        Effect of the High Frequency Chest Wall Oscillation (HFCWO) on Pulmonary Function and Walking Ability in Stroke Patients

        Purpose: This investigation aimed to determine the effects of treadmill training (TT) and high frequency chest wall oscillation (HFCWO) on pulmonary function and walking ability in stroke patients as well as propose an exercise program to improve cardiovascular function. Methods: Twenty hemiplegic stroke patients were randomized to either the control group (CG) (n=10) or the experimental group (EG), which received TT and HFCWO (n=10). Pulmonary function was quantitated using patient forced vital capacity (FVC) and forced expiratory volume at one second (FEV1) while walking speed was assessed by the 10m walking test (10MWT). Further, walking endurance was determined utilizing the 6-minute walk test (6MWT). Subjects of the EG performed the study protocol for 60 minutes, five times a week for six weeks; CG patients did not participate in regular exercise. To determine significance for the differences observed before and after exercise, within-group and between-group comparisons were conducted utilizing paired and independent t-tests, respectively, with the level of significance set at α=0.05. Results: Within-groups, significant differences were observed in both FVC and FEV1 (p<0.01) following completion of the study protocol. Further, between-group comparisons demonstrated significant differences in both FVC (p<0.05) and FEV1 (p<0.01). Post-exercise, significant changes in the 10MWT and 6MWT score were observed between the EG and CG (p<0.01). Further, statistically significant differences were observed in 6MWT scores between-groups (p<0.05). Conclusion: The TT and HFCWO effectively improved pulmonary function and walking ability in subjects with stroke. The proposed program can be applied to stroke patients as a useful therapy.

        Effects of 4-week PNF Exercise Program on Activity, Posture, and Muscle Strength in a Patient with Abdominalis Weakness after Cesarean Section: A Single-Subject Study

        Purpose: The purpose of this study was to determine the effect of proprioceptive neuromuscular facilitation (PNF) abdominal muscle strengthening exercise on the activity, posture, and body function of clients with abdominal muscle weakness after cesarean section. Methods: PNF abdominal muscle strengthening exercise was applied to one patient with abdominal muscle weakness after cesarean sec- tion. PNF abdominal muscle strengthening exercise was applied five times a week for four weeks. The 5-time supine-to-long sitting test (5-TSLST) and supine-to-stand test (SST) were used to measure activity, and the pelvis tilt angle test (PTAT) and anterior head translation test (AHTT) were used to measure posture. Body function was assessed using the transverse abdominis muscle strength test (TAMST), the internal oblique and external oblique muscle strength test (IOEOMST), and the rectus abdominis muscle strength test (RAMST). Results: Activity, posture, and body function were improved post-intervention. Conclusion: The study verified that PNF abdominal muscle strengthening exercises are effective when applied to patients with abdomi- nal muscle weakness after cesarean section. The findings of this study provide useful data for future interventions in patients with ab- dominal muscle weakness after cesarean section.

        시내버스 운행여건과 서비스 수준에 관한 연구 - 울산광역시 사례를 중심으로 -

        본 연구는 시내버스 서비스 수준을 의미하는 노선별 차량 운행대수의 영향을 미치는 운행여건과 상관성 분석에 관한 연구이다. 분석방법은 회귀분석방법을 적용하였으며, 분석대상은 울산시 2013년 기준 98개 노선으로 한정하였다. 상관성분석을 위한 모형 도출을 위해 먼저 상관분석을 실시하여 종속변수인 운행대수와 독립변수인 운행거리, 운송수입금, 운송원가간의 관계를 분석한 결과, 아주 높은 유의수분에서 상관성이 있는 것으로 확인되었다. 회귀분석은 모형을 3가지 유형으로 구분하여 실시하였으며, 그 결과는 다음과 같다. 첫 번째 울산시 98개 버스노선 전체를 대상으로 도출한 모형은 Y(운행대수)=$-4.532+0.00002877*X_1$(운송수입금)인데, 이 의미는 운행대수 1대 증차를 위한 필요조건은 1일 승객 수가 140명 정도 증가하여야 시행할 수 있다는 점을 알려준다. 두 번째는 일반형, 좌석형, 중형으로 구분되는 버스 운행형태별 모형은 일반형의 경우 Y(운행대수)=$-10.954+0.00004283*X_1$(운송수입금), 일반형 시내버스를 1대 증차하기 위해서는 1일 승객 수가 약 153명 정도 증가되어야 시행할 수 있음을 보여주며, 중형의 모형은 Y(운행대수)=$-0.859+0.00001438*X_1$(운송수입금)인데, 이 의미는 1일 승객 수가 약 52명 정도 증가되어야 시행 가능함을 보여준다. 세 번째는 버스노선 운행체계별 모형 분석의 경우, 공동배차제의 경우 Y(운송대수)=$-4.786+0.00003028*X_1$(운송수입금)인데, 이 의미는 운행대수 1대 증차를 위한 필요조건은 1일 승객 수가 약 140명 정도 증가해야만 시행할 수 있으며, 개별노선제의 경우 Y(운행대수)=$-2.339+0.00002030*X_1$(운송수입금)인데, 이 의미는 운행대수 1대 증차를 위한 필요조건은 1일 승객수가 약 140명 정도 증가되어야 시행할 수 있음을 보여준다. 따라서 버스노선 운영체계별로 모형을 도출하여 운영체계가 시내버스 노선별 차량 운행대수에 미치는 영향을 분석한 결과, 두 가지 운영체계 모두 차량 증차를 위한 추진할 경우, 1일 승객 수가 140명 정도는 확보되어야 한다는 공통적인 결론에 도달되었으며, 노선운영체계 특성에 따른 노선별 차량 운영대수 차이는 거의 없는 것으로 분석되었다. This study made an attempt to analyse the relationship between operation environment and fleet size per route which represents the level of service for inner-city bus service. Regression analysis method has been adopted as main analysis tool and 98 routes of bus operation status in Ulsan city as of 2013 has also been selected for analysis target. Correlation analysis was performed to identify the relationship between dependent and independent variables. There are three types of model for whole sample, type operation, and bus route operation system. These are the results of the current study. 1. The model developed for whole sample of 98 routes is as follows. Y(Fleet Size)=$-4.532+0.00002877*X_1$(Revenue). This model shows that it is necessary to have more than 140 passengers per day to increase fleet size of each bus route in Ulsan. 2. Models developed by type of operation (which are standard, express, and middle sized) are shown below. Stand Bus : Y(Fleet Size)=$-10.954+0.00004283*X_1$(Revenue). It is identified that more than 153 passengers need to use standard bus to increase fleet size per each standard bus, Middle Sized Bus : Y(Fleet Size)=-0.859+0.00001438*X1(Revenue). For middle sized bus, at least 52 daily passengers are needed to increase number of bus in each route. 3. Models developed for each route operation systems are as belows. Joint Operation Group : Y(Fleet Size)=$-4.786+0.00003028*X_1$(Revenue). Individual Operation Group : Y(Fleet Size)=$-2.339+0.00002030*X_1$(Revenue). These model provide similar result which 140 people is the minimum number of passenger to raise the number of vehicles in each route. This result shows that the route operation systems does not affect the raise number of cars significantly.

        Effects of Respiratory Muscle Strengthening Exercise on Respiratory Function and Activities of Daily Living in Stroke Patients

        Purpose: This study aimed to demonstrate reduction in stroke symptoms by analyzing the changes in respiratory function and activities of daily living (ADL) after respiratory muscle strengthening exercise in patients who had a stroke and thereby, propose an efficient exercise method. Methods: Twenty patients with hemiplegic stroke were divided into two groups, with 10 patients in each. The control group (CG) received the traditional exercise therapy, and the experimental group (EG) received the traditional exercise therapy combined with expiratory muscle strengthening training. The training continued for 6 weeks, 5 days a week. Forced vital capacity (FVC) and forced expiratory volume at 1 second (FEV1) were measured with a spirometer, SpO2 was measured with a pulse oximeter, and ADL were assessed by using the modified Barthel index (MBI). A paired t test was applied to compare the differences before and after the intervention, and an independent t test was used to compare the differences between the groups. The level of statistical significance was set as α=0.05. Results: The changes in the FVC and FEV1 values within the group showed significant differences only in the EG (p<0.01). The betweengroup difference was statistically significant only for FVC and FEV1 in the EG (p<0.01). The changes in SpO2 and MBI within the group showed significant differences only in the EG and CG (p<0.01). Between-group differences were statistically significant only for SpO2 and MBI in the EG (p<0.05). Conclusion: The interventions with active patient involvement and combined breathing exercises had a positive impact on all the functions investigated in this study.

        앞쪽머리자세가 있는 만성 목통증 환자에게 PNF를 이용한 목안정화운동 적용이 자세와 기능 장애에 미치는 효과

        The study aimed to determine the effect of the proprioceptive neuromuscular facilitation (PNF) cervical stabilization exercise on pain, cranio vertebral angle (CVA), and neck disability index (NDI) in chronic neck pain patients with forward head posture. Following baseline measurements, 26 subjects (n=26) with chronic neck pain were randomized into two groups: the experimental group (n=13), which received PNF cervical stabilization exercise, and the control group (n=13), which received traditional physical therapy. Each group participated in the intervention for three times per week, for six weeks. The visual analogue scales (VAS) for pain, CVA, and NDI Korean version (KNDI) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention period compared with pre-intervention, and independent t-tests were used to analyze differences in the dependent variables between the two groups. After the six-week intervention, both groups experienced significantly decreased VAS and KNDI (p<.05) and significantly increased CVA (p<.05). The experimental group that received the PNF cervical stabilization exercise showed greater improvements in pain, CVA, and KNDI than those of the control group (p<.05). These results suggest that the PNF cervical stabilization exercise reduces neck pain and functional disability and enhances CVA in chronic neck pain patients with forward head posture.

        Effect of Stabilizing Reversal Technique of Proprioceptive Neuromuscular Facilitation and Taping Convergence on Wrist Pain and Grip Strength

        This study investigated the effect of wrist taping (WT) after application of stabilizing reversal technique (SRT) of proprioceptive neuromuscular facilitation (PNF) on pain and grip strength (GS) in patients with wrist pain (WP). Twenty patients with WP were randomly assigned to an experimental group (n=10) that received WT after application of SRT, and a control group (n=10) that received WT after application of stretching. The total intervention time consisted of a maximum of 10 minutes including breaks, and was performed 5 times a week for 2 weeks. Pain reduction was measured using a visual analogue scale. GS was measured using a dynamometer. Within group changes in pain and GS were significant in both experimental and control groups (p<0.01). Between group changes in pain and GS were greater in the experimental group than in the control group (p<0.01). This findings indicate that SRT of PNF and WT convergence can be an effective intervention for patients with WP. Continued development of convergence interventions for patients with WP various conditions in practice, is suggested.

        Jeonbuk Tri-Pull Taping과 고유수용성신경근촉진법 운동이 뇌졸중 환자의 어깨관절 가동범위, 통증, 아탈구, 팔 기능 및 일상생활수행능력에 미치는 영향 -사례연구

        Purpose: This study aims to determine the effect of Jeonbuk tri-pull taping and proprioceptive neuromuscular facilitation (PNF) exercise on the shoulder’s active range of motion, pain, subluxation, upper extremity function, and activities of daily living in patients with stroke. Methods: In this study, Jeonbuk tri-pull taping and PNF exercise were applied to three patients with stroke and subluxation. The tape was removed and new tape applied for two days every Monday, Wednesday, and Friday over six consecutive weeks. PNF exercise was applied five times a week for six weeks. To measure the range of motion, a smart phone clinometer application was used, and the degree of pain was measured using a visual analogue scale (VAS). A jig measuring method was employed to measure the distance of subluxation. The Fugl-Meyer Assessment (FMA) was used to evaluate arm function, and the modified Barthel Index (MBI) was employed to evaluate the activities of daily living. Results: The shoulder’s active range of motion was improved in the patients compared to the range of pre-tests, and the pain and subluxation distance were reduced compared to those of pre-tests. Arm function and activities of daily living were increased compared to those of pre-tests. Conclusion: The study results verified that Jeonbuk tri-pull taping and PNF exercise are useful when applied to patients with subluxation and stroke.

        하네스를 착용한 상하지 협응 패턴운동이 만성 뇌졸중 환자의 보행능력과 균형능력에 미치는 영향

        Purpose: The current study seeks to examine the effect of coordinative pattern exercises of the upper and lower extremities using harnesses and walking rails on the walking and balance abilities of chronic stroke patients, and to develop effective programs and training methods to improve the functions of such patients. Methods: Subjects included 16 patients with hemiplegia caused by stroke. The subjects were randomly divided into an experimental group (n=8), on which coordinative pattern exercises of the upper and lower extremities were conducted, and a comparison group (n=8) that received typical exercise therapy. The experimental group underwent 30 minutes of typical exercise therapy and 30 minutes of coordinative pattern exercises of the upper and lower extremities, while the comparison group underwent typical exercise therapy for 30 minutes twice a day, five days per week for a six-week period. To evaluate walking ability, 10-m walking tests (10MWT) and 6-m walking tests (6MWT) were conducted. To assess balance ability, timed up and go tests (TUG) were performed. Results: After the intervention, significant (p<0.05) differences were seen in the 10MWT, 6MWT, and TUG in both the experimental and comparison groups. As for the 10MWT, the experimental group showed more significant improvement than the comparison group (p<0.05). In terms of the 6MWT, no significant differences were found between the groups, while the experimental group showed more significant differences than the comparison group in the TUG (p<0.05). Conclusion: The results from the current research indicate that training programs that apply coordinative pattern exercises of the upper and lower extremities with harnesses are extremely effective for improving the walking and balance abilities of chronic stroke patients.

        PNF를 이용한 작은가슴근 신장과 아래등세모근 강화 운동이 둥근 어깨 자세를 가진 돌림근띠 복원술 환자의 자세변화와 어깨 관절가동범위 및 통증에 미치는 영향

        Purpose: This study aims to determine the effect of proprioceptive neuromuscular facilitation (PNF) pectoralis minor stretching and lower trapezius strengthening exercise on posture change, shoulder range of motion (ROM), and pain in rotator cuff repair patients with a rounded shoulder posture (RSP). Methods: Following baseline measurements, the subjects (n = 25) with rotator cuff repair were randomized into two groups: the PNF group (n = 13), which engaged in PNF pectoralis minor stretching and lower trapezius strengthening exercise, and the control group (n = 12), which received traditional physical therapy. Each group participated in the intervention five times per weekfor two weeks. The RSP values for the postural changes, flexion and abduction ROM, and visual analogue scales (VASs) of both groups were recorded at both pre- and post-intervention. In addition, before and after the intervention, a paired t-test was conducted to compare the within-group changes and an independent t-test to compare the between-group differences. Results: After the two-week intervention, both groups experienced significantly decreased RSP and VAS values (p < 0.05) and a significantly increased ROM (p < 0.05). Meanwhile, the PNF group showed greater improvements in RSP, ROM, and VAS than the control group (p < 0.05). Conclusion: These results suggest that PNF pectoralis minor stretching and lower trapezius strengthening exercise reduce posture and shoulder pain and enhance ROM in rotator cuff repair patients with RSP.

        고유수용성신경근촉진법을 사용한 몸통 강화 및 안정화 운동이 기저핵 뇌졸중 환자의 균형과 보행 능력에 미치는 영향

        This study aimed to determine the effects of trunk strengthening and stabilization exercise using proprioceptive neuromuscular facilitation (PNF) on the balance and walking ability in patients with basal ganglia stroke. Five patients with basal ganglia stroke volunteered for this study. PNF trunk strengthening and stabilization exercise was performed for average 30 minutes per day, five times a week for four weeks (a total of 20 sessions). Outcome measures included one-leg standing test (OLST), Berg balance scale (BBS), and tandem walking test (TWT) for balance function, and 10 meters walk test (10WMT) for walking function. Significant improvements were found for the OLST, BBS, TWT, and 10WMT scores after the intervention (p<.05). These results suggest that trunk strengthening and stabilization exercise using the PNF may be favorably used for patients with basal ganglia stroke.
