were superior to noncurative-intent treatments (P<0.001). TACE or HAIC was superior to sorafenib or systemic chemotherapy (P<0.001). Combining radiotherapy to TACE or HAIC did not provide additional benefit on OS (P=0.096). Conclusions: Treatment modalities as well as baseline factors significantly influenced on OS of HCC patients with PVTT. Whenever possible, curative intent treatments should be preferentially considered. If unable, locoregional therapy would be a better choice than systemic therapy in HCC patients with PVTT.         정상임신태반에 있어서의 Alkaline Phosphatase 및 Acid Phosphatases의 활성도

        50예의 만기임신태반과 25예의 조기임신태반에서 ALPdhk ACP의 활성도를 조직화학적 방 법으로 검색한 결과 다음과 같은 결론을 얻었다. 1) ALP (1) 임신 제 6주에 중등도로 syncytium brush border에 나타난 효소활성은 제 9주경부터 점 차 약화되었다가 제 14주 이후에 다시 나타나나 이때는 syncytium과 cytotrophoblast의 세 포질에서 공히 관찰되며 임신이 진행됨에 따라 더욱 증가하여 만삭시에 그 절정에 도달한 다. (2) 혈관, 융모 및 융모막판의 기질조직, 모든 세포의 핵에서는 만삭 및 조기태반 공히 효소 활성을 발견할 수 없었다. 2) ACP (1) 임신 제6주에 매우 강한 활성도를 모든 trophoblast의 세포질에서 볼 수 있었으며 이는 임신 제10-14주에 그 절정에 도달하고 그 뒤 임신이 지속됨에 따라 조금씩 약회되어 만삭 태반에서는 중등도의 활성을 보였다. (2) 융모 및 융모막판의 기질세포들에서 미약한 활성이 있었고 혈관중 모세혈관은 무활성이 고 보다 큰 혈관중 특히 소동맥내피세포에서는 중등도로 Hofbarer 제포에서는 상당히 강한 활성을 거의 임신 전기간 동안 관찰할 수 있었다. Two hydrolytic enzymes were studied histochemically in 25 early and 50 term human placentas. In alkaline phosphatase, the syncytiotrophoblast has had abundant enzyme activity in the first 6 weeks of pregnancy. The activities decreased until 10-14 weeks, and then gradually increased until it reached the peak at term. The cytotrophoblast, on the other hand, in the first trimester had no enzyme activity but certain of these cells acquired alkaline phosphatase activity in abundance in the second and third trimester. In acid phosphatase, there has been marked enzyme activity in the cytopasms of trophoblast and Hofbauer cells at 6th week and reached their greatest activities during 9-13 weeks of pregnancy. During the second and third trimester their activities in thevilli gradually decreased.

        폐경후 난관조직의 조직학적 연구

        A histochemical study on the postmenopausal fallopian tubes was carried out in order to observe the postmenopausal changes of a few meterials and enzymes and compare with the cyclic changes of the normal fallopian tubes. Nine tubal specimens were obtained from laparotomy cases without any salpingeal disease. Two pieces of the tube, one from the isthmic and the other from the ampullar portion, were used in this study in order to find out any difference between them. The results of this study were summarized as follows: 1. The mean age of these postmenopausal cases was 56.3 years and the most frequent laparotomy indication of these cases was chronic cervicitis with cervical erosion. 2. Fat was identified in slight to mild degree in the cytoplasms of the tubal epithelial cells. And fat content in muscle tissue was slight, but negative in the stroma and blood vessel endothelium. 3. Glycogen was seen as granule forms in the perinuclear region of the tubal epithelium, the content of which was mild. And the stromal and muscular glycogen content was mild, too. 4. Diastase resistant periodic acid Schiff reactive material was scattered mildly in the apical parts of the secretory cells of the tubal epithelium. But the contents in the basement membrane, stroma and muscle tissue were moderate to marked. 5. Mucin was not identified in the salpingeal tissue except slight amount in the apical portion of the epithelium. 6. Alkaline phosphatase activity was identified along the apical portion of the secretory cells in the tubal epithelium. The staining intensity of the above portion was slight, but moderate in the capillary and arteriole endothelium. 7. Acid phosphatase activity was observed in the whole cytoplasm of the tubal epithelial cells with diffuse and granular forms. The enzyme activity of the epithelium was mild in the apical portion and trace in the basal portion. 8. Beta-glucuronidase activity was mild in the apical portion of the tubal epithelium, but trace in the basal portion of it and blood vessel

        간세포암에서 분자표적치료제의 현황

        Hepatocellular carcinoma (HCC) is one of the leading causes of cancer death in Korea. Curative treatment is only possible when the disease is diagnosed at the early stage. The prognosis of patients with HCC is even dismal in advanced stages. No systemic cytotoxic chemotherapy has proven to be beneficial in overall survival. Recently, the understanding of the molecular pathogenesis led to the development of new therapies. With the evidence of dysregulation of critical genes associated with cellular proliferation, growth factor signaling, cell cycling, apoptosis, and angiogenesis in HCC, a number of molecular target agents are under clinical trials. Sorafenib is the first systemic anticancer drug which has proven to gain survival benefit in the global as well as Asia-Pacific trials. However, the survival gain is still modest, and further efforts to improve outcomes in patients with HCC are necessary by developing novel drugs or combining other forms of therapies. This article will review signaling pathways in HCC and introduce molecular target agents under investigation currently. (Korean J Gastroenterol 2013;61:136-146)

        A histochemical study on the normal human fallopian tubes was performed in order to observe the cyclic changes of a few materials and enzymes and compare with the cyclic changes of the endometrium. Seventy four tubal specimens were obtained from tubal sterilization cases by minilaparotomy. Two pieces of the tube, one from the isthmic and one from the ampullar portion, were used in this study in order to find any difference between them. The results of this study were summarized as follows: 1. The mean age of these tubal sterilization cases was 31.7 years and their average parity was 2.9. 2. Fat was present not only diffusely distributed in the cytoplasm, but also in the form of granules, which was most marked in the tubal epithelium of the early luteal phase. 3. Glycogen was seen as granule form in the perinuclear region of the tubal epithelium and the content was most marked in the late follicular phase. The amount of glycogen was decreased after ovulation when the ciliary activity in the tube was known to be increased. 4. Diastase resistant periodic acid Schiff reactive material scattered in the basement membrane, stroma and muscle tissue with marked intensity without any cyclic changes. But the amount in the apical parts of the secretory cells in the tubal epithelium has reached its maximum in the early luteal phase. 5. Mucin content in the secretory cells of tubal epithelium was slight throughout the menstrual cycles without change. But it was not found in any other sites of the tube. 6. Alkaline phosphatase activity was observed along the apical portion of secretory cells. The staining intensity was low in the luteal phase, and moderate in the late follicular phase in which the enzyme activity was maximum. 7. Acid phosphatase activity was observed in the whole cytoplasm of the tubal epithelial cells with diffuse and granule forms. But the enzyme activity of cellular survace was more intense than that of the basal portion. The peak activity was in the early luteal phase. 8. Beta-glucuronidase activity wass
