        의과대학 조기임상경험의 성찰일지 내용 분석

        This study qualitatively analyzed medical students’ engagement of pre-clerkship early clinical experience and their implications based on reflective journals. The subjects were 60 third-year students who had taken the early clinical experience course offered by the Catholic Kwandong University College of Medicine. Students had to complete and submit a reflective journal at the end of the course. This study used the reflective journals students submitted as analysis data. Data were analyzed using Elo and Kyngäs’ inductive qualitative analysis. As a result of study, a total of 127 unique significant keywords and statements were extracted. These statements generated 11 formulated sub-categories and five theme categories. Five main categories were derived from the students’ reflective journals: understanding of patients, understanding of professionalism, understanding of other occupations, understanding of the hospital system, and motivation to learn. Students understood patients’ perspectives on diseases and improved their ability to empathize; they gained confidence in communication and clinical skills; they became more positive toward the underprivileged; and their interest in the medical system grew. The results suggest that early clinical experience can be applied as one beneficial educational program for medical students in the pre-clerkship curriculum. 본 연구의 목적은 임상실습 전(pre-clerkship)에 병원에서 임상을 경험한 의과대학생들의 성찰일지를분석하여 조기임상경험의 의미를 알아보는 데에 있다. 연구는 가톨릭관동대학교 의과대학 의학과 1학년에서 시행한 임상실습 전 조기임상경험 프로그램을 수강한 60명 학생들의 성찰일지를 대상으로 하였다. 성찰일지 내용은 Elo와 Kyngäs의 귀납적 질적 내용분석 방법으로 분석하였다. 분석 결과 성찰일지에나타난 의미 있는 키워드와 문장은 총 127개였으며, 범주화 형성을 통해 11개의 하위범주로 분류하였다. 최종적으로 환자에 대한 이해, 전문직업성의 이해, 타직종의 이해, 병원시스템 이해, 학습동기 등 5개의주범주를 도출하였다. 조기임상경험 프로그램을 통해서 학생들은 질병에 대한 환자의 관점을 이해하고공감능력이 향상되며, 의사소통과 임상적 기술에 자신감을 갖게 되며, 소외 계층에 대한 보다 전향적인자세를 갖게 되었으며, 의료체계에 대한 관심이 높아지는 학습 성과를 얻었다는 것을 알 수 있었다. 본 연구는 조기임상경험이 임상실습 전 교육과정에 있는 의과대학생들을 위한 유익한 교육 프로그램으로 적용될 수 있을 가능성을 시사하였다.

The purpose of this study is to find out useful parameters oftrunk accelerometry in evaluating gait abnormalities of stroke patients with hemiparesis by comparing it withother characteristics of hemiparetic gait. Methods: Nine hemiparetic stroke patients were enrolled in the study. Aportable accelerometer was attached between L3,4 intervertebral area, and vertical and medio-lateral accelerationwere measured during 10m walking of each subject. Other gait characteristics such as linear parameters, foot con-tact area and force, center of pressure were measured by F-scan. Results: Swing symmetry showed significantcorrelation with mean value of peak I of unaffected side, standard deviation of step time of unaffected side, steptime ratio and walking velocity. Stance symmetry showed significant correlation with standard deviation of steptime of unaffected side, step time ratio and walking velocity. (p〈0.05) Anteroposterior distance of center of pres-sure, maximal force and foot contact area did not show significant correlation with accelerometric parameters.C o n c l u s i o n s: In evaluating gait abnormalities of hemiparetic stroke patients by trunk accelerometry, step timeratio, standard deviation of step time and mean of peak I in unaffected side can be used as useful parameters thatreflect gait symmetry.대한뇌졸중학회지제6권제1호2 0 0 4년5월

        노인 자동차 운전자들의 운전 실태, 운전 습관 및 안전성

        Objective: To investigate the actual driving status and driving related safety issues of older drivers aged 65 years or older. Method: We conducted a survey on 56 elderly drivers about driving status, driving habits, safe driver self check list, and medical conditions that may affect safe driving. For comparison, 50 younger drivers aged between third and fifth decades were also surveyed. Results: The mean age of total 56 elderly drivers was 69.96 years old. Their mean driving time in life was 23.6 years and the purposes of driving were shopping (23.2%), religion activity (21.4%) and leisure (16.1%). Most vehicles were equipped with automatic transmission (83.9%) and power steering (91.1%) and the most common type of vehicle was a sedan (82.1%). The incidence of their motor vehicle accidents in recent 2 years was 21.4%, which was higher than that of control group (18.0%) without statistical significance. The driving habits questionnaire revealed elderly drivers tend to avoid rush-hour traffic, driving at night, and high-traffic roads in 75.0%, 69.6%, 51.8% of subjects respectively. The result of safe driver self check list showed that mean demerit score of elderly drivers was 4.3, which was higher than that of younger drivers (3.2). Elderly drivers had hypertension (33.9%), diabetes mellitus (17.9%), eye disorders (8.9%), heart disease (8.9%), arthritis (8.9%), stroke (7.1%), and respiratory disease (5.4%). Conclusion: Compared to younger drivers, elderly drivers tend to avoid unsafe driving situations and reported more safety problems in the safe driver self check list. Elderly drivers also had more medical conditions that may affect safe driving, such as eye disorders, cardiovascular, and metabolic diseases. (J Korean Acad Rehab Med 2010; 34: 570-576)

      • V. Vulnificus Cytolysin에 대한 Sandwich ELISH

        A sensitive Enzyme-Linked Immunosorbent Assay(ELISA) for Vibrio vulnificus cytolysin was established. Cytolytic activity in V. vulnificus culture supernatant reached to the maximum at about 4 hours and decreased rapidly afterwards. V. vulnificus cytolysin was completely purified from culture supernatants by ammonium sulfate precipitation, DEAE-cellulose chromatography and phenyl-Sepharose chromatography. The polyclonal antibodies against the purified cytolysin were produced from a chicken and a rabbit. Resulting antibodies showed a high antigen-specificity for cytolysin, visualizing a band of 51 kDa when the V. vulnificus culture supernatants were on immunoblottings. Anti-cytolysin polyclonal rabbit and chicken antibodies were used as a capture or a detector antibody in sandwich ELISA after further purification with cytlysin-AFFi-prep immunoaffinity chromatography. The limit of detection was 10 ng of V. vulnificus cytolysin, with saturation achieved at 50 ng.

        노인 자동차 운전자들의 운전 능력 평가

        Objective: To assess the driving ability of older drivers, their visual function, cognitive-perceptual function, motor function and driving performance were evaluated. Method: Subjects were 55 drivers aged 65 years or older. Visual function test included visual acuity, visual field, color vision and contrast sensitivity. Cognitive perceptual function was evaluated with the cognitive perceptual assessment for driving (CPAD) and clock drawing test. For motor function, muscle strength and range of motion were evaluated. Driving performance was evaluated by virtual reality based driving simulator. For comparision, 48 younger drivers aged between late twenties and early forties underwent the same evaluation. Results: Among older drivers, 21 (38.2%) had visual acuity less than 20/40, 3 (5.5%) had visual field narrower than 140o bilaterally. Contrast sensitivity was significantly decreased in both day and night with glare light conditions. In cognitive- perceptual function assessment, 20 subjects (36.4%) passed CPAD test, 3 subjects (5.5%) failed, and 32 subjects (58.1%) fell into borderline group. Mean CPAD score was 50.65± 5.62, which was significantly lower than that of younger drivers. 18 subjects (32.7%) were incorrect in clock drawing test. In motor function assessment, 4 subjects (7.3%) in older drivers showed hemiparesis secondary to stroke. In driving simulator, 21 subjects (38.2%) failed whereas only 4 subjects (8.3%) did in younger drivers. Average demerit score was 24.09±15.53 and was significantly higher than that of younger drivers. Conclusion: Older drivers showed significantly higher incidence of visual and cognitive-perceptual dysfunction, and poorer driving performance compared to younger drivers group.
