N/A Recently, epidermal growth factor receptor(EGFR) has been examined in many tumors such as lung cancer, breast cancer, brain tumor and stomach cancer. And it has been known to be associated with the development and progression of such tumors. However, there is no agreernent of the expression of EGFR with clinical staging and .ne histological differentiation of colon cancer of which incidence is increasing progressively in Korea. This study was initiat- ed to clarify the importance of expression of EGFR on clinical staging and clinicopathologic parameters including the depth of invasion, the histologic differentiation of cancer cells, tumor site, tumor size and the blood level of carcinoembryonic antigen(CEA). The expression of EGFR was examined immunohistochemically using monoclonal antibody against EGFR in total of 49 colon cancer tissues in the patients undertaken colectomy due to colon cancer and 10 metastatic lymph nodes in the same patients and 18 bnign tumors such as polyps and adenomas and 13 normal colon tissues in the patients undertaken colectomy due to t.raumatic colon injury or inflammatory disease. The immunoreactivity of EGFR was detected in 36.7% (18/49) of the colon cancers, while it was observed in 5.6%(1/18) of the benign tumors, the incidence between the two being signif- icantly different(P<0.05). However, there was no expression of EGFR in normal colon tis- sues. And the immunoreactivity of EGFR in metastatic lymph nodes was detected in 40.0% (4 /10) of the metastatic lymph nodes. The immunoreactivity of EGFR of human colon cancers was gradually increased from 12.5%(1/8) of Dukes-Astler-Coilers stage A, 22.2%(2/9) of stage Bl, 16.7%(2/12) of stage B2, 42.9%(3/7) of stage Cl to 76.9%(10/13) of stage C2(P <0.05). And the immunoreactivity of EGFR was related to their depth of invasion of cancer cells(P<0.05). There was no relation between the immunoreactivity of EGFR, cancer size, cancer site and level of carcinoembryonic antigen(P>0.05). The EGFR was expressed only in moderately-differentiated cancers and well-different.iated cancers, while not expressed in poor- ly-defferentiated cancers(P<0.05). The immunohistochemical staining intensity of EGFR was gradually increased according to clinical stages(P<0.05). These results suggested that the expression of EGFR may p)ay an !mportant role in the growth and differentiation of colon cancer. And the expression of EGFR using immunohistochemical stain was useful for diagnosis and treatment of colon cancer and it also may serve as a prognostic indicator.(Korean J Gastroenterol 1994; 26: 637 646)

N/A The plasma atrial naturiuretic polypeptide (ANP) was repeatedly measured in each clinical phases in 20 patients (20.8±7.2yr) with Korean Hemorrhagic Fever (KHF). The study purpose was to see whether there is any relationship between the clinical course of KHF and the changes of plasma level of ANP. In three patients the plasma ANP could be repeatedly measured from the early oliguric to diruetic phases. In these three patients the plasma level of renin activity and aldosterone were very high in the early oliguric phase. Then the plasma renin activity and aldosterone level decreased steeply in remarkable degree to near normal level while the patient is still in oliguric phase and stayed at this level during the diuretic phase. The plasma ANP level was low (30-80pg/ml) in the early oliguric phase. Then with the sudden remarkable decrease of plasma renin activity and aldosterone the plasma ANP increased rapidly to high level (230-280pg/ml), then was decreased to near normal level (90-190pg/ml) making a narrow peaked plasma ANP concentration curve, and then the level stayed at near normal level during the diuretic phase. Around the time when the plasma renin activity and aldosterone concentrations were decreased to their low level and the plasma ANP was increased to it's peak level the oliguric phase was changed to the diuretic phase. The changes of plasma cortisol level followed the pattern of changes of plasma renin and aldosterone. It is possible that the increase of plasma ANP level might have had some role in the initiation of diuretic phase in these patients. It is not certain whether these changes are the unique changes seen only in patients with KHF or the changes seen also in patients with acute renal failure of other causes.

We report a case of pseudomembranous colitis with severe diarrhea, abdominal pain, generalized edema, elevated serum carcinoembryonic antigen (CEA), and probable spontaneous bacterial peritoni- tis. All symptoms and signs were completely relieved after 10 days of treatment with oral vancomcin, 500 mg daily. Serum CEA level was also normalized.

N/A Objective : The golden standard of serodiagnosis of HFRS is IFA(indirect immunofluorescence antibody test). However, positive IFA for IgG antibody does not always correlate with current infection because IFA may be positive due to past exposure to Hantaan virus such as subclincal infection or due to vaccination in endemic area. So supplementary diagnostic method is needed to diagnosis of HFRS with more accuracy. Methods : In this study, the usefulness of IgG avidity assay with 8 mole/l urea in diagnosis of HFRS was investigated. Serum samples from 45 patients with acute phase HFRS and 79 residents of endemic area, who had HFRS antibody were tested for IgG avidity. Results : The distribution of IgG avidity index of HFRS antibody was different acute phase of HFRS patients from endemic area residents(p<0.001). Patients with acute phase HFRS exhibited lower avidity of Hantaan virus-specific IgG (mode of 64 of avidity index), in contrast endemic area resident had a higher avidity (mode of 4 of avidity index). Conclusion : The IgG avidity assay should assist in the diagnosis of acute phase HFRS and may be used to identify recent infection and past exposure to Hantaan virus.

N/A Background: The enteroviruses are the most common etiologic agent of aseptic meningitis in adults and children. The incidence of enteroviral meningitis in childhood meningitis is up to 80%, but in adults is not known, worldwidely. In Korea, where tuberculosis is endemic, the rapid and accurate diagnostic method for enteroviral meningitis is required especially because early differential diagnosis of viral meningitis from tuberculous meningitis is very important. The aims of this study were the demonstration of enteroviruses from cerebrospinal fluid (CSF) of adult patients with aseptic meningitis by PCR/Southern hybridization and the verification of the usefulness of PCR/southern hybridization as a rapid diagnostic tool. Methods: From July 1992 to June 1995, total 34 CSF samples (10 from children, 24 from adults) of patients with aseptic meningitis were studied. As a control group, 15 patients with tuberculous meningitis and 15 patients with bacterial meningitis were studied. Viral RNA was extracted from CSF, reverse transcriptied into cDNA and amplified. The PCR products were Southern hybridizied with enteroviruses-specific digoxigenin-labelled probe. Results: 16/24(66.7%) samples of adult patients with aseptic meningitis were positive for enteroviruses, while in child patients with aseptic meningitis, 9/10(90%) samples were positive. And in one patient, PCR was positive from asymptomatic, onset-7th day CSF sample. Conclusion: Enteroviruses were the most common causative organisms of adult aseptic meningitis in Korea. And, this study showed the usefulness of PCR/Southern hybridization of enteroviruses from CSF for etiologic diagnosis of adult aseptic meningitis in subclinical, asymptomatic period.
