        급성기 편마비 뇌졸중 환자에서 재활 치료에 따른 호흡 기능의 변화

        Objective: To investigate the change of respiratory function and relationship between pulmonary function and functional improvement following rehabilitation in acute hemiplegic stroke patients. Method: Thirteen stroke patients were enrolled in this study. To evaluate hemi-diaphragmatic paralysis, we checked chest radiographs in the inspiration and expiration position of all patients. When diaphragmatic paralysis was suspected, fluoroscopy of diaphragm was conducted. To evaluate respiratory function, pulmonary function test and peak cough flow (PCF) were performed before and 3 weeks after conventional rehabilitation period. To evaluate correlation of functional performance, motor and pulmonary function, Motricity index and modified Barthel index were checked also before and 3 weeks after the period. Results: Only one of 13 acute stroke patients was diagnosed as diaphragmatic paralysis. At initial evaluation of pulmonary function test, restrictive pattern was observed in 5 of 13 patients and female and patients with restrictive pattern generally showed lower pulmonary function scores. After rehabilitation, forced inspiratory vital capacity (FIVC), peak expiratory flow rate (PEF), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP) and PCF were significantly improved. Improvements of forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1), MIP and FIVC were correlated with those of MBI. Conclusion: Decline in respiratory function could be occurred in acute hemiplegic stroke patients. Also, respiratory function could be improved with conventional rehabilitation and may be correlated with functional improvement. Therefore, the evaluation of respiratory function and active rehabilitation therapy should be conducted in acute stage to prevent pulmonary complications and promote functional recovery.         파킨슨병 환자에서 Tetrax를 이용한 균형 능력의 평가

        Objective: To investigate the postural stability of patients with mild to moderate stage of idiopathic parkinson’s disease (IPD) using Tetra-ataxiometric posturography (Tetrax) and to evaluate the correlation between balance and activity of daily living (ADL). Method: Twenty patients with IPD at mild to moderate stage, twenty young controls and twenty elderly controls were enrolled in this study. In order to evaluate postural stability, we checked stability index, fourier index, weight distribution index, synchronization index, and fall index at eight positions using Tetrax. We also assessed Berg Balance Scale, Postural Instability and Gait Difficulty (PIGD) score of Unified Parkinson Disease Rating Scale (UPDRS). In order to evaluate the correlation between balance and functional performance, the ADL score of UPDRS was checked in the patients with IPD. Results: The patients with IPD showed a significantly higher fall index, weight distribution indices in five positions than the elderly controls. The stability indices of the patients with IPD were higher in all positions, but significantly higher in only one position than the elderly controls. The fallers in the IPD patients had a higher fall index and lower synchronization indices than the non-fallers. The balance and functional performance showed significant correlations in the IPD patients. Conclusion: We could identify a high falling risk and uneven weight distribution in the IPD patients and a close correlation between balance and functional performance. Therefore the evaluation of postural stability should be considered from mild stage because postural instability might impair activity of daily living and increase fall risk.         텔레매틱스 서비스를 위한 많은 어플리케이션들이 개발 됨에 인해 테스트를 위한 도로 네트워크 기반의 이동객체 궤적데이터의 필요성이 증대되고 있다. 본 논문에서는 도로 네트워크 상의이동객체들의 실 궤적 데이터와 유사한 합성 궤적 데이터를 구축하기 위한 방법론을 제안한다. 그리고 실제 구현 결과와 실 데이터와의 속도 패턴을 비교하여 실 데이터와의 유사성을 보인다.

      • 과학과의 진로선택과목 선택기준과 수업 만족도 요인 분석

        과학과의 진로선택과목에는 대학수학능력시험을 실시하지 않는 과목들이 있다. 이 과목들은 개별 학교에서 학생들의 선택에 의해 개설이 결정된다. 그래서 학생들이 생각하는 진로선택과목 선택기준과 수업 만족도 요인을 아는 것은 해당 과목의 개설 가능성에 도움이 된다. 이번 연구는 문헌조사를 통해 설문지 개발에 필요한 과목 선택기준과 수업 만족도 요인을 도출하였고, 경남 G시에 소재한 S고등학교 2학년 102명을 대상으로 설문조사를 실시하였다. 본 연구의 연구 결과는 다음과 같았다. 첫째, 과학과의 진로선택과목 선택에는 타인의 권유와 같은 외부기준보다는 자신의 선호에 의한 다양한 내부기준이 더 많은 영향을 미쳤다. 둘째, 대학수학능력시험을 실시하지 않는 진로선택과목의 수업 만족도를 높이기 위해서는 '흥미', '진로와 연관성'과 같은 학생 요인을 높여야 한다. 이상의 결과로부터, 대학수학능력시험을 실시하지 않는 과학과의 진로선택과목은 교사가 학생들의 흥미, 진로와 관련된 내용으로 수업을 진행해야 학생의 많은 선택을 받을 수 있음을 알 수 있었다. 앞으로 학생들이 적성과 진로에 맞는 진로선택과목을 선택할 수 있게 개별 과목에 대한 흥미, 진로 연관성 정보제공 등의 적극적인 지원이 필요하며, 이를 통해 만족스러운 교육과정을 이수할 수 있게 해야 한다. Some subjects do not take the Korean Scholastic Ability Test(KSAT) for career elective subjects in the science curriculum. In individual schools, the student's choice decides these courses. Therefore, knowing the criteria for choosing a career elective subject and the factors of satisfaction with the class that students think about the possibility of opening the subject is helpful. In this study, the researchers derived the subject selection criteria and class satisfaction factors required for questionnaire development through literature research. They conducted a questionnaire survey on 102-second graders of S high school in G-si, Gyeongsangnam-do. The study results of this study were as follows. First, various internal criteria based on one's preference rather than external criteria such as the recommendation of others significantly influenced the selection of career elective subjects in the science curriculum. Second, student factors such as 'interest' and 'relevance to career path' should be increased to improve class satisfaction in the career elective subjects that do not take the KSAT. From the above results, the researchers found that, for career elective subjects in the science curriculum that do not take the KSAT, teachers can receive many choices from students only if they teach content related to their interests and career paths. In the future, active support such as providing information about interest in individual subjects and career-related information is needed so that students can choose career elective subjects that fit their interests and career paths. Through this, they should be able to complete a good curriculum.

        Effects of Adjuvant Hydrotherapy on Functional Status and Mental Relaxation in Patients with Knee Osteoarthritis: Preliminary Study

        Background: The aim of this study is to test if adjuvant hydrotherapy with viscosupplement is beneficial on management of pain, stiffness, function, and mental relaxation in knee osteoarthritis (OA). Methods: Nineteen patients with OA were randomly assigned to hydrotherapy or control groups. All patients received viscosupplement injections once a week for 3 weeks. Hydrotherapy group (n=9) had a spa containing green tea, 3 days a week for 2 weeks. The control group (n=10) received only injections. All patients were assessed at baseline and after the third injection. All were assessed for pain (visual analog scale, VAS), pain severity, and functional status (Western Ontario and McMaster Universities osteoarthritis index, WOMAC), emotional status, quality of life (Euro quality of life health-related quality of life inventory five dimension, EQ-5D), and relative spectral power of alpha waves in electroencephalography (EEG). Results: Both groups reported a statistically significant reduction of pain at the end of treatment and detailed assessment of pain, and function in WOMAC and the relative power of alpha in electroencephalogram showed statistical significant difference only in the hydrotherapy group. However, there were no significant intergroup differences, except for VAS score. Conclusion: Adjuvant 2-week hydrotherapy to viscosupplement might have a modest role in the management of pain and functional disability and the mental relaxation in patients with knee OA. Larger, randomized controlled trials with intervention of long term period to determine efficacy in treating knee OA are warranted.

        Effects of Early Combined Eccentric-Concentric Versus Concentric Resistance Training Following Total Knee Arthroplasty

        Objective To investigate the effects of early combined eccentric-concentric (ECC-CON) or concentric (CON) resistance training following total knee arthroplasty (TKA).Methods Patients who underwent a primary TKA were randomly assigned to an ECC-CON group (n=16) or a CON group (n=18). All patients received early, progressive resistance training with five sessions per week for 2 weeks starting 2 weeks after TKA. Isometric knee flexor and extensor strength of the surgical and non-surgical knees, instrumental gait analysis for spatiotemporal parameters, 6-Minute Walk Test (6MWT), Timed Up and Go Test (TUG), Timed Stair Climbing Test (SCT) were used to evaluate performance-based physical function. The Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) and EuroQOL five dimensions (EQ-5D) questionnaire were used to evaluate self-reported physical function and self-reported quality of life. All patients underwent these evaluations before and 1 month after TKA.Results The ECC-CON group showed clinically meaningful improvements in extensor peak torque (PT) of the non-surgical knee, gait speed, and 6MWT from preoperative values. The CON group had an increase in H/Q ratio of the surgical knee and improvement in SCT-ascent postoperatively. Both groups showed significant improvements in WOMAC-Pain, function, and EQ-5D scores. Although extensor PT of the surgical knee did not reach the preoperative level in either group, the postoperative change was significantly less in the ECC-CON group than the CON group.Conclusion Early combined ECC-CON resistance training minimizes the loss in quadriceps strength of the surgical knee and improves endurance and gait speed after TKA.

        The Effect of Dual-Task Training on Balance and Cognition in Patients With Subacute Post-Stroke

        Objective To investigate the effect of dual-task training on the recovery of balance ability and cognitive function in patients with subacute stroke.Methods Twenty patients (12 males and eight females; average age, 59.70 years) with subacute stroke were enrolled in this study. All participants were randomly assigned to one of two groups, the dual-task group (n=10) or the control group (n=10). The dual task was simultaneous balance and cognitive training using the BioRescue. All patients were evaluated with posturographic parameters and the Berg Balance Scale for balance ability, a computerized neuropsychological test and the Korean version of the Mini-Mental State Examination for cognitive function, the Fugl-Meyer Assessment for motor function, and the Korean-Modified Barthel Index for activities of daily living (ADL) function before and after 4 weeks of rehabilitation.Results The dual-task group showed significant improvements in the pressure of the weight distribution index (WDI), surface area, and length of the stability index during the eyes-open condition; surface area of the limit of stability (LOS) on the hemiparetic and intact sides, and the auditory continuous performance test and backward visual span test after rehabilitation. Although no significant difference was observed for the changes in balance ability or cognitive, motor, and ADL functions between the groups, changes in the WDI pressure during the eyes-open condition and in the area ratio of LOS (hemiparetic/intact) showed a tendency to improve in the dual-task group.Conclusion Our findings suggest that dual-task training could be as effective as conventional balance training for improving balance and cognition in subacute post-stroke patients.

        The anterior interosseous nerve syndrome is a rare focal neuropathy with typical clinical and electromyographic features. A number of etiologies have been implicated in this disorder and most commonly reported etiologies include spontaneous (idiopathic), fracture of the forearm and arm, trauma, or prolonged pressure on the forearm. However, we experienced one case of anterior interoeesous nerve syndrome in the postpartum period, which was not related to trauma. To the best of our knowledge, there are a few case reports in Korea.
