        Succinylcholine is a depolarising muscle relaxant. Because of its quick onset of action, it is particularly used in situations which require urgent intubation. However, there have been several reports of cardiac arrest after administration of succinylcholine. We report a case of a 49-year-old male patient with spinal cord injury who developed cardiac asystole following succinylcholine administration. The authors discuss the possible causes and its prevention with a review of the relevant literature.

        The high-voltage discharge/oxidation catalyst filter was designed to reduce the particulate emission in automobile. The filter was located on the exhaust pipe of engine. The electrical power of low current which have the high voltage was applied. The experimental measurement on smoke density was performed by the implementation of the automobile named, "Carnival", a product of KIA Motor ㏇ at the uniform engine speed and brake on. The smoke opacity in optical absorption measurement showed that the high voltage discharge/oxidation catalyst filter leads to reduce the amount of emission. To characterize the performance of the filter, we used various experimental equipment. The results showed that the adoption of filter reduced the amount of the particulate matters.

        Updated Clinical Practice Guidelines for the Diagnosis and Management of Long COVID

        "Long COVID" is a term used to describe a condition when the symptoms and signs associated with coronavirus disease 2019 (COVID-19) persist for more than three months among patients infected with COVID-19; this condition has been reported globally and poses a serious public health issue. Long COVID can manifest in various forms, highlighting the need for appropriate evaluation and management by experts from various fields. However, due to the lack of clear clinical definitions, knowledge of pathophysiology, diagnostic methods, and treatment protocols, it is necessary to develop the best standard clinical guidelines based on the scientific evidence reported to date. We developed this clinical guideline for diagnosing and treating long COVID by analyzing the latest research data collected from the start of the COVID-19 pandemic until June 2023, along with the consensus of expert opinions. This guideline provides recommendations for diagnosis and treatment that can be applied in clinical practice, based on a total of 32 key questions related to patients with long COVID. The evaluation of patients with long COVID should be comprehensive, including medical history, physical examination, blood tests, imaging studies, and functional tests. To reduce the risk of developing long COVID, vaccination and antiviral treatment during the acute phase are recommended. This guideline will be revised when there is a reasonable need for updates based on the availability of new knowledge on the diagnosis and treatment of long COVID.

      • Usability of diaphragm ultrasonography in lung transplantation

        Purpose: For patients with end-stage lung disease, the diaphragm is a major inspiratory muscle and has a significant impact on postoperative recovery. Among the various diagnostic tools available for evaluating the diaphragm, ultrasound has recently proved to be useful tool because it is portable and non-invasive and provides real-time images. The aim of the study is to determine how ultrasound can be used in LTx recipients. Methods: Ten patients who underwent LTx from April 2017 to April 2019 were included. We performed PFT, diaphragm ultrasound before the LTx, immediately and 1 month after the LTx. Ultrasound evaluation was done by expert in M-mode to visualize the diaphragm excursion at resting and at forceful respiration. B-mode is used to check the change in diaphragm thickness during breathing, the thickness is measured at the zone of apposition of the anterior axillary line. The diaphragm thickness at the end of inspiration and at the end of expiration were measured. The diaphragmatic thickness fraction was calculated by the following formula: DTf = . Results: DTf is associated with MIP and VC (p=0.001 and p=0.012). The diaphragm excursion showed no statistically relationship with PFT parameters but showed linear correlation with DTf (p=0.006). Immediately after LTx, both resting and forceful excursions decreased by an average of 49% (p=0.024), 50% (p=0.017). After a month after LTx, both resting and forceful excursions increased by an average of 120% (p=0.016), 123% (p=0.000). Conclusion: MIP and VC are the most commonly used parameters reflecting diaphragm function, however it’s inaccurate and hard to perform in the early postoperative state due to tracheostomy status and poor cooperation. DTf correlates well with PFT parameters and diaphragm excursion in ultrasound. Thus, considering portability and non-invasiveness of the ultrasound, diaphragm ultrasonography has a great advantage in the serial evaluation of the diaphragm function after LTx.

        Analysis of Equivalence and Reliability of Korean Translated Abbreviated Profile of Hearing Aid Benefits

        For using the translated questionnaires, an appropriate translation should be accomplished for establishing the equivalence. The equivalence is divided into two categories, formal equivalence regarding words and grammar and dynamic equivalence meaning the naturalness of the context with the consideration of the cultural and emotional characteristics of the region. The purpose of this study was to verify formal and dynamic equivalences and statistical reliability of the widely used translated questionnaire in Korea, abbreviated profile of hearing aid benefits (APHAB). The identification of equivalence was conducted in formal and dynamic categories and the statistical verification was performed for 6 translated APHAB which are currently utilized in Korea. None of 6 translated APHAB had formal and dynamic equivalences for all 24 items when the score ‘2’ was given to the completed equivalence of each item. Two translated APHAB showed ‘11’ as the best score and one of them showed ‘0’ for the formal equivalence identification. For the dynamic equivalence, the two translated APHAB scored ‘20’ as the best score. The one scored ‘0’ for the formal equivalence showed ‘0’ also in the dynamic equivalence. And only two translated APHAB presented the statistical validation using Cronbach’s alpha and factor analysis. It was confirmed that any of 6 translated APHAB in Korea could not reproduce the original contents with the appropriate equivalence and statistical validation. As the translated version was found to have a limitation in delivering the original contents due to language, emotional and cultural differences, a Korean questionnaire should be developed.

        CONSTRUCTION OF THE RURAL DEVELOPMENT INDEX: THE CASE OF VIETNAM

        The objective of this research is to analyze the current status and process of Vietnam`s rural development to draw lessons for improving the quality of rural life and achieving sustainable rural development in Vietnam. The research was carried out by the following methods. First, the paper reviews previous studies on concepts and theoretical perspectives on rural development and comes up with a concrete definition of the term `rural development` for this research to build a Rural Development Index (RDI). Second, the RDI is developed as a tool to evaluate the current status and process of rural development. Third, the paper examines the current status and development process of Vietnam`s rural development using the RDI. The scope of this study covered 63 regions (58 provinces and 5 municipalities) in Vietnam. This research can be used to establish midand long-term visions and strategies of rural development policies in Vietnam.

      • Clinical Values of Diaphragmatic Movement in Patients with Chronic Obstructive Pulmonary Disease

        Background Diaphragmatic movement affects dyspnea in the COPD patient. Research Question: Which of the diaphragm-related factors are closely related to Pulmonary Function test in the COPD patients? Study design and Methods We enrolled 60 patients with COPD who attend respiratory outpatients. The diaphragm evaluation was performed using ultrasound, and PFT, 6MWT was performed in a respiratory outpatient clinic. Dyspnea scare, BMI, PFT, 6MWT and diaphragmatic factors collected and statistically analyzed. Results In total patients, the right forced-excursion at supine positon was 6.9±2.000, left forced-excursion at supine position was 7.6±1.649. Right forced-excursion at supine position (p=0.003, R2=0.137), left forced-excursion at supine position (p=0.048, R2=0.074) and thickness of right end-expiratory (p=0.016, R2=0.094) were significantly associated with FEV1. When the group with right diaphragmatic excursion ≥ 6.7 was further divided into groups according to FEV1 (< 50% or ≥ 50%), FEV1/FVC, MIP, portable peak flow meter, 6-minute walk distance, SpO2 before and after the 6MWT, and right diaphragmatic thickness at endexpiration subgroups were significantly different between the two groups. Interpretation Right diaphragmatic forced excursion was closely related to FEV1, and analysis according to cut-off value which based on Right diaphragmatic forced excursion showed a large difference between groups. When the diaphragm function was maintained, MIP, portable peak flow meter, 6-minute walk distance, SpO2 before and after the 6MWT, and right diaphragmatic thickness at end-expiration were different between the two FEV1 groups.

      • Fast B<sub>1</sub> <sup>+</sup> mapping using three consecutive RF pulses and balanced gradients for improved bSSFP imaging

        <P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>To develop a B<SUB>1</SUB> <SUP>+</SUP> mapping during the transient phase of balanced steady state free precession (bSSFP) imaging which can be used for subsequent B<SUB>1</SUB> <SUP>+</SUP> inhomogeneity compensation.</P> <P><B>Methods</B></P> <P>Two images with different flip angles (FA) are acquired using single-shot spiral technique during the transient phase of bSSFP with three consecutive RF pulses and balanced gradients. Under the assumptions that the transmit (B<SUB>1</SUB> <SUP>+</SUP>) field varies slowly in spatial domain and T<SUB>1</SUB> and T<SUB>2</SUB> relaxation effects are negligible during 2·TR, B<SUB>1</SUB> <SUP>+</SUP> was estimated using the two magnitude images and bSSFP data was sequentially acquired. B<SUB>1</SUB> <SUP>+</SUP> estimation error due to the assumptions and other factors such as FA and off-resonance were assessed using Bloch simulation. Phantom and in vivo experiments were performed with α-2α-3α scheme.</P> <P><B>Results</B></P> <P>The simulation results indicated that the proposed method was less sensitive to T<SUB>1</SUB> relaxation and B<SUB>1</SUB> <SUP>+</SUP> mapping FA (α) of approximately 60° produced minimum estimation error. The B<SUB>1</SUB> <SUP>+</SUP>-induced intensity variation was reduced with the proposed method in the phantom experiment. For both the phantom and in vivo experiments, the estimated B<SUB>1</SUB> <SUP>+</SUP> map showed comparable to the conventional B<SUB>1</SUB> <SUP>+</SUP> map using spin-echo DAM.</P> <P><B>Conclusion</B></P> <P>B<SUB>1</SUB> <SUP>+</SUP> map was estimated during the transient phase of bSSFP and subsequently compensated bSSFP images. There was no scan time increment and hence the technique can be used in a prescan manner for B<SUB>1</SUB> <SUP>+</SUP> mapping or shimming.</P>
