Cirrhotic patients have bleeding tendencies due to the lack of coagulation factors and thrombocytopenia. However, decreased levels of procoagulants are also accompanied by decreased levels of natural anticoagulants. However, there have been contrasting reports. It has been reported that patients with cirrhosis are at risk for thrombotic complications, including portal vein thrombosis and venous thromboembolism. Physicians consider active anticoagulation for prophylaxis and treatment of portal vein thrombosis and/or venous thromboembolism in cirrhotic patients with high risk of thrombosis. Concurrently, there are safety concerns regarding the risk of bleeding from anticoagulants in people with advanced liver disease. Further prospective studies are required to determine not only if cirrhotic patients benefit from receiving anticoagulation therapy for preventing thrombotic complications, but also to determine which prophylactic regimen is most appropriate. 본 논문에서는 통신망 설계시 신뢰성을 높이며 계산량을 감소시키기 위하려 기존의 Cut-Saturation 알고리즘에 절점차수(node degree)를 적용하여 서바이버블(survivable)한 통신망을 설계하는 알고리즘을 제안하였다. 먼저, 초기 토폴로지(initial topology)를 결정한 다음, 망의 성능을 평가하고 통신망의 정보처리량이 요구량보다 적을 경우 최소 절점차수와 지로 길이(link distance)를 이용하여 최적의 위치에 지로를 연결하였다. 그렇지 않은 경우 외대 절점차수 및 지로 길이와 지로 사용도(link utilization)를 이용하여 지로를 제거하였다. 이 과정을 통신망의 트래픽(traffic) 처리량과 트래픽 요구량이 일치할 때까지 반복하였다. 본 알고리즘에 의해 설계된 통신망은 서비이버블하며, 설계비용을 최소화하고, 트래픽 요구량을 모두 처리할 수 있고, 특히 계산량이 적어 초기 토폴로지 선택이 자유롭고 통신망의 크기에 큰 영향을 받지 않는다는 것이 장점이다. 또한 본 논문에서 제안한 알고리즘을 분석하기 위하여 Kris와 Pramod의 결과와 비교하였다. This paper propose a survivable communication network design process using node degree that augments the usual traffic flow and cost analyses with previously ignored topological survivability and computing time considerations. At first, decide a initial topology, and then measure a throughput of network. If the throughput is smaller than the required traffic, add edge to the optimum place by using minimum node degree and link distance. Otherwise, drop useless edge by using maximum node degree, link distance and link utilization. This process is repeated until throughput equals to the required traffics. This Process designs a survivable communication network with the minimized cost and computing time and usual traffic flow. The design proceses that minimized computing time are freely select initial topology and easily design a large network. And these results of algorithm are compared with the Kris and Pramod's in order to analyses the perfmance of the designed network.
Extended Application of Endoscopic Repair for Frontal Sinus Fractures

Purpose: The coronal approach for repair of frontal sinus fractures is associated with significant adverse sequelae including a long scar, alopecia, paresthesias, and, uncommonly, facial nerve injury. To minimize these complications, an endoscopic approach for repair of frontal sinus fractures was developed. The authors now present the results of an endoscopy-assisted approach for the treatment of frontal sinus fractures. Methods: From 2002 to 2009, five patients with frontal sinus fracture underwent endoscopic repair. Two slit incisions were placed in the scalp, and one or two stab incisions directly over the fractures were placed in the forehead. After subperiosteal dissection, fracture segments were reduced under direct vision and fixed with microplates or fibrin glue. Results: All patients had good cosmetic results and remained free of sinus complaints. There were no perioperative complications reported. Conclusion: Endoscopic repair of frontal sinus fractures is an efficacious technique that significantly reduces patient morbidity. A relatively wide range of anterior table fractures can be reduced using an endoscope. In cases of complicated comminuted fractures, fibrin glue helps to achieve satisfactory endoscopic reduction. Endoscopic repair is an alternative treatment for various anterior table fractures of the frontal sinus.
병원행정직 간호사의 특성에 관한 탐색적 연구 - 성격, 직무만족, 업무적합성 인식을 중심으로 -

Objective: This study was performed to find out the characteristics of nurse administrators and the relationships among their personality types, administrative work appropriateness and job satisfaction. Methods: Participants were 112 nurse administrators who had worked for more than 1 year in a hospital administrative position in Seoul and Gyeonggi area. Data were collected form April 14 to Jun 17, 2016, using questionnaires. For the analysis, SPSS WIN 20.0 program was used. Results: Personality types are related to job satisfaction and administrative work appropriateness. Among big-5 personality types, the neurotic level is negatively related to job satisfaction. However, extroversion is positively related. For the appropriateness of administrative work, the way of transfer and working department show no significant relations but age shows significant relation. The influence of age needs further study. Conclusion: The personalty types are related to nurse administrators` job satisfaction and administrative work appropriateness.
