Pleomorphic adenoma is the most comon benign tumor of the salivary gland. It occurs mainly in the major salivary gland, but extremely rare. Recently, we have experienced a huge pleomorphic adenoma originated in the maxillary sinus. The patient was 64 years old woman and the tumor in the left maxillary sinus extended to the infratemporal fossa, pterygopalatine fossa, and parapharyngeal space through destructed maxillary sinus bony wall. Surgical resection was performed via midfacial degloving approach and the result of surgery was successful. We report this case with the review of literature.

Background and Objectives:It has been known that imune reaction plays important roles in the pathogenesis of otitis media with efusion (OME)changes of middle ear mucosal inflammation induced by lipopolysacharide (LPS) following administration of imunosuppres-sive agent, Cyclosporin A (CsA) in experimental animals. Materials and Method:35 μl LPS was injected into the midle ears of 30 rats. These rats were divided into groups of 10, each receiving CsA, Dexamethasone, and no treatment, respectively, daily. Five rats from each gr Results:Both the CsA-treated and the steroid-treated groups showed lesser inflammatory changes than the untreated groups, however, there were no significant differences betwen the CsA-treated and the steroid-treated groups. Both on day 2 & day 6, the CsA-treated group showed a decrease of mucosal metaplasia and secretory epithelium, compared with the untreated and steroid-treated group. Conclusion:The results suggest that CsA may be effective in decreasing inflammation with particular significance in mucosal mataplasia and the secretion of effusion in OME. Background and Objectives:Surgery and postoperative radiation therapy have been the main stream management of tonsil cancer. There is a variety of surgical approach methods are used for the resection of tonsil cancer, however, there have only been of our surgical treatments of tonsil cancer. Subjects and Method:32 medical records, from 1994 April to 2003 June, of patients surgicaly treated for tonsil cancer were reviewed. Evaluated were disease status, surgical approach methods, types of the neck dissection, surgical margin, reconstruction methods, postoperative complications, time of oral diet and decannulation, recurence, and survival rate. Results:Mean follow up periods were 2.3 years. There was 1 patient in Stage I, 5 in Stage II, 2 in Stage III, pharyngotomy approach. Surgical defects were primary closed in 5 patients and reconstructed with pectoralis major myocutaneous flap in 7 patients and radial forearm fasciocutaneous free flap in 20 patients. There were 2 local recurences, 5 regional recurrences, and one distant metastasis. The mean time of the beginning of oral diet/decannulation after operation were 19.8/11.3 days in lateral pharyngotomy and 25.3/12.9 days in mandibulotomy. The mean survival period was 41.4 months. Conclusion:Because pharyngotomy approach had priority to other surgical approaches in selected cases except T4. It has advantages of full range of vision for resection, better functional status and less postoperative morbidity. (Korean J Otolaryngol 2004 ;47 :168-73)

Background and Objectives:Mast cells are the main effector cells in the immediate phase of the allergic responses. To evaluatethe role of mast cells on the development of allergen-induced eosinophilic inflammation in a murine model of allergic rhinitis(AR), we compared the responses of mast cell deficient (W/Wv) mice with congenial normal (W/W+) mice. Materials and Method:Mice were sensitized intranasally with an extract of Aspergillus fumigatue (Af) for 3 weeks, and challenged intranasallywith a single dose (acute model) or single dose every day for 5 consecutive days (chronic model). Interleukin (IL)-5 in nasallavage fluids (NLF), histologic examination and Af-specific immunoglobulins (Ig) were examined in W/Wv mice and W/W+mice. Results:In the sensitized W/Wv mice, the number of eosinophils in the nasal mucosa and IL-5 in NLF after repeatedallergen challenge were not significantly different from those of W/W+ mice. In contrast, the number of eosinophils in the nasalmucosa and IL-5 in NLF after single allergen challenge were partially attenuated in W/Wv mice. The levels of Af-specific IgEand IgG1 in both acute and chronic models were not different between W/Wv mice and W/W+ mice. Conclusion:These findingssuggest that eosinophilic inflammation caused by repeated allergen challenge are independent of mast cells in this AR model.(Korean J Otolaryngol 2006;49:912-7)
조기 성문암에서 수술과 방사선치료 후 음성과 연하기능 비교

Background and Objectives:Laryngeal neoplasms such as early glottic cancer can be effectively treated with several methods, namely laser surgery, conservation laryngeal surgery and radiotherapy. Therefore, the preservation of the ability to speak and swallow normally is another important consideration in the selection of therapy for early glottic cancer. The purpose of this study is to compare the functional results of voice and deglutition for early glottic cancer patients treated with surgery or radiation. Subjects and Method:Between January 2002 and September 2004, 17 patients with the early glottic cancer who had undergone radiotherapy (n=6) or surgery (n=11) were retrospectively investigated. All the patients were phonetically analysed using CSL and Fo, Jitter, Shimmer and NHR were evaluated. For aerodynamic test, maximum phonation time (MPT) and mean air flow rate (MFR) were also evaluated. Modified barium swallow (MBS) was performed to evaluate swallowing function. Results: There were no significant difference in all the vocal parameters between radiotherapy group and surgery group. But Shimmer differed significantly between radiotherapy group and laser cordectomy group. In aerodynamic study, MPT was significantly longer in the radiotherapy group than in the surgery group, but no significant difference was observed between the radiotherapy group and laser cordectomy group. MBS showed better results in the surgery group than in the radiotherapy group. Conclusion: Therapeutic modality for patients with early glottic cancer should be chosen in consideration of voice quality and swallowing, and according to multiple parameters of the patients’ conditions.
