혈액투석 시 투석액 속도가 투석적절도와 피로에 미치는 효과

Purpose: In this single repeated measures study, an examination was done on the effects of dialysate flow rate on dialysis adequacy and fatigue in patients receiving hemodialysis. Methods: This study was a prospective single center study in which repeated measures analysis of variance were used to compare Kt/V urea (Kt/V) and urea reduction ratio (URR) as dialysis adequacy measures and level of fatigue at different dialysate flow rates: twice as fast as the participant’s own blood flow, 500 mL/min, and 700 mL/min. Thirty-seven hemodialysis patients received all three dialysate flow rates using counterbalancing. Results: The Kt/V (M±SD) was 1.40±0.25 at twice the blood flow rate, 1.41±0.23 at 500 mL/min, and 1.46±0.24 at 700 mL/min. The URR (M±SD) was 68.20±5.90 at twice the blood flow rate, 68.67±5.22 at 500 mL/min, and 70.11±5.13 at 700 mL/min. When dialysate flow rate was increased from twice the blood flow rate to 700 mL/min and from 500 mL/min to 700 mL/min, Kt/V and URR showed relative gains. There was no difference in fatigue according to dialysate flow rate. Conclusion: Increasing the dialysate flow rate to 700 mL/min is associated with a significant nicrease in dialysis adequacy. Hemodialysis with a dialysate flow rate of 700 mL/min should be considered in selected patients not achieving adequacy despite extended treatment times and optimized blood flow rate.

영양교육프로그램이 항암화학요법을 받는 소화기암환자의 오심구토, 식욕부진, 식이섭취 및 영양상태에 미치는 효과

Purpose: The purpose of this study was to identify the effects of nutritional education program (NEP) on nausea and vomiting, anorexia, food intake, and nutritional status among GI cancer patients undergoing chemotherapy. Methods: A nonequivalent, non-synchronized posttest design was used. Study subjects were conveniently selected 30 patients (15 subjects for each group) who were undergoing chemotherapy. The subjects of the experimental group received NEP which consisted of 2 educations and 1 counselling while they were hospitalized, and telephone counselling with nutritional supports after discharge for four times. Results: The score of anorexia was lower and the amount of food intake was higher in the experimental group than those of the control group. However, there were no significant differences in levels of nausea and vomiting, weight, hemoglobin, albumin, and total protein level between the experimental and the control group. Conclusion: The findings demonstrate that the NEP was effective in alleviating anorexia and improving subjective food intake of GI cancer patients undergoing chemotherapy. These findings suggest that NEP can be a useful nursing intervention for preventing nutritional disorders for patients with GI cancer after chemotherapy.

혁신도시 개발정책 평가 : 지역 부동산 가격을 중심으로

This study investigates the impact of the innovative city development policy aimed at the agglomeration economy from two perspectives, focusing on land and apartment prices. These are whether the increase in local income through the agglomeration economy drive up the rise in real estate prices and/or whether real estate prices rise due to expectations for simple development profit. The former was analyzed by using macroeconomic indicators such as GRDP and per capita regional income. In the latter case, by using year-dummy variables, we explore the change of influence by each of policy implementation stage. From 2007 to 2019, as a result of panel regression analysis of 175 cities, counties, and districts, it is found that land and apartment prices rose without income increase in the innovative cities. In the case of annual apartment price change, it increased significantly in 2011, just before public institution relocation was actively executed. This can be said to reflect the expectation that housing demand will increase after the completion of relocation of public institutions. In the case of land prices, the magnitude of the increase in land prices was not that large during the research period when compared with apartment prices. We conjecture that the research period of this study is the time when the innovative city site development projects had already begun. When implementing such a large scale city development policies in the future, more sophisticated policy formation and management are required to effectively realize the economy of agglomeration, and at the same time, it is necessary to prepare a smart measure to prevent speculation in both land and apartments.

이 소설은 익히 알려진 대로 소설가 구보가 정오에서 새벽 두시에 이르기까지 경성시내를 산책하는 내용이다. 의식의 흐름 기법으로 장소를 이동할 때마다 연상되는 사건들이 과거와 현재를 넘나들며 단편적으로 그려지고 있다. 구보가 산책을 통해 추구하는 것은 ‘행복 찾기’ 이다. 그 전제조건으로 자기가 바라는 행복이 과연 무엇인가를 성찰하지 않을 수 없다. 소설가 구보는 자신이 바라는 것을 알 수 없다고 한다. 그러나 단편적인 회상들을 통해 구보는 자신이 그토록 경멸했던 소시민적인 생활, 즉 애인, 여자, 아내가 있는 가정을 소망하고 있음을 알 수 있다. 일정한 직업이 없는 지성인 소설가 구보는 물질주의에 만연된 황금광 시대의 사람들과 그들의 속물성을 경멸한다. 그러나 생활인 구보에게 황금과 아내는 필요하다. 고독을 사랑하면서도 두려워하는 구보의 이중성은 물질주의를 경멸하면서도 부러워하는 이중성과 중첩된다. 그는 산책을 통하여 자신 안에 숨겨졌던 진정한 욕망, 즉 동경에서 헤어진 여자를 잡았어야 한다는 회한을 인정하면서 아들이 가정을 이루기를 바라는 어머니의 욕망을 핑계삼아 생활에 안착하고 싶은 마음을 드러낸다. 구보의 욕망은 지식인/ 속물, 공책/금시계, 아내/여자(계집) 등의 이분법적인 경계를 넘어서 아내, 아버지, 딸과 같은 기호가 상징하는 생활의 욕망으로 변증법적으로 통합된다. 이 소설은 구보가 집을 나갔다가 돌아오는 원점회귀적 소설이지만 집에 돌아올 때의 구보는 나갈 때와 동일한 구보가 아니다. 그는 산책을 하는 동안 자기 안에 억압했던 욕망을 솔직하게 인정하고 수용하면서 중단했던 글쓰기를 다시 시작할 것과 결혼제의를 거절하지 않겠다고 결심한다. 즉 그에게 이분법적으로 보였던 창조의 욕망과 성적인 욕망이 더 높은 차원의 불멸의 욕망으로 화해되고 있다. 아버지가 되고 싶다고 고백하는 구보에게 자식이나 작품은 그의 분신이며 불멸의 욕망의 상징이기 때문이다. This novel consists of reflections and scenes observed by Gubo during his stroll in Seoul from noon to 2 o`` clock in the morning. The pasts reminded by several places where he visited penetrate his present time with techniques of stream of consciousness. The novelist Gubo longs for a happiness through his promenade. To do so, he has to find out what kind of happiness that he wants. He says that he doesn`t know what he really wants. As he walked around Seoul, he met many people, friends and girls. He visited several places like cafe, department store , bank and Seoul station. etc. In the streetcar, he met a lady with parasol who reminded him of ex-girl friend in Tokyo. Gubo loved her really but he did`nt have enough courage to merry her. Because she was once fiancee of his schoolmate. He decided to forget her and oppressed all memories about her. Nevertheless, the suppressed memories sprung out unexpectedly and disguisedly. Gubo disguised his desire for woman and money, in addition to it, he disdained this kind of snobby and bourgeois desire. He seemed to much value on writing novels to disguise his snobby desire. These two dichotomous desires, ``desire of love`` and ``desire of art`` are not harmonious in his mind.. He was not able to keep the balance between love and art, so he suffered from many diseases like amblyopia, headaches, tympanitis. Ending his stroll, he accepted the desire of love as his, he decided to merry with some woman by following his mother`s advice. Finally, he will overcome these competitive two desires and sublimate sexual impulse in art. Gubo tries to be a father both for his future children and for his novels based on immortal desire. Now two competitive desires are in harmony with immortal desire, Gubo is able to rewrite novels interrupted for three years.
