        경동맥 스텐트 삽입술로 완치된 안허혈증후군 1례

        Purpose: To report a case of ocular ischemic syndrome successfully treated with delayed carotid angioplasty and stenting (CAS). Case summary: A 52-year-old male was admitted to our hospital because of amaurosis fugax- like symptoms in the right eye for several months. His visual acuity was 0.8 in the right eye and he did not have rubeosis iridis. Neovascularization of the disc, narrowing of the retinal artery and multiple retinal hemorrhages were diagnosed by fundus examination. Fluorescein angiography showed delayed choroidal filling, a delayed arm-to-retina time, prolongation of arteriovenous transit time, neovascularization of the disc, retinal capillary nonperfusion, and staining of the retinal vessels. MR angiography showed severe stenosis in the proximal portion of the right carotid artery. We diagnosed this case as ocular ischemic syndrome. The patient was recommended carotid angioplasty and stenting at the severely narrowed portion of the right carotid artery, but it was postponed about six months after diagnosis because of personal problems. At the final follow-up, 24 months after stenting, the amaurosis fugax symptoms had disappeared, the patient had an improved visual acuity of 1.0, and the new vessels on the disc changed to fibrous tissue. Fluorescein angiography showed resolution of the delayed arm-to-retina time and prolongation of the arteriovenous transit time, disappearances of the leakage around the disc and the retinal capillary nonperfusion.         신생혈관녹내장에서 베바시주맙의 전방과 유리체내 동시 주입과 유리체내 단독 주입의 치료 효과

        <b>Purpose:</b> To compare the clinical efficacy of simultaneous intracameral and intravitreal injection and intravitreal single injection of bevacizumab in patients with neovascular glaucoma (NVG). <b>Methods:</b> The medical records of 43 eyes of 43 patients, who had treated with simultaneous intracameral and intravitreal injection (Group I) or intravitreal single injection (Group II) of bevacizumab 1.25 mg from January 2010 to December 2012, were retrospectively reviewed. The best corrected visual acuity (BCVA), intraocular pressure (IOP), regression time of new vessel in the iris (NVI) and anterior chamber angle (NVA), progression of peripheral anterior synechiae (PAS), and corneal parameters were measured preoperatively and one day, three days, 1 week, 1 month, and 3 months postoperatively. <b>Results:</b> There was significant changes of IOP between the two groups at 1, 3 days postoperatively (<em>p</em> = 0.001, <em>p</em> < 0.001). The regression time of NVI and NVA in Group I was significantly faster than Group II (<em>p</em> = 0.026, <em>p</em> = 0.033). In the phakic eyes, regression time of NVI and NVA was significantly longer than aphakic and pseudophakic eyes in group II (<em>p</em> = 0.006, <em>p </em>= 0.005). Also, in the phakic NVG patients, the formation of PAS in Group I was significantly less than in Group II for the postoperative three months (<em>p </em>= 0.020). <b>Conclusions:</b> Simultaneous intravitreal and intracameral injection of bevacizumab seem to be more effective for the early lowering of IOP and regression of NVI and NVA, and inhibiting further PAS formation in NVG patients, especially in the phakic eyes. Therefore, simultaneous intracameral and intravitreal injection of bevacizumab may be considered as an adjunct to management of NVG in the phakic eyes. J Korean Ophthalmol Soc 2014;55(7):1039-1048

        유리체강 내 항혈관내피세포성장인자 주입술 전의 예방적 안압강하제의 효과

        Purpose: The purpose of the present study was to investigate the prophylactic effects of a fixed combination of dorsolamide/ timolol on intraocular pressure (IOP) elevation before intravitreal anti-VEGF injection. Methods: A prospective, randomized clinical trial was conducted on 91 eyes of 91 patients undergoing intravitreal anti- VEGF injection. The eyes were randomly divided into 2 groups, the eyes which had used the fixed prophylactic dorsolamide/ timolol combination (group 1, 58 eyes) and the eyes which had not used the combination (group 2, 15 eyes). The IOP was measured one hour and 5 minutes prior to the procedure, 5 minutes interval up to 30 minutes after the procedure and one hour, 1 day, 7 days, and one month after the procedure. The IOP changes were analyzed. Results: The mean 5 minutes and 30 minutes postoperative IOPs were 14.12 ± 4.18 mm Hg and 10.87 ± 1.58 mm Hg in group 1 and 28.21 ± 3.16 mm Hg and 17.48 ± 2.34 mm Hg in group 2, respectively. After IVBI, the mean 5 min postoperative IOP was 12.17 ± 1.13 mm Hg in group 1 and 27.12 ± 3.35 mm Hg in group 2. After IVRI, the mean 5 minutes postoperative IOP was 15.98 ± 4.14 mm Hg in group 1 and 25.19 ± 1.04 mm Hg in group 2. Conclusions: Prophylactic use of a fixed dorsolamide/timolol combination before intravitreal anti-VEGF injection is an easy and safe method of preventing IOP elevation immediately after intravitreal injection of bevacizumab or ranibizumab.         펜디메트라진에 의해 발생한 안 합병증 2예

        Purpose: The authors of the present study report treatment experience of acute myopia and branch retinal vein occlusion associated with phendimetrazine, a drug used for weight reduction. Case summary: Case 1: A 32-year-old woman, previously devoid of ocular problems, visited our hospital with bilateral visual disturbance after taking phendimetrazine for weight reduction. Ciliochoroidal effusion and anterior shifting of the lens-iris diaphragm were observed, which resulted in a shallow anterior chamber, myopic shifting and an increase in intraocular pressure due to angle closure. The symptoms were relieved by discontinuing the use of phendimetrazine and administration of intraocular pressure-lowering agents. Case 2: A 26-year-old woman, previously devoid of ocular problems, visited our hospital with left superior visual field disturbance after taking phendimetrazine for weight reduction. The examinations revealed papilledema, disc hemorrhage and tortuous vascular changes in her left eye. Fluorescein angiography was performed, and retinal vein occlusion was diagnosed. The patient discontinued weight reduction agents and recovered while under observation. Conclusions: Phendimetrazine, used for weight reduction, can cause acute myopia via prostaglandin synthesis and retinal venous occlusion due to vascular constriction.         Purpose: To report the case of a 36-year-old patient with nanophthalmic uveal effusion was treated with scleral window surgery and topical administration of mitomycin C (MMC). Case summary: A 36-year-old woman presented with decreased visual acuity and blurred vision in the both eyes during 3 months. Fundus examination revealed choroidal effusion and retinal detachment with thickend sclera wall and short axial lengh. Partial-thickness sclera flap with deep sclerostomy was performed and topical MMC was administered to one quadrant of the equatorial sclera. The subretinal fluid resorbed gradually and visual acuity improved. Conclusions: Scleral window surgery and topical mitomycin C might relieve the blocked transscleral outflow of intraocular fluid in the small area of a sclerostomy in young patient with nanophthlamos.         Purpose: To report the manifestation of ocular syphilis combined with neurosyphilis in patients who present with uveitis clinically and are diagnosed initially by an ophthalmologist. Methods: This study is a retrospective, clinical investigation of seven male patients with ocular syphilis from a single tertiary center between 2009 and 2012. Results: The average age of the patients at onset was 44.4 years (range, 33-71 years). Posterior segment involvement was found in all patients. Two patients had papillitis. In serologic testing, all patients had positive responses to Venereal Disease Research Laboratory (VDRL), fluorescent treponemal antibody-absorption test (FTA-ABS) and Treponema pallidum hemagglutination (TPHA). In fluorescein angiography (FAG), retinal vascular and disc leakage was the most common finding. Cerebrospinal fluid (CSF) analysis was performed for six patients and demonstrated positive CSF FTA-ABS and CSF TPHA tests in all patients. CSF VDRL was reactive in 2 patients (33.3%). Conclusions: All patients with ocular syphilis were diagnosed with neurosyphilis based on the analysis of CSF with FTA-ABS and TPHA.         Drechslera 균종에 의한 진균각막염 1예

        Purpose: To report a case of fungal keratitis caused by Drechslera species. Case summary: A 77-year-old man visited our clinic complaining of left ocular pain and decrease of visual acuity for 1 week On slit-lamp examination, epithelial defect and stromal infiltration on the corneal center with numerous inflammatory cells in the anterior chamber were found. There was no improvement after routine antibiotic treatment. A corneal biopsy and culture were performed on the corneal lesion. The KOH smear study reported hyphae, thus the patient was treated with 0.25% amphotericin B, 0.2% fluconazole and 5% natamycin eye drops. A clinical improvement was observed on the corneal lesion and Drechslera species was identified by the culture study. Conclusions: The authors report an experience of fungal keratitis caused by Drechslera species. J Korean Ophthalmol Soc 2011;52(11):1366-1369

        스마트워치의 생리 데이터 기반 질병 모니터링 체계를 활용한 COVID-19 이상탐지

        Real-Time vital-sign from patients are important information that implies the current health status and behavior of patients. Recently, Mishra et al. have shown that COVID-19 can be detected by analyzing the patient’s vital signs and behaviors, i.e., heart rates and steps, using anomaly detection techniques. This paper presents a medical IoT platform, called MiT Eco-platform, which is designed to gather patient’s physiological data through a smartwatch and to increase the efficiency of data labeling for building an AI model for medical diagnosis and treatment. Furthermore, we present a real-time COVID-19 detection approach advanced from the approach of using anomaly detection Mishra et al. that will be run on MiT Eco-platform. As a result, we show performance evaluation results of preemptively detecting the COVID-19 infection for the same samples of the COVID-19 infected ones of Mishra et al., comparing with the anomaly detection approach of Mishra et al.. We expect that physiological data through smartwatches on daily life can be continuously gathered and effectively labeled by the MiT Eco-platform for various studies in medical area.

        당뇨황반부종에서 베바시주맙 단일주입과 저용량 베바시주맙-트리암시놀론 혼합주입 단기효과 비교

        Purpose: To report the effects and intraocular pressure results of intravitreal bevacizumab alone injection compared with intravitreal low-dose bevacizumab combined with low-dose triamcinolone injection in patients with diabetic macular edema. Methods: In total, 40 eyes of 40 patients diagnosed with diabetic macular edema were evaluated. Of these, 20 eyes of 20 patients were injected with intravitreal bevacizumab (1.25 mg/0.05 mL) and 20 eyes of 20 patients were injected with low-dose bevacizumab (0.625 mg/0.025 mL) combined with low-dose triamcinolone (1 mg/0.025 mL). The best corrected visual acuity (BCVA), central macular thickness, and intraocular pressure of treated eyes were measured before injection and at 1 month, 2 months, and 3 months after injection. Results: In both the intravitreal bevacizumab and the low-dose bevacizumab combined with low-dose triamcinolone groups, BCVA increased significantly at 1 month, 2 months, and 3 months after injection (<em>p </em>< 0.05). In addition, in both groups, neither intraocular pressure (IOP) nor central macular thickness increased significantly at 1 month, 2 months, or 3 months after injection (<em>p</em> > 0.05). The BCVA, IOP, and central macular thickness (CMT) at 1 month, 2 months, and 3 months after injection showed no significant differences between the intravitreal bevacizumab group and the low-dose bevacizumab combined with low-dose triamcinolone group (<em>p</em> > 0.05). Conclusions: The BCVA of both groups increased significantly, and the CMT of both groups decreased significantly in patients with diabetic macular edema. The injection of low-dose intravitreal bevacizumab combined with low-dose intravitreal triamcinolone may be useful for the treatment of diabetic macular edema.
