        Diagnosis and Treatment of Dyssynergic Defecation

        Dyssynergic defecation is common and affects up to one half of patients with chronic constipation. This acquired behavioral problem is due to the inability to coordinate the abdominal and pelvic floor muscles to evacuate stools. A detailed history, prospective stool diaries, and a careful digital rectal examination will not only identify the nature of bowel dysfunction, but also raise the index of suspicion for this evacuation disorder. Anorectal physiology tests and balloon expulsion test are essential for a diagnosis. Newer techniques such as high-resolution manometry and magnetic resonance defecography can provide mechanistic insights. Recently, randomized controlled trials have shown that biofeedback therapy is more effective than laxatives and other modalities, both in the short term and long term, without side effects. Also, symptom improvements correlated with changes in underlying pathophysiology. Biofeedback therapy has been recommended as the first-line of treatment for dyssynergic defecation. Here, we provide an overview of the burden of illness and pathophysiology of dyssynergic defecation, and how to diagnose and treat this condition with biofeedback therapy.         Evaluation and Identification of Promising Bivoltine Double Hybrids of the Silkworm Bombyx mori L. for Tropics Through Large Scale In-House Testing

        An attempt was made to assess the potentiality of bivoltine double hybrids under simulated conditions of farmers to identify the suitable bivoltine double hybrid combination. Four bivoltine double hybrids developed at Central Sericultural Research and Training Institute (CSRTI), Mysore along with popular single hybrid, CSR2 x CSR4 as control was assessed for economic traits. The rearing results showed significant improvement of 20-24% in fecundity of the double hybrids studied over single hybrid. Among the double hybrids, [D7 ×S5]×[D13×S1] recorded significantly higher survival (89.58 %), cocoon yield (76.328 kg/50,000 eggs), cocoon price (Rs. 180.87/kg) and lower cocoon leaf ratio of 1: 21.80. The performance of the reeling traits were also found significantly superior in [D7×S5]×[D13×S1] with higher filament length (1100 m), reelability (88%), raw silk (18.55%) and neatness (92 points) compared to CSR2 × CSR4 and other double hybrids evaluated. Besides, the cocoons of [D7×S5]×[D13×S1] exhibit uniformity in size with a standard deviation of < 8. Overall data indicated the superiority of [D7×S5] × [D13×S1] compared to the other hybrids evaluated and it has profound influence in expressing the full potentiality in the field.

        Prevalence of Disaccharidase Deficiency in Adults With Unexplained Gastrointestinal Symptoms

        Background/Aims Disaccharidase assay is used for assessing carbohydrate intolerance in children, but its usefulness in adults is not known. The aim of this study is to assess the prevalence of disaccharidase deficiency in patients with unexplained gastrointestinal symptoms. Methods A retrospective review of adults with chronic (> 1 year) abdominal symptoms and negative imaging and endoscopy/colonoscopy and who completed bowel symptom questionnaire and duodenal biopsy for lactase, maltase, sucrase, and palatinase was performed. A subset also underwent 25 g lactose breath test (LBT). Results One hundred twenty patients (females = 83) were evaluated, of whom 48 also underwent LBT. Fifty-six (46.7%) patients had enzyme deficiency; 44 (36.7%) had single (either lactase or maltase), 1 had 3 enzyme deficiencies, 11 (9.2 %) had all 4 disaccharidase enzyme (pan-disaccharidase) deficiency, and 64 (53.0%) had normal enzyme levels. Baseline prevalence and severity of 11 gastrointestinal symptoms were similar between normal and single enzyme deficiency groups. The sensitivity and specificity of LBT was 78.3% and 72.0%, respectively and overall agreement with lactase deficiency was 75.0%. Conclusions Isolated disaccharidase deficiency occurs in adults, usually lactase and rarely maltase, and pan-disaccharidase deficiency is rare. Baseline symptoms or its severity did not predict enzyme deficiency. (J Neurogastroenterol Motil 2020;26:384-390)

        Update on the Pathophysiology and Management of Anorectal Disorders

        Anorectal disorders are common and present with overlapping symptoms. They include several disorders with both structural and functional dysfunction(s). Because symptoms alone are poor predictors of the underlying pathophysiology, a diagnosis should only be made after evaluating symptoms and physiologic and structural abnormalities. A detailed history, a thorough physical and digital rectal examination and a systematic evaluation with high resolution and/or high definition three-dimensional (3D) anorectal manometry, 3D anal ultrasonography, magnetic resonance defecography and neurophysiology tests are essential to correctly identify these conditions. These physiological and imaging tests play a key role in facilitating a precise diagnosis and in providing a better understanding of the pathophysiology and functional anatomy. In turn, this leads to better and more comprehensive management using medical, behavioral and surgical approaches. For example, patients presenting with difficult defecation may demonstrate dyssynergic defecation and will benefit from biofeedback therapy before considering surgical treatment of coexisting anomalies such as rectoceles or intussusception. Similarly, patients with significant rectal prolapse and pelvic floor dysfunction or patients with complex enteroceles and pelvic organ prolapse may benefit from combined behavioral and surgical approaches, including an open, laparoscopic, transabdominal or transanal, and/or robotic-assisted surgery. Here, we provide an update on the pathophysiology, diagnosis, and management of selected common anorectal disorders. (Gut Liver 2018;12:375-384)

        An Empirical Formula for Mode-II Fracture Energy of Concrete

        This paper presents an experimental investigation to understand the influence of Maximum Size of the Aggregate (MSA) andcompressive strength of concrete on the Mode-II fracture parameters (Fracture energy (GIIf) and Brittleness number) of normal and highstrength of concrete. An empirical formula for Mode-II fracture energy (GIIf) based on the Bazant’s size effect law is developed in termsof maximum size of aggregate and the compressive strength of concrete. A linear constitutive relationship between shear stress and slipi.e., the linear softening curve for Mode-II fracture of concrete is presented by conducting experiments on the double edge notchedspecimen. The experimental results indicated that increase in the grade of concrete (compressive strength) increases the brittleness of themember and Mode-II fracture energy and also increase in the maximum size of aggregate increases the Mode-II fracture energy.

        Parametric Performance Investigation on Membraneless Microfluidic Paper Fuel Cell with Graphite Composed Pencil Stoke Electrodes

        Membraneless Microfluidic Paper Fuel Cells (MMPFCs) are promising devices to harvest energy for various portable applications. This is primarily because such devices eliminate the need for membrane, and external pumps due to the builtin co-laminar flow, and embedded capillaries, respectively. However, these devices have enormous scope to optimize their performance. Herein, investigations on MMPFC have been carried out with varying electrode separation, whereby cellulose papers acts as the microchannel of MMPFC. Here, various grades of carbon-supported graphite pencils serve as electrodes, formic acid acts as a fuel, and sulphuric acid works as an electrolyte. Oxygen from the atmospheric air acts as an oxidant for ionic exchange under co-laminar fl ow in the MMPFC. These MMPFCs with the optimized configuration provide enhanced energy densities with different grades of the microchannel material along with variation in electrode separation. Leveraging grade 1 Whatman cellulose paper as MMPFC microchannel with 1 mm electrode separation, the MMPFC generates energy with a maximum open-circuit potential of 0.49 V, a maximum current density of 2.45 mAcm −2 and power density of 0.26 mWcm −2 .

        Bile Reflux Gastropathy and Functional Dyspepsia

        Background/Aims The pathoetiology of functional dyspepsia remains unclear; one mechanism could be chemical gastropathy from chronic bile reflux. We aim to examine the association of bile reflux gastropathy with functional dyspepsia and identify predisposing factors. Methods In a retrospective study, patients with functional dyspepsia (Rome III) who completed symptom assessment, esophagogastroduodenoscopy, and biopsies were categorized into 3 groups; bile gastropathy (BG), non-bile gastropathy (NBG), and no gastropathy (NG). Demographics, symptoms, endoscopy, and motility data were compared between groups. Multivariate analysis identified clinical factors associated with BG. Results Of 262 patients (77.5% female), 90 had BG, 121 had NBG, and 51 had NG. Baseline demographics were similar, however, patients with BG reported significantly more severe abdominal pain than NBG or NG groups (P = 0.018). Gastric erythema was significantly more common in BG vs NBG groups (P < 0.001). Cholecystectomy was significantly associated (OR, 6.6; P = 0.003) with the presence of gastropathy in BG compared to NBG or NG group. Patients with cholecystectomy had significantly more severe abdominal pain (P < 0.05), gastric erythema (P < 0.03), and gastritis (P < 0.05), and were more likely to be prescribed narcotic medications (P < 0.004) than patients without cholecystectomy. Conclusions Bile reflux gastropathy is associated with functional dyspepsia and causes more severe symptoms. Cholecystectomy predisposes to BG and abnormal pain, and could contribute to the pathogenesis of functional dyspepsia. (J Neurogastroenterol Motil 2021;27:400-407)

      • High Cell Density Cultivation for the Production of Industrially Important Engineered Bi-functional Recombinant Staphylokinase Variant from Salt Inducible Escherichia coli GJ1158

        Most of the bacterial and other simple non glycosylated recombinant proteins were conventionally produced from IPTG inducible Escherichia coli BL21(DE3). Considering the factors like cost and toxic nature of IPTG, as an alternative, salt inducible Escherichia coli GJ1158 was used in this study for the over production of staphylokinase variant (sak – hirulog) using fed batch fermentation, cost effectively. Optimization of physico chemical factors viz., dissolved oxygen (DO), carbon, nitrogen and phosphate sources on bacterial growth for the production of recombinant sak – hirulog using batch and fed batch fermentation was studied. In batch culture, increased DO at more than 30 % did not influence the enhanced expression of sak – hirulog, but significant improvement was observed in fed batch cultivation. Supplementation of production medium with different nutrient sources like dextrose, yeast extract and dipotassium hydrogen phosphate (K2HPO4) enhanced the sak – hirulog expression in fed batch fermentation process even without disturbing the cell growth by providing 50 % DO. Approximately 1178 mg/L of specific protein was obtained using cost effective modified glucose yeast exgtract (GYE) media devoid of sodium chloride. This study also reports the highest concentration of recombinant protein from salt inducible expression host till to date, which manages to satisfy the production of bifunctional staphaphylokinase variant using economically feasible bacterial expression host Escherichia coli GJ1158.
