        원추척수에 발생한 유피낭포 1예

        Dermoid cysts are very infrequent benign tumors of the spinal canal. They arise 1mm epithelial cell rests that become sequestered during closure of the neural tube between the third and fifth fetal weeks. Consequently, they occupy midline or near midline position characteristically. If the cleavage of the cutaneous and neural ectoderm persists locally anywhere along the neuraxis, a dermal sinus is formed connecting the skin with neuraxis. We present a case of dermoid cyst at the conus medullaris which was confirmed on histologic examination. An eleven years old female patient was admitted to our department because of urinary incontinence and sensory disturbance at the saddle area. Motor weakness was absent. In the plain spine A-P view, widening of the interpedicular distance and flattening and erosion of the pedicle were noted at the L1 and L2 vertebra. The lumbar myelogram A-P view shows almost total obstruction with meniscus side at the level of the L1 and L₂ interspace. In the myelogram lateral view, the dye column was indented in a round fashion from the dorsal side at the L1 and L2 interspace. Under the impression of the intradural extramedullary mass, we performed total laminectomy and removal of the mass.

        토끼에서 자가대퇴근막을 이용한 뇌경막 성형술에 관한 조직학적 연구

        Defects in the dura mater can be result from craniocerebral trauma, neoplastic or inflammatory destruction surgical removal and congenital absence. Materials used for closure of dural defects were variable. Of these, autogenous fascia lata have been used for a long time. It seems interesting to study the histologic fate of the autogenous grafted fascia lata in the dural defects. After autogenous fascia lata graft to dural defect in 18 rabbits, the histologic fate of the autogenous grafted fascia lata was observed microscopically. The results are as follows: 1) The adhesions among the grafted fascia lata, arachnoid, pia mater and brain cortex were usually mild. This result may be due to either short period of observation or autogenous substitutional material. 2) The grafted autogenous fascia lata was revealed mininal inflammatory response and was ultimately reabsorbed. 3) The grafted autogenous fascia lata was replaced by granulation tissue and fibroblast origined from the host mesenchymal tissue.

        두개골에 발생한 원발성 골육종 1예

        Osteosarcoma is the most frequently encountered primary malignant tumor of the bone. But sarcoma of the skull are rare tumor to be observed even in an extensive neuropathological practice. We presents a case of skull neoplasm identified as osteosarcoma. A twelve year old male patient was admitted to our department because of the large palpable mass on the posterior parietal and occipital region with headache, nausea and vomiting. On admission moderate papilledema was noted in the both eyes. Other neurologic finding were within normal limits. Chest film and long bone series showed no evidence of abnormality. Skull films revealed a large round destructive area on the posterior parietal and occipital region and the right carotid angiogram revealed patch densities in the mass lesion which was supplied by the right occipital artery and possibly the right middle meningeal artery. Serum Alkaline Phosphatase was 14 BL Units. The findings on the pathology slide was compatible with osteosarcoma.

        소아에서 시상하부에 발생한 모양세포성 신경교종 : 증례 보고 Case Report

        The Juvenile pilocytic astrocytoma has histologically the same features as the adult type one. Contrary to the adult type, the parallel arrangement of fibrils in long thin tapering cells of the tumor is peculiarly independent of the environment in which the tumor is growing and it gives rise to the main tumor mass that is usually well circumscribed. The overall incidence is relatively rare of all the brain tumor, it occurs in children mainly from three to five of age. Its predilection sites are the hypothalamus and the optic chiasm. We are reporting an experiance of a juvenile pilocytic astrocytoma in a three years old female patient, involving the hypothalamus, basal ganglia and deep white matter of the temporoparietal lobe of the left side.

        Forestier씨 병 : 증례 보고 Case Report

        The characteristic features of the Forestier's disease, ankylosing hyperostosis of the spine, consist essentially in marginal proliferation at the vertebral edges which are apt to unit their homologues to form bridges and to extend as a bony layer which thickens the vertebral cortex with an overgrowth of bone spreading like a flow. The disease had been long looked upon as vertebral spondylosis or other spinal diseases such as ankylosing spondylitis before Forestier first defined it as a new disease entity. The bony changes could be occured everywhere on the spinal column, but are mainly confined to the-anterior part of the thoracic spine, especially in the right side. It clinically may be of little significance because of little symptoms. Forestier reported 88% of the patients was over 50 years old and 65% was male of 245 cases. Although the pathogenesis is uncertain, a spinal expression of more diffuse ossifying diathesis may be considered. We are reporting a case of the Forestier's disease which was incidentally found on a 55 years old paraplegic patient who had suffered from back trauma.

        성인에 발생한 두개인두종 치험 1예

        The craniopharyngima has been known to be congenital origin and primary tumor of childhood. The craniopharyngioma may be found at all ages, though nearly one half in the first twenty years of life and equally in either sex. Important differences exist between younger and older patients. In children, signs of increased intracranial pressure, endocrine abnormality and radiological calcification are common. In adults, mental disturbance becomes of increasing importance as well as remaining visual failure and endocrine disturbance. Because of slow growing rate of tumor in adult, subtotal excision is recommended. A 39-year-old male patient, suffering from agitation memory disturbance and almost total blindness, was diagnosed craniopharyngioma by neuroradiologic examinations. After surgical treatment, mental symptoms and visual acuity improved much more than before operation.

        척추간반탈출증 수술후에 발생한 척추간반염의 재발 1예 보고

        The etiology of the postoperative discitis is not fully known, but most likely, the cause seems to be of a low grade infection and chemical and mechanical injuries. The incidence of these complication is from 0.12% to 2.6% of all cases. The characteristics of this disease are recurrence of severe lower back and gluteal pain, spasm of lumbar paravertebral muscle and elevated E.S.R. following the asymptomatic postoperative period, a roentgenogram showed an early destructive change in the adjacent vertebra, the narrowing of the intervertebral disc space, sclerotic changes in the vertebral bone and occasional fusion of the spine, and relatively good prognosis. We present a case of recurrent postoperative discitis with E.S.R. change and roentgenographic change.

        대구 경북지역의 중추신경계 종양현황

        The authors analized statistically 403 cases of C.N.S. tumors confirmed by pathology in the boundary of Daegu city and Kyungbook province from January 1984 to December 1986. The results were as follows: 1) The each cases of male and female were 220 (54.6%) and 183 (45.4%). 2) Intracranial tumors were 288 cases (71.5%), spinal tumors 75 (18.6%), and others 40 (9.9%). 3) Numbers of C.N.S. tumors were 8.7/100,000 population in city and 6.3/100,000 in country. 4) Among the intracranial tumors, glioma (30.9%) was the most common and followed by meningioma (16.3%) and metastatic tumor (8.7%). Among the spinal tumors, metastatic tumors was 34.6%, neurofibroma 30.7%, and meningioma 5.3%. 5) The majority of intracranial tumors were found in supratentorial region (76.3%), in which area frequency of tumor incidence were as follows; glioma, meningioma, metastatic tumor. Among the infratentorial area (22.6%), the sequence of tumor incidence were glioma, medulloblastoma, and neurilemmoma. 6) Tumors predominantly in male were oligodendroglioma (2:1), metastatic tumor (1.8:1), and medulloblastoma (1.7:1), and in female were meningioma (l:2), neurilemmoma (l:1.7), and pituitary tumor (1:1.5). 7) Peak age incidence of glioma, meingioma, and neurilemmoma were from 40 to 60 years, pituitary tumor and oligodendroglioma were 30 to SO years, craniopharyngioma and ependymoma were around 10 years old, and metastatic tumor was 50 to 70 years old. 8) In the spinal tumors, the most favorable site of longitudinal axis was thoracic area (62.7%) and transverse location was epidural space (45.3%). 9) The most common site of metastatic tumor were came from lung (29.1%) in brain metastasis and prostate (19.2%) in spine.
