        한국판 청소년 부모 스트레스 척도 타당화

        This study was conducted to exam the validity and reliability of Korean version of 70 item–SIPA which was produced from the preliminary study. Data were collected and analysed from 350 caregivers whose children aged from 11 – 19 residing in the area of Seoul and Kyunggi district. Through the process of study, first, reliability was verified by demonstrating the convergency of items and internal consistency. Second, the validity of construct was confirmed through the confirmatory factor analysis. Third, concurrent validity was examined by using referenced scales such as K-CBCL, parents’ role stress, parent efficacy, and family cohesion. Fourth, hierarchical liner regression was analysed to predict the significant sub-scales of SIPA by utilizing dependent variables of referenced scales. Consequently, it was evident that K-SIPA is a reliable and validate tool for assessing parenting stress among Korean care givers. At the end, limitation and significancy of this study and future suggestion were discussed.

        Polyopes lancifolia Extracts Protect INS-1 Pancreatic β Cells against High Glucose Level Induced Apoptosis

        Polyopes lancifolia extracts were investigatedfor protection of INS-1 pancreatic β cells against highglucose level-induced apoptosis using cell viability, lipidperoxidation generation, intracellular ROS, and NO assays. Expressions of the Bax and Bcl-2 proteins in INS-1 cellswere evaluated using western blotting. Apoptosis inducedby high glucose levels was determined based on analysis ofFITC-annexin V/PI stained cells. A high glucose leveltreatmentinduced INS-1 cells death. Cells treated with P. lancifolia extracts showed markedly reduced cytotoxicityin a dose-dependent manner, compared to the high glucoselevel-treated cells. Treatment with extracts dose-dependentlydecreased lipid peroxidation and intracellular ROS and NOlevels, and increased cell viability in high glucose levelpretreatedINS-1 cell, compared to the high glucose leveltreatedcells. Extract treatment significantly (p<0.05) reducedthe Bax protein level and increased the anti-apoptotic Bcl-2 protein level, compared with the high glucose leveltreatedcells. P. lancifolia extract treatment reduced theapoptosis that is induced by a high level glucose-treatment,based on annexin V/PI staining.

        선형 및 비선형역학적 방법에 의한 당뇨병 환자의 술전 심박수 변이도 분석

        Analysis of Heart Rate Variability in Preoperative Diabetic Patients by Linear and Nonlinear Dynamics Seong-Won Min, M.D. Department of Anesthesiology, Seoul Municipal Boramae Hospital, Seoul, Korea Background: The aim of this study was to investigate the underlying changes in heart rate variability(HRV) of preoperative diabetic patients using five analytical methods; SDNN (standard deviation of normal to normal intervals), SDANN (standard deviation of the mean of normal RR intervals for each 5 min period of the entire electrocardiographic recording), RMSSD (root mean squate successive difference, the squre root of the mean of the sum of the squares of differences between adjacent normal RR intervals over the entire electrocardiographic recording), PNN50 (percent of difference between adjacent normal RR intervals that are greater than 50 ms computed over the entire electrocardiographic recording) for linear time domain analysis and approximate entropy for nonlinear complexity analysis. Methods: HRV values analyzed by five different measures were compared between a control group of ten nondiabetics without any significant systemic disease and a diabetic group of ten patients from the preperative ambulatory electrocardiographic recordings. Results: Approximate entropy, SDNN and SDANN values were significantly lower in the diabetic group than those of the control group (P < 0.01). Conclusions: Significantly decreased approximate entropy, SDNN and SDANN could provide information about decreased cardiovascular complexity and sympathetic output, suggesting the nature of dysfunction of the diabetic cardiovascular system.         Polyopes lancifolia Extract, a Potent α-Glucosidase Inhibitor, Alleviates Postprandial Hyperglycemia in Diabetic Mice

        This study was designed to investigate the inhibitory effects of Polyopes lancifolia extract (PLE) on α-glucosidase activity, α-amylase activitiy, and postprandial hyperglycemia in streptozotocin (STZ)-induced diabetic mice. The results of this study revealed a marked inhibitory effect of PLE on α-glucosidase and α-amylase activities. The IC50s of PLE against α-glucosidase and α-amylase were 0.20 mg/mL and 0.35 mg/mL, respectively. PLE was a more effective inhibitor of α-glucosidase and α-amylase activities than acarbose, the positive control. The postprandial blood glucose levels of STZ-induced diabetic mice were significantly lower in the PLE treated group than in the control group. Moreover, PLE administration was associated with a decreased area under the curve for the glucose response in diabetic mice. These results indicate that PLE may be a potent inhibitor of α-glucosidase and α-amylase activities and may suppress postprandial hyperglycemia.

        Power Spectral Analysis 를 이용한 당뇨병성 자율신경병증의 술전 평가

        Background: The aim of this study is to investigate the changes in the patterns of power spectra of R-R interval variability of diabetic patients who are subject to autonomic neuropathy. Methods: The changes in power spectra of eight diabetic patients were compared with those of eight normal persons while changing positions from supine to Trendelenburg and from supine to standing each. Results: Low, high and total frequency power densities of diabetic patient group were significantly lower than those of control group in resting supine position. Low frequency power density and ratio of low frequency power to high frequency power of control group increased significantly while changing position from supine to standing compared with those of diabetic patient group. And low frequency power density of control group decreased significantly compared with that of diabetic patient group while changing position from supine to Trendelenburg. Conclusions: The decrease in power densities of low, high and total frequency suggest depressed overall autonomic activities in diabetic patient group and significantly decreased changes in low frequency power and ratio of low frequency power to high frequency power while changing positions from supine to standing suggest attenuated sympathetic activity in diabetic patient group, reflecting autonomic changes in diabetic group rapidly.         Linear and Nonlinear Analysis of Heart Rate Variability in Postoperative Myocardial Ischemic Patients: Time Domain Analysis versus Approximate Entropy Seong0Won Min, M.D. Department of Anesthesiology, Seoul Municipal Boramae Hospital, Seoul, Korea Background: Postoperative myocardial ischemia has been regarded as one of the major predictors of adverse cardiac outcomes after noncardiac surgery in high risk patients. Many schemes have been proposed to stratify the potential risk of this patient group in more noninvasive and cost-effective ways and analysis of heart rate variability (HRV) is one of them. To uncover the underlying changes in HRV with postoperative myocardial ischemia five analytical methods were introduced; SDNN (standard deviation of normal to normal intervals), SDANN (standard deviation of the mean of normal RR intervals for each 5 min period of the entire electrocardiographic recording), RMSSD (root mean square successive difference, the squre root of the mean of the sum of the squares of differences between adjacent normal RR intervals over the entire electrocardiographic recording), PNN50 (percent of difference between adjacent normal RR intervals that are greater than 50 ms computed over the entire electrocardiographic recording) for linear time domain analysis and approximate entropy for nonlinear complexity analysis. Methods: Sixteen vascular surgical patients were monitored by an ambulatory electrocardiogram preoperatively and during the first postoperative day (PODI). HRV values analyzed by five different measures were compared between a control group (C group) of eight patients with no postoperative ischemia and a postoperative ischemic group (PI group) of eight with ischemia on PODI. Results: Approximate entropy was the only measure of HRV which was significantly lower in PI group than that of C group (P < 0.01) on PODI. Conclusions: Approximate entropy, a complexity measure could provide more sensitive information about the physiologic changes associated with postoperative ischemia which could not be obtained from the conventional HRV measures. Time domain analyses can be used as adjuvant measures providing information about the cardiac autonomic regulation. (Korean J Anesthesiol 2001; 41: 141~147)

        Effect of Polyopes lancifolia Extract on Oxidative Stress in Human Umbilical Vein Endothelial Cells Induced by High Glucose

        The protective effect of Polyopes lancifolia extract on high glucose-induced oxidative stress was investigated using human umbilical vein endothelial cells (HUVECs). High concentration of glucose (30 mM) treatment induced HUVECs cell death, but Polyopes lancifolia extract, at concentrations of 25, 50, and 100 μg/mL, protected cells from high glucose-induced damage. Furthermore, thiobarbituric acid reactive substances, intracellular reactive oxygen species, and nitric oxide levels increased by high glucose treatment were effectively decreased by treatment with Polyopes lancifolia extract in a dose-dependent manner. Also, Polyopes lancifolia extract treatment reduced the overexpressions of inducible nitric oxide synthase, cyclooxygenase-2, and nuclear factor-kappa B proteins activation that was induced by high glucose in HUVECs. These results indicate that Polyopes lancifolia extract is a potential therapeutic material that will reduce the damage caused by high glucose-induced-oxidative stress associated with diabetes.
