      • 소아간질환자에서 Valproate 투여군과 Carbamazepine 투여군의 혈청 Leptin 및 Insulin농도에 관한 비교연구

        ■ Objectives Valproate(VPA) is an effective anticonvulsant in many type of epilepsy but weight gain has been recognized as an adverse effect of valproate therapy and the mechanism is inevitable. To evaluate the effect of prolonged anticonvulsant therapy on obesity, we compared obesity Index, body mass index(BMI) and serum levels of leptin and insulin in both VPA and carbamazepine(CBZ) groups. ■ Methods and Methods Serum leptin and insulin levels were measured in 103 epileptic patients recieving VPA monotherapy(72 males. 31 females) and 66 patients recieving CBZ monotherapy(35 males, 31 females). Obesity index and BMI were measured by Inbody 3.0(Micromedic System Inc., U.S.A.) Serum samples were gained after overnight fast and were frozen -20degreesC until analyzed. Radioimmunoassay using Human leptin and insulin RIA kit (Linco Research co., St. Charles, U.S.A.) was performed. ■ Results 1) Body mass indicies were significatly higher in VPA group(19.62±3.75㎏/㎡) than CBZ group(18.10±3.17㎏/㎡)(p=0.009). 2) Serum leptin levels were significantly higher in VPA group(6.49±4.42ng/Mℓ) than CBZ group(2.27±2.77ng/Mℓ) (p=0.007). 3) Serum insulin levels were significantly higher in VPA group(23.28±19.47mU/L) than CBZ group(20.49±13.90mU/L) (P=0.03). ■ Conclusions Weight gain might be long-term complication of valproate monotherapy and correlate with the elevation of serum leptin and insulin levels.

        「성매매 알선 등 행위의 처벌에 관한 법률」의 개정방향 : 성매매 여성의 권익보호를 중심으로

        The controversy over the effectiveness of The Act on the Punishment of Intermediating in Sex Trade and Associated Acts has been going on since the legislation and enforcement of this law. Especially in the spring of 2011 the attempts to eradicate sex trade in the red light districts in Yeongdeungpo, mainly led by Seoul metropolitan police agency met with failure because of organized responses and fierce protests from pimps and women involved in sex trade. Their main argument is the law just makes ordinary people criminals and is no use restraining or reducing sex trade industry. In addition, in December 2012 The Seoul Northern District Court has requested The Constitutional Court to rule on the constitutionality of the penal provision that punish ‘anyone who (voluntarily) trade sex’. This incident raised issues not only about the effectiveness of law but the invasion of constitutional rights. But sex trade is fundamentally not the same as general trade because sex trade is very serious human rights violation. So the intermediaries (for typical example, pimps) who gained immense profits from this business and clients who buy sex all deserve to be severly punished, then we shouldn’t deny the need for this law. After this request of the court anti sex trade agencies anticipate the constitutional court will decide to decriminalize only women(part of providing sex) and on the other hand make much efforts to revise this provision in the direction of decriminalizing women. By the way the law says ‘victims of the sex trade shall not be subjected to punishment for sex trade’. So Anti sex trade agencies also try to revise the definition of victims of sex trade toward enlarging the concept of victims and rearranging the provisions related to protective procedure for women involved in sex trade during police investigation and trial. This is the second-best solution in preparation for possibilities that the decision of Constitutional Court doesn’t meet our hope or the movement for revision in the direction of decriminalizing women is frustrated. Also it is necessary to reinforce punishment of a person who intermediate in sex trade and associated acts, employ or recruit another person for the purpose of sex selling, introduce work with knowledge that sex trade occurs and advertise activities or agencies where such activities are carried out above mentioned. And it’s needed to adopt the new penal provision that regulate the advertisement on sex trade through the internet. This paper is mainly about the revision direction of act on the punishment of intermediating in sex trade and associated acts for the right of women involved in sex trade. 「성매매 알선 등 행위의 처벌에 관한 법률」의 실효성 논란은 법 제정 및 시행이후 계속되어 왔다. 특히 2011년 봄에는 경찰이 주축이 된 집결지의 집중단속을 통한 영업 폐쇄 계획에 반대하는 성매매 알선업주들의 연합체인 한터의 조직적 대응 및 성매매 여성들의 격렬한 시위로 이러한 논란이 증폭되었었고, 또한 2012년 12월에는 서울북부지방법원이 위 법률의 성매매자 처벌조항에 대해 위헌제청을 함으로써 법 자체의 실효성은 물론 법의 기본권 침해 논의까지도 본격적으로 불거졌다. 그러나 성매매는 중대한 인권침해행위라는 점에서 근본적으로 일반적인 매매계약과는 다르고 업주 등 성매매 알선행위자들 및 성구매자는 중대하게 처벌받아야 한다는 점에서 법 자체의 실효성을 부정할 수는 없다. 이번 위헌제청을 계기로 성매매 피해상담소는 성매매 여성 즉, 성을 구매하는 자의 상대방이 된 자는 처벌하지 않는 헌법재판소의 일부위헌결정을 기대하며 한편으로는 위와 같은 성매매 여성의 비범죄화 취지를 반영한 법률 개정을 위한 노력을하고 있다. 그런데 아직 헌법재판소의 결정이나 법률개정여부가 확정되지 않은 상황 속에서는 특정한 요건이 충족되면 성매매 여성도 처벌받지 않도록 하고 있는 현행법상 성매매 피해자 조항을 확대하고 수사절차 등에서 성매매 여성을 보호하는 규정을 정비하는 노력도 필요하다. 여성만의 비범죄화를 인정하는 취지의 헌법재판소의 결정이나 그러한 내용의 법률개정이 이루어지지 못할 가능성에 대비한 차선책이라고 할 수 있다. 또한 업주 등의 처벌조항의 법정형을 상향조정하고 인터넷을 통한 성매매 광고 등에 대한 처벌 조항을 도입하여 업주 등의 확실한 처벌을 통한 성산업 근절 방안도 마련해야 한다. 이 논문은 성매매 여성의 비범죄화 및 차선책으로서의 성매매 피해자 조항의 내실화 등 성매매 여성을 보호하기 위한 법률개정을 중점적으로 논의하고 있다.

        ANATOMICAL EXPANDER와 IMPLANT를 이용한 유방 재건의 경험

        The use of a tissue expander and implant is the simplest option for breast reconstruction after mastectomy. Use of a round dome-shaped prosthesis and the commonly used one-stage technique with a Becker type prosthesis, however, often produces an undesirable upper pole fullness. To overcome this and to achieve improved aesthetic results, we started using an anatomically-shaped expander and implant, as described by Maxwell, as a two-stage breast reconstruction. We reviewed the results of our 21 reconstructed breasts in 22 patients who were deemed suitable for reconstruction using this technique since January 1995. The most commonly used expander was 350cc (range 350-550cc) and an average of 4.2 inflations were required before replacing the expander with a permanent implant. All the expanders were placed in submuscular pockets and implant volume. The longest follow-up was 36 months. Few complications developed and most patients were satisfied with the results. We found that the anatomically-shaped expander and implant produced better aesthetic results compared to a done-shaped prosthesis.

        Running title: Cases of anesthesia for extremely low birth weight infant. Extremely low birth weight infants (birth weight < 1000 g) are prone to various morbidities such as respiratory distress syndrome, intraventricular hemorrhage, periventricular leukomalacia, patent ductus arteriosus, necrotizing enterocolitis and retinopathy. To accomplish successful anesthetic management, many precautions must be continuously taken during the operation. First, inspired oxygen concentration should be adjusted to avoid oxygen toxicity. Second, body temperature must be maintained adequately. Third, hemodynamic parameters should be kept stable. We report 2 cases of successful anesthetic management for extremely low birth weight infant who underwent ligation of patent ductus arteriosus at the neonatal intensive care unit.

      • Highly efficient plasmonic organic optoelectronic devices based on a conducting polymer electrode incorporated with silver nanoparticles

        <P>Highly efficient ITO-free polymeric electronic devices were successfully demonstrated by replacement of the ITO electrode with a solution-processed PEDOT:PSS electrode containing Ag nanoparticles (NPs). Polymer solar cells (PSCs) and light emitting diodes (PLEDs) were fabricated based on poly(5,6-bis(octyloxy)-4-(thiophen-2-yl)benzo[<I>c</I>][1,2,5]thiadiazole) (PTBT):PC<SUB>61</SUB>BM and Super Yellow as a photoactive layer, respectively. The surface plasmon resonance (SPR) effect and improved electrical conductivity by the Ag NPs clearly contributed to increments in light absorption/emission in the active layer as well as the conductivity of the PEDOT:PSS electrode in PSCs and PLEDs. The ITO-free bulk heterojunction PSCs showed a 1% absolute enhancement in the power conversion efficiency (3.27 to 4.31%), and the power efficiency of the PLEDs was improved by 124% (3.75 to 8.4 lm W<SUP>−1</SUP>) compared to the reference devices without Ag NPs. The solution-processable conducting polymer, PEDOT:PSS with Ag NPs, can be a promising electrode for large area and flexible optoelectronic devices with a low-cost fabrication process.</P> <P>Graphic Abstract</P><P>A solution-processed PEDOT:PSS electrode incorporated with silver nanoparticles can be a promising candidate for replacing indium-tin-oxide in organic optoelectronic devices. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c3ee40190a'> </P>

        Clinical Research Article : Comparison of the effects of acetaminophen to ketorolac when added to Lidocaine for intravenous regional anesthesia

        Background: This study was done to evaluate the effect on pain relief when acetaminophen was added to lidocaine for intravenous regional anesthesia (IVRA). Methods: Sixty patients undergoing hand or forearm surgery received IVRA were assigned to three groups: Group C received 0.5% lidocaine diluted with 0.9% normal saline to a total volume of 40 ml (n=20), Group P received 0.5% lidocaine diluted with intravenous acetaminophen 300 mg to a total volume of 40 ml (n=20) and Group K received 0.5% lidocaine diluted with 0.9% normal saline plus ketorolac 10 mg made up to a total volume of 40 ml (n=20). Sensory block onset time, tourniquet pain onset time, which was defined as the time from tourniquet application to fentanyl administration for relieving tourniquet pain and amount of analgesic consumption during surgery were recorded. Following deflation of tourniquet sensory recovery time, postoperative pain and quantity of analgesic uses in post-anesthesia care unit were assessed. Results: Sensory block onset time was shorter in Group P compared to Group C (P<0.05). Tourniquet pain onset time was delayed in Group P when compared with group C (P<0.05). Postoperative pain and analgesic consumption were reduced in Group P and Group K compared to Group C (P<0.001). Conclusions: The addition of acetaminophen to lidocaine for IVRA shortens the onset time of sensory block and delays tourniquet pain onset time, but not with ketorolac. Both acetaminophen and ketorolac reduce postoperative pain and analgesic consumption. (Korean J Anesthesiol 2010; 58: 357-361)

        Phytochemical Constituents from the Leaves of Soybean [Glycine max (L.) Merr.]

        Phytochemicals study from the leaves of soybean [Glycine max (L.) Merr.], one of Korean edible plant materials were investigated through various chromatographic procedures. The methanolic leaves extracts of soybean yielded 16 phytochemicals, including 5 isoflavones 1-5, 3 flavones 6-8, 1 flavonol 9, 2 pterocarpans 10 and 11, 2 phenolic compounds 12 and 13, 2 phytosterols 14 and 15, and 1 sugar alcohol 16. The structures were fully characterized by analysis of physical and spectral data and were defined clearly as 4,5,7-trihydroxyisoflavone (1), 4,5,7-trihydroxyisoflavone-7-O-β-D-glucopyranoside (2), 4,7-dihydroxy-6-methoxyisoflavone (3), 4,7-dihydroxyisoflavone (4), 4,7-dihydroxyisoflavone-7-O-β-D-glucopyranoside (5), 5,7,4"-trihydroxyflavone (6), 3",4",5,7-tetrahydroxyflavone (7), 3",4",5-trihydroxyflavone-7-O-β-D-glucopyranoside (8), 3,4",5,7-tetrahydroxyflavonol (9), coumestrol (10), glyceofuran (11), 4-hydroxybenzoic acid (12), methyl-4-hydroxybenzoate (13), soyasapogenol B (14), stigmasterol (15), and D-mannitol (16), respectively. Among them, phytochemicals 7-16 were reported for the first time on the isolation and confirmation from the leaves of this species. These results suggest that the leaves extracts of soybean may possess possible health related benefits to human due to the isolated phytochemicals 1-16 which have been well known potential effects on various chronic diseases.

        Effects of 3-Dimensional Lumbar Stabilization Training for Balance in Chronic Hemiplegic Stroke Patients: A Randomized Controlled Trial

        Objective To investigate the effects of the newly developed Spine Balance 3D system on the balance and gait abilities of hemiplegic stroke patients.Methods Twenty-eight hemiplegic patients with chronic stroke were randomly assigned to an experimental (n=14) or control group (n=14). The experimental and control groups performed balance training by using the newly developed Spine Balance 3D system and the well-known Biodex Balance System 30 minutes per day, three times a week for 7 weeks. The Berg Balance Scale (BBS), 10-m walking test (10mWT), Timed Up and Go Test (TUG), Functional Reach Test (FRT), the Korean version of the Fall Efficacy Scale-International (KFES-I), trunk muscle strength and stability were evaluated before and after 7 weeks of intervention.Results The 10mWT improved significantly (p=0.001) in the experimental group (using the Spine Balance 3D system) but not in the control group, and core muscle strength, which we checked using Spine Balance 3D system evaluation program, improved more in the experimental group as well. The results of the BBS, FRT, TUG, KFES-I, and Biodex Balance System evaluation program improved in both groups after 7 weeks of balance training.Conclusion We suggest that the newly-developed Spine Balance 3D system can be a more useful therapeutic tool for gait and dynamic balance rehabilitation in hemiplegic patients than a conventional 2D-based balance training system. A large-scale randomized controlled study is needed to prove the effect of this system.
