양성돌발성체위성현훈을 동반한 돌발성감각신경성난청의 임상적 특성

Objectives: Idiopathic unilateral sudden sensorineural hearing loss (SSNHL) with simultaneous benign paroxysmal vertigo (BPPV) is known to be associated with poor hearing recovery. We aimed to investigate clinical findings in patients with SSNHL with BPPV and analyze prognostic factors including presence of BPPV related to hearing outcome.Methods: We retrospectively reviewed the electronic medical data of 14 patients with concurrent SSNHL and BPPV (combined group). We selected 52 patients without BPPV as a control group who have matched initial threshold of pure tone audiometry and age of each patient in the combined group. We evaluated clinical characteristics of all participants and compared hearing outcomes between the 2 groups. A multivariate logistic regression analysis was performed to investi-gate the factors related to hearing recovery.Results: Initial mean pure tone audiometry (PTA) threshold of combined group was 90.36±26.2 dB. Posterior canal was most commonly involved (n=7, 50%), and 8 (57%) patients showed abnormal video head impulse test results. There was no significant difference between hearing recovery rates of combined and control group (p=0.237) and mean pure tone audiometry threshold changes were not significantly different between the 2 groups (p=0.942). Old age (≥60 years), high initial PTA threshold (>90 dB), and obesity (body mass index ≥ 25 kg/m2) were poor prognostic predictors. Conclusions: There was no difference between hearing outcomes of combined group and profound SSNHL only group. BPPV was not a significant prognostic factor of SSNHL patients.
Bacteroides fragilis Roid8의 $\alpha-fucosidase$ 조효소 특성

Bacteroides fragiles Roid8은 인간의 장내에서 가장 우세한 균종에 속하는 것으로서 효소활성을 측정해본 결과 다른 균들에 비해 $\alpha-fucosidase$의 활성이 높은 것으로 나타났다. 본 연구실에서 조사된 $\alpha-fucosidase$의 생산 최적배지 조성은 1L 배지당 BHI 37g, hemin 10mg, cysteine 0.5g, resazurin 1mg, vitamin K 1mg, starch 5g을 함유하였다. 또한 DEAE-sepharose와 CM-cellulose 크로마토그래피를 거쳐 조효소액을 분리하여 조사한 결과 효소의 최적 반응온도는 $40^{\circ}C$, 최적반응 pH는 7.0이었다. 무기 금속이온의 영향은 $Co^{++}\;and\;Zn^{++}$ 존재 하에서 효소역가가 낮아졌다. Bacteroides is the most predominant bacteria in the human large intestinal tract. Bacteroides fragilis RoidB isolated from a Korean faecal sample showed high $\alpha-fucosidase$ activity compared to other bacterial species. The optimized meidum for the production of $\alpha-fucosidase$ from 8. fragilis contained BHI 37g, hemin 10mg, cysteine 0.5g, resazurin 1mg, vitamin K1mg, and starch 5g per 1 L. The crude $\alpha-fucosidase$ obtained through DEAE-sepharose and CM- cellulose chromatography showed optimum temperature and pH at $40^{\circ}C$ and 7.0, respectively. Among several metals, $Co^{++}\;and\;Zn^{++}$ showed strong inhibition on enzyme activity.
연령에 따른 설소대 절제술 후 혀 길이와 운동성 및 자음정확도의 변화에 대한 연구

Background and Objectives Ankyloglossia often results in articulation disorders, whichdepends on age, articulation ability, and mobility of the tongue. A primary treatment strategyincludes surgery and/or speech therapy. This study aimed to evaluate the changes of thetongue length, tongue mobility, and percentage of consonants correct (PCC) after frenotomy. We investigated the optimal surgery timing for ankyloglossia. Subjects and Method A total of 136 patients underwent frenotomy for the tongue tie betweenJanuary 2016 and December 2019. There were 90 males and 46 females, with the meanage of 5.23, which ranged from 2 to 10 years. The length of the tongue was measured for all patientsduring the operation preoperatively and postoperatively. The mobility of the tongue andPCC were performed preoperatively and 1 month after surgery by two speech therapists. Patientswere divided according to age into three groups: 2-3 years, 4-5 years, and over 6 years old. Results The mean tongue length of all age groups was significantly elongated after surgery(p<0.05). After frenotomy, the mobility of tongue, including horizontal protrusion, protrusionwith upward pointing, circumlocution, and lateral movement, was improved. Also, PCCs ofall age groups were significantly improved after surgery (p<0.05). Conclusion Frenotomy can be an effective procedure for children with ankyloglossia in allage groups. After frenotomy, length of the tongue, mobility of the tongue and PCC were significantlyimproved.
Inhibitory Effects of Genistein and Water Extracts of Various Herbs on Histamine Release

Water extracts of eighteen herbal materials and genistein were used to investigate their effects on compound 48/80-induced histamine release in rat peritoneal mast cells (RPMCs). The inhibition effects of histamine release by the water extracts of Ulmus pumila Linne (UP), Lonicera japonica Thunberg (LJT), Angelica gigas Nakai (AGN), Cordyceps militaris (CM), Rehmannia glutinosa Liboschitz (RGL), Liriope platyphylla Wang et Tang (LPWT), Zizyphus jujuba var. inermis Rehder (ZJ) were evaluated by a dose-response assay. LPWT, ZJ, and genistein showed dose-dependent decrease in histamine release at the concentrations of 1 to 1,000 mg/mL, and the inhibition percentage reached up to 31.0, 35.9, and 46.2%, respectively. UP, LJT, AGN, CM, and RGL significantly decreased histamine release at 1 to 100 μg/mL. Among the various combinations, mixture of UP and genistein showed an enhanced inhibition on histamine release.
프락토올리고당을 함유한 야콘 추출물의 비피더스균에 대한 잠재적인 프리바이오틱 효과

본 연구에서는 야콘 덩이뿌리 추출물이 프로바이오틱스인 비피더스균과 락토바실러스균의 생장에 미치는 영향을 구명하고자 하였다. 야콘의 당함량을 HPLC로 분석한 결과, 생체 중 100 g 당프락토올리고당 함량이 7.60 g으로 가장 많았으며, 유리당은 수크로스가 0.72 g, 프럭토스 0.34 g, 글루코스 0.26 g 순의 함량을 보였다. 야콘 추출물이 장내 유산균 활성에 미치는 영향을 구명하기 위해 락토바실러스 3종(L. acidophilus AD031, L. plantarum BH02 and L. fermentum BH03)과 비피더스 3종(B. bifidum BGN4, B. longum BORI, B. lactis AD011) 등 6종을 대상으로 대조군과 야콘처리군 배지에서 유산균의 생장속도를 비교하였다. 락토바실러스 3종의 균주들은 대조군과 야콘처리군에서 대수기에 도달하는 시간이 비슷하였고, 생균수에서도 비슷하거나 감소하는 경향을 보였다. 하지만, 비피더스 3종의 경우 야콘처리군에서 모두 생장이 증가되었고, 특히, B. longum BORI 균주는 배양10시간 이후부터 대조군과의 확연한 차이를 나타냈다. 이와 같은 결과로, 야콘 추출물은 장내 유익균 중에서도 비피더스균에 특이적인 증식 효과를 보이며, 야콘의 프락토올리고당이 비피더스균 증식에 적합한 영양원임을 확인하였다. This study was conducted to investigate the potential prebiotic activity of an extract from yacon tuberous root on the growth of probiotic strains Bifidobacterium and Lactobacillus. Results showed that the amount of fructooligosaccharides per 100 g fresh root was the highest (7.60 g), followed by sucrose (0.72 g), fructose (0.34 g), and glucose (0.26 g). The in vitro culture test of intestinal beneficial bacteria, including Bifidobacterium longum BORI, B. bifidum BGN4, and B. lactis AD011 showed effective growth on the MRS-Yacon medium containing yacon extract, whereas the growth of Lactobacillus acidophilus AD031, L. plantarum BH02, and L. fermentum BH03 did not differ from that of the control groups. In particular, B. longum BORI showed better growth than the control group after 10 h of incubation. These results indicate that yacon can be a natural prebiotic source of fructooligosaccharides, which can exert a prebiotic effect on intestinal microflora by selectively enriching Bifidobacterium.
