유체유동을 고려한 추체모델에서 하중형상에 따른 응답특성 분석

추체의 해면골 구조의 유체유동 분석을 위하여 섬유주 형상과 골수를 포함한 2 차원 정상 주체 모델을 개발하였다. 유체-고체 상호작용(Fluid-Structure Interaction) 조건을 적용하였고, 같은 충격량을 갖는 두 가지 다른 형상의 충격하중을 가해주었다. 유체의 압력분포와 흐름, 구조에 작용하는 응력분포의 특성을 분석하였다. To analyze the fluid flow in the trabecula bone structure of the vertebral body, two-dimensional intact vertebral body including trabecula shape and bone marrow was developed. Fluid-Structure Interaction option between structure and fluid was applied. Flow pattern and stress distribution of the vertebral body structure were analyzed for two different shapes of impulsive force having same impulse.
중증 과립구 감소증을 동반한 환자에서 Fluconazole의 진균감염 예방 효과

Fungal infection is one of the major cause of mortality in neutropenic patients. We performed clinical studies to assess the effectiviveness and toxicity of fluconazole for prophylaxis of fungal infection in 20 neutropenic patients, who were admitted to Kyung Hee University Hospital from June 1991 to June 1992 and who had a high risk of fungal infection. The results were as follows. 1) In 18 case, there was no evidence of fungal infection during the hospital course. One patient had suspicious fungal infection on chest X-ray and clinical finding, and the other case had fungemia confirmed by blood culture. Two patients died of septicemia. 2) Elevation of AST, LAT (15%) G-I trouble (10%) were obserbed as toxicities of fluconazole, but renal dysfuntion, skin rash, or seizure was not obserbed.
과립구 감소증을 동반한 발열 환자에서의 MIPENEM/CILASTATIN(Tienam) 단일요법

The need for prompt antibiotic therapy in the management of febrile neutropenic patients is well established. Imipenem is the first representitive of the new thienamycin class of antibiotics. It has the broadest antibacterial activity of all antibiotics available for systemic use in humans. We performed clinical studies to assess the efficacy and tolerability of Imipenem/Cilastatin as monotherapy in 20 febrile neutropenic patients. Clinical response rate was 80%(16/20). Microbiologically documented infections account for 55%(11/20). Microbiologic response rate was 86%(6/7) for G(+) and 75%(3/4) for G(-) infection. No serious side effects were noted in any patients. In conclusion, Imipenem/Cilastatin is a safe and effective antibiotics as monotherapy in febrile granulocytopenic patients.

Recent Diesel Engine usually uses main bearing cap to reduce mass, to improve assembly line efficiency and to secure sufficient stiffness for crank shaft support. If main bearing cap doesn’t have sufficient stiffness and durability performance, there is a potential to cause many problems such as oil leakage and block crack. In this paper, an effective way is introduced to achieve mass reduction, to reduce bearing distortion and to improve durability of main bearing cap using the design of experiments and computer simulation. Based on the results from this study, the best effective factor is material property for bearing performance. And the mass of the best condition is reduced by 20.7% when compared with the mass of base model.
PCR 및 Western Blot 으로 HTLV-1 감염이 증명된 성인 T-세포 백혈병 1 예

Adult T-cell leukemia (ATL) is an often aggressive and fatal malignancy of mature CD4+ T lymphocytes, characterized by leukemia, lymphadenopathy, tumor infiltration of the skin, hepatomegaly, hypercalcemia and lytic bone lesions. ATL is an endemic disease in southwestern Japan and the Caribbean, but occurs sporadically in most of the world. HTLV-I is recognized as the etiologic agent of adult T-cell leukemia. We report the 2nd case of ATL in Korea, which was confirmed by typical clinical findings and western blot analysis of the serum. A-44-year-old man was admitted to the Department of Internal Medicine, Kyung Hee University Hospital because of abdominal fullness, dyspnea and itching sensation. He denied any history of blood transfusion, drug abuse or travel abroad. On admission, he had multiple lymphadenopathy, hepatomegaly, erythematous nodules on anterior chest wall and both upper extremity, and rales on left lower lung fields. His peripheral blood leukocyte count was 287,000/㎜³ with 87%, atypical lymphocytes containing pleomorphic and lobulated nucleus. In the bone marow, 90% of nucleated cells were leukemic cells. The surface phenotype of these abnormal cells were CD2+, CD3+, CD4+, CD5+, CD8-. Anti-HTLV antibody was positive, PCR (polymerase chain reaction) analysis of the DNA from leukemic cells showed HTLV-I provirus integration. Western blotting analysis of patient serum confirmed HTLV-I infection. Despite of intensive supportive care, he died of respiratory failure on 21st hospital day.
