두경부 표피낭종 치료에서 펀치 절개술과 방추형 절제술의 효과 및 재발률에 관한 비교 연구

Background: The removal of epidermal cysts from the head and neck can sometimes be challenging for dermatologists, due to the associated cosmetic effects. Punch incision is an alternative method to the traditional elliptical excision for the removal of epidermal cysts. It is simple to perform with commonly available instruments, and easy to learn. However, there is currently no randomized study directly comparing the long-term results of these two methods in Korea. Objective: The purpose of this study was to evaluate the usefulness of punch incision as compared to elliptical excision for the removal of epidermal cysts, and to identify the characteristics of the lesions most suitable for punch incision. Methods: We conducted a prospective randomized study. A total of 109 patients with non-infected epidermal cysts on the head and neck were randomly assigned to treatment with either punch incision or elliptical excision, between January 2012 and January 2013. Statistical analysis was performed to compare the differences between the two procedures with respect to the demographic data of the patients; size and location of the lesions; operative time; complications; recurrence; cosmetic results; and patient satisfaction. Results: The mean operative time for the punch incision group was significantly shorter than for the elliptical excision group (p<0.05). The cosmetic result scores at 6 months after the operation were significantly higher in the punch incision group, compared to the elliptical excision group (p<0.05). No significant differences were observed between the groups with regards to age, sex, disease duration, location, size, prevalence of complications, and recurrence rate (p>0.05). Conclusion: Punch incision requires less time than elliptical excision, and is associated with superior cosmetic results. Regardless of different demographic characteristics, duration, location, and size, any epidermal cyst on the head and neck can be effectively treated with punch incision. Thus, we suggest that punch incision may be a useful method for removing epidermal cysts of the head and neck.

Atypical lymphocytic lobular panniculitis (ALLP) is a recently described entity characterized by self-limiting plaques that manifest a waxing and waning course. Although ALLP is known as a benign condition, it needs adequate follow-up to determine the risk of progression to subcutaneous panniculitis-like T-cell lymphoma (SPTCL) because ALLP has morphologic and biologic similarities with SPTCL. We report a case of ALLP in a 13-year-old girl who presented with an unusual tumor-like erythematous mass on the left thigh but without systemic symptoms or laboratory abnormalities. The histopathologic findings showed a dense and diffuse infiltration of the fat lobules by small-to-medium-sized atypical lymphocytes including focal atypical mitosis. Some cells showed characteristic peripheral rimming of individual adipocytes. However, prominent lymphoid atypia, fat necrosis, vasculitis, vascular thrombosis, erythrophagocytosis, mucin deposition, and the striking dominance of CD8+ lymphocytes seen in SPTCL were not detected. The lesion resolved spontaneously in 6 months and there was no recurrence over a 24-month follow-up period. (Korean J Dermatol 2015;53(3):222∼225)
증례 : 감염 ; 수막염을 합병한 폐렴알균 심내막염 1예

페니실린이 개발된 이래로 폐렴알균에 의한 감염성 심내막염은 매우 드문 질환으로, 전형적으로는 알코올중독자에서 폐렴에 합병되어 발생하고 수막염을 일으키기도 한다. 현재까지 국내에는 성인에서 폐렴알균에 의한 감염성 심내막염 1예가 보고되어 있으며, Austrian 증후군이나 수막염이 동반된 증례는 보고가 없다. 감염성 색전 물질이 뇌혈관이나 수막혈관에 살포되어 수막염을 일으키기도 한다. 저자들은 아직까지 국내에 보고된 바 없는 색전뇌경색증과 수막염을 합병한 폐렴알균 심내막염 1예를 경험하였기에 보고하는 바이다. 심장 수술을 하지 않고 항균제 치료만 지속하다가 심장 합병증으로 사망하였으며, 폐렴알균 심내막염은 급속히 진행하고 국소적 전신적 합병증이 잘 발생하므로 심장 수술을 하지 않고는 치료가 어렵다는 것을 보여주는 증례이다. Streptococcus pneumoniae is an infrequent cause of infective endocarditis in the penicillin era. This infection typically follows pneumonia in the setting of chronic alcoholism and may be complicated by meningitis. Only one adult case of pneumococcal endocarditis (PE) has been reported in Korea. We report the case of a 73-year-old woman who suffered from PE complicated by embolic cerebral infarction and meningitis. This is the first Korean case of PE complicated with meningitis. The patient was treated medically and died from a cardiac complication. This case shows that PE is difficult to treat without surgery because its evolution is usually acute and very aggressive; it has a high rate of local and systemic complications despite the administration of appropriate antibiotics. Mushroom fly, Lycoriella mali (Diptera: Sciaridae), is the primary pest in imported mushrooms. The amount of Tricholoma matsutake imported from China increases every fall when it is harvested. When importing T. matsutake, disinfestation using methyl bromide (MB) or phosphine ( PH3) is performed to prevent the introduction of L. mali. However, MB will be phased out due to ozone-depletion, chronic toxicity to workers, and residual issues. PH3 fumigation in mushroom disinfestation requires a long exposure time (24 h). In this study, we used ethyl formate (EF), which can replace MB and reduce exposure time. The efficacy of EF, PH3 and EF + PH3 on L. mali was evaluated. Using 4-h EF fumigation at 5 °C, the 3rd and 4th instar was the most tolerant stage in terms of 99% killed lethal concentration × time products ( LCt99%). When 4-h EF fumigation at 5 °C was applied on all stages of L. mali, the LCt99% values of EF were 73.1 g h/m3 to the 1st and 2nd instar, 112.9 g h/m3 to the 3rd and 4th instar, 68.9 g h/m3 to pupae, and 20.1 g h/ m3 to adult. It was confirmed that combination treatment with EF + PH3 had a synergistic effect on L. mali. The LCt99% of EF + 0.5 g/m3 of PH3 to the 3rd and 4th instar was 48.3 g h/m3. When only 140 g/m3 of EF was applied for 4 h at > 5 °C and 35 g/m3 of EF + 0.5 g/m3 of PH3 for 4 h at > 5 °C in commercial trials containing T. matsutake, proven efficacy (100%) on L. mali was confirmed. In the case of EF treatment only, phytotoxic damage occurred due to high Ct products, and there was no phytotoxic damage in combination treatment with EF + PH3. This study provides a new guideline for EF + PH3 combination treatment within a shorter exposure time (4 h) than existing PH3 treatment (24 h) and replacement of MB use.
