        9세 소아에서 진단된 흰점망막병증

        Purpose: We report a case of fundus albipunctatus discovered in a young patient. Case summary: A 7.6-year-old female showed numerous small whitish-yellow flecks in the perimacular area and retinal periphery. Dark adapted 0.01 electroretinography (ERG) and dark adapted 3.0 ERG were profoundly reduced. At 26 months after the first visit, the best-corrected visual acuities were 1.0 right eye and 0.9 left eye. There were no pigmented lesions, atrophic lesions, or vascular abnormalities in the retina. Humphrey and Goldmann visual field tests were performed, but neither of the tests revealed any scotomas or other visual field defect. The number and size of characteristic numerous small whitish-yellow retinal flecks seemed almost unchanged. In spectral domain-optical coherence tomography (SD-OCT), the subretinal hyperreflective lesions spanned the retinal pigment epithelium and the external limiting membrane. ERG showed improved dark adapted responses (dark adapted 0.01 ERG and dark adapted 3.0 ERG) after prolonged dark adaptation (2.5 hours). No family member showed any abnormal findings. Conclusions: Fundus albipunctatus is a rare disease in Koreans. We report a case diagnosed using fundus photography, SD-OCT, visual field tests, and ERG after prolonged dark adaptation (2.5 hours). 목적: 소아에서 발견된 흰점망막병증 증례를 문헌 고찰과 함께 보고하고자 한다. 증례요약: 만 7세 7개월인 여아에서 망막중간부와 주변부에 수많은 흰색-노란색의 반점들이 관찰되었으며, 망막전위도검사에서는 암순응 0.01 망막전위도와 암순응 3.0 망막전위도가 현저하게 감소하였다. 2년 2개월 뒤 양안 모두 망막에 색소침착이나 위축성병변은 관찰되지 않았고, 시신경유두와 혈관도 정상이었다. 시야검사에서 이상은 발견되지 않았고, 반점들은 변화가 거의 없었다. 빛간섭단층촬영검사에서 반점들이 있는 곳에서 고반사의 병변이 망막색소상피층 위쪽부터 외경계막에 걸쳐 관찰되었다. 새로 측정한 망막전위도검사에서 2년 전과 유사한 결과를 보였지만, 암순응 시간을 150분으로 늘려서 다시 시행한 결과, 암순응 0.01 망막전위도와 암순응 3.0 망막전위도가 정상 범위로 호전되었다. 결론: 흰점망막병증은 드문 질환으로, 소아에서 특징적인 안저 소견과 망막전위도검사, 장시간 암순응 후 망막전위도검사, 시야검사, 빛간섭단층촬영검사 그리고 가계도 조사 등의 결과로 흰점망막병증을 진단할 수 있었다.

        Prediction of Relative Stability between TACE/Gelastatin and TACE/Gelastatin Hydroxamate

        A gelastatins (1), natural MMP inhibitors, and their hydroxamate analogues (2) in TACE enzyme evaluated for discovery of potent TACE inhibitors. We have employed molecular dynamics simulations to compute the relative free energy of hydration and binding to TACE for gelastatin (1) and its hydroxamate analogue (2). The relative free energy difference is directly described in this article using the free energy perturbation approach as a means to accurately predict the TACE inhibitor of gelastatin analogues. The results show that the good agreement between the experimental and theoretical relative free energies of binding, gelastatin hydroxamate (2) binds stronger to TACE by ‒3.37 kcal/mol. The desolvation energy costs significantly reduced binding affinity, hydroxamate group associated with high desolvation energy formed strong favorable interactions with TACE with more than compensated for the solvation costs and therefore led to an improvement in relative binding affinity.

        Computational Drug Discovery Approach Based on Nuclear Factor-κB Pathway Dynamics

        The NF-κB system of transcription factors plays a crucial role in inflammatory diseases, making it an important drug target. We combined quantitative structure activity relationships for predicting the activity of new compounds and quantitative dynamic models for the NF-κB network with intracellular concentration models. GFA-MLR QSAR analysis was employed to determine the optimal QSAR equation. To validate the predictability of the IKKβ QSAR model for an external set of inhibitors, a set of ordinary differential equations and mass action kinetics were used for modeling the NF-κB dynamic system. The reaction parameters were obtained from previously reported research. In the IKKb QSAR model, good cross-validated q^2 (0.782) and conventional r^2 (0.808) values demonstrated the correlation between the descriptors and each of their activities and reliably predicted the IKKβ activities. Using a developed simulation model of the NF-κB signaling pathway, we demonstrated differences in IκB mRNA expression between normal and different inhibitory states. When the inhibition efficiency increased, inhibitor 1 (PS-1145) led to long-term oscillations. The combined computational modeling and NF-κB dynamic simulations can be used to understand the inhibition mechanisms and thereby result in the design of mechanism-based inhibitors.

        일스병 환자에서 경과관찰 중 발생한 망막중심정맥폐쇄

        Purpose: To report the occurrence of central vein occlusion in an Eales disease patient. Case summary: A 23-year-old man presented with decreased left eye visual acuity and was diagnosed with bilateral Eales disease after ophthalmic evaluations. The patient received laser photocoagulation and visual acuity in his left eye improved 1 month after treatment. He was followed up regularly for 3 years and had no specific eye problems. Subsequently, the patient visited our clinic because of visual disturbance in his right eye. The patient’s visual acuity was 0.6 in his right eye, and 1.0 in his left eye. On right eye fundus examination, there were multiple flame shape hemorrhages and retinal vascular tortuosity was observed. Arteriovenous transit time was extended on fluorescein angiography. Therefore, the patient was diagnosed with central retinal vein occlusion and underwent an internal medical examination to reveal a possible systemic cause of the central retinal vein occlusion; however, there were no systemic problems. Macular edema was observed on optical coherence tomography and the patient received an intravitreal bevacizumab injection. Six months after treatment, the right eye visual acuity and macular edema improved. Conclusions: Reports of branched retinal vein occlusion on the peripheral retina are common in Eales disease patients. However, the authors experienced and report a case of central retinal vein occlusion occurring in Eales disease. J Korean Ophthalmol Soc 2012;53(8):1181-1185

        눈물 흘림을 호소하는 결막이완증 환자의 수술적 치료에서 파이브린 글루의 유용성

        Purpose: To investigate the efficacy of fibrin glue used in conjunctival resection for conjunctivochalasis with epiphora Methods: Twenty-three patients (42 eyes) with conjunctivochalasis without nasolacrimal duct obstruction underwent conjunctival resection using either absorbable sutures (11 patients, 20 eyes, Group 1) or fibrin glue (12 patients, 22 eyes, Group 2) to attach the conjunctiva to the sclera. Outcomes recorded were improvement of epiphora, postoperative discomfort, and operation time. Postoperative discomfort was analyzed only in one eye (right eye) in case that the both eyes were operated. Results: Epiphora completely improved in 6 eyes (30%) in Group 1 and 8 eyes (36.4%) in Group 2, partially improved in 9 eyes (45%) and 8 eyes (36.4%), and did not improved in 5 eyes (25%) and 6 eyes (27.2%), respectively (p=1.000). On the first day postoperatively, postoperative eye discomfort developedin 7 eyes (63.6%) in Group 1 and 5 eyes (41.7%) in Group 2 (p=0.414). Throughout the following week, the discomfort lasted in 6 eyes (54.5%) in Group 1 and 1 eye (13.6%) in Group 2 (p=0.027). The mean operation time was 25.0 (±2.6) minutes in Group 1 and 12.0 (±2.4) minutes in Group 2 (p<0.001). Conclusions: The success rates were similar in the two groups. However, the use of fibrin glue significantly reduces the postoperative discomfort and the operation time. Therefore, the use of fibrin glue in conjunctival resection of conjunctivochalasis seems to be an effective method. J Korean Ophthalmol Soc 2010;51(4):498-503
