        Two-Year Follow-up on the Use of Absorbable Mesh Plates in the Treatment of Medial Orbital Wall Fractures

        Background Absorbable materials offer many advantages in the reconstruction of orbital walls; however, the possibility of postoperative enophthalmos after complete absorption cannot be excluded. We evaluated the postoperative results of absorbable mesh plates used as onlay implanting on the medial orbital wall to determine whether they are suitable for medial orbital wall reconstruction. Methods The study included 20 patients with medial orbital wall fractures who were followed up for more than 2 years postoperatively. We used absorbable mesh plates in all of the patients. We measured the following: the changes in the expanded orbital volume by comparing the preoperative and postoperative computed tomography (CT) scans and the degree of clinical enophthalmos. Results There were no major complications associated with the use of absorbable materials such as infection, migration, or extrusion of mesh plates during the long-term follow-up. The orbital volumetric changes between the preoperative and postoperative CT scans were not statistically significant. However, the expanded orbital volume was not related to the degree of clinical enophthalmos. Conclusions The reconstructed orbital wall may provide supportive scar tissue to the orbital contents even after the absorbable materials have dissolved completely. Absorbable mesh plates could be another option for the reconstruction of the medial orbital wall.

        Treadmill exercise with bone marrow stromal cells transplantation potentiates recovery of locomotor function after spinal cord injury in rats

        Transplantation of bone marrow stromal cells (BMSCs) is regarded as a promising candidate for the spinal cord injury (SCI). In the present study, we investigated whether treadmill exercise potentiate the effect of BM-SCs transplantation on the functional recovery in the SCI rats. The spi-nal cord contusion injury applied at the T9–T10 level using the impactor. Cultured BMSCs were transplanted into the lesion at 1 week after SCI induction. Treadmill exercise was conducted for 6 weeks. Basso-Beat-tie-Bresnahan (BBB) scale for locomotor function was determined. Sprouting axons in the lesion cavity were detected by immunofluores-cence staining for neurofilament-200. Brain-derived neurotrophic factor (BDNF) and synapsin-I expressions were analyzed using western blot-ting. BMSCs transplantation improved BBB score and increased ex-pressions of neurofilament-200, BDNF, and synapsin-I in the SCI rats. Treadmill exercise potentiated the improving effect of BMSCs trans-plantation on BBB score in the SCI rats. This potentiating effect of treadmill exercise could be ascribed to the enhancement of BDNF ex-pression in the SCI rats.

        Associated Factors for Asthma Severity in Korean Children: A Korean Childhood Asthma Study

        Purpose: Childhood asthma has a considerable social impact and economic burden, especially in severe asthma. This study aimed to identify the proportion of childhood asthma severity and to evaluate associated factors for greater asthma severity. Methods: This study was performed on 667 children aged 5–15 years with asthma from the nationwide 19 hospitals in the Korean childhood Asthma Study (KAS). Asthma was classified as mild intermittent, mild persistent, and moderate/severe persistent groups according to the National Asthma Education and Prevention Program recommendations. Multinomial logistic regression models were used to identify the associated factors for greater asthma severity. Results: Mild persistent asthma was most prevalent (39.0%), followed by mild intermittent (37.6%), moderate persistent (22.8%), and severe persistent asthma (0.6%). Onset later than 6 years of age (adjusted odds ratio [aOR], 1.69 for mild persistent asthma; aOR, 1.92 for moderate/severe persistent asthma) tended to increase asthma severity. Exposure to environmental tobacco smoke (aOR, 1.53 for mild persistent asthma; aOR, 1.85 for moderate/severe persistent asthma), and current dog ownership with sensitization to dog dander (aOR, 5.86 for mild persistent asthma; aOR, 6.90 for moderate/severe persistent asthma) showed increasing trends with greater asthma severity. Lower maternal education levels (aOR, 2.32) and no usage of an air purifier in exposure to high levels of outdoor air pollution (aOR, 1.76) were associated with moderate/severe persistent asthma. Conclusions: Modification of identified environmental factors associated with greater asthma severity might help better control childhood asthma, thereby reducing the disease burden due to childhood asthma.

        Case Report : A Case of Acute Calcium Pyrophosphate Crystal Arthritis in the Lumbar Facet Joint

        Calcium pyrophosphate dihydrate deposition disease most commonly presents with acute arthritis of the peripheral joints. Infrequently, a mass effect of this disease can cause axial symptoms, such as spinal stenosis, radiculopathy, or myelopathy. Herein, we report on the first Korean case of acute arthritis in the lumbar facet joint due to calcium pyrophosphate dihydrate crystal deposition disease. A 73-year-old female presented with acute fever, severe lumbago, and knee arthralgia, 11 days after partial parathyroidectomy. Plain radiographs showed multiple chondrocalcinosis, while a bone scan, computed tomography, and magnetic resonance imaging showed right L5-S1 facet arthritis. In synovial fluid from the facet and knee joints, positively birefringent calcium pyrophosphate dihydrate crystals were observed under polarized light microscopy. Under the diagnosis of acute calcium pyrophosphate dihydrate crystal arthritis (formerly known as ‘pseudogout’) in the facet joint, an intra articular triamcinolone injection was administered, which resulted in dramatic improvement of the symptoms within 24 hours.         Background: Effect of concurrent chemotherapy regimen on patients undergoing preoperative chemoradiotherapy for locally advanced rectal cancer was contradictory in recently published long-term results of two large prospective randomized trials. This study was undertaken to examine the differential effect of chemotherapy regimen in single center cohort through retrospective analysis. Methods: Medical records of 279 patients who underwent preoperative concurrent chemoradiotherapy, followed by radical surgery for locally advanced rectal cancer from July 2003 to December 2010 were retrospectively reviewed. Radiation dose ranged from 45 to 54 Gy. Thirty-four patients were treated with one cycle of iv bolus 5-FU, 500mg/m<sup>2</sup> on D1-3 (group A), 214 patients with two cycles of 5-FU with identical schedule (group B) and 31 patients with oral capecitabine, 1650mg/ m<sup>2</sup> daily on days with radiotherapy (group C). Radical surgery was performed median 57 days from the end of radiotherapy. Sphincter preserving surgery was performed in 261 patients, while 18 underwent abdominoperineal resection. Results: Median follow-up duration was 60.1 months. Patients in group A had poor performance compared to other groups (p=0.015). five year overall survival (OS), disease free survival (DFS), locoregional free survival (LRFS), distant metastasis free survival (DMFS) were 83.2%, 76.0%, 80.6% and 77.2%, respectively. Sphincter preserving surgery rate (SPSR) for tumors located below 5 cm from anal verge was inferior in group A (76.9% vs. 93.7% vs. 89.3%, p=0.027, respectively). However, chemotherapy regimen was not related with other end points. In addition, there was no grade 3 or higher toxicity during treatment in all patients. Conclusions: Although SPSR was inferior for patients treated with one cycle of iv bolus 5-FU, there was no difference in other endpoints with acceptable toxicity. Results from current analysis may provide guidance in the treatment selection for patients with compromised performance.

        Background: Rheumatoid arthritis is a chronic illness that affects people both physically and psychologically. To date, many studies have been conducted for evaluation of the burden of caregivers in chronic disease such as stroke, dementia, however, RA has not been evaluated. A population-based study was conducted to evaluate the psychosocial impact of RA patients on their family members. Methods: From the fifth Korea National Health and Nutrition Examination Surveys (KNHANES V) (2010-2012) dataset, we identifi ed 452 RA patients and then selectedfamily members of these patients who were aged 20 years or older (n=515). The control group was sampled from members of families without RA patient with matching for sex and age (n=1,545). Serial conditional logistic regression models were used for comparison of characteristics between family members of RA patients and control group, and prediction of psychosocial impact, such as history of depression, of RA patients on their family memebers. Results: The mean age was 51. 9±18. 8 years. Family members of RA patients were more employed (63. 6% vs. 60. 2%, p = 0. 037), and had higher household income (p=0. 037) compared with sex and age-matched control subjects. No signifi cant differences in comorbidity between two groups. Family members of RA patients also had a signifi cantly higher level of stress (29% vs. 24. 3%, p=0. 046), history of depression (17. 3% vs. 11. 9%, p=0. 004). The presence of a RA patient in the family showed an association with history of depression (odds ratio, 1. 60; 95% confi dence interval, 1. 19 to 2. 16; p=0. 002), after adjustment for household income, education level, and employment status. Conclusions: Our data shows that family members of RA patients are more susceptible to depression. We suggest that physicians or rheumatologists who treat RA patients should pay attention to psychosocial burden of family members of their patients.

        Objectives: To examine the associations between chronic periodontitis (CP) and ankylosing spondylitis (AS) or between CP and clinical features of AS, and the effect of anti-TNFa therapy on periodontal treatment in Korean patients with AS. Methods: 84 AS patients and 84 age- and sex-matched healthy controls were enrolled. Clinical manifestations, disease activity and mobility parameters of AS, and periodontal parameters including probing pocket depth (PPD), clinical attachment loss (CAL), plaque index (PI), bleeding on probing (BOP) were assessed at baseline. Those with 4mm=PPD<7 mm on any tooth site had additional examinations at weeks 12 or 24 after periodontal treatment. Potential risk factors for CP including P. gingivalis infection were sought and adjusted. Results: No associations were found between CP and AS or clinical phenotypes of AS. No difference was found in PPD, CAL, PI, BOP, P. gingivalis carrier frequencies, or anti-P. gingivalis titers between patients and controls at baseline. No correlation was found between disease activities and periodontal parameters. However, those with CP showed decreased chest expansion and increased BASMI scores in univariate analysis with the latter being signifi cant in multivariate analysis. Both patients and controls showed improvement in PPD and CAL after periodontal treatment, irrespective of anti-TNFa therapy. The improvement was independently associated with AS group, P. gingivalis carriage, and baseline PPD>2. 6 mm. Conclusions: we could neither fi nd any association between CP and AS nor any correlation between disease activities and periodontal parameters. CP was not associated with any clinical phenotypes of AS, except for decreased mobility. Periodontal parameters improved after treatment in both patients and controls but more frequently in patients. Anti-TNFa therapy was not harmful to AS patients who had conservative periodontal treatment.

        Background: Endoscopic ultrasound (EUS) is widely used in recent days. However, there was no defi nite guideline for the management and the follow up period of small subepithelial tumor (SET) in upper gastrointestinal tract (GIT). Therefore, we evaluated the natural course of SELs using EUS and the characteristics of growing SETs. Methods: From October 2004 to June 2014, we retrospectively investigated the size changes, echogenicity, echogenic foci, and origin layer of SETs less than 30 mm in EUS fi ndings. The signifi cant growth of SET defi ned as increasing more than 25% of the longest diameter in the last EUS fi nding comparing the initial study. Results: A total of 131 upper GIT SETs in 122 patients were examined two more times using EUS. The median follow up interval for SETs was 25 months (range, 3 to 124 months). The location of SETs was 31 (23.7%), 93 (71.0%), 7 (5.3%) in esophagus, stomach, and duodenum, respectively. The majority of SETs were located in the 4th layer (90/131, 68.7%), and 117 SETs (89.3%) had hypogenicity and 107 SETs (81.1%) had homogenecity. Among 131 SETs, 28 SETs (21.4%) showed signifi cant increase in follow up EUS. However, initial size, echogenicity, presence of echogenic foci, layer of origin, and marginal regularity were not signifi cantly associated with the growth of the tumor. Conclusions: Although there were no significant relative factors about the SETs growth, however, about one fi fth of the SETs showed the size changes. Therefore, regular observation of SETs by using EUS might be needed.

        Roles of Rac and p38 kinase in the activation of cytosolic phospholipase A2 in response to PMA.

        <P>The roles of Rac and p38 kinase in the activation of cPLA2 (cytosolic PLA2) in Rat-2 fibroblasts were investigated. In the present study, we found that PMA activates cPLA2 by a Rac-p38 kinase-dependent pathway. Consistent with this, Rac, if activated, was shown to stimulate cPLA2 in a p38 kinase-dependent manner. In another experiment to understand the signalling mechanism by which the Rac-p38 kinase cascade mediates cPLA2 activation in response to PMA, we observed that PMA-induced cPLA2 translocation to the perinuclear region is completely inhibited by the expression of Rac1N17 or treatment with SB203580 (inhibitor of p38 kinase), suggesting that Rac-p38 kinase cascade acts in this instance by mediating the translocation of cPLA2. The mediatory role of p38 kinase in cPLA2 activation was further demonstrated after a treatment with anisomycin, a very effective activator of p38 kinase. Consistent with the mediatory role of p38 kinase in stimulating cPLA2, anisomycin induced the translocation and activation of cPLA2 in a p38 kinase-dependent manner.</P>
