        瘀血病態模型 개발에 관한 硏究

        현재까지 개발된 어혈 동물모형들은 각각 그 특성이 달라 약물의 활혈화어 효과를 검증하는데 일괄적으로 적용시킬 수 없는 난점이 있었고, 실험상 변수의 통제가 어렵고 정확한 기전을 밝히기도 어려울 뿐 아니라 비용이 많이 들어 대규모적인 스크리닝을 하기가 힘들었다. 따라서 이들 모형의 차이점을 비교검토할 필요가 있고 이 문제점들을 보완해줄 in vitro에서의 새로운 병태모형이 필요하여 본 연구를 실행하였다. 동물을 이용한 어혈 모형의 경우, 혈전증모형은 출혈기전과 응고기전과 관련있는 어혈모형으로 생각되며, HA와 dextran으로 유발한 고점도혈증은 응고기전과 직접 관련되는 모형이라기 보다는 혈액의 점성이 증가되어 혈의 흐름이 정체되어 나타나는 혈어모형에 가깝다고 볼 수 있다. 압상어혈모형은 대퇴부의 압박으로 인한 국소 어혈모형이며, 전신의 혈류상태에는 큰 영향을 미치지 않았다. 또한, in vitro에서는 ECV304 cell line과 ECVPAR cell line에서의 세포부착효과, Angiogenic lumen formation, MMP-9, uPA의 발현양상, Integrin αvβ3, LFA-1, ELAM-1의 발현 등을 지표로 삼아 혈관신생을 통한 어혈병태모형을 구축하였으며, 이 모형은 기존 동물에서의 어혈모형의 문제점들을 보완할 수 있을 것으로 생각된다. This experimental study was carried out to characterize in modern medical understanding the concept of Blood Stasis as proposed in the traditional medical system of Korea, Japan, and China. Another objective was to construct a new model of blood stasis via in vitro. We divided the original model of blood stasis into three catergories: thrombosis, high blood viscorsity and trauma. The thrombosis model was induced by endotoxin and is related to the mechanisms of bleeding and coagulation. the high blood viscosity model was induced by hydrocortisone acetate and dextran and is related to the hemodynamic derangement rather than the bleeding and coagulation. the trauma model was induced by compression and is related to the local and hematological change by the compression of the thigh of rats. Finally, we induced an in vitro blood stasis model using ECV304 and ECVPAR cell lines. The focus being inducing angiogenesis and correlating the mitigating factors of cell adhesion, angiogenic lumen formation, mRNA expression of MMP-9 and uPA, and expressions of integrin αvβ3, LFA-1 and ELAM-1. We expect that this model may complement the problems associated with the original models.

      • Squalene Adjuvant를 이용한 단백질 항원의 세포성 면역반응 유도

        To investigate the role of adjuvant for cytotoxic T lymphocytes (CTL) induction, squalene-based adjuvant was studied its capability eliciting to the induction of cellular immunity as well as humoral immunity to exogeneous proteins. Ovalbumin (OVA) was used as model proteins. It was demonstrated that antigen formulation consisted of metabolizable oil squalene mixed with Tween 80 and pluronic L121 (S/TJ121) could induced the antigen specific CTL responses and antibodies irrespective of immunization routes in mice. We also demonstrated that this antigen formulation was a inducer of CD8+, major histocompatibility complex(MHC) class I-restricted, and antigen-specific C;1'Ls. These data suggest that the squalene-based emulsion system is a potent adjuvant inducing cellular immunity as well as humoral immunity.

        상아질 접착제의 미세누출과 변연부 혼화층

        The purpose of this study was to compare microleakage and marginal hybrid layer in class V restorations using two one-bottle adhesives and one self-etching adhesive. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on buccal and lingual surfaces of 30 extracted human molar teeth. Prepared teeth were randomly divided into there treatment groups (n=30) and restored with three adhesives and composites: Single Bond/Filtek Z-250 (Group 1), Prime&BondNT/EsthetㆍX(Group 2), Unifil Bond/Unifil F (Group 3). For microleakage, samples were stored in room temperature water for 24 hours, thermocycled, stained with 2% methylene blue dye, sectioned into halves, scored and analysed using Mann-whitney test and Wilcoxon singed rank sum test. For marginal hybrid layer, samples were sectioned into halves with 10% phosphoric acid for 5 seconds, stored in 5% NaOCL solution for 24 hours, dried and gold coated. Occlusal and gingival margins of each sample were inspected under SEM. The results of this study were as follows; 1. Microleakage at the occlusal margins was not evident in group 1 and group 2, but it showed in group 3(p<0.05). 2. Microleakage in group 1 and group 3 was significantly lower than in group 2 at gingival margins (p<0.05). 3. Microleakage at gingival margins was greater than at occlusal margins in group 1 and group 2, but microleakage at occlusal margins was greater than at gingival margins in group 3 (p<0.05). 4. In group 1 and group 2, no gaps at occlusal margins showed. But gaps showed in group 3. Occlusal margins were free from a hybrid layer in all groups. 5. The thickness of the marginal hybrid layers was 2.5~5㎛ thick in group 5㎛ thick in group 2 and 1.5㎛ thick in group 3. 6. There was no corelation between microleakage and thickness of marginal hybrid layer. In coclusion, the effect of dentin adhesives on microleakge in class V composite restorations was excellent when one-bottle adhesives were applied on enamel margin, and it was good when a self-etching adhesive was applied on dentinal margin. There was no corelation between microleakage and thickness of marginal hybrid layer.

        The diagnostic requirements were suggested and explained regarding the systems of differentiation of symptoms and signs in the second year study of standardization and unification of the terms and conditions used for diagnosis in oriental medicine. The systems were as follows; - differential diagnosis according to condition of body fluid, differentiation of syndromes according to the state of qi and blood, differential diagnosis according to relative excessiveness or deficiency of yin and yang(氣血陰陽津液辨證) - differentiation of diseases according to pathological changes of the viscera and their interrelation - analyzing and differentiating of febrile diseases in accordance with the theory of the six channels(傷寒辨證) The individual diagnosis pattern was arranged by the diagnostic requirements in the following order : another name(異名), notion of diagnosis pattern, index of differentiation of symptoms and signs(辨證指標), the main point of diagnosis, analysis of diagnosis pattern(證候分析), discrimination of diagnosis pattern(證候鑑別), a way of curing a diseases(治法), prescription(處方), herb in common use(常用藥物), diseases appearing the diagnosis pattern(常見疾病), documents(文獻調査). This study was carried out on the basis of the Chinese documents and references.

        Treatment of Stevens-Johnson syndrome

        Stevens-Johnson syndrome with oral cavity, skin, ocular, genital involvement appears to be a form of erythema multiforme. The etiology of erythema multiforme is not clear, but it is related to infection, food allergy, drug allergy, radiation therapy, and so on. For treatment, drugs like corticosteroids and acyclovir are mainly used. This study presents a case report of a male patient with Stevens-Johnson syndrome which was well controlled by medication (steroid).

        Evaluation of Gustatory Function in Patients with Sleep Disordered Breathing

        Purpose: The aim of this study is to evaluate the difference between gustatory functions in a sleep disordered breathing (SDB) group and a control group. The pathogenesis of SDB has not been fully understood. Though the precise contributions of neuromuscular and anatomical factors on SDB pathogenesis are still debated, we hypothesized that the gustatory dysfunction could be predisposed to SDB. Methods: All patients were diagnosed as SDB by polysomnography (PSG). On the basis of PSG results, patients were divided into 3 groups: snoring, mixed, and obstructive sleep apnea (OSA). The control group comprised healthy volunteers who were the same age as those of the SDB group and whose breathing was verified as normal using a portable sleep monitor device. The patient group and the control group were evaluated for gustatory functions with an electrogustometry (EGM). The electrical taste thresholds were measured in the anterior, midlateral, and posterior sides of the tongue and soft palatal regions, both sides. To find out the difference in EGM scores, statistical analysis was performed using the Kruskal-wallis and Mann-Whitney U test with 95% confidence interval and p<0.05 significance level. Results: The patients with SDB had higher EGM scores than the control group at all spots tested, except for the right midlateral of the tongue, and there was a statistical significance in the comparison between the control group and the divided SDB groups, respectively. Among the divided SDB groups, the snoring group had the most significant differences in the number of the measured spots, but there was no difference among the snoring, mixed, and OSA groups. Conclusions: These results may suggest that neurologic alterations with sleep disordered breathing could be associated with gustatory dysfunction. In the future, further systemic studies will be needed to confirm this study.

        EFFECTS OF CYCLIC NUCLEOTIDES AND CALCIUM ON THE TRANSMEMBRANE POTENTIAL IN THE RAT SUBMAXILLATY GLAND

        Cyclic nucleotide 농도와 영향을 주는 칼슘과 약물이 흰쥐 악하선의 막전압에 미치는 효과를 흰주 악하선의 절편을 Krebs-Henseleit용액이 관류되는 perplex vessel내에 고정하고 세포내 미세전극을 이용하였다. 안정 세포막 전압은 -28.7mV±0.76(S.E)이었다. 10mM theophylline이 함유된 용액 관류시에는 2분이내에 안정막 전압이 감소되었으며,ac-etylcholine으로 자극한 결과 이상성 전압변화를 보였는데 처음에 탈분극이었고 후기는 과분극 현상을 보였다. 10mM nicotinic acid용액으로 관류시에는 안정막 전압이 증가하였고, acetylcholine에 의해 이상성 전압변화를 나타내었다. Dibytyryl cyclic AMP와 8-bromo cyclic GMP(10^-4M)는 전압변화를 일으키지 않았으나 관류하는 동안 지속되지는 않았다. 10^-4M EGTA가 함유된 calcium-free용액 관류시에는 안정막 전압이 다소 감소되었지만 acetylcholine에 의해서는 변동되지 않았다. Cyclic nucleotides 유도체는 calcium free용액에서 안정막 전압이 아무런 영향도 주지 못했다. The effects of calcium and drugs which affect the concentration of cyclic nucleotides, as well as the direct actions of cyclic nucleotide derivatives, on the membrane potential in the rat submaxillary gland were investigated. The membrane potential recordings were carried out using an intracellular micro-electrode in acinar cells of rat submaxillary glands mounted in a perspex vessel through which oxygenated Krebs-Henseleit solution was perfused. The resting acinar cell membrane potential in Krebs-Henseleit solution was -28.7mV ± 0.76 (S.E). When the perfusion solution contained 10 mM theophylline, the resting acinar cell membrane potential decreased within two minutes, and acetylcholine stimulation evoked biphasic potential changes. The initial response was depolarization; the latter, hyperpolarization. During perfusion with a 10 mM nicotinic acid solution, the resting membrane potential increased, and a biphasic potential change was revealed by acetylcholine. Dibutyryl cyclic AMP and 8-bromo cyclic GMP (10^4 M) did not induce the potential changes, but dibutyryl cyclic GMP (10^4 M) caused transient changes in the resting membrane potential, which were not sustained during the perfusion period. In calcium-free solution with 10-^4M EGTA, the resting acinar cell membrane potential was slightly reduced and was not changed by acetylcholine. Cyclic nucleotide derivatives had little effect on resting acinar cell membrane potential changes in calcium-free solution.
