        침습성 아스페르길수스증 진단을 위항 갈락토만난 항원검사의 유용성 평가

        Background : Invasive aspergillosis (IA) is an important cause of morbidity and mortality among immunocompromised patients. However, 1A is difficult to diagnose, especially in the aforementioned patient group. Recently, galactomannan assay (GMA) using enzyme immunoassay has been introduced in Korea. We evaluated its diagnostic usefulness in the diagnosis of 1A and we analyzed the results according to the underlying diseases. Materials and Methods : All patients who underwent GMA during the period from October 2007 to June 2008 were evaluated retrospectively. According to the criteria of European Organization for Research and Treatment of Cancer/Mycoses Study Group, IA was classified into four clinical categories: proven', probable', possible', and 'non' IA. Patients with 'proven' and 'probable' IA were used as the reference standards for IA. GMA was performed using Platelia Aspergillus EIA (Bio-Rad, Hercules, CA, USA). Optical density index ≥0.5 was considered positive. Results : Of the 144 patients who underwent GMA, two patients were classificed as proven' IA and sixteen patients were probable' 1A. Overall sensitivity, specificity, positive predictive value, and negative predictive value of the assay were 50% (95% confidence interval [C195), 26-74%), 88% (C195, 81-93%), 38% (CI95, 19-59%), and 93% (CI95, 86-97%), respectively. Among the 99 patients with underlying hematologic diseases, GMA showed 67% (C195, 35-90%) sensitivity and 89% (C195, 80-94%) specificity, whereas in 45 patients with underlying diseases other than hematologic diseases, sensitivity and specificity of the assay were 17% (CI95, 0-64%) and 87% (CI95, 73-96%), respectively. Conclusions : GMA showed high specificity, irrespective of the patient population. However, sensitivity of GMA was low and the assay was less sensitive in patients with underlying diseases other than hematologic diseases than in patients with hematologic diseases.

        The purpose of this study is to analyze walking accessibility to multi-family houses area and apartment complexes area using space syntax and discuss it from the viewpoint of child care environment. For this study, Bokdae1-dong and Gagyeong-dong that have the most infants in Cheongju City were selected as target places. Auto Cad 2018, depthmapX_0.35_windows, and SPSS 10.0 were used for analysis. Findings of this study are as follows: 1) Apartment complexes area is wider than multi-family houses area in a range of integration level. 2) For multi-family houses area, park/playground showed the highest integration level whereas children"s hospital showed the lowest one. For apartment complexes area, children"s hospital showed the highest integration level whereas park/playground showed the lowest one. However, there was no statistically significant difference. 3) When analyzing accessibility to spaces in multi-family houses area and apartment complexes area respectively according to blocks, it was found that there was a statistically significant difference in integration level of day care center, children"s hospital, and park/playground.

      • 정부 및 지자체의 귀농·귀촌을 위한 주거환경 지원정책의 유형 분석 - ‘2012 귀농·귀촌 페스티발’의 지자체 자료를 중심으로 -

        The purpose of this study was to suggest direction of government policy to support residential environment. For the purpose, the policies of central and local governments and materials issued for the introduction of policies to persons with plans to return to farm were collected and analyzed in order to explore focal policies. This study utilized content analysis method. In the first step, information on support policies for the return farm planners were collected from Internet. In this step, the residential environment support policies were categorized into two types; village support type and household support type. The village support was classified into central government support and local government support. Types and their characteristics of the policies were analyzed. In the second step, the materials disseminated at the ‘Return-Farm Festival, 2012’, held in Seoul between May 4 and 6, 2012, were analyzed. In the festival, 9 provincial governments and 39 local governments participated. Findings are as follows: (1) Governments were more active to introduce their policy online than offline such as participation in the festival; (2) The materials disseminated at the festival had more information on the household support policies than the village support policies. Local governments, however, emphasized their village support policies. The household support policies were mainly focused on repair support and purchase loan.

        지역커뮤니티 중심시설로서의 농촌폐교 활용에 관한 연구 : 일본의 농촌폐교활용 사례분석 Implications of Japanese Cases

        This study has its purpose to establish a model for the use of closed schools as core facility of rural community. The closed schools would be used for various activities that residents want, just like health care, medical care, welfare, side-job training and convenience upgrading activities. Japanese cases in this field give us good implications, so that we visited 5 closed schools in Totsuki-ken and Ibaraki-ken for interviews. The results of interviews with facility managers are as follows. (1) For the decision on the usage of facilities, district administration officers, residents, private companies and NPO(Non-Profit Organization) join the procedure. (2) The usage is focused to enhance elderly welfare, to increase household income, and to enforce community relationship. There are facilities with mixed usage of various functions.Because closed schools have symbolic meanings for the community, sufficient discussion with residents is essential for the decision. And social supports as NPO law and nursing insurance of Japan would be indispensible for the successful implementation.

        농촌지역 폐교의 노인복지시설 및 주민 커뮤니티센터 활용에 관한 연구 : 거주자의 생활패턴과 지역시설 이용실태를 중심으로 Life Pattern and Use of Facilities of Rural Residents

        Closed schools in rural areas are hardly used for the residents. The purpose of this study is to suggest the possibility to use these facilities for elderly welfare spaces or community centers of residents. For this purpose this article studied life pattern and use of facilities of rural residents. Interviews on the use of leisure time, additional jobs other than agriculture and the use of facilities were conducted on 91 residents living in the educational area of 2 preliminary schools which were just closed. The 2 schools-Seongdae branch school of Baekgok preliminary school and Ohsang branch school of Gujeong preliminary school- are located in Jincheon-gun, Chuncheongbuk-do. Rural residents have relatively much spare time more than 6 hours a day, but their leisure activities are very restrictive in diversity. Economic burden, lack of time in their agricultural period and inconvenient traffic ask closed schools near and familiar to the residents to be converted to the compound community center with diverse functions and leisure programs. It would improve the life quality of agricultural residents and lessen their economic burden.

        반복적인 부비동염으로 내원한 Good 증후군 1예

        Good 증후군은 흉선종과 면역결핍이 동반되는 질환으로, B세포의 감소로 인한 범저감마글로불린혈증과 CD4^(+)T세포의 감소로 인한 세포성 면역저하가 동시에 나타나는 것을 특징으로 한다. 본 증례는 18개월 전부터 발생한 반복적인 부비동염을 주소로 내원한 43세 남자 환자로, 내원 1년 전 흉선종절제수술을 시행받았으며, 면역학적 검사상 범저감마글로불린혈증과 T세포의 감소 등의 이상 소견이 발견되었다. 정주 면역글로불린과 경험적 항생제(amoxicillin/clavulanic acid) 사용 후 부비동염이 호전되었으며, 예방접종과 정기적인 면역글로불린 투여 후 특이 합병증없이 경과 관찰 중이다. Good's syndrome is the association of thymoma with immunodeficiency, characterized by hypogammaglobulinemia, B-cell lymphopenia and variably defects in cellular immunity with CD4^(+) T-cell lymphopenia and an inverted CD4^(+):CD8^(+) T-cell ratio. We report a 43-year-old male patient who presented with a 18-month history of productive cough and postnasal drip. One year ago, he underwent the operation for resection of a thymoma. Despite of appropriate management, sinusitis relapsed multiple times. He was found to have hypogammaglobulinemia with nearly absent B cells(4/μL). The CD4^(+) T-cell count was 554/μL with an inverted CD4^(+):CD8^(+) T-cell ratio of 0.6. His symptoms and signs improved with antibiotic treatment and monthly administration of intravenous immunoglobulin (IVIG, 400 mg/kg).

      • MYC quantitation in cell-free plasma DNA by real-time PCR for gastric cancer diagnosis

        <P>BACKGROUND: Detection of tumor-associated genetic alterations in plasma of cancer patients has recently been suggested to be an accurate method for detecting early or recurrent cancer. METHODS: We performed quantitative real-time PCR for MYC and GAPDH in tissue and plasma samples of 57 patients with gastric cancer and in plasma of 79 cancer-free individuals. We also performed two-color MYC fluorescence in situ hybridization (FISH) in tissue from the 57 patients with gastric cancer. RESULTS: The tissue MYC/GAPDH ratio by real-time PCR was significantly correlated with MYC status by FISH (p<0.001). The mean ratio of plasma MYC/GAPDH was 5.226+/-3.578 (range: 1.25-18.35) in gastric cancer patients, and 2.436+/-0.881 (range: 1.00-5.00) in the healthy volunteers (p<0.001). We used receiver-operating characteristics (ROC) curve analysis to select two optimal plasma MYC/GAPDH cut-offs of 2.725 and 5.225. The sensitivity and specificity were 75.4% and 76.9% at 2.725, 38.6% and 100% at 5.225, respectively. The plasma MYC/GAPDH ratio from cancer patients was significantly correlated with the tissue MYC/GAPDH ratio (p=0.009), and tissue MYC status by FISH (p=0.024). CONCLUSIONS: These findings suggest that the plasma MYC/GAPDH ratio, as determined by real-time PCR, may be an alternative non-invasive approach for detecting gastric cancer.</P>

      • 멀티미디어 교육용 콘텐츠의 정성적 평가지침 개발

        현재의 지식 사회에 대응하기 위하여 교육인적자원부, 시·도 교육청 각종 교육관련 기관은 다량의 교육용 콘텐츠를 개발하고 보급하였다. 그러나 교육용 콘텐츠에 대한 질 관리에 대해서는 개념도 불명확하고 법이나 규정도 미비하여 국가수준의 질 관리방법이나 절차도 체계성을 가지지 못하고 있어 양질의 자료 선별과 품질이 관리된 특화된 정보를 사용자에게 제공하는 기회가 부족하였다. 본 연구는 양질의 콘텐츠를 선별 탑재하기위해 기존의 정량적 평가 방법에 정성적 평가 방법을 보완하여 콘텐츠 특성에 맞는 평가를 할 수 있는 지침을 마련하였다.

        수도권 타운하우스 분양성 제고를 위한 마케팅 전략

        This study has a purpose of suggesting marketing strategies leading to the successful sales of the town houses which witness a boom recently as a new type of residence. Nineteen town house complexes, presented in the market in 2004 to 2007, are the object of this study, and their physical characteristics along with supply conditions were studied from the viewpoint of the marketing mix of 'product', 'price', 'place' and 'promotion'. The questionnaire was accompanied with the study, asking demand side of customers. It has comprised fifty persons of middle and upper class with more than 5 million wons of monthly income. Following is what we suggest as marketing strategies. (1) Product: The residence for undisturbed life is preferred, so that detached type or maisonette(duplex) type residences have to be supplied. The parking space for at least three vehicles has to be allotted to each house. The space for common use and service system for life has to be emphasized. The unit size of house has to be large enough with 60 to 70 pyeongs. The interior is recommended to have cozy, natural and elegant image, and eco-friendly facilities are preferred more. (2) Price: The sales price per 3.3㎡ should be lower than 14 million wons. (3) Place: The considerations for the location of town houses have to be surrounding nature, conveniences and traffic. (4) Promotion : The way of selling town houses has to be the 'open market' type. Big constructors have chances to establish their own private brands, whereas smaller constructors have to try to enhance consumers' recognition by means of inviting well-known architects to their projects.
