Objectives; This study was performed to examine the status of antibiotic use and the effect of antibiotic usage management in a military hospital. Methods; Quality assessment data reported from 14 Republic of Korea Armed Forces hospitals were retrospectively reviewed. We compared preoperative antibiotic dose timing and selection of antimicrobial agents before and after management. Results; From 2013 to the first half of 2019, a total of 5,155 surgeries were performed and the rate of administration of antibiotic within 1hour before skin incision was lowest at 94.8% in 2013 when the quality assessment indicator was first introduced. Since the introduction of quality assessment indicator the rate of administration of antibiotic within 1hour before skin incision was tended to increase. (p-trend<0.05) Of the 1,926 surgeries performed during the period from 2013 to the first half of 2019, the rate of administration of third-generation cephalosporin was 14%. From 2016 to 2018, the rate of third-generation cephalosporin administration was 14.9%(257/1,718) and the rate of third-generation cephalosporin administration in the first half of 2019 since the introduction of the quality assessment indicator was 6.3%(13/208) (p-value<0.05) Conclusion; This study shows timely antibiotic administration and appropriate antibiotic selection were improved through the quality-improvement efforts. Further research is needed to examine whether the improved outcomes are associated with a reduction in surgical site infection.

총 부하손의 20%를 차지하는 표유부하손은 대전력 변압기일수록 그 크기도 무시 할 수 없을 만큼 커지고 hot spot 온도 상승으로 변압기 수명에도 악영향을 줄 수 있는 요인이 된다. 이러한 표유부하손실은 측정하기도 쉽지 않고, 설계시 고려하기 위해 계산해내는 것은 더욱 어렵다. 본 논문은 3MVA 몰드 변압기의 프레임에서의 표유부하손을 2차원 FEM 수치해석을 통해 계산해내고 표유부하손이 발생하는 하나의 구조물로서 프레임 형상을 최적화하는 연구를 수행하였다.

Oligospermia is a deficiency in spermatogenesis with a low sperm concentration, less than 20 million per 1 ml semem. This is one of many important causes of male infertility. The number of sperm of average male has been decreased since last several decades. It is known that environmental hormones and drug abuse have incurred oligospermia continually. Oligospermia is divided into three types;spermatogenesis failure, sperm-carrying defect and idiopathic type. Until now, drug therapy has not proved itself to be effective, and ART(Assisted Reproductive Technique) is not a radical method in treating oligospermia, quantatatively and/or qualitatively, having scientific and/or ethical limit. The object of this study is to investigate current trend in oligospermia treatment and search out effective treatment method. In oriental medicine, oligospermia was referred to defective ejaculation and/or thinness of seminal fluid, and the cause of the disease was recognized as insufficiency of Kidney Essence. Therapeutic target was supplementing the Kidney Essence. Sperm density is increased and fertility rate increased. Therapeutic method is divided into drug and acupuncture theraphy. When, in drug formulae, Supplementing Kidney Essence medicinals become main stream, and Circulating Blood and Extinguishing Bloodstasis medicinals sidestream, the formula's effects was better. In fact, the effects of these methods was proved excellent. It is known that some medicinals and acupuncture correc thypothalamus-pituitary gland-sexual gland axis disturbance, recover testis function, and invigorate seminal production. Besides above methods, there are other methods to treat oligospermia-pasting umblicus with herb medicinals, controlling food intake and psychotic theraphy.

수치해석을 사용하여 2㎹A 몰드 변압기의 정전용량을 시뮬레이션(FLUX 2D)하고 계산하였다. 산출한 정전 용량 값을 변압기 모델링 회로에 적용하여 switching surge 등 이상전압이 인가되었을 때 변압기 내부에 있는 권선들의 전압분포를 예상하고 절연설계에 적용 할 수 있음을 보였다.
Elevation of S100A9 Protein in Sputum of Neutrophilic Inflammation in Severe Uncontrolled Asthma

Background: Neutrophilic airway inflammation is frequently observed in severe uncontrolled asthma as well as in controlled asthma. However, there is no sputum biomarker to differentiate the two conditions. Objective: To identify biomarkers of severe uncontrolled asthma with neutrophilic airway inflammation. Methods: Sputum with a >70% neutrophil content was pooled from 5 severe uncontrolled asthmatics (UA) and from 10 controlled asthmatics (CA). 2-DE was adopted for differential display proteomics, and candidate proteins were identified using MALDI-TOF MS analysis. S100 calcium binding protein A9 (S100A9) was identified by western blot and its level was measured in sputum from asthmatics with varying disease severity, COPD, and normal controls, by ELISA. Results: 14 protein spots exhibited differences in intensity between severe UA and CA. MALDI-TOF/TOF of these spots revealed an increase in HNP- 2, S100A9, α-amylase, NGAL, 4-aminobutyrate transaminase, and cystatin SA in patients with UA compared to patients with CA. There was a decrease in PLUNC, C3 component, immunoglobulin heavy chain variable region, GFAP1, IgM kappa IIIb SON, MLL-AF4 der (11) fusion protein, cytokeratin 8, and recombinant IgG4 heavy chain. S100A9 was detected at a higher level in western blots of neutrophilic sputum from UA, and significantly increased as measured by ELISA, in neutrophilic UA compared to CA, and COPD. Conclusions: S100A9 in sputum may be a biomarker of neutrophilic inflammation in severe uncontrolled asthma.
