        신이식후 발생하는 감염에 대한 고찰

        N/A Objectives: Renal transplantation is a major therapeutic advance for patients with chronic renal failure. But recipients of renal transplantation are prone to infection with both common and unusual organisms. And infectious diseases remain a major cause of morbidity and mortality in renal transplant recipients. This study was to analyze the infections in renal transplant recipients; its occurrence according to sites and organisms; comparison among immunosuppressive agents; prognostic differences of urinary tract infections(UTI) between those developed during 1 month after transplantation and those not developed; graft outcome; and patients mortality. Mothods: 181 renal transplant recipients were examined. They received renal transplantion between january 1979 and December 1992 at the Kyung-Hee University Hospital. 158 of them received transplantation from living-related donors, 22 from living-unrelated donors, and 1 from cadaver donor. Their age at the time of transplantation ranged from 14 to 66 years(mean 35.6 years), and the male to female ratio was 2.3: 1. The observation period ranged from 1 to 144 months(41.26±31.71, mean±S. D.). Results: 1) 110 recipients(60.7% of total) had 232 episodes of infectious complications whereas 71(39.3% of total) had no infectious episodes. About half of infectious episodes(128 episodes, 55.2% ) occurred during 1 month after transplantation. 2) The most common site of infection was urinary tract(138 episodes, 59.2%) whth the next sites coming in this order, bacteremia(32 episodes, 13.8%), pulmonary(21 episodes, 9.1%), and skin(21 episodes, 9.1%). 3) The causative organisms of UTI ranked in this order E. coli 21.7%, staphylococcus spp 14.5%, and pseudomonas spp 13.0%. 4) The causative organisms of bacteremia ranked in this order E. coli 34.4% salmonella spp 18.8%, pseudomonas spp 12.5 %, and staphylococcus spp 12.5%. 5) There was no significant differences of infectious episodes among immunosuppressive regimens. 6) Early UTI group(UTI occurred during 1 month after transplantation) recorded significantly higher incidence of urinary tract infections after 1 month that followed than non-UTI group. But there was no differences between two groups on graft and patients outcome. 7) The major cause of death was life-threatening infections(63.2 %). Conclusion: Most infections due to various organisms may occur during 1 month after transplantation. And still they constitute a major cause of death in renal transplant recipients.

        대학 골프 선수의 Pitching wedge 스윙동작의 운동학적 특성 분석

        The purposes of present study were to determine the major check-points of golf swing from the review of previous studies, and to suggest additional information on the teaching theory of golf. The golf swing motion of 6 male and female elite university golf players were filmed with 16mm Locam II high speed cameras at the speed of 200f/s, and variables such as time, displacement, angle, velocity were calculated and analyzed by 3D Cinematography using DLT method. The results were: 1. Differences were shown in the ratio of weight distribution on the feet, cocking angle, take-back velocity, club-head velocity at impact depending upon the physical characteristics and club used for swing. 2. Time for the down-swing and impact were $0.27{\sim}0.29s$ in men and $0.29{\sim}0.32s$ in women, which was 1/3 of the time for the back-swing. Women showed longer total swing time than men because of longer time in back-swing, follow-through and finish. 3. Men showed larger range of motion in shoulder and knee joints than women, on the other hand women showed larger range of motion in hip joint than men. 4. Cocking motion and right elbow flexion were occurred at the top of back-swing and cocking release was occurred at the moment of impact. Maximum rotations of shoulder and hip joints were found between the top of back-swing and down-swing phase. 5. Women showed lower back-swing velocity than men, and men showed higher club velocity(men: $38.2{\sim}38.6m/s$, women: $35.1{\sim}36.4m/s$) than women.

        Objectives For the excellent differentiation of syndromes, we study on the individual characteristic factor by the inspection of body shape. Methods To the subject of diagnosis special books and diagnostics textbook of korean medicine, we arrange the individual characteristic factor by the four methods of diagnosis. Results The individual characteristic factor analyzed by the inspection of the body shape in the 『Huangdineijing 黃帝內經』 and 『Donguisusebowon 東醫壽世保元』 was observed stereotyped. The inspection of the body shape means to diagnose the patient by examining the individual physical conditions of the patient. The body depends on visceral essence to nourish, while the functions of the viscera and the conditions of visceral essence may be reflected by the body. The inspection of individual physical strength and weakness may enable one to know the functions of the viscera and the conditions of ki(氣) and blood. And also the inspection of local regions is used to closely examine some regional areas to obtain necessary clinical data on the individual characteristic factor. Conclusion As the results, the individual characteristic factor by the inspection of body shape is very important item of the four methods of diagnosis(四診) and the differentiation of syndromes(辨證). And therefore the process on four methods of diagnosis and differentiation of syndromesis is necessary to divide the signs of individual characteristic factor and the signs of disease.

        Zipeprol 남용환자에서 뇌 단일광자방출 전산화단층촬영술의 임상적 의의

        N/A Drug abuse is widespread in worldwide and has been associated with neurologic complication. Zipeprol is one of drugs which been abused for psychological satisfaction in some adolescents. This agent is non-opioid antitussive agent, which is not legally considered as being capable of creating dependence or abuse liability at therapeutic serum levels. But it has been reported that acute or chronic overdose create neurologic complication such as convulsion as well as dependence. Recently we experienced six zipeprol abusers who admitted due to convulsion and variable neurologic symptoms. The aim of our study was to determine the role of Tc-99m- HMPAO brain SPECT in those patients. EEG and brain CT showed no abnormal finding, but brain SPECT showed focal or multiple perfusion abnormalities in frontal, parietal, occipital cortex, basal ganglia, thalamus and especially at temporal cortex. These results suggest that brain SPECT may be a useful diagnostic tool to evaluate the cerebral dysfunction indused by zipeprol abuse.

      • 엘리트 높이뛰기 선수들의 발구름 동작에 관한 운동학적 분석

        This study was attempted to kinematical characteristics of the take-off motion in elite high jumpers for the purpose of improving jumping height. The subjects were three males who were 2002 Busan Asian Games medalist in high jump. The three dimensional motion analysis with DLT method was executed using three video cameras of analyzing the actual competition situation. In point of analyzing the actual competition situation, it is expected that high jumpers and coaches have the effective informations, and the following conclusion had resulted. The take-off time shows that this study was longer than the other former studies. This result comes from wet ground condition, because of cold day. The ratio of height that center of gravity was 52.8±4.3 % in touch-down phase and 72.3±5.3% in take-off phase. Also vertical height of increasing between touch-down and take-off was 19.5±1.5%, maximum height was 124.3±2.8%. In order to improve jumping height the vertical and medial-lateral velocity of center of gravity should be increased as possible. And the lead leg and both arms contribute to the increase of center of gravity's velocity. It is appeared that the knee joint angle the lead leg at touch-down phase 162.2±8.89 degree, at take-off phase was 167.7±8.38degree, the angle of maximum flextion was 174.0±3.58 degree. The take-off leg were 117.4±10.6, 66.9±8.0, 146.8±13.1degree respectively.

        철봉 어깨 틀어 휘돌아 다시 잡기(el-grip swing with one turn to el-grip) 동작의 운동학적 분석

This study was attempted to kinematical characteristics of the El-grip swing with 1turn to el-grip in elite horizontal bar for the purpose of improving performance. The subjects were three males who were 2002 Busan Asian Games in men's team. The three dimensional motion analysis with DLT method was executed using three video cameras of analyzing the actual competition situation. In point of analyzing the actual competition situation, it is expected that gymnastics and coaches have the effective informations, and the following conclusion had resulted. 1. In case of release, It is impotant to make fast horizontal velocity of CM, high vertical position of CM, large hip and shoulder angle. Also It should be performed release motion of trunk rotation angle(+). 2. During LHR the action should be made at higher position than the CM and the shoulder joint is moving within 127±16.82. It is important to make large trunk rotation angle. 3. During Hop, the RHR motion should be done in high position with short time and fast twisting action and to reduce the vertical speed is important.
