        Aneurysmal Subarachnoid Hemorrhage Following Intravenous Thrombolysis in Acute Ischemic Stroke

        Background: An incidental finding of unruptured aneurysm, which is a contraindication to the recombinant tissue plasminogen activator (rtPA), is common in patients with acute ischemic strokes. However, reports describing the rupture of intracranial aneurysm following the administration of rtPA are extremely rare. Case Report: A 51-year-old man presented to the emergency room with global aphasia. A computed tomography (CT) of the brain revealed no intracranial hemorrhage. Since global aphasia occurred in an hour, rtPA was administrated intravenously. A CT angiography was performed 2 hours after an infusion of rtPA, which despite the absence of neurological deterioration and blood pressure surge, revealed subarachnoid hemorrhage in the right cerebral hemisphere, in addition to a 3-mm saccular aneurysm with a bleb in the right middle cerebral artery. Conclusions: Aneurysmal subarachnoid hemorrhage can develop following the infusion of rtPA. Hence, unruptured aneurysm may not simply be an “incidental finding” in stroke patients receiving rtPA.

        후박(厚朴)이 ob/ob 마우스의 대사성 염증과 인슐린 저항성에 미치는 영향 및 관련기전에 대한 연구

        Objective: This study was undertaken to investigate how magnolia bark extract affects ob/ob mouse in terms of metabolic inflammation and insulin resistance. Methods: Leptin-deficient ob/ob mice were divided into 2 groups (n=5): a normal saline treatment (=control) and magnolia bark treatment. Wild type mice were the lean group (n=5). After 5 weeks, we measured fasting blood sugar (FBS) and conducted oral glucose tolerance tests (OGTTs) in each group. After 6 weeks, we measured body weight, epididymal fat pad weight, liver weight, serum glucose, serum insulin, and gene expression of tumor necrosis factor-${\alpha}$, interferon-${\gamma}$, and interleukin-6. We characterized the phenotype of adipose tissue macrophages (ATMs) and analyzed fractions of the phenotype in each group by flow cytometry. Results: In the magnolia bark group, fasting blood sugar, oral glucose tolerance levels, and insulin resistance (HOMA-IR) were significantly decreased. The population and proportion of ATMs among leukocytes in adipose tissue were significantly decreased in the magnolia bark group. The population and proportion of M1 type ATMs among ATMs were significantly decreased in the magnolia bark group. Gene expression of tumor necrosis factor-${\alpha}$ was significantly decreased in the magnolia bark group. Conclusions: These results support a positive effect of magnolia bark on metabolic diseases such as insulin resistance and metabolic inflammation in leptin-deficient ob/ob mice.

        마황약침(麻黃藥鍼)이 비만 유발 생쥐의 지방대사에 미치는 영향

        Objectives: This study aimed to investigate the impact of Ephedra sinica pharmacopuncture on the weight and lipid metabolism of obese mice.Methods: Obesity was induced in male C57BL/6 mice by a 60% fat diet. The animals were divided into three groups (n=5) fed a normal diet, high-fat diet, and high-fat diet with Ephedra sinica pharmacopuncture. After 13 wk, fasting blood sugar levels were measured in each group, and oral glucose tolerance tests were conducted. After 15 wk, body weight, epididymal fat pad weight, subcutaneous fat pad weight, and serum lipid and gene expression of hormone sensitive lipase (HSL), adipose triglyceride lipase (ATGL), monoacylglycerol lipase (MGL), perilipin, and peroxisome proliferator-activated receptor (PPAR)-γ were measured in each group.Results: In the Ephedra group, body weight, fasting blood sugar, and oral glucose tolerance were significantly decreased. In addition, in the Ephedra group, the gene expression of HSL was significantly increased, whereas that of perilipin was significantly decreased.Conclusions: These results provide evidence that E. sinicapharmacopuncture affects obesity and obesity-induced metabolic syndrome, including insulin resistance and dyslipidemia, by activating lipolysis via the HSL pathway in adipose tissue.

        Objective: To evaluate the etiology, clinical and electrodiagnostic manifestation of winged scapula in Korean soldiers. Method: Reports of conscripts with winged scapula were reviewed retrospectively on details associated with the condition onset, symptoms, signs, and electrodiagnostic study results. Results: We identified 36 patients with winged scapula. The causal events were backpack carriage in 10, closed trauma in 10, surgical trauma in 4, osteochondroma in 1, and idiopathic in 11 patients. The accessory nerves were affected in 16, the long thoracic nerves in 12, the upper brachial plexus in 5, and the dorsal scapula nerve in 3 patients. Poor outcome was rated as 14 (37.8%) and good outcome rated as 20 (54.1%) patients. Conclusion: The winged scapula was most commonly caused by nerve compression associated with backpack carriage in Korean soldiers. Careful physical examination for winged scapula should be performed in patients with shoulder problems associated with backpack carriage.

        복잡계의 행위자기반모형을 활용한 종합병원의 진료부 공간구조 분석에 관한 연구

        (연구배경 및 목적) 현재 우리나라는 사회·문화적 성장과 함께 다양한 질병과 감염 등에 노출되어 있는 실정이며, 특히, 2010년 이후부터 더욱 다양한 형태의 질병 등을 격게 되면서 종합병원 의료체계의 중요성과 필요성이 함께 대두되게 되었다. 특히 종합병원은 응급환자들과 의료진, 일반 환자들의 의료공간의 구조체계가 사회, 문화, 경제적 혼란 발생시키는 등의 피해가 발생되고 있는 실정이다. 이에 발맞춰 국내에서 운영되고 있는 종합병원의 의료 공간에서 다양한 형태의 질병과 환자의 피해를 줄이기 위하여 의료기관의 진료부 공간구조을 분석하고자 한다. (연구방법) 본 연구는 보건복지부에서 정의하는 국내의 수도권에 위치하고 있는 상급종합병원에서 하루 5,000명 이상의 외래환자가 내원하는 사례를 기준으로 분석하였으며, 병원 내 진료부의 공간구조체계가 어떠한 형태로 구성되어 있는지를 분석하고. 분석된 공간구조의 문제점을 파악하는 연구를 진행하였으며, 복잡계의 행위자기반모형을 기준으로 공간의 문제점을 심층 분석하여 해결방안을 제안하였다. 연구 조사방법으로는 선정된 사례의 병원 진료부를 직접 방문하여 담당자와 인터뷰, 문헌, 웹, 연감 조사등을 함께 비교하여 본 연구의 분석을 진행하였다. (결과)본 연구의 분석진행 결과를 살펴보면 다음과 같다. 국내에서 운영중인 종합병원의 진료부 공간구조 시스템은 기 방문 환자에게는 다양한 의료서비스의 형태로 공간을 구성하여 운영되고 있는 반면에 병원에 처음 방문한 환자에게는 복잡한 진료부 의료체계와 시스템으로 인해 많은 혼선과 공간의 명료성이 떨어지는 실정으로 분석되었다. 진료부 공간의 구조는 의료체계별 영역별로 구성되어 있으며, 결정된 종합병원의 공간규모에서 각각의 진료부서에서 필요한 공간의 규모를 조직화 하여 상호 소통관계를 가지고 있기 때문에 외래환자가 처음 병원에 방문 할 시에는 공간을 인지하는데 매우 취약한 것으로 분석되었다. 이에 보다 전문으로 환자를 대응할 수 있는 공간의 구성이 필요 된다. (결론) 본 연구에서는 우리나라에서 운영 중인 종합병원의 진료부공간에 대한 사례분석을 토대로 의료공간의 구조형태와 문제점을 분석하여 기존 내원 환자와 의료진, 처음 내원한 환자 등, 다양한 사용자들이 의료공간의 구조에 합리적으로 사용할 수 있는 공간구조의 형태를 제안하고, 복잡계의 행위자기반 모형 분석을 이용하여 보다 효과적이고 합리적인 형태의 공간 구조 모형의 기초적 데이터를 제안하는 것을 본 연구의 의의를 두고자 한다. (Background and Purpose) South Korea is exposed to various diseases and infections due to the country' social and cultural growth, increasing the necessity of the medical system. However, general hospitals in Korea have been negatively affected by social, cultural, and economic disruptions caused by the current organizational spatial structure for emergency patients, medical staff, and general patients. Thus, this study analyzed the space compositions of departments in medical institutions which reduce various types of diseases and damages of patients in the medical spaces operated in Korea.(Method) This study was analyzed based on cases where more than 5,000 outpatients visit each day at the upper general hospital located in the capital area of Korea as defined by the Ministry of Health and Welfare, and the spatial structure system of the medical department in the hospital is analyzed. A study was conducted to identify problems in the analyzed spatial structure, and a solution was proposed by analyzing the problems of space in depth based on the acting magnetic half model of the complex system. As a method of research investigation, we visited the hospital care department of the selected case in person and compared interviews, literature, web, and almanac investigation with the person in charge to conduct an analysis of this study.(Results)The analysis results of this study are as follows. The space structure system of the medical department of the general hospital operated in Korea consists of various medical services in the form of various medical services for the patients who visited the hospital. It was analyzed that the situation is not clear. The structure of medical department space is organized by the area of medical system, and when the outpatient visits to the hospital for the first time since it has a mutual communication relationship by organizing the amount of space required by each medical department in the determined size of the general hospital It was analyzed to be very vulnerable to the perception of space. This requires the construction of a space that can deal with patients more professionally. (Conclusions) The study analyzes the form and problems of medical spaces based on the case analysis of the medical department spaces of general hospitals operated in Korea. The purpose of this study is to propose a spatial composition that can be used reasonably and to propose basic data of more effective and reasonable form of spatial composition model using agent-based model analysis of a complex system.

        태양전지 묘듈용 솔드 합금의 산화 특성

        Photovoltaic(PV) cell is considered as one of the finest ways to utilize the solar power. A study of improving solar cell"s efficiency is important because the lifetime of solar cell is determined by photovoltaic module technology. Therefore, oxidation (and/or corrosion) of solder materials will be one of the primary yield and long-term reliability risk factor. Recently, the development of lead-free solder alloy has been done actively about lead-free solder alloys of the thermodynamic and mechanical properties. However, the oxidation behavior have rarely been investigated. In this study, the oxidations of 60 wt% Sn-40 wt% Pb, 62 wt% Sn–36 wt% Pb-2 wt% Ag, 50wt% Sn-48 wt% Bi-2wt% Ag alloys for the interconnect ribbon after exposure in atmosphere at 100℃ for several times were investigated. The wettability of 62 wt% Sn-36 wt% Pb-2wt% Ag and 50 wt% Sn-48 wt% Bi-2 wt% Ag solders was also studied to compare with that of 60 wt% Sn-40 wt% Pb alloy. The result showed that the zero cross time and the wetting time of 50 wt% Sn-48 wt% Bi-2 wt% Ag solder were better than other two samples. The surface of tested samples was analyzed by XPS. The XPS result showed that in all samples, SnO grew first and then the mixture of SnO and SnO₂ was detected. SnO₂ grew predominantly for the long time aging. Moreover XPS depth profile analysis has found surface enrichment of tin oxide.
