Muscular involvement in polymyalgia rheumatica

Objective: Synovitis and bursitis have been demonstrated in polymyalgia rheumatica (PMR). But myositis has been rarely reported in PMR. Magnetic resonance imaging (MRI) has been used in PMR because of its potential to localize inflamed tissues. We sought to investigate the muscular involvement in PMR. Methods: We analyzed MRI findings of shoulder and pelvic girdle of 12 patients diagnosed by Chuang`s diagnostic criteria for PMR at Kyungpook National University Hospital. Inflammations in joint, bursa and muscle were examined. Results: The male to female ratio was 1:1. Mean age at diagnosis was 64.2 years. MRI of shoulder (5 patients) showed subacromial subdeltoid bursitis (60%), synovitis of gleno-humeral joint (40%), synovitis of acromioclavicular joint (20%) and juxtaentheseal myopathy (20%). MRI of pelvis (10 patients) showed synovitis of hip joint (20%), trochanteric bursitis (30%), ischial bursitis (20%), juxtaentheseal myopathy (60%) and myopathy (20%). Overall muscular involvement rate of PMR was 58.3%. Iliopsoas and hip rotators were the most common involved muscles and hamstring, gluteus maximus, and gluteus medius were followed in order of frequency. Muscular involvement were characterized by juxtaentheseal myopathy which was defined as inflammation of muscle around the site of bone attachment and near the fascia and by patch distribution of small areas of muscle fibers. Conclusions: MRI of PMR revealed characteristic findings of juxtaentheseal myopathy in addition to bursitis and synovitis. This study suggest that juxtaentheseal myopathy in pelvis may contribute to the diffuse pain and stiffness in the pelvic girdle observed in patients with PMR at least partially.

Facilitated Ion Diffusion in Multiscale Porous Particles: Application in Battery Separators

<P>Polyethylene (PE) separators have been the most popular option for commercial Li-ion batteries because of their uniform pore size, high tensile strength, low cost, and electrochemical stability. Unfortunately, PE separators generally suffer from significant dimensional changes at high temperatures, which frequently results in serious safety problems. In this regard, the integration of inorganic nanoparticles with PE separators has been considered to be a promising approach. Here, inorganic nanoparticles with a hierarchical pore structure were coated on a conventional polymer separator. The resultant composite separator exhibited superior Li ion transportation compared with separators coated with mesopore-only nanoparticles or conventional nonporous nanoparticles. In this paper we consider strongly polynomial variations of the auction algorithm for the single origin/all destinations shortest path problem. These variations are based on the idea of graph reduction, that is, deleting unnecessary arcs of the graph by using certain bounds naturally obtained in the course of the algorithm. We study the structure of the reduced graph and we exploit thus structure to obtain algorithm with O(n min {m, nlog n}) and O(n^(2)) running time.

<P>Injecting human mesenchymal stem cells (hMSCs) at wound sites is known to have a therapeutic effect; however, hMSCs have several limitations, such as low viability and poor engraftment after injection, as well as a potential risk of oncogenesis. The use of a conditioned medium (CM) was suggested as an alternative method for treating various wounds instead of direct hMSC administration. In addition to not having the adverse effects associated with hMSCs, a CM can be easily mass produced and can be stored for long-term, thereby making it useful for clinical applications. In general, a CM is collected from hMSCs with low passage number; whereas, the hMSCs with high passage number are usually discarded because of their low therapeutic efficacy as a result of reduced angiogenic factor secretion. Herein, we used a CM collected from high passage number (passage 12, P12) hMSCs treated with gold-iron nanoparticles (AuFe NPs). Our AuFe NPs were designed to release the iron ion intracellularly via endocytosis. Endosomes with low pH can dissolve iron from AuFe NPs, and thus, the intracellularly released iron ions up-regulate the hypoxia-inducible factor 1α and vascular endothelial growth factor (VEGF) expression. Through this mechanism, AuFe NPs improve the amount of VEGF expression from P12 hMSCs so that it is comparable to the amount of VEGF expression from low passage number (passage 6, P6), without treatment. Furthermore, we injected the CM retrieved from P12 MSCs treated with AuFe NPs in the mouse skin wound model (AuFe P12 group). AuFe P12 group revealed significantly enhanced angiogenesis in the mouse skin wound model compared to the high passage hMSC CM-injected group. Moreover, the result from the AuFe P12 group was similar to that of the low passage hMSC CM-injected group. Both the AuFe P12 group and low passage hMSC CM-injected group presented significantly enhanced re-epithelization, angiogenesis, and tissue remodeling compared to the high passage hMSC CM-injected group. This study reveals a new strategy for tissue regeneration based on CM injection without considering the high cell passage count.</P>
