        실험적 급성 허혈성 신부전에서 Verapamil 이 혈중 Polyamine 동태에 미치는 영향

        N/A It has been proposd that calcium entry from an external medium increases intracellular free calcium to toxic levels during ischemic acute renal failure, and verapamil (ARF) has been suggested as the agentblocking calicium entry into renal cells and protecting renal function during ischemic injury. Polyamines, anorganic cations that play various roles in normal cellular proliferation and differentiation, accumulate in renal failare. Alsa it has been suggested tht the synthesis and metabolism of polyamine are influence by acute changes of the general condition, such as acute renal failure, and are mediated by a calcium influx into the cells. The study was designed to study the protective effects of systemic verapamil pretreatment on renal function and the influence on polyamine metabolism in experimental ischemic ARF in cats. For these purpose, the experimental animals were divided into 2 groups. While Group I (n=5) was an ischemic ARF model by renal artery clamping for 60 minutes, Group II (n=5) was ischemic ARF with systemic verapamil (5 ml/min/kg) pretreatment. The creatinine clearance and plasma and urinary polyamine were measured in each group before and after the renal artery clamp. The results were as follows: 1) Creatinine clearance before and after the renal artery clamp were 10.64±7.18 ml/min/kg and 2.09±1. 70 ml/min/kg in Group I, 4.47±3.38 ml min/kg and 0.60±0.79ml/min/kg in Group II, respectively, So creatinine clearance decreased more significantly in Group II campared with Group I. 2) Plasma polyamine increased after ischemia in Group I. In group I, plasma levels of putrescine, spermidine, and spermine before ischemia were 4.75±0.40 nmol/ml, 0.69±0.09 nmol/ml, and 0.83±0.63 nmol/ml, were elevated to 7.17±2.91 nmol/ml, 9.83±1.46 nmol/ ml, and 2.64±1.14nmol/ml after ischemia. But in Group II, the plasma level of polyamine was not changed, and especially, spermine decreased significantly from 0.83±0.27 before ischemia to 0.49±0.23 nmol/ml after ischenmine (p=0.033). 3) Urine polyamine excretion decreased after ischemia in Group I and Group II. In Group II, urinary excretion of spermidine and spermine before ischemia, 0.13±0.10 nmol/min and 0.17±0.13nmol/min, decreased after ischemia to 0.01±0.01nmol/min (p=0.019) and 0.032±0.26 nmol/min (p=0.0257). 4) In renal tissue, spermine content vas highest. In Group II, preischemic spermine were 397.20 nmol/g and increased to 646.66nmol/g after ischema, But there were no significant changes in the polyamine contents in Group II. From these data, it was suggested that systemic verapamil pretreatment exerts no protective effect on ischemic ARF. Plasma polyamines are elevated in ischemic ARF, and verapamil may protect these elevations.

      • 간경변 환자에서 Oro-cecal Transit Time의 측정

        Background: Neurotransmitter like substances in the gut have been proposed to contribute to the overall portal systemic encephalopathy (PSE). The source of the ammonia in patients with PSE is mainly the small bowel, as a result of the conversion of glutamine in the diet to glutamate and ammonia. PSE is highly responsive to changes in diet, to antibiotic therapy and to ingestion of nondigestible disaccharides such as lactulose. This study was performed to evaluate the relationship between PSE and oro-cecal transit time. In addition, performed to evaluate the effect of severity, cause, complications of liver cirrhosis on oro-cecal transit time. Methods: Between August 2006 and February 2007, 48 cirrhotic patients consecutively was enrolled in study their oro-cecal transit time after informed consent was obtained. A non-invasive hydrogen breath test was used to study oro-cecal transit time (OCTT). Result: The OCTT were delayed in cirrhotic patients with PSE compared without PSE (123.3 ± 42.7 min vs 170.0 ± 58.3 min, p<0.05). OCTT tended to prolongate further as the damage in the liver got worse, nevertheless the result was statistically meaningless (p<0.111). Neither presence of ascites or cause of PSE had any influence on gastrointestinal transit. Conclusions: Cirrhotic patients with PSE have more delayed OCTT than without PSE. Further research is needed to find out the relationship between OCTT and severity of liver cirrhosis.

        Effect of Impact Velocity on Interface Characteristics of HT‑9 Steel Joints Fabricated by Magnetic Pulse Welding

        The weldability window in magnetic pulse welding (MPW) is proposed for the end-closure joining of HT-9 steel tubularworkpieces. Based on both theoretical and experimental investigations, the impact velocity of the flyer tube at the momentof collision had a dominant effect on the bonding characteristics of the resultant joints and the optimal impact velocity wasestimated to be in the range of 265–290 m/s in this alloy system. Under the optimal process conditions, no leak was detectedduring helium leak testing. Moreover, the integrity of the joint interface was preserved until the tube section ruptured duringinternal pressure burst testing. For HT-9 steel, the ductile δ-ferrite phase produced by the localized interfacial melting duringMPW acted as an effective obstacle to crack propagation, thereby enhancing the fracture resistance of the joint interface. Allof the results demonstrated the high integrity and durability of the HT-9 steel tube assembly fabricated by the MPW process.

      • Outsole 경도에 따른 피로도 분석에 관한 연구

        대형 할인업체간의 경쟁이 치열해지면서 계산원들의 업무가 복잡해졌다. 계산원의 업무 특성상 장시간 동안 한정된 장소에서 정적 기립자세를 유발하며 국소적 근육활동의 사용은 근육통과 피로누적 등 근골격계질환 발생 가능성을 가지고 있다. 본 연구에서는 이러한 문제점을 보완하기 위하여 근전도 측정을 통해 outsole의 경도에 따른 피로도를 비교분석하고자 하고자 한다. Outsole은 C형 경도계를 이용하여 경도(hardness)가 30~35인 경우를 Soft라 하고, 경도가 45~50을 Hard라 하였다. 계산원의 작업시간에 따른 변화를 알아보기 위해 작업시간은 0시간, 2시간, 4시간으로 구분하였고, 측정부위는 종아리(비복근, gastrocnemius), 허벅지(외측광근, vastus lateralis), 허리(척주기립근, erector spine)로 선정하였다. Outsole 경도에 따른 근전도 분석은 MF(Median Frequency), MPF(Mean Power Frequency), ZCR(Zero Crossing Rate) 3부류로 나누어 분석하였고 측정 부위별, outsole의 경도별 그리고 시간별 간의 통계적 유의성 분석을 실시하였다. MF, MPF, ZCR 분석 결과 세 측정 부위 중 종아리에서만 90% 유의 수준에서 유의성이 나타났으며 허벅지와 허리는 50%~80%정도의 유의 수준에서 유의한 차이를 보였다. Outsole의 경도별 분석은 Hard보다 Soft의 값이 현저히 낮게 나타났음을 알 수 있었고, 작업 시간간의 통계적 분석 결과는 20~30% 유의 수준에서 유의한 차이를 보였다. 이는 4시간 이내의 작업에서는 현 계산원들의 작업 시 허리 및 하지에 미치는 피로도 간의 차이가 크게 나타나지 않음을 알 수 있으며 허벅지와 허리보다 종아리에 있어서 피로도 감소 효과가 크게 나타남을 알 수 있다. 또한 45~50정도의 경도를 가진 outsole(hard)의 영향보다 30~35정도의 경도를 가지는 outsole(soft)이 피로감소에 따른 효과가 큼을 알 수 있다. 따라서 장시간 정적인 작업 자세에 있어서 단순 반복작업을 요하는 작업에서는 경도가 30~35정도의 soft type의 outsole 사용이 권장된다.

        This study attempts to investigate whether 11 weeks' rhythmic exercise-three times a week, intensity at 50~70%HRmax-affects body composition, health-related fitness, and respiratory function in the elderly women. As for body composition, in excess body fat group/over 30% body fat), weight and %body fat were reduced, but, there was no significant difference, Under body fat group/under 30%body fat) showed a tendency to reduce in weight and %body fat significantly. As for health-related fitness, excess body fat group/over 30% body fat) showed a increase in the muscular strength of the lower limbs, but there were no significant differences in the muscular strength of the upper limbs. flexibility, quickness, balance, and endurance. However, the muscular strength of the lower(upper) limbs, quickness, and balance were significantly reduced in under body fat group. As for pulmonary ventilation in under body fat group, FVC and %FVC increased but there was no significant difference. In contrast, Fev1 was significantly increased. In conclusion, regularly rhythmic exercise may improve body composition and health-related fitness. Moreover, it could be one of the best ways for the elder to live actively.

        치과용 고무인상재의 영구변형 및 압축시 변형률

        This study investigated the permanent deformation of dental rubber impression materials in order to evaluate their degree of recovery from displacement during removal through undercut area. We also investigated the stain in compression in order to evaluate the materials' flexibility during removal from the month, or during removal of the cast from the impression. Additional silicone, polyether, and polysulfied rubber impression materials were tested for their properties of permanent deformation and strain in compression according to the testing methods of the ADA specificaltion no. 19. 1.The permanent deformation values were as follows: polysulfide (2.25%), polyether (2.14%), and additional silicone (0.28∼1.21%, 2.38% for Perfect F). Examix, Express and Provil, which are additional silicone rubbers, showed lower permanent deformation values of 0.29%, 0.28% and 0.34%, respectively. Perfect F, although it is an additional silicone rubber, showed the highest permanent deformation value (2.38%) within the tested materials. 2.The strain in compression values were as follows: polysulfide (10.63%), additional silicone (2.88∼10.09%), and polyether (2.38%). The additional silicone rubber samples showed various values. Contrary to our expectations, it is also notable that Vinylate, which is used when taking an one-step impression, had higher stain in compression value (10.09%) than the light body impression materials even though it is classified as a heavy body material. Express (a.s.) had a low strain in compression (2.88%) which is similar to that (2.83%) of Impregum F (p.e.). In light body rubber materials, Permlastic had the highest value (p.s. 10.63%), followed by Perfect F (a.s. 7.38%), Correct VPS (a.s. 7.13%), Examix (a.s. 6.04%), Provil (a.s. 3.82%) and Express (a.s. 2.88%). Although the tested materials showed various values, all of them satisfied the requirements of A.D.A. Specification No. 19, which require that permanent deformation be less than 2.5%, and strain in compression be between 2∼20%.

        In this paper, we have developed a client/server diagnosis system that process the information of motion picture real time. Development of motion analysis and diagnosis system can be used in medical clinic. Users manages clients only while the expensive image processing equipments are managed by server manager. Very high speed network is used for the communication between server and client. In this paper we propose a real time marker detection algorithm for remote real time system of analysis for information of medical image sequence.

      • 좌측흉수로 발현된 Boerhaave 증후군 1예

        구토 후 발생한 토혈을 주소로 내원한 환자에서 좌측 늑막 삼출 동반 시 식도 파열의 가능성을 고려하여 흉수 천자 및 지속적인 흉부 단순 촬영의 추적관찰이 필요하며 식도 파열이 배제된 후 환자의 생체 징후 및 토혈 양을 파악하여 내시경 검사 시기를 신중히 결정하는 것이 좋을 것으로 생각된다. 본 증례는 토혈을 주소로 내원하여 Mallory-Weiss 증후군 의심 하에 내시경을 통한 치료 시행 후 추적 관찰에 의해 진단된 Boerhaave 증후군을 경험하였기에 보고하는 바이다. Boerhaave's syndrome is a spontaneous perforation of the esophagus most commonly resulting from a sudden increase in intraesophageal pressure combined with negative intrathoracic pressure caused by straining or vomiting. Surgery should generally be performed within 24 hours because mortality approached 30 to 50 percent in which surgery was delayed. Therefore, early diagnosis and prompt surgical repair are critical for survival. We have experienced a case of Boerhaave's syndrome in a patient presenting with hematemesis with left pleural effusion without any signs or symptoms of pneumomediastinum or pneumothorax.

      • 집배 Mobile Containers(운반도구)의 생체역학적 부하 개선에 관한 연구

        집배원 업무량 증가에 따른 업무환경 개선이 수반되지 못하고 있어, 근골격계 질환등 많은 문제점들이 제기되고 있다. 특히 집배실에서 이루어지는 집배 분류작업, 운반작업과 포장작업 등의 중량물 취급작업은 작업자들에게 요통과 같은 근골격계 질환을 야기 시킨다. 본 연구에서는 집배시 사용되는 운반도구(롤 파렛트, 대차, 운반함)에 대하여 인간공학적 분석프로그램(3DSSPP, NLE, OWAS, RULA, REBA)을 사용하여 발생되는 문제점들을 분석하였다. 롤 파렛트는 많은 우편물을 적재하기 위해 크기와 무게가 인간공학적 기준치를 벗어나 우편물을 싣고 내릴 경우와 밀고 당기는 작업시 신체적인 부하를 야기시킨다. 또한, 중량물 취급 시 작업자의 작업영역은 과도한 크기로 적정 작업영역을 벗어나게 함으로써 작업부하를 더욱 가중시키기 된다. 대차의 경우 역시 우편물을 싣고 내릴 때 수직의 적정 기준보다 현저히 낮은 위치로 반복적인 허리굽힘 자세가 요구되어 허리에 부하를 가중시키며생체역학적 기준을 초과하고 있다. 운반함의 경우, 운반함을 겹쳐쌓기 위한 들기작업, 바닥에서 이루어지는 우편물 운반작업과 우편물 분류작업 역시 요구되어지는 작업자세가 인간공학적 기준치를 초과하고 있다. 인간공학적 분석 프로그램을 통하여 분석한 결과, 롤 파렛트는 재원을 줄여주고 견고하고 가벼운 소재를 이용하여 재설계되어야 하며 밀고 당기는작업을 효율적으로 하기 위한 손잡이를 제공해야 한다. 대차는 높이 조절식 형태나 지게차형태의 대차를 사용하므로 과도한 허리굽힘 자세를 예방할 수 있으며 운반함의 경우 우편물 무게에 따른 높이 조절식 운반함으로 설계/재설계함으로 생체역학적 작업부하를 감소시킬 것이다.
