腦動脈瘤手術時 自家輸血法의 適用 : Perioperative Blood Collection and Acute Hemodilution

뇌지주막하 출혈을 일으킨 뇌동맥류 환자 29명중 16명에서 개두술시 마취상태에서 수술직전에 채혈한 동맥혈액으로써 수술중에 자가 수혈법을 적용하고, 13명의 뇌동맥류 환자를 대조군으로 하여 결과를 비교 관찰하였다. 대조군 13명의 동맥류환자에서 Hemoglbin과 Hematocrit의 평균치는 개두술전이 각각 12.5gm%, 40% 이었고, 수술후가 각각 12.2gm%, 39%로써 수술동안 공혈자에 의한 저장혈액을 사용한 평균 수혈량은 1104㎖였다. 자가수혈군 16명의 동맥류 환자에서 Hemoglobin과 Hematocrit의 평균치는 자가수혈을 위한 동맥혈액의 채혈전이 각각 12.6gm%, 38%이었고, 평균 채혈량 850㎖를 채혈한 후 각각 9.6gm%, 28%로써 저하되었다. 수술동안 자가수혈법의 적용과 공혈자에 의한 저장혈의 평균 수혈량이 438㎖의 보충으로써 수술직후 Hemoglobin과 Hematocrit의 평균치는 11.gm%, 37%로써 채혈전 Hemoglobin치와 Hematocrit치에 도달하였으며, 대조군에서 사용한 저장혈액의 수혈량과를 비교할 때 약 60%가 절약되었다. Autotransfusion has been known to have many benefits in patients who might have anticipated or unanticipated hemorrhage during elective or emergency surgical procedures. Twenty-nine patients with intracranial aneurysms with subarachnoid hemorrhage were enrolled in this study. Sixteen aneurysmal patients were randomly selected for autotransfusion group, whose hematocrit value ranged between 21 to 39% after aspiration of mean arterial blood volume of 850㎖. for auto-transfusion, and 13 patients were given only homologous banked blood during surgical operation as control group. During surgical operation for direct clipping on aneurysmal neck in autotransfusion group, autotransfusion was applied by the method of perioperative blood collection and acute hemodilution of 30% hematocrit value. In 13 control aneurysmal patients who need homologous banked blood during surgical operation to maintain normal blood pressure and intravascular blood volume, average mean volume of banked blood required was 1104㎖. In 16 aneurysmal autotransfusion group, it was only 438㎖ of banked blood, and furthermore in 4 patients among them, homologous banked blood was not needed at all.
회충 각피에 대한 호산구와 조직 비만 세포의 반응에 관한 실험적 연구

The tissue mast cell is important cell in the IgE-mediated hypersensitivity reaction and it's functions are partly regulated by the eosinophil. The author had carried out an experiment using cuticular antigen obtained from adult Ascaris lumbricoides to observe the changes of the tissue mast cell, neutrophil, macrophage and eosinophil at the injection site of the adult albino rats. The primary responses of the above cells were observed at 1,3,5,7 and 9 days after injection and the secondary responses were observed 30 minutes and 24 hours after reinjection at 9 day after primary injection. The results obtained were as follows: 1. In the primary response, the neutrophils were increased at the 5th and 7th day after injection and the macrophages were markedly increased at 9th day. 2. Only minimal increases of eosinophils and lymphocytes were noted in primary response. 3. The tissue mast cells were progressively increased to 5th day in primary response, and at 5th day the maximum increase was noted. 4. In secondary response, marked and immediate increase of eosinophil was found after reinjection, while the number of tissue mast cell was decreased. Above responses of albino rat to cuticular antigen of Ascaris lumbricoides seems to be mediated by Type Ⅰ hypersensitivity.

綜合的으로 보아 種類에 따라 多少 利用上 問題를 남기는 것도 있으나 廐 堆肥와 지렁이 糞은 堆肥와 同等하거나 그 以上의 利用效果를 認定할 수 있다. 그리고 用土의 混入程度는 Flower Cavage는 지렁이糞을 細粒子의 單用이거나 1/2程度의 모래를 섞어 쓰는 것이 有用하며 廐 堆肥에서는 細粒子와 모래 또는 흙을 같은 量을 섞어 쓰는 것이 좋다. Pansy Cineraria에서는 廐 堆肥와 모래 또는 흙을 같은 量 섞어 쓰는 것이 有利하다고 生覺된다. The aim of the present study is to develop a new soil for flower pot. Three flowers (Flower Cavage, Pansy, Cineraria) planted by industry’s waste, earth-worm dropping, barnyard manure, barnyard manure with saw dust and compost. The result obtained are summarized as follows: 1. Effect of barnyard manure and earthworm dropping equalize compost or over this. 2. A soil For flower mixed equally the amount of fine particle in barnyard, sand, soil was effective. 3. One utilization of fine particle in earth worm dropping, the other utilization of fine particle in earth worm dropping with a half sand were advantage to soil for Flower Cavage. 4. The soil for Pansy and Cineraria mixed equally barnyard, and soil was advantage.

A number of ways for solving the nation's fundamental food problems might be thought out, and I intended,for one device, to clarify if Nahada originated in Egypt is worth recommending to farmers, by examining the respective ripening times, planting densities, resistances to lodging and resistances to diseases of Nongnim No.6, Nongnim No.25, Nongnim No.29, Gumnampung, Eunbangju, Palgweng, Nonggwang which are the recommended varieties in this province, as well as newly imported Nahada. The results acquainted by comparing each other's characteristic are as follows: (1) Having examined their ripening times, differentiating in time of seeding and transplanting-extremely early seeding and transplanting, early seeding and timely seeding and transplanting-I found no difference in their growing, but the most yield from the timely seeded and transplanted, and the longer period they got frosted, the poorer yield they brought comparing those of the common varieties. (2) As for the planting densities, I transplanted 72 stubs, 108 stubs and 135 stubs in 3.3 M^2 each, and found almost no difference in their ripening times, but the best condition in the 72 stubs lot, and the 108 stubs lot and the 135 stubs lot followed. (3) No significance was found in their resistances to lodging according to the different kinds of seed by different??ting the applied amount of fertilizer with the ratio of 1.7 in the 1st lot, 1.4 in the 2nd lot and 1.0 in the 3rd lot. Nahada and Nongnim No.6, however, brought the most yield from the 2nd lot Nongnim No.25 produced more yield from the 1st lot, and the 2nd and 3rd lots followed. The resistance to lodging was weak in the 1st lot of Nahada. (4) Concerning their resistances to diseases, I have examined by comparing seven kinds of seed as well as Nahada each other, applying two times of nitrogen, and found that Nongnim No.6 suffered most severely and Nongnim No.29 and Palgwang least. Although there was almost no difference in their growing, the most yield was brought from Gumnampung, and almost same yields from the others, but Nahada did not produce a satisfactory yield because of its ripening time. The above results show that newly imported Nahada is not worth recommending.
