        The nevus sebaceus is a congenital hamartoma of epithelial and adnexal components. Secondary neoplasms of nevus sebaceus occur in 6.5-30% of adults. Most of them are benign tumors such as trichoblastoma, syringocystadenoma papilliferum, but malignant transformations rarely present. Sebaceous carcinoma is one of uncommon secondary neoplasm developing in nevus sebaceus. The cause of sebaceous carcinoma is unknown, but there have been cases associated with Muir-Torre syndrome, radiation, diuretic agents, and nevus sebaceus. A 77-year-old woman who had congenital nevus sebaceus presented 2.5 X 3.0 cm sized erythematous dome shape elevated nodule with ulcerative crust within the nevus. Histologic features revealed irregular asymmetric multiple lobules extending into subcutaneous layer. Tumor lobules compromised atypical cells with foamy cytoplasm and undifferentiated cells. Herein, we report a case of malignant change of nevus sebaceus and we suggest careful observation should be warranted for patient with nevus sebaceus.

Dermoscopy has recently proved to be a useful, noninvasive technique for the diagnosis of cutaneous lesion, but their dermoscopic features of EP have not been clarified. Objectives: To evaluate dermoscopic features of nonpigmented & pigmented eccrine poroma. Methods: We have reviewed the medical records, clinical photographs and dermoscopic features of histologically proven cases of EP at our clinic between 2008 and 2016. Results: 10 cases were studied. Nonpigmented EP were 6 cases, and pigmented EP were 4 cases. Vascular structures that were glomerular vessels, hairpin vessels and arborizing-like vessels were observed in 7 of 10 cases(70%). Glomerular vessels were found in most nonpigmented EP(5 cases, 83%). In pigmented EP, Glomerular vessels were also commonly observed(2 cases, 50%). Frog egg-like appearance were observed 4 cases in nonpigmented EP(67%). Comedo-like openings were observed in 3 cases of pigmented EP(75%). Conclusion: Vascular structures are important features on dermoscopic findings of EP. Frog egg-like appearance and comedo-like openings were charateristically observed respectively in nonpigmented & pigmented EP. Due to the diversity of dermoscopic patterns of EP, further studies are required.

      • A case of linear eruptions with overlapping features of lichen striatus and lupus erythematosus

        Linear eruptions showing both features of lichen stratus and lupus erythematosus have rarely been reported. Lichen striatus is an inflammatory skin disorder characterized by unilateral and linear skin lesions following the lines of Blaschko mostly on extremities. Linear cutaneous lupus erythematosus is a rare subtype of cutaneous lupus erythematosus that the lesions follow the lines of Blaschko. A 9-year-old male patient presented with erythematous linearly arrayed elevated papules and plaques from the glabella to the ala nasi. Histologic features revealed focal hydropic degeneration of basal cells, spongiosis, and exocytosis in epidermis. Perivascular and periappendageal dense lymphohistiocytic infiltration is seen in entire dermis. We first treated the patient with 0.03% tacrolimus ointment for 2 months. Because there was no significant improvement, we added hydroxychloroquine 200mg/day for 7 months. And then the lesions improved with some pigmentation. Herein, we report a case of linear eruptions which follow the lines of Blaschko with overlapping features of lichen striatus and lupus erythematosus.

      • Effect of polydeoxyribonucleotide(PDRN) on herpes zoster related pain

        Background: Although many analgesic drugs are used to treat herpes zoster related pain, they are not enough for pain relief. To improve quality of life of the patients, research for adjuvant treatments for pain has been performed. However, study about PDRN on herpes zoster has not been performed. Objectives: The aim of this study is to investigate the effect of PDRN intradermal injection on herpes zoster related pain. Methods: Twenty nine inpatients were evaluated in regard to age, sex, dermatomal distribution, dermatology quality of life index(DLQI), pain severity. The injetion group consisted of fifteen patients and the non-injection group consisted of fourteen patients. We divided the patients depending on the numeric rating scale(NRS) : severe(7 or more), mild to moderate(0 to 6). PDRN was intradermaly injected on the first day of hospitalization. Pain levels were assessed on first day of hospitalization, 3 days and 28 days after discharge. When the treatment was complete 28 days ago, the patient was measured at the last visit. DLQIs were investigated on first day of hospitalization and 3 days after discharge. Results: The NRS was 8.17 on initial day and 2.33 on 28 days after discharge in PDRN injected severe group. The result of NRS after 28 day was statistically significant compared to treatment without PDRN(p<0.05), but not significant in mild to moderate pain. Conclusion: This study suggests that PDRN injection is more effective in prevention of chronic pain than severe acute pain.

      • A case of worsened giant keratoacanthoma after withdrawal of acitretin and review of treatment

        Giant keratoacanthoma is a subtype of keratoacanthoma where the size is greater than 2~3cm. In most cases, giant keratoacanthoma regresses spontaneously. But, in some cases, different kinds of treatment modalities are being used including complete excision when it is expected to cause anatomical disfigurement. Other treatment modalities like radiation, chemotherapy, retinoids, and cryotherapy are being considered when the affected area is cosmetically important or challenging to do surgical excision. A 69-year-old male patient presented with 3 X 5cm sized pink colored irregular shaped protruding bridge-like mass with yellow crusted bumpy surface on sacral area. On histological analysis, numerous hyperkeratotic small craters filled with keratin plug and papillomatous hyperplasia were seen in epidermis. In dermis, hyperplastic epithelium with eosinophilic glassy keratinocytes were seen. The patient was treated with acitretin 20mg/day for 3 months and partial regression was achieved in shape and size. Then the patient withdrawed the medication by himself for 10 months and the lesion suddenly exacerbated. Herein, we report a case of worsened giant keratoacanthoma after achieving partial regression on withdrawal of acitretin.

      • A case of morpheaform cutaneous sarcoidosis on the face

        Sarcoidosis is an idiopathic mulitsystem granulomatous disease. Morpheaform sarcoidosis is a less common manifestation of sarcoidosis. Indurated and infiltrated plaques turn into erythematous or purplish appearance with the passage of time. For evaluate systemic disease, it is focused patient's history and review of system and complete physical examination. A 15-year-old boy presented child palm sized erythematous indurated patch on the right cheek for a month. He had no remarkable medical history and exposure like zirconium, beryllium. Chest radiograph and angiotensin-converting enzyme level were all within normal limits, but 24 hours urine calcium was elevated. Histologic examination showed well circumscribed, non-caseating granulomatous inflammatory structures with only a sparse lymphocytic infiltration in dermis and subcutaneous layer. The patient were treated with oral hydrochloroquine and minocycline. The lesions remarkably improved. Herein, we report a uncommon case of morpheaform cutaneous sarcoidosis on the face.

        Seasonal Changes in the Microbial Communities on Lettuce ( Lactuca sativa L.) in Chungcheong-do, South Korea

        Lettuce is one of the most consumed vegetables worldwide. However, it has potential risks associated with pathogenic bacterial contamination because it is usually consumed raw. In this study, we investigated the changes in the bacterial community on lettuce (Lactuca sativa L.) in Chungcheong-do, South Korea, and the prevalence of foodborne pathogens on lettuce in different seasons using 16S rRNA gene-based sequencing. Our data revealed that the Shannon diversity index showed the same tendency in term of the number of OTUs, with the index being greatest for summer samples in comparison to other seasons. Moreover, the microbial communities were significantly different between the four seasons. The relative abundance of Actinobacteriota varied according to the season. Family Micrococcaceae was most dominant in all samples except summer, and Rhizobiaceae was predominant in the microbiome of the summer sample. At the genus level, the relative abundance of Bacillus was greatest in spring samples, whereas Pseudomonas was greatest in winter samples. Potential pathogens, such as Staphylococcus and Clostridium, were detected with low relative abundance in all lettuce samples. We also performed metagenome shotgun sequencing analysis on the selected summer and winter samples, which were expected to be contaminated with foodborne pathogens, to support 16S rRNA gene-based sequencing dataset. Moreover, we could detect seasonal biomarkers and microbial association networks of microbiota on lettuce samples. Our results suggest that seasonal characteristics of lettuce microbial communities, which include diverse potential pathogens, can be used as basic data for food safety management to predict and prevent future outbreaks.

      • Priming nanoparticle-guided diagnostics and therapeutics towards human organs-on-chips microphysiological system

        Nanotechnology and bioengineering have converged over the past decades, by which the application of multi-functional nanoparticles (NPs) has been emerged in clinical and biomedical fields. The NPs primed to detect disease-specific biomarkers or to deliver biopharmaceutical compounds have beena validated in conventional in vitro culture models including two dimensional (2D) cell cultures or 3D organoid models. However, a lack of experimental models that have strong human physiological relevance has hampered accurate validation of the safety and functionality of NPs. Alternatively, biomimetic human “Organs-on-Chips” microphysiological systems have recapitulated the mechanically dynamic 3D tissue interface of human organ microenvironment, in which the transport, cytotoxicity, biocompatibility, and therapeutic efficacy of NPs and their conjugates may be more accurately validated. Finally, integration of NP-guided diagnostic detection and targeted nanotherapeutics in conjunction with human organs-on-chips can provide a novel avenue to accelerate the NP-based drug development process as well as the rapid detection of cellular secretomes associated with pathophysiological processes.
