      • LC, Acute : O-061 ; Mode of cirrhosis related complication in Korean patients with liver cirrhosis -A prospective multicenter cohort study

        Background: Cirrhosis-related complications are indications for transplantation. However, the mode of complication has not been evaluated so far. To better address this issue, we investigated the mode of cirrhosis related complication and impact of these complications on survival in prospective, multicenter, inception cohort study of subjects with liver cirrhosis presenting with the first onset of decompensated complication. Methods: In this observational study, 1515 patients with the confirmed onset time of the first decompensated complication were enrolled. The mortality risk of cirrhosis-related complications, including ascites, portal hypertensive gastrointestinal bleeding (PHGB), spontaneous bacterial peritonitis (SBP), hepatic encephalopathy (HE), hepatorenal syndrome (HRS), was analyzed in these cirrhotic patients. Result: As the first complication, ascites was the most frequent (53.8%), followed PHGB (36.2%), HE (7.6%), SBP (1.7%) and HRS (0.7%). During follow-up (mean 32±47 months), 484 (31.9%) experienced subsequent decompensations and the proportion of subsequent complication was consistently changed; the proportion of HE, SBP, HRS increased as time progressed. The interval between subsequent decompensative events was progressively shortened. The mortality rate according to the first complication was as follows; Ascites =PHGB> HE=SBP>HRS. The mortality rate of patients experienceing subsequent complication was significantly higher than those without subsequent complication. According to the kinds of subsequent complication, survival of HE was also higher than HRS but lower than ascites or PHGB and similar to that of SBP and this difference of survival disappeared after repeated complication. Conclusion: The mortality risk correlated with initial complication and the risk increases as the number of complication episodes is repeated. Patient`s mean age was 56±11.50 years and 72.1% was male. The cause of liver cirrhosis is hepatitis B virus (38.6%), alcohol (35.5%) and hepatitis C virus (8.9%).

      • Preclinical safety evaluation of hepatic arterial infusion of oncolytic poxvirus

        <P><B>Purpose</B></P><P>Oncolytic poxvirus has shown promise in treating various solid tumors, such as liver cancer, and administration of oncolytic poxvirus via the hepatic artery may provide more survival benefits than other routes of administration. However, there is a lack of safety information to guide the application of hepatic arterial infusion (HAI) of oncolytic poxvirus in human studies. To investigate the acute and chronic toxicity of HAI administration of oncolytic poxvirus in animals and provide safety information for future human studies.</P><P><B>Methods</B></P><P>VV<SUP>tk−</SUP>, a vaccinia poxvirus with inactivated <I>thymidine kinase</I> gene, was administered via HAI to rabbits with normal liver function under angiography (1×10<SUP>8</SUP> or 1×10<SUP>9</SUP> pfu), and rats with N-nitrosomorpholine-induced precancerous liver cirrhosis under open surgery (1×10<SUP>8</SUP> pfu). Body weights and survival were monitored and blood samples were collected for hematological and biochemical tests. Distribution of A56 (a specific marker for poxvirus infection) in rabbit organs was evaluated using immunofluorescence assays.</P><P><B>Results</B></P><P>HAI of high doses of VV<SUP>tk−</SUP> did not cause any acute or chronic changes in body weight, survival or in biochemical, hematological tests in the 2 animal models, and none of the changes showed dose dependency (in rabbit study), or were influenced by liver cirrhosis (in rat study). A56 was not detected in any of the major rabbit organs.</P><P><B>Conclusion</B></P><P>HAI may provide a safe alternative route of oncolytic poxvirus administration for human studies.</P>

        5-FU 내성 대장암 세포주에서 생물학적 지표로서 단백 분해효소발현의 유용성

        N/A There have been many reports that colon cancer responds poorly to chemotherapy. Several classes of matrix metalloproteinases(MMPs) have been implicated in the process of invasion of epithelial and endothelial basement membranes in several steps of tumor invasion and metastasis. This study was performed to determine the biologic behavior and the histopathological characteristics of a 5-FU resistant colon cancer cell line. Methods : We performed several biologic assays including liver colonization assay, cell adhesion assay, invasion assay and zymogram for protease activity using parental HT29 cell and 5-FU resistant HT29 cell (HT29-FU cell). Results : In liver colonization assay, HT29-FU cell revealed a 2.5-fold increase in the liver weight and tumor burden compared with HT29 cell. HT29-FU cell showed moderate increase in adhesion and invasion assays in comparison to HT29 cell. HT29-FU cell revealed increased activity of MMPs and serine protease. Xenograft tumors of HT29-FU cell formed moderately differentiated adenocarcinoma with more glandular formations of mucin. Conclusion : The increased expression of MMPs in 5-FU resistant colon cancer cell can explain poor prognosis. These are potentially poor prognostic indicators in 5-FU resistant colon cancer. Consequently, it can be suggested that modulation of MMPs is needed to prevent invasion and metastasis in colon cancer by using inhibitors of these enzymes. (Korean J Med 57:323-332, 1999)

        우췌양혈관종 ( Verrucous Hemangioma ) 의 2 예

        Verrucous hemangioma is a variant of a capillary, cavernous, or mixed hemangioma which undergoes a reactive proliferative epidermal change. Most verrucous hemangiomas appear on the lower extremities, and most are present at birth or appear during childhood. We observed two patients who showed typical clinical and histopathological findings of verrucous hemangioma. Treatment was done by wide surgical excision and skin graft. The first male patient had a good therapeutic result but the second female patient had a recurrence at one month post-operative.

      • 라미부딘 내성 만성 B형간염 환자에서 아데포비어 치료 중 HBV polymerase 의 변이양상

        Purpose: Adefovir dipivoxil (ADV) is a nucleotide analogue that inhibits wild-type hepatitis B virus (HBV) and lamivudine (LMV)-resistant HBV mutants. ADV-resistant mutation has been reported to occur frequently in patients with LMV-resistance. The aim of this study was to evaluate the frequency of ADV-resistant mutations and pattern of mutants in the patients with LMV-resistant chronic HBV infections. Subject and Methods: This study consisted of 124 chronic HBV infected patients who had LMV-resistant HBV mutants. The duration of LMV therapy was 27.5 (±14.0) months, and that of ADV was 26.6 (±6.9) months. Serum HBV DNA was tested by Cobas Amplicor HBV Monitor test (Roche Diagnostics System, Basel, Switzerland). The mutations of the HBV DNA reverse transcriptase/polymerase were tested by direct sequence analysis. Results: The serum HBV DNA level was decreased below 2x10⁴IU/mL (10^(5) copies/mL) in 77.7% and below 2x10² IV/mL (10³ copies/mL) in 42.6% after 12 months of ADV therapy. The median level of serum HBV DNA was 3.3log10 and 3.lloglO at 6 months and 12 months of ADV therapy, which were significantly lower than pretreatment level (6.05log10). The mutant rtN236T developed in 25%, rtA181T/V in 64%, rtV84H in 4%, and rtQ215S in 7% of the patients during 12 months. Conclusions: Although ADV therapy is effective, it leads high rate of mutations of HBV DNA reverse transcriptase/polymeerase in patients with chronic HBV infections who have LMV-resistant HBV mutants.

      • 성견에서 임프란트 매식시 골형성단백 사용에 따른 골재생에 관한 연구

        The purpose of this study is to evaluate the efffect of the bone morphogenetic protein, bone matrix gelatin and collagen matrix on the amount and shape of generating new bone adjacent to the implant. Implants were inserted in the mandible of adult dogs at 2 months after teeth extraction. Artificial bony defects, 3mm in width and 4mm in depth were made at the mesial and distal side of implant. Experimental groups were divided into three groups; Group 1 : Defects filled with collagen matrix and bone morphogenetic protein. Group 2; Defects filled with bone matrix gelatin, Control group: Defects filled with only collagen matrix. After implantation, the animals were sacrificed at 1, 3, 5 and 10 weeks for light microscopic examination. For the fluorescent microscopic examination, each tetracycline Hcl and calcein were injected at 1, 3, 5, 8 and 10 weeks after implantation. The results obtained were as follows: 1. The molecular weight of bovine BMP was about 18,100 by hydroxyapatite chromatography. 2. Osseointegration was observed in experimental groups 1 & 2, and BMG and BMP had an excellent bone forming capability as a filling materials to the repair of the bone defects. 3. The degree of healing of bone defect area, the experimental group 1 showed more prominent bone formation than control group, and the control group showed fibrous connective tissue between the implant and the bone. 4. In the fluorescent microscopic findings, bone remodelling was observed regenerative lamellar bone at defect area in experimental group 1, and partial remodelling in experiment group 2. In the control group, fibrous connective tissue was observed between the implant and bone surface and sign of remodelling was not appeared. Above results suggest that BMP has rapid osteoinductive property and can be used clinically as a bone substitute on bone defects around implants.

        Haemolymph and soluble proteins in several tissues of several lepidoteran species were investigated electrophoretically by native-PAGE and SDS-PAGE. The eletrophoretic profiles of the soluble proteins in the haemolymph, fat-body, silkgland, midgut and integument of the wild silkmoth, Actias gnoma, were monitered very differently. Also in the comparison of haemolymph proteins among lepidopteran insects, the electrophoretic patterns varied greatly. These results reveals that different tissues or organs present in a same insect species can have their own soluble protein patterns, and even though in a same tissue (haemolymph), speciesdependant electrophoretic patterns can be generated.

        D-ribose섭취와 저항성운동이 신체구성, 무산소운동능력 변화, 근부상효소 및 스트레스호르몬에 미치는 영향

        This study aims to examine the effects of 12 weeks of D-ribose supplementation and resistance training on body composition, changes in anaerobic exercise capacity (Wingate test), stress hormones and muscle damage enzymes. Thus, it examined female university students in their 20s who have no regular exercising experience by dividing them into RI(D-ribose intake) group of 6 members and PL (Placebo) group of 6 members, and came up with the following conclusion. Steady and regular exercise prevented muscle damage in carrying out acute exercise, and steady intake of D-ribose plays the role of increasing the ability to recover from acute exercise. Steady D-ribose intake clearly helped improve physical fitness, but it must be noted that low-intensity and little exercise may lead to weight gain.
