      • 조혈모세포이식을 받은 백혈병환자의 희망정도와 영향요인

        Purpose: The purpose of the present study was variables in patients hemopoietic stem cell transplantation(HSCT) for leukemia. Method: A total of 88 participants were recruited at St. Mary's Hospital. The data were gathered from July, 2001 to June, 2002 using questionnaires. Result: The mean score of hope was 3.39±0.35. Positive relationships were found between hope and self-esteem, physician's support, nurse's support, and satisfaction of life. And negative relationships were found between hope and depression, trait-anxiety, and state-anxiety. The major variable, that associated with hope, was the state-anxiety, explained 35.4% in the variance of the hope. Conclusion: Leukemic patients undergoing<br/> HSCT tended to have a high level of hope. Higher levels of self-esteem, physician's support, nurse's support and satisfaction of life were related with higher level of hope. And, higher the levels of depression and anxiety were related with a lower level of hope.

        혈액종양 관련 국내 간호연구 분석

        Purpose: This study was conducted in order to analyze themes, concepts, research methods and results of previous domestic research on the nursing of hematologic patients conducted through the last 10 yr, to find trends in the research, and to provide basic materials for setting the direction of future research on the nursing of hematologic patients. Methods: This study analyzed a total of 72 nursing theses related to hematology sampled from domestic theses for a master’s or doctoral degree and papers published in six nursing journals registered in Korea Research Foundation from January 2000 to July 2009. Results: Of the 72 theses, 51 were for a master’s degree, 7 for a doctoral degree, and 14 not for an academic degree. The concept covered most frequently in correlation research was ‘quality of life’ and concepts found in comparative research were stress and quality of life. In experimental research, the most common nursing intervention was oral care. The scale used most frequently was Spielberger’s Anxiety Scale. Conclusion: It is necessary to expand experimental research applying nursing interventions, and to use objective physiological indexes for more effective assessment in experimental research. Furthermore, interdisciplinary research is required for enhancing the quality of clinical nursing research. 본 연구는 2000년 1월부터 2009년 7월까지 발표된 국내 석,박사 간호학위 논문과 한국 학술 진흥재단에 등재(등재 후보지포함)된 간호학 학술지 6개에 발표된 간호학 논문 중 혈액종양관련 연구 72편을 대상으로 하였다. 1) 혈액종양 관련 국내 연구의 출처는 석사학위 논문 51편,박사학위 논문 7편, 비학위 논문 14편으로, 비학위 논문은 종양간호학회지 6편, 아동간호학회지 4편, 임상간호연구 3편, 정신간호학회지 1편이었다. 환자를 대상으로 한 연구가 64편으로10편을 제외하고는 성인을 대상으로 한 연구가 대부분이었고,치료유형으로는 조혈모세포 이식환자 대상이 34편, 항암화학요법 환자 대상이 30편이었다. 환자 이외의 대상자로는 암환자가족이 4편, 간호사 3편, 골수 공여자가 1편이었다. 2) 연구 설계는 양적연구가 65편으로 실험연구가 18편, 비실험 연구가 47편이었다. 실험연구는 17편이 유사 실험연구였으며, 비실험 연구는 조사연구가 20편으로 가장 많고 다음이 상관관계 연구 13편, 방법론적 연구 8편, 비교연구 6편 순이었다. 질적 연구는 총 7편으로 현상학적 연구 5편, 근거 이론 연구 1편, 문화기술지 1편이었다. 3) 상관관계 연구에서 가장 많이 사용된 개념은 삶의 질이었으며, 그 외 자가간호행위, 우울, 희망, 건강신념, 피로 등이 사용되었다. 4) 비교연구에서 나타난 개념은 스트레스, 삶의 질, 불안, 오심, 구토였다. 5) 방법론적 연구는 모두 8편으로 치료단계별 간호중재 프로토콜, 동종 조혈모세포이식환자의 삶의 질, 조혈모세포이식환자 가족의 요구사정, 조혈모세포이식병동 성과측정지표, 동종조혈모세포이식 공여자 간호 표준 임상지침서, 조혈모세포이식 간호업무표준 및 간호활동이 있었다. 또한 교육자료 개발에관한 연구로 조혈모세포이식 환아 가족 교육자료, 조혈모세포이식환자 교육지침서 개발이 있었다. 6) 실험연구에서 간호중재는 구강간호가 6편으로 가장 많았고, 손, 발, 등 마사지가 각 1편, 지시적 심상요법이 2편이었다. 7) 질적 연구는 7편으로 현상학적 연구 5편, 근거 이론 연구1편, 문화기술지 1편이었다. 연구주제는 재발경험, 청소년기 암환자의 경험이나 심리사회적 적응, 조혈모세포이식환자의 직계 간호 제공자의 경험 등이었다. 8) 측정도구는 대부분 외국의 도구를 번안하여 수정, 보완하여 사용한 것으로 나타났다. 사용된 측정도구는 Spielberger의불안이 11편으로 가장 많았으며, Eilers, burger & Peterrsen 의 구내염과 이순교의 삶의 질이 각각 5편에서 사용되었다.

      • 조혈모세포이식을 받은 환자의 불안

        Purpose: The purpose of this study was to identify the influencing factors on anxiety of patients who had received hemopoietic stem cell transplantation(HSCT). Method: A total of 81 patients who had undergone a HSCT at a university hospital were recruited from July of 2001 to June of 2002. They were asked to complete questionnaires about anxiety, selfesteem, family support, doctor's support and nurse's support. Result: The mean score of anxiety, self-esteem, family support, doctor's support and nurse's support was 41.9, 3.0, 4.6, 7.6 and 7.3, respectively. A significant negative correlation was found between anxiety and self-esteem. Self-esteem was positively correlated with family support, doctor's support and nurse's support. The major variable that influenced anxiety was the self-esteem, explaining 25.4% of the anxiety. Conclusion: On the basis of these results, it is necessary for HSCT patients to develop a nursing intervention for the enhancement of self esteem. Also, family support, doctor's support<br/> and nurse's support should be considered as a main support system in the nursing strategy for improving the quality of life of patients undergone HSCT.

        신규간호사의 긍정심리자본, 조직몰입, 소진의 변화

        Purpose: The purpose of this study was to describe changes in positive psychological capital, organizational commitment and burnout according to work periods for new nurses. Methods: A longitudinal study was done using surveys of 91 nurses from a tertiary hospital located in Seoul, South Korea. Data were collected from these newly graduated nurses during the period from July 2018 to September 2019, three times (2 months, 4 months and 6 months after starting to work). Results: Hope of positive psychological capital decreased after 6 months of work (F=7.11, p=.001) and organizational commitment decreased after 4 months of work (F=15.30, p<.001). Factors influencing participants' burnout included the optimism of positive psychological capital and continuance of organizational commitment. These factors explained 51.3% of the variance in burnout (F=48.47, p<.001). Conclusion: Organizational support for new nurses is urgently needed. It is necessary to develop programs to strengthen the positive psychological capital of newly graduated nurses to reduce their burnout and to help their job adaptation.
