Standard open procedures for resection of the first rib in thoracic outlet syndrome can prove to be quite difficult with extensive incisions. A minimal invasive procedure can also be painstaking, but provides an attractive alternative to the more radical open procedures. We report the details of the technique with direct video footage of the procedure performed in a 41-year-old man with thoracic outlet syndrome done entirely by thoracoscopic methods. Effect of Masticatory Movement Using Gum on Walking and Cycling: A Randomized Crossover Design

PURPOSE: Gum chewing stimulates the sympathetic nervous system and increases energy consumption. However, the effect of mastication on physical activity remains unclear. This study aimed to investigate the effect of gum masticatory movement on physiological markers related to walking and muscle activation during cycling in different patient groups. METHODS: Using a randomized crossover design, 25 participants participated in walking trials with a 1-week washout; the trials included chewing gum (GUM), taking a candy with the same ingredients as the gum (CAN), and no ingestion (CON). Energy expenditure (EE), metabolic equivalent (MET), oxygen consumption (VO<sub>2</sub>), and heart rate were measured using a portable metabolic device. The walking distance was also calculated. In the cycling experiment, the other 19 participants randomly completed 7 minutes of the three trials (GUM, CAN, CON) with a 15-minute break. The mean cycling period (MCP), cycle number (CN), coefficient of variation of the cycling period (CV), and integrated electromyography (iEMG) results were measured using the Delsys Trigno™ Wireless EMG System. RESULTS: The walking distance was significantly higher in the GUM group than in the CAN and CON groups by an average of 78 m (7.4%, p<.05). Comparing the GUM and CON groups, EE, METs, and VO<sub>2</sub> demonstrated a partially significant increase after 15 minutes. In the cycling experiment, there were no significant differences in the effects of the trials on cycling performance (MCP, CN, CV). However, significant differences were observed in the GUM group for the iEMG results. CONCLUSIONS: Our study results suggest that gum chewing improves physical performance, such as walking distance, and improves energy metabolism, such as EE and METs. Additionally, it can influence the improvement in the lower limb muscle activity during cycling.
Current-carrying capacity of double-wall carbon nanotubes

<P>We have studied electrical transport characteristics of individual double-wall carbon nanotubes (DWNT) under high bias voltages. As the bias voltage applied to the carbon nanotubes increases, the outermost shell of the DWNTs broke down sequentially, which enabled us to determine the current-carrying capacity of each shell. The maximum current-carrying capacity per shell was about 150 µA, which is well above that of any other previous reports. </P>

Background: The regional lymph nodal status is the most powerful independent predictor of survival for patients with clinical N0 primary cutaneous malignant melanoma. Objective: We wanted to evaluate the feasibility and morbidity of the sentinel lymph node biopsy (SLNB) staging using a multidisciplinary team approach, in cooperation with other surgical departments, at a university hospital setting. Methods: Twenty two patients with cutaneous melanoma and who were treated at Kyungpook National University Hospital were included in this study. They all received SLNB, which was done by the Departments of Dermatology and General Surgery. We evaluated the feasibility and side effects of SLNB. Results: Pathologically-positive sentinel nodes were found in 7 of the 22 cases (31.8%) and all 7 patients were consequently upstaged. The whole process involved in SLNB was well tolerated by nearly all the patients, with only mild and transient complications being observed. Conclusion: We suggest that in a Korean setting, utilizing SLNB with a multi-disciplinary team approach is a technically feasible procedure that is able to detect occult nodal metastasis with low morbidity rates in patients with cutaneous malignant melanoma. 아밀로이드의 심근 침윤은 심각한 예후를 초래할 수 있는 진행성 심근병증 뿐만 아니라 울혈성 심부전, 심장 판막 질환, 부정맥 등과 같은 다양한 심장질환을 일으킬 수 있다. 우리는 처음에 심근 아밀로이드증으로 내원하였으나 신드롬 X로 잘못 진단되었던 환자가 재발성 흉통 및 진행성 심부전으로 인해 심장 이식을 시행한 증례가 있어, 이를 보고하는 바이다.

A rapid, sensitive and selective analytical method was developed and validated for the determination of compound K, a major intestinal bacterial metabolite of ginsenosides in human plasma. Liquid-liquid extraction was used for sample preparation and analysis, followed by liquid chromatography tandem spectrometric analysis and an electrospray-ionization interface. Compound K was analyzed on a Phenomenex Luna C18 column ($100{\times}2.00$ mm, 3 ${\mu}m$) with the mobile phase run isocratically with 10 mM ammonium acetate-methanol-acetonitrile (5:47.5:47.5, v/v/v) at a flow rate of 0.5 mL/min. The method was validated for accuracy (relative error <12.63%), precision (coefficient of variation <9.14%), linearity, and recovery. The assay was linear over the entire range of calibration standards i.e., a concentration range of 1 ng/mL to 1,000 ng/mL ($r^2$ >0.9968). The recoveries of compound K after liquid-liquid extraction at 1, 2, 400, and 800 ng/mL were $106.00{\pm}0.08%$, $103.50{\pm}0.19%$, $111.45{\pm}5.21%$, and $89.62{\pm}34.46%$ for intra-day and $85.40{\pm}0.08%$, $94.50{\pm}0.09%$, $112.50{\pm}5.21%$, and $95.87{\pm}34.46%$ for inter-day, respectively. The lower limit of quantification of the analytical method of compound K was 1 ng/mL in human plasma. The developed method was successfully applied to a pharmacokinetic study of compound K after oral administration in ten of healthy human subjects.

Background: The study investigated virtual reality (VR) immersion in alleviating procedure-related pain in patients with chronic pain undergoing fluoroscopy-guided minimally-invasive intervention in a prone position at an outpatient clinic. Methods: In this prospective randomized controlled study, 38 patients undergoing lumbar sympathetic ganglion block were randomized into either the VR or the control group. In the VR group, procedure-related pain was controlled via infiltration of local anesthetics while watching a 30-minute VR hypnotic program. In the control group, the skin infiltration alone was used, with the VR device switched off. The primary endpoint was an 11-point score on the numerical rating scale, indicating procedure-related pain. Patients’ satisfaction with pain control, anxiety levels, the need for additional local anesthetics during the procedure, hemodynamic stability, and any adverse events were assessed. Results: Procedure-related pain was significantly lower in the VR group (3.7 ± 1.4) than in the control group (5.5 ± 1.7; P = 0.002). Post-procedural anxiety was lower in the VR group than in the control group (P = 0.025), with a significant reduction from pre-procedural anxiety (P < 0.001). Although patients’ satisfaction did not differ significantly (P = 0.158) between the groups, a higher number of patients required additional local anesthetics in the control group (n = 13) than in the VR group (n = 4; P = 0.001). No severe adverse events occurred in either group during the study. Conclusions: VR immersion can be safely used as a novel adjunct to reduce procedural pain and anxiety during fluoroscopic pain intervention.
