心臟에 原發한 纖維肉腫 : 症例報告 Report of A Case ana Review of Literature

Malignant tumors primarily in the heart are rare, and moreover fibrosarcoma of heart is of much rare occurrence. In 1700 & 1762, Boneti & Morgagni, at first, reported the primary fibrosarcoma of heart. Mahaim(1945) Collected 329 cases of the tumors of hearts& 84 Cases of the tumors of pericardium, in the literatures, and 87 cases of a total 413 cases were in the heart & 20 cases were also sarcoma in the pericardium. In 1931, Yater reported 10 cases of primary tumors of pericardium. A case of clinically and pathologically diagnosed fibrosarcoma, 49 year-old American female is reported, because of its rare occurrence anb also our first expreience of this type of primary malignant tumor of the heart. Initial symptoms and signs were chest discomfort, anemia and an episode of shock state. She had a history of myopia and thyroidectomy 8 years ago, and of cardiac tamponade 5 months prior to admission. She went to a methodst hospital and was diagnosed by biopsy, fibrosarcoma of Rt. atrium, 5 months prior to admission. Autopsy examination disclosed massively enlarged heart weighing 2,510 gm. The external surface of heart showed shaggy white gray fibrous appearanec of the tumor. The tumor was completely replacing the epicardium, pericardium, both auricles and ventricles, extending to the ascending aorta and myocardium, but not to the endocardium. The tumor tissue was generally grayish white with jelly-like myxoid appearance and hemorrhage. The circumf erences of the valves were; tricuspid, 9.5; mitral, 7.5; aortic, 6. 5 and pulmonic, 6.0 cm. The right lung was also involved, weighing around 1, 010gm., in which the tumor was spreading along the pleural surface, mainly into the interlobular septum and also into the small areas of parenclhynia. The left lung was also involved weighing around 540 gm. and the pleural surface of the diaphragm was also involved. Histopathology of the tumor showed fibrosarcoma with scattered areas of my xornatous degeneration and hemorrhage.

The histogenesis of the adenomyosis uteri has heretofore been the theme of much controversy. And in spite of the relatively frequent studies on adenomyosis that have appeared in the literature, comparatively little attention had been given to its occurrence in conjunction with carcinoma or tuberculosis of the uterus. This report consist of two cases of adenomyosis, associated with tuberculosis in one and endometrial carcinoma in another. The first patient was a 30 years old female who had complained abdominal discomfort, vaginal bleeding, difficulties in urination and defecation for 4 months duration. She -was a nulli-para and her chest was clear on X-ray examination. At operation the uterus was in normal position but two masses, one in the left wall and another in the anterior wall of the uterine body, were noted. The cut surface of the mass showed fibrous and whirl like tissue. Microscopically the lesion was a typical adenomyosis but scattered granulomatous reaction in the stroma of isolated endometrial tissue in the myometrium. The second patient was a 55 years old female who had complained of vaginal spotting and lumbago for 9 months. At operation a large mass is found in the right wall of the uterine body. Examination of removed uterus showed several small and large myoma masses in the myometrium. The endometrium was irregular and partly friable. Microscopically the endometrium showed well differentiated adenocarcinoma and multiple islands of carcinomatous glands in the myometrium accompanied by normal endometrial stroma. At some area, the islands of endometrial tissue showed no evidence of malignant change but a typical adenomyosis.

The authors analysed 220 patients (225 cases) of complications including implant failure and refracture in fracture treatment which had been requested for medical consultation by the Korean Automobile, Fire k Marine Insurance Co., Ltd. from July 1984 to December 1992. The results were as follows: 1. The patients were consisted of 178(81%) males and 42(19%) females. 2. The average age of patients was 36.9 years. 3. There were 140(60%) cases of femur, 63(29%) cases of tibia and 12(5%) cases of humerus. 4. The most common previous mode of treatment was plate and screw fixation (183 cases, 83%). 5. There were 171(78%) cases of metal failure, 14 cases of secondary fracture and 14 cases of refracture. 6. Among the causes of complications, improper postoperative care (156 cases, 71%) was the most common causes of complications. 7. It is recommended that in the operative treatment of fracture, orthopaedic surgeon should obey the AO principles and proper postoperative care and also consider the conservative treatment as a good counter part of the operative treatment.
