        광섬유 브래그 격자가 포함된 측면 연마된 광섬유를 이용한 매질의 온도와 굴절률 동시 측정

        We proposed and demonstrated a simultaneous measurement method to detect the refractive index and temperature of a medium using a side-polished fiber involving FBG (fiber Bragg grating). The temperature of a medium was obtained by using the Bragg wavelength shift of FBG, while the refractive index of medium were calculated by using the transmission loss. The Bragg wavelength is independent on the refractive index of the covering medium placed on surface of side-polished fiber, while the transmission loss at off-Bragg wavelength highly depends on the environmental temperature because of thermo-optic effect of the medium.

        기관 협착에서 레이저와 방사선 치료의 적용

        기관협착에서 기관절제 문합술을 적용할 수 없을 경우 레이저 소작술은 고식적 치료수기 중 하나로 사용 되고 있다. 저자들이 1995년부터 1996년 5월까지 기도폐쇄병변에서 기도재건수술 전 또는 후에 KTP (Potassium Titanyl Phosphate)레이저를 이용하여 굴곡성 기관지경의 생검용 채널로레이저 소작술을 시행한 치료결과를 보고한다. 레이저 소작술받은 환자는 모두 7명이며, 이중 남자는 3명 여자는 4명이었으며, 평균연 령은 43.7세이었다. 대상환자의 기관협착의 일차 원인은 외상에 의한 기관절개술후 협착의 3례와 폐혈증으로 인한 장기간의 기관튜브삽관이 원인이된 1례, 기관 결핵이 1례, 그외, 폐암이 기관내에 국소 재발한 2례이었다. 레이저 소작술의 치료 대상이 된 기도협착의 경우는 절제 문합후 문합부의 협착재발이 2례, 기관절개소공의 협착이 3 례, 폐암이 국소 재발한 2례이었다. 기관협착기 부위는 삽관의 풍선부위(기관절개공이 형성된 자리로 부터 3-4 cm 하부, carina 상방 2~3 cm)가 1례, 문합부위가 5 mm이하로 좁아진 경우가 4례있었으며, 기관내의 협착랭변은 전벽에서 2명, 후벽에서 1명, 기관전주위(circumferential)협착인 경우가 2명이였다. 레이져 소작술은 평균 25.4$\pm$5.9분의 시간이 걸렸으며, 1768$\pm$365J의 에너지가 조사되었다. 레이저 소작술 실시 후 환자들은 현저한 증상의 호전을 볼 수있었으며, 기관지 내시경하에서 기관의 내경이 7mm이상 커진 것을 확인 할 수 있었다. 레이저소작술 후 합병증으로는 경미한 기관지경축(bronchospasm)이 1례있었다. 레이저소작술을 시행한 후 협착이 재발한 환자 3명에서 보조적 방사선치료를 병행하였다. 방사선 치료가 섬유아 세포의 거대성장을 예방하는 것에 착안하여 기관지내시경하 레이저소작술실시 직후, 4시간 안에 방 사선을 외부조사하였다. 보조적 방사선치료 후 환자들은 현재 10개월이 지나는 동안 재협착이 발생되지 않았다. Tracheal stenosis is a difficult disease entity to manage. Laser ablation is one effective treatment for treacheal stenosis and can be utilized if tracheal reconstructive surgery is impossible. Potassium titanyl phosphate laser, transmitted via flexible quartz fiber, can be precisely manipulated through flexible bronchoscope under local anesthesia. We treated 7 patients with trach al and broncheal lesion under local anesthesia with KTP laser from January 1995 to July 1996. The patients included three males and four females. The age of patients ranged from 22 to 66 years with a mean of 43.7 years The etiology of tracheal stenosis in patients was stenosis after tracheostomy(3 cases), prolong inturbation in cases of sepsis(1 cases), and the recurrence of lung cancer within endobronchial lesion(2 cases). In the cases of tracheal stenosis treated with laser ablation, there were 2 cases of recurrence of stenosis at the anastomosis site after the operation, 3 cases of stenosis at tracheostomy site, and 2 cases of local recurrence of lung cancer. The site of the tracheal stenosis was the balloon site of the tracheostomy tube(3-4cm inferior to the tracheostomy site, 2-3cm superior to the carina) and the anastomosis site that were narrowed to less than 5mm(4 cases). For the stenosis lesion in the endobronchial area, there were 2 patients with a lesion at the anterior wa l, 1 patient with a lesion at the posterior wall, 2 patients with circumferential stenosis. Laser ablation time was 25.4 $\pm$5.9min and used energy was 1768 $\pm$365J. We have used KTP laser via (lexible bronchoscope without major complications. Adjuvant radiation therapy may prevent fibroblast proliferation which leads to restenosis. In three patients of restenosis after laser ablation, adjuvant irradiation started within 4 hours after laser ablation, and the radiation doses were 1500cGy given in five fraction. In patients with adjuvant radiation therapy, stenosis has not recurred

        Multimodal Curvature Discrimination of 3D Objects

        As virtual reality technologies are advanced rapidly, how to render 3D objects across modalities is becoming an important issue. This study is therefore aimed to investigate human discriminability on the curvature of 3D polygonal surfaces with focusing on the vision and touch senses because they are most dominant when explore 3D shapes. For the study, we designed a psychophysical experiment using signal detection theory to determine curvature discrimination for three conditions: haptic only, visual only, and both haptic and visual. The results show that there is no statistically significant difference among the conditions although the threshold in the haptic condition is the lowest. The results also indicate that rendering using both visual and haptic channels could degrade the performance of discrimination on a 3D global shape. These results must be considered when a multimodal rendering system is designed in near future.

        기관 무명 동맥루 -1례 보고-

        기관무명동맥루는 기관절개술후 드믈게 발생하는 합병증으로 사망률이 높은 질환이며, 치료는 신속한 외과적 처치가 관건이 된다. 환자는 과거력상 30년전에 폐결핵으로 좌측전폐절제술을 시행받았으며 그 동안 경과 양호하였으나 최근 2개월간 급성호흡부전으로 기관절개술을 시행받고 호전되어 11 mm 실리콘 Montgomery T-tube로 교환 후, 3일째 발생한 기관무명동맥루에 의한 절개창주위의 다량의 출혈과 기도폐쇄에 의한 심정지를 일으켰다. 기관무명동맥루에서 Utley maneuver와 무명동맥을 절단봉합하여 지혈에 성공하였고, 이에 지혈방법, 수술수기에 관하여 보고하는 바이다. Tracheoinnominate artery fistula is a rare complication that can happen after tracheostomy, the mortality rate is high and it reqiures urgent surgical management. The patient had received a left pneumonectomy 30 years ago and post-operative course was in uneventful. And tracheostomy was performed for acute respiratory failure due to trachea stenosis for 2 months in recent. She was improved in general condition and changed to a 11 mm silicone Montgomery T-tube. On the 3rd day after the tube changed, she had cardiac arrest due to the excessive hemorrhaging due to tracheoinnominate artery fistula. We report an successusful experience for control of bleeding by an innominate artery fistula division and the Utley maneuver for the tracheoinnominate artery fistula. We report the operation method of bleeding control.

        알루미늄 지지대에 고정된 융착 광섬유 커플러의 열팽창을 이용한 온도 센서

        We have investigated a temperature sensor on a thermal expansion effect of a fused optical fiber coupler. Both side of the fused tapered region of the coupler were fixed on a metal support to induce the high thermal expansion effect. The sensor showed that the peak coupling wavelengths were shifted to shorted wavelength region with increased of environmental temperature. The sensitivity of the sensor was $0.12nm/^{\circ}C$.

        Fiber-Optic Temperature Sensor Based on Single Mode Fused Fiber Coupler

        This paper reports a fiber-optic temperature sensor using a single mode fused fiber coupler incorporating a thermo-optic external medium. The spectral transmission was altered by changing the refractive index of the external thermo-optic medium. A theoretical and experimental investigation was carried out with the aim of achieving high sensitivity. The measured sensitivity for the environmental temperature was as high as -1.5 nm/℃.

        Do Rotational Shifts Affect Micturition Patterns in Real Practice? A Pilot Study in Healthy, Young Female Nurses

        Purpose: Healthy, young individuals are known to exhibit circadian variation in urinary functions. However, the effects ofchronic circadian disturbance on voiding functions are largely unknown. The present work compared the effects of rotationalshifts on the micturition patterns of female nurses to that in female nurses with routine daytime shifts. Methods: A total of 19 nurses without lower urinary tract symptoms who worked rotational shifts for an average duration of 2years were recruited. A voiding diary was kept for 9 consecutive days, and the overactive bladder symptom score (OABSS) questionnairewas completed three times, starting 3 days before their night duties until 3 days after completion of their night duties. For comparison, seven nurses with regular shifts completed a 3-day voiding diary and the OABSS questionnaire. Results: Female nurses working rotational shifts had lower overall urine production and had decreased urination frequencyand nocturia than female nurses working regular shifts, even when the nurses who worked rotational shifts had a regular night’ssleep for at least 7 days. Upon reinitiation of night duty, overall urine production increased significantly, with no significantchanges in urgency and frequency. When these nurses returned to daytime duty, the volume of urine decreased but nocturnalurine production remained high, and the incidence of nocturia also increased significantly. However, the effects on OABSSscore were not significant under the study design used. Conclusions: Long-term rotational shifts resulted in adaptive changes such as decreased urine production and frequency inhealthy, young female nurses. In addition, their micturition patterns were significantly affected by abrupt changes in their workschedules. Although working in shifts did not increase urgency or frequency of urination in healthy, young female nurses workingrotational shifts for an average 2 years, large-scale studies are needed to systematically analyze the influence of shift worktimings on micturition in humans.

        Transmission and Sensing Characteristics of the Biconically Tapered Cladded Multimode Fibers

        This paper presents a theoretical and experimental investigation of the transmission and sensing characteristics of the biconically tapered cladded multimode fibers. The beam propagation method was used to examine the transmission characteristics with various structural parameters. The results show that the transmission of the biconically tapered cladded multimode fibers is sensitive to the mode of the input optical beam and the refractive index of the external medium. A refractive index sensor for the external medium was proposed based on the theoretical analysis, and its feasibility was demonstrated experimentally.
