      • Pseudocarcinomatous hyperplasia of the Fallopian tube mimicking Tubal cancer: Radiological and Pathological diagnostic challenge

        목적: Pseudocarcinomatous hyperplasia is a term which is applied to the florid form of epithelial hyperplasia with atypical features, that mimicks tubal adenocarcinoma. This report presents the history and details of a woman with bilateral adnexal masses and a highly elevated serum CA-125 level. 방법: A 22-year-old nulliparous woman presented with lower abdominal pain and vaginal spotting. She was referred to our hospital due to bilateral ovarian masses and a highly elevated CA-125 level. Ultrasonography of the pelvis showed bilateral ovarian masses. Contrast-enhanced fat-suppressed T1-weigthed MR images show the enhancement of the papillary projection in the right adnexal mass and the enhancement of the irregular thick wall in the left adnexal mass. The putative preoperative diagnosis based MRI findings was fallopian tube cancer. Based on the above-mentioned imaging findings and the much elevated serum CA125 level, she underwent laparotomy with a preoperative diagnosis of tubal malignancy. Intraoperative frozen section from right salpinx revealed acute and chronic inflammation. Bilateral salpingectomy and partial omentectomy were performed. Pathologic findings of excised specimens led to a diagnosis of pseudocarcinomatous hyperplasia of tubal epithelium associated with acute and chronic salpingitis in both tubes, and endometriosis in left tube and omentum. Postoperative recovery was uneventful, and the patient was treated with dienogest. 결과: We report a case of pseudocarcinomatous hyperplasia of the fallopian tubes, which histologically mimicked adenocarcinoma. 결론: Such cases need to be differentiated morphologically from adenocarcinoma of the fallopian tubes to avoid overtreatment of the benign condition.

        Identification of Microorganisms in Duck Meat Products Available in Korea and the Effect of High Hydrostatic Pressure

        The objective of this study was to investigate the microbial count of duck meat and duck meat products commercially available in Korea. High hydrostatic pressure (HHP) treatment was applied at 0.1, 300, 400, and 500 MPa for 5 min to enhance the microbiological safety of duck meats. The levels of total aerobic bacteria were in the ranges of 3.53-6.19 and 3.62-6.85 Log CFU/g in raw and smoked duck products, respectively. By DNA sequence analysis, we identified microorganisms responsible for spoilage, with the most common species in the raw and smoked duck products being Aeromonas spp. or Pseudomonas spp. and Leuconostoc mesenteroides, respectively. HHP treatment significantly reduced the levels of total aerobic bacteria in raw and smoked duck products. This study demonstrates that HHP treatment may be used to effectively improve the safety of raw and smoked duck meat products.

        Assessment of Breed- and Sex-based Variation in Flavor-related Compounds of Duck Meat in Korea

        & & 본 연구는 오리의 종 및 성별에 따른 육 내 풍미 관련 물질의 영향을 구명하고자 수행되었다. 동일한 조건에서 사육된 6주령의 한국 토종오리 및 일반오리(Cherry Valley) 총 12수를 공시하여 가슴육의 일반성분, 핵산관련물질, 지방산 조성 및 유리아미노산 등을 분석하였다. 그 결과, 일반오리와 암컷오리가 각각 토종오리와 수컷오리에 비해 수분함량이 유의적으로 높았다. 육 내 IMP 함량은 수컷보다 암컷이 더 높았으며, 일반오리보다 토종오리 내 inosine 함량이 높은 결과를 보였다(P<0.05). 또한, 수컷 오리 가슴육은 암컷오리에 비해 alanine, aspartic acid, glycine, histidine, leucine, serine, valine, glutamic acid, cysteine 등 유리아미노산 함량이 높았다(P<0.05). 연구결과, 종에 따른 토종오리와 일반오리의 풍미 관련 물질의 차이는 미미한 것으로 확인되어, 토종오리 만의 우수성과 차별성을 확보하기 위해 오리육 내 풍미 관련 물질에 영향을 미치는 인자들에 관한 연구와 이를 통한 품종 개량이 필요할 것으로 사료된다. & & The objective of present research is to evaluate the effect of breed and sex on flavor-related compounds of duck meat in Korea. From the results, it was found that breed has no significant impact while sex has influenced the flavor-related compounds, especially, IMP and most of the free amino acids. In order to get breed-specific advantages, KND should be developed based on flavor aspect and further studies using KND with known genetic confirmation should be conducted for the extrinsic and intrinsic flavor influencing factors.

        Comparison of Bioactive Compounds and Quality Traits of Breast Meat from Korean Native Ducks and Commercial Ducks

        The aim of this research was to compare the bioactive compound content and quality traits of breast meat from male and female Korean native ducks (KND) and commercial ducks (CD, Cherry Valley). Meat from three 6-wk old birds of each sex from KND and CD were evaluated for carcass and breast weights, pH, color, cooking loss, shear force, and bioactive compound (creatine, carnosine, anserine, betaine, and L-carnitine) content. KND showed significantly higher carcass weights than CD whereas no such difference (p>0.05) was found between male and female ducks. The breed and sex had no significant effects on the breast weight, pH value, and shear force. However, KND had significantly lower cooking loss values than did CD. Creatine, anserine, and L-carnitine contents were significantly higher in KND than in CD and were predominant in female ducks compared to males. The results of this study provide rare information regarding the amounts and the determinants of several bioactive compounds in duck meat, which can be useful for selection and breeding programs, and for popularizing indigenous duck meat.

      • Pseudomyxoma Peritonei associated with synchronous mucinous tumors of the ovary and the appendix: A case report

        The coexistence of mucinous ovarian and appendiceal tumors in association with pseudomyxoma peritonei (PMP) is well established. However, there has been considerable debate regarding to the origin of the tumor in such cases. PMP was thought to originate from the ovary or peritoneal cavity. Based on clinicopathological, immunohistochemical, and molecular data, mucinous tumors involving the appendix and ovaries in women with PMP are clonal and derived from a single site, most likely the appendix. We report a case of a 73-year-old woman who had complained of lower abdominal distension. A magnetic resonance imaging of the abdomen and pelvis demonstrated a 21.8x10.0 cm cystic mass with thin enhancing septa arising from left ovary, and ascites. Additionally, tumor markers, cancer antigen-125 and carcinoembryonic antigen (CEA) were elevated to 196 U/ml and 14.5 ng/ml, respectively. On laparotomy the patient was found to have a ruptured huge right ovarian mass and amputed appnedix with mucus spillage. Subsequent surgical pathology results confirmed two separate malignant processes: borderline mucinous tumor and low-grade appendiceal mucinous neoplasm, ruptured. Immunohistochemical examination was performed on the surgical specimens. The appendiceal tumor stained negative for CK-7 and positive for CK-20, whereas the ovarian sample stained positive for CK-7 and negative for CK-20, consistent with malignant processes of independent origins.         골반 방선균증의 임상적 고찰

        N/A Objective : Actinomycosis is a rare entity, especially in the female genital tract, which presents some difficulties in establishing a correct preoperative diagnosis. Pelvic actinomycosis can mimick pelvic or intra- abdominal malignancy leading to mutilating surgical exercise. The authors surveyed 12 cases of pelvic actinomycosis for advice to detection and treatment of the pelvic actinomycosis. Methods : The authors studied retrospectively 12 cases which have admitted to Department of Obstetrics and Gynecology, College of Medicine, University of Ulsan, Asan Medical Center for pelvic actinomycosis from January. 1, 1991 to December. 31, 2000. Results : 41.7% of the cases occurred in 31-40 years age group, 91.7% of cases associated with intrauterine devices for 5-21 years. Most common complaints were abdominal pain and palpable mass, and other complaints were vaginal discharge, bowel habit change, nausea, vomiting and fever. The actinomycosis lesions involved one or both ovaries in all 12 cases. In 11 cases, the lesions extended to other areas, such as the uterus, parametrium, pelvic walls, cul-de-sac, colon and bladder. All patients underwent surgery that included removal of the lesions with ipsilateral or bilateral adnexa and, in specific cases, with extension of the lesions, hysterectomy, colostomy and primary repair of bladder or rectum. After surgery, 9 patients were treated with penicillin and the duration of treatment was 12 months in 2 patients, 6 months in 3, ≤3 months in 3 and one patient was being treated for 2 months. Other 3 patients were treated with metronidazole, cephalosporin and aminoglycoside during 1-3 weeks. All patients were alive and well. Conclusion : It is needed to make an earlier and more correct diagnosis of actinomycosis, and high-dose intravenous antibiotic therapy can reduce the risk of nearby pelvic structure injuries. In cases of pelvic actinomycosis where the abscess can be completely resectable, a shorter period of antibiotic therapy can be required.

      • Complete hydatidiform mole with massive intrauterine hemorrhage in an adolescent girl

        Hydatidiform mole (HM) is a premalignant proliferative trophoblastic disorder. Vaginal bleeding is the most common presenting symptom in women with HM. A 17-year-old nulliparous adolescent girl presented with progressive lower abdominal pain and pronounced nausea and vomiting of about a week’s duration. Her gestational age was 12 weeks and 4 days according to her last menstruation date. Pelvic examination revealed a closed cervical os and no uterine bleeding. Subsequent ultrasound and magnetic resonance imaging (MRI) depicted a T2-hyperintense, huge intrauterine mass, numerous cystic spaces, and massive peri-lesional hemorrhage. Immediately after introducing a forcep into the uterine cavity for suction curettage, a large amount of trophoblastic tissue was expelled and sudden gushes of blood with clots were passed. Histological examination and p57 immunohistochemistry resulted in a final diagnosis of complete hydatidiform mole. Massive intrauterine concealed hemorrhage in an adolescent is a rare complication of complete hydatidiform mole.       • The regulation of steroid hormones and its receptors in women with preeclampsia

        Objective: The aim of this study is to investigate the altered concentration and expression of steroid hormones, its receptors and steroidogenic enzymes in placenta and serum from normal and preeclamptic women. Methods: Placental tissues and blood samples were collected from normal (n=21) and preeclampsia (n=20). For the comparison of steroidogenesis between two groups, three analyses were conducted. First, the concentration of steroid hormones such as pregnenolone (PG), progesterone (P4), dehydroepiandrosterone (DHEA), estrogen (E2), testosterone (T) were analyzed by ELISA. Second, the mRNA and protein expressions of CYP11A1, CYP17A1, HSD17B3, HSD3B1 and CYP19A1 were examined by RT-qPCR and western blotting. The expression levels of estrogen receptor 1, 2 (ESR1, 2) and progesterone receptor (PR) were evaluated in vitro and in vivo. The expressions of mRNA and protein of these receptors were analyzed by RT-qPCR, western blotting and immunocytochemistry respectively. Results: P4 and E2 concentrations were significantly decreased in PE serum and placenta. The mRNA and protein levels of HSD3B1 were decreased, whereas those of CYP19A1 were elevated by 2 fold in the PE placenta. The protein level of HSD17B3 was different compared with the mRNA level, which was downregulated by 2.7 fold in the PE placenta. In steroid receptors analysis, mRNA and protein expression levels of ESR1 were reduced (1.4-fold), whereas ESR2 and PGR were elevated in PE placenta (1.5 and 1.6-fold). Also, expression levels of these receptors after hypoxic stress in BeWo cells demonstrated similar regulation to those in PE placenta. Conclusion: The present study suggested that the levels of steroid hormones (P4 and E2) in the serum and placenta of women with PE are downregulated, which may be mediated by the regulation of steroidogenic enzyme expression in the PE placenta. In addition, reduced ESR1 expression and inactivation of ESR2 and PGR proteins may affect the physiological complications of preeclampsia in the placenta.
