척추 압박 골절의 임상 양상 및 노인 기능에 미치는 영향

연구배경 : 척추 압박 골절은 노인에 있어 신체적, 정서적 활동 및 일상 생활에 제한을 가져올 수 있다. 국내에서는 아직까지 척추 압박 골절이 신체적, 정서적 활동 및 일상 생활에 어떠한 영향을 미치는지에 대한 연구는 아직까지 미흡한 실정이다. 이에 본 연구에서는 척추 압박 골절의 임상 양상과 노인 기능에 미치는 영향을 알아보고자 하였다. 연구방법 : 1995년 9월부터 1999년 6월까지 분당의 모 대학병원 가정의학과에 입원한 60세 이상의 환자들 중에서 과거 척추 압박 골절로 진단된 68명을 대상으로 척추 압박 골절의 부위와 골절수, 요통유무, 골대사율(골흡수율, 골생성율) 및 골밀도를 조사하였다. 신체 기능 평가를 위하여 일상 생활 동작 수행 능력(ADL)과 도구적 생활 동작 수행 능력(IADL) 및 노인 우울 척도(GDS)를 평가하였고, 척추 압박 골절과의 관계를 분석하였다. 결과 : 연구 대상군의 평균 연령은 74.16(±6.88)세였고, 68명중 여자가 62명(91.2%)이었다. 낙상의 병력이 있는 경우가 77.8%, 요통이 있는 경우는 71.2% 이었으며, 휠체어와 지팡이를 사용하는 경우가 각각 6.1%, 12.2%였다. 평균 척추 압박 골절수는 2.63(±0.29)개였으며 척추 압박 골절이 가장 많은 부위는 제1요추였으며 다음으로 많은 부위는 제 12 흉추였다. 척추 압박 골절수의 증가에 따라서 골밀도가 감소하였으나 통계학적 의의는 없었고(age adjusted, r= -0.179, p=0.239) 동통에 따른 척추 압박 골절 수의 유의한 차이를 보였으나(P<.01) 다발성 척추 압박 골절군(2개 이상)과 1개의 척추 압박 골절 군에 따른 일상 생활 동작 수행 능력, 도구적 생활 동작 수행 능력 및 우울 정도와의 차이는 없었다(P>.05). 결론 : 척추 압박 골절수에 따른 기능의 유의한 차이는 없었으나 동통에 따른 척추 압박 골절수의 유의한 차이를 보였다. 따라서 고위험군이나 이미 척추 압박 골절이 있는 대상자들에서 이에 대한 진단 및 예방을 위한 적극적인 노력이 필요함을 제시한다. Background : Vertebral compression fractures cause the limitation of physical, emotional, and daily living activity in elderly persons. There is no work that evaluates the effects of vertebral compression fractures on the physical, emotional, and daily living activity among Korean elderly patients. Thus, we undertook this study to learn about the clinical consequences and the functional impairment among elderly persons with vertebral compression fractures. Methods : The 68 patients over 60 years of age who had vertebral comperssion fractures documented in their medical records and admitted to the department of family medicine, Pundang CHA general hospital from September 1995 to June 1999 were enrolled in this study. We reviewed their medical records and investigated the site and the number of vertebral compression fractures, the presence of the associated pain, bone turnover markers and bone mineral density(BMD). The Katz Activities of Daily Living Scale(ADL) and Instrumental Activities of Daily Living Scale(IADL) were used to asses the patient's physical function, and Geriatric depression scale(GDS) was used to asses the patient's depression. Results: 62 women and 6 men comprised the study population. Their mean(±SD) age was 74.16(±6.88) years. 77.8%, 71.2%, 6.1%, and 12.2% of the patients had documented the past medical history of fall, back pain, use of wheel chair and cane, respectively. The mean number of vertebral compression fractures was 2.63(±0.29). Vertebral compression fractures were most commonly located in the L1 and Tl2 spines in sequence. The number of vertebral compression fracture was negatively correlated with BMD, but there was no significance(age adjusted, r=-0.179, p=0.239). There showed no difference of ADL, IADL, and GDS between single fracture and multiple fractures groups(p>.05). Conclusions : These data showed that the most common sites of vertebral compression fracture were LI and Tl2 spine. And there was no significant difference of functional status between single fracture and multiple fractures groups. But, significant difference of the number of vertebral compression fracture between the two groups who had or dindn't have back pain was noted in this study population.

For the purpose of studying the change of mechanical properties of weld parts, shielded metal are welding, one-pole and two-pole submerged arc welding were accomplished weldability on $60kg/mm^2$ quenched and tempered high strength steel. Charpy impact values of the weld metal in welded parts by SMAW and SAW were lower than those of the heat affected zone and increased in order of bond, coarsened, refined and carbon spheroidized regions in the heat affected zone. Grain size of prior austenite or M-A constituent did not significantly affect toughness of welded parts, but precipitated carbide films which forms at the grain boundaries or within matrix and volume fraction of pearilte were most important factor for toughness.

Shaped Charge's penetration performance is depended on the shape of warhead and explosive, liner materials. The liner that manufactured to small homogeneous grain increase the penetration performance and decrease the deviation of penetration performance. This texture is obtained by forging process but, creating the process that remove crack and get small homogeneous grain is very hard. In this study, We success to get the homogeneous small grain texture through appling the most suitable process by DEFORM CODE analysis.

For the purpose of studying the influence of stress relieving on microstructures and mechanical properties of weld metal, manual arc welding, onepole and twopole submerged arc welding were accomplished on 60㎏/㎟ quenched and tempered high strength steel. After stress relieving, a lot of carbides were precipitated, developed and subsequently coarsened at the grain boundaries and within matrix due to multiple tempering effect in manual arc welding, resulting in deterioated toughness. Meanwhile pearlite and cementite films were spheroidized and shortened in submerged arc welding, resulting in improved considerable toughness. It was observed that main effect of stress relieving was to reduce solut supersaturation by nucleation and growth of carbide precipitates, and stress relieving led to some reduction in the yield and tensile strength but did not significantly affect elongation.

P/M coppers are subjected to the isothermal compression tests at the strain rate ranging from 0.01 to 10.0 $s^{-1}$ and the temperature from 200 to $800^{\circ}C$. The processing map reveals the dynamic recrystallization (DRX) domain in the following temperature and strain rate ranges: $600-800^{\circ}C$ and 0.01-10.0 $s^{-1}$, respectively. In the domain, the region at temperature of $600^{\circ}C$ and strain rate of $10^{-2}s^{-1}$ shows peak efficiency. From the kinetic analysis, the apparent activation energy in the DRX domain is 190.67 kJ/mol and it suggests that lattice self-diffusion is the rate controlling mechanism.

The heating unit of direct heating method manufactured as the plasma spray coating of $TiO_2/NiCr$ conductive heating material on the surface of heating unit in order to improve the disadvantages of indirect heating method. $TiO_2$ and NiCr (80wt.%Ni-20wt.%Cr) that had the properties of conduction and heating was chosen for the conductive heating material. The compositions of the composite powders were studied $TiO_2-30wt.%NiCr\;and\;TiO_2-10wt.%NiCr$. As the heating temperature was increased, the hardness of heating layer was increased because of the fine microstructure and the decrease of porosity. The adhesion strength was decreased for coarsening and connection of voids in the insulation layer, and the electrical resistivity of heating layer was increased for fine crack formation and growth. In this study, the best efficient sprayed coatings with heating unit was concluded as the plasma sprayed $TiO_2-10wt.%NiCr$ coatings that was heat treated at $300^{\circ}C$.

As a rule, high temperature superconducting coated conductors have multi-layered buffers consisting of seed, diffusion barrier and cap layers. Multi-buffer layer deposition requires longer fabrication time. This is one of main reasons which increases fabrication cost. Thus, single buffer layer deposition seems to be important for practical coated conductor process. In this study, a single layered buffer deposition of $CeO_2$ for low cost coated conductors has been tried using thermal evaporation technique. 100 nm-thick $CeO_2$ layers deposited by thermal evaporation were found to act as a diffusion layer. $1\;{\mu}m-thick$ SmBCO superconducting layers were deposited by thermal co-evaporation on the $CeO_2$ buffered Ni-5%W substrate. Critical current of 90 A/cm was obtained for the SmBCO coated conductors.

The Friction Stir Welding (FSW) has mainly been used for making butt joints in Al alloys. Development of FSW would expand the number of applications. The FSW is a relatively solid-state joining process. This study possibility of a welding between C1020 and Al6063 by means of FSW has fundamentally clarified. The primary process parameters, such as a rotating speed, rotating direction of tool and off-set of pin periphery from materials interface were optimize, respectively.
