빈안장증후군과 단안 제육뇌신경마비로 나타난 소아의 특발 두개내압상승

목적: 뇌자기공명영상검사에서 빈안장증후군이 동반된 양안의 시신경유두부종 및 단안의 제육뇌신경마비를 보이는 특발두개내압상승 환아를 경험하였기에 이를 보고하고자 한다. 증례요약: 9세 정상 체중 남자 환아가 4일 전 발생한 두통과 복시를 주소로 내원하였다. 교대프리즘가림검사에서 원거리 16프리즘디옵터의 내사시가 있었으며 안구운동검사에서 우안의 -1 외전장애가 있었다. 안저검사에서 심한 양안의 시신경유두부종 및 유두 경계부 출혈이 있었으며 시야검사에서 우안의 생리적 맹점 확대가 관찰되었다. 뇌자기공명영상에서 빈안장 및 후공막편평 등 뇌척수액압상승 소견들이 보여 특발두개내압상승을 진단하고 치료하였다. 치료 4달째 양안의 시신경유두부종과 유두 경계부 출혈은 호전되었고 우안의 외전장애도 호전되었다. 뇌자기공명영상에서 빈안장이 호전되었으며, 치료 8달째 재발 소견 없이 경과 관찰 중이다. 결론: 특발두개내압상승이 의심되는 환아가 내원하였으나 요추 천자검사를 하기 어려운 상황인 경우 뇌자기공명영상검사를 적극적으로 활용해야 하며, 뇌자기공명영상에서 이상이 발견된 경우 뇌자기공명영상검사가 이후 질병의 경과를 판단하는 데 도움을 줄 수 있다. Purpose: We report the case of a child with idiopathic intracranial hypertension who presented with binocular papillary edema and monocular sixth cranial nerve palsy accompanied by empty sella syndrome evident on brain magnetic resonance imaging. Case summary: A 9-year-old, normal-weight male patient visited the emergency room complaining of headache and diplopia 4 days in duration. The alternative prism cover test revealed esotropia of 16 prism diopters and a -1 right lateral gaze limitation. A fundus examination revealed papilledema and peripapillary hemorrhages in both eyes, and a visual field examination an enlarged, physiological blind spot in the right eye. Brain magnetic resonance imaging revealed elevated cerebrospinal fluid pressure, an empty sella, and posterior scleral flattening. We diagnosed and treated idiopathic intracranial hypertension. After 4 months, the papilledema and peripapillary hemorrhages of both eyes resolved, and the right lateral gaze limitation improved. The empty sella improved on brain magnetic resonance imaging, and we noted no recurrence 8 months after treatment. Conclusions: If a child with suspected idiopathic intracranial hypertension visits a hospital, but it is difficult to perform a lumbar puncture, brain magnetic resonance imaging should be scheduled. If abnormalities are found, these help to determine the course of disease.

Although 'Cinnamon' has been widely used for the food and biophamacy in the world, it's toxicity was not screened completely. Major component of 'cinnamon' is CB-OH and CB-PH. CBPH has been reported to have antimutagenic effect. To investigate the toxicity of 2- o-Benzoylcinnamaldehyde (CB-PH), repeated dose (4 weeks) oral toxicity test performed in SD rats. Results of repeated dose oral toxicity tests for 4 weeks (CB-PH; 500, 1000, 2000 mg/kg/day) suggested that the CB-PH treated group showed no significant toxicological findings with body weights, organ weights, hematological and histopathological findings. Therefore, these data indicated that the maximum tolerated dose of CB-PH was 2000 mg above/kg/day in the rats.
오미자약침액(五味子藥鍼液)의 NO, DPPH 소거(消去) 및 IL-4에 대한 억제효과(抑制效果)

Backgrounds : It has been investigated about ageing theory. However, ageing mechanism still remains to be unknown. Since Harman proposes that ageing and ageing related diseases might be due to oxidative damage and these are modifiable by genetic and environmental factors. For designing an optimal nutritional countermeasure against ageing and ageing related disease, it is necessary to understand the ageing mechanism and other reactive species. Objectives : Schizandra Chinesis has been clinically used to treat brain disease, respiratory or inflammatory disease etc. in Oriental medicine. The purpose of this study is to investigate the scavenging effect of Schizandrae Fructus herbal-acupuncture solution(SFHA) on NO, DPPH and IL-4. Results : The results are summarized as follows; (1) There is a significant scavenging effect of SFHA on NO in 0.1, 1, 10mg/ml group after 24hrs. (2) There is a significant scavenging effect of SFHA on DPPH in 1, $10{\mu}g/ml$ group. (3) There is a significant inhibitory effect of SFHA on IL-4 in 1, 10, $100{\mu}g/ml$ group. Conclusions : These findings indicate that SFHA can be used as antioxidant or antiimflammatory drug. But further study is needed about the effect of SFHA.
