BNT162b2 접종과 면역관문억제제 치료 이후 발생한 이상 반응 1예

COVID-19 vaccination is essential in cancer patients. However, there is limited evidence of the prognosis of these patients, especially for those taking immune checkpoint inhibitors (ICIs). We present a patient on pembrolizumab for advanced endometrioid adenocarcinoma experiencing continuous diarrhea and subsequent episodes of fever with pain in multiple joints following a second dose of the BNT162b2 mRNA COVID-19 vaccine. An ICI-induced immune-related adverse effect (irAE) was the main diagnosis; cytokine release syndrome and rheumatoid arthritis were also considered. Notably, the novel irAE occurred after the 19th pembrolizumab trial, highlighting the potential effect of changes in systemic immunogenicity after BNT162b2 vaccination. Ultimately, the patient was treated with steroid, which alleviated her symptoms. Here, we report a rare adverse effect after COVID-19 vaccination in an endometrioid carcinoma patient on ICI therapy. This report shows that there is a need to consider and investigate vaccine-related adverse events. (Korean J Med 2023;98:93-97)

Eeok Park(1937-2000), who was an important musician of Busan city, died at April lst, last year. He worked through life as a conductor, a player, and also as a composer. Especially, Eeok Park devoted his life to wind orchestra. He established three big wind orchestra, that is Busan Symphonic Band Orchestra, Myungusng symphonic Band Orchestra, and Busan Police Wind Orchestra, and made a decisive contribution to introducing marching band in Korea. Eeok Park worked for several important musical organizations such as Korea Band Association(KBA) as a vice president, Bussan Branch of KBA as a president, and Korea Marching Band Directors Association(KMBDA) as a chairman of a board of directors. Conclusively, Eeok Park was a glorious and proud musician of Busan. He was born, educated in Busan, and lived through his life in Busan. But he played a nationwide role in Korean music culture as well as a local role in Busan music culture. He was a leading hero in the history of wind music of Busan in the second half of the 20th century.

Dissolution of the irradiated UO₂ target is the first step in process for separation and purification of fission ^(99)Mo, the only parent nuclide of ^(99m)Tc an extremely useful tool for medical diagnosis. In this study, the effects of concentration of sulfuric acid (0.5∼2N) and nitric acid(0.01∼0.1N), temperature on UO₂ pellets dissolution rate were investigated. The results of experiments showed that dissolution rate proportioned to the power 2.597 of H₂SO₄ concentration and 1.241 of HNO₃ concentration, and activation energy was evaluated to be 105.2 kJ/㏖. And also, the optimum operating condition for UO₂ target dissolution was suggested.
Molybdenum - benzoinoxime complex 로부터 molybdenum 의 분리

Generally, a sodium hydroxide solution containing hydrogen peroxide is used to dissolve molybdenum-benzoinoxime complex in the α-benzoinoxime precipitation method. However, the existence of residual hydrogen peroxide in the solution will interfere with the adsorption of ^(99)Mo by alumina. The separation of molybdenum with only sodium hydroxide solution was carried out in this work. A simulated solution consisted of 50 ppm molybdenum and 1 M nitric acid was used. Adding benzoinoxime with the 20-weight ratio of benzoinoxime to molybdenum produced molybdenum-benzoinoxime complex. Molybdenum was separated from molybdenum-benzoinoxime complex by reaction during 5 minutes using distillated water,0.1 N,0.4 N sodium hydroxide solution, and sodium hydroxide solution containing 1 wt% hydrogen peroxide. As results, molybdenum was separated over 97 %, and the separation efficiency was not different with the case of solution containing hydrogen peroxide. It is estimated that hydrogen peroxide acts to accelerate dissolution rate of benzoinoxime and does not affect to separate molybdenum from molybdenum-benzoinoxime complex.
α-Benzoin Oxime 에 의한 황산 용액 내 Mo 침천 분리 연구

Tc-99m is an extremely useful tool for medical diagnosis because it has good nuclear properties, and the radioisotope Mo-99 is the only parent nuclide. Mo precipitation process by using α-benzoin oxime can be used as one of processes for separation & purification of fission ^99Mo. In this study, the recovery yield of Mo in sulfuric acid solution by precipitation reaction were investigated to obtain optimum operating condition and process design data. In the results of experiments, recovery yield of Mo could be maintained above 95% in condition of 1-1.5N free acidity of Mo solution, 25 B.O./Mo weight ratio and dissolution time 25min.
