제주도 지하수 질산염 농도의 시·공간적 변화 특성: 장기(1993-2015) 모니터링 자료의 평가

1993년부터 2015년까지 관측된 제주도 지하수 장기모니터링 관측정(N = 4,835)에서 수집된 지하수 수질자료(N = 21,568)를 기반으로 질산성질소의 시공간적 변동 특성을 평가하였다. 제주도 지하수의 질산성질소 농도의 중앙값은 2.5 mg/L로서 다른 국가나 대륙의 조사 결과에 비해 다소 높거나 유사한 것으로 나타났다. 또한 지하수 용도, 행정구역 및 고도 별로 유의한 차이를 보였다. 특히, 산간 지역에 비해 저지대 해안가에 위치한 농업 및 주거지역에서 농도가 높음을 확인하였다. Mann-Kendall 및 Sen’s slope 분석을 활용한 질산성질소 농도의 추세 분석 결과, 하류 저지 대에 비해 중산간지역에서의 질산성질소 농도 증가 경향이 뚜렷하였다. 제주도 내 토지 피복의 시계열 변화 특성과 결부 지어 보면, 중산간지역의 오염 증가 추세는 농업지역의 확장 등 인위적 활동 증가에 기인한 결과로 판단된다. 반면,기지정된 지하수자원특별관리구역에서는 전반적으로 질산성질소 농도의 감소 경향이 나타났는데, 이는 지하수 관리 측면에서 수질관리를 위한 적극적인 정책이 유효함을 시사한다. 본 연구에서는 제주도 지하수의 질산성질소 오염관리를 위한 적정 방안을 제안한다. The spatio-temporal variations of nitrate concentrations in groundwater of Jeju Island were evaluated by an analysis of time series groundwater quality data (N = 21,568) that were collected from regional groundwater monitoring (number of wells = 4,835) for up to 20 years between 1993 and 2015. The median concentration of NO 3 -N is 2.5 mg/L, which is slightly higher than those reported from regional surveys in other countries. Nitrate concentrations of groundwater in wells tend to significantly vary according to different water usage (of the well), administrative districts, and topographic elevations: nitrate level is higher in low-lying agricultural and residential areas than those in high mountainous areas. The Mann-Kendall trend test and Sen’s slope analysis show that nitrate concentration in mid-mountainous areas tends to increase, possibly due to the expansion of agricultural areas toward highland. On the other hand, nitrate concentrations in the Specially Designated Groundwater Quality Protection Zones show the temporally decreasing trend, which implies the efficiency of groundwater management actions in Jeju. Proper measures for sustainable groundwater quality management are suggested in this study.

Oregonin, a diarylheptanoid derivative from Alrus hirsuta Turcz, Betulaceae was evaluated on the antitumor activity. Oregonin, a diarylheptarioid zyloside is a novel immunomodulator augmenting macrophage and natural killer cell activity related anti-tumor functions. To investigate cytotoxicity of oregonin on tumor cells, the effect of the substance on three cell lines(P815, Yac-1, B16-F10) was examined. The result was somewhat different between suspension and adhesion cells. The growth-rate of suspension cells(P815, Yac-1) was not cut down significantly, but in contrast, the inhibitory activity on adhesion cell(B16-F10) was effective. Its ED50 is 20.09㎍/㎖(4.19×10^-5mol/l)

Pedunculagin, an ellagitannin from Alrus hirsuta var. microphylla, Betulaceae was evaluated on the antitumor activity. To investigate cytotoxicity of pedunculagin on tumor cells, the effect of the substance on three cell lines(P815, Yac-1, B16-F20) was examined. And the result was somewhat different among three cell lines. The growth-rate of P815 was not cut down significantly, but in contrast, the inhibitory activities on Yac-1 and B16-F10 were effective. ED_50s of pedunculagin against P815, Yac-1 and B16-F10 were 12.16, 1.89, 3.30㎍/㎖ respectively.
Dermoscopic features of acral dysplastic nevus

Background: Comparing to nevus in other regions, acral melanocytic nevi (AMN) have characteristic dermoscopic features due to their anatomical localization. The major dermoscopic pattern of parallel furrow, latticelike, and fibrillar patterns in acral nevus is well known. Dermoscopic patterns of dysplastic nevus in non-acral area classified into three types, namely, reticular, globular, and homogenous. But, there are limited data on the dermoscopic characteristics of dysplastic nevus on the acral area. Objectives: To investigate characteristic dermoscopic patterns of acral dysplastic nevus (ADN), and to find distinctive features that can differentiate them from other AMN. Methods: Dermoscopic patterns were evaluated in 13 patients with ADN and 19 patients with AMN. Results: Dermoscopic patterns of 13 ADN and 20 AMN were compared. ADN presented more frequently central structureless area (p < 0.05), central hyperpigmentation (p < 0.05), and mixed benign pattern than AMN (p < 0.05). Other findings of ADN, that present a higher incidence than AMN, include latticelike and homogeneous pattern related to histopathologic features such as bridging of rete ridges. Conclusion: Central hyperpigmentation, central structureless area, and mixed benign pattern were common features of ADN. We believe that dermoscopic features of ADN can provide a clue of differential diagnosis separating from AMN.

Background: Lupus erythematosus profundus (LEP) is a variant of chronic cutaneous lupus erythematosus and association between LEP and systemic lupus erythematosus (SLE) has been described as from 10% to 50% in the previous studies. Objectives: To investigate the association of SLE in LEP patients. Methods: This study was conducted on 32 patients diagnosed as LEP in the authors’ hospital and we reviewed previous studies using PubMed searches (Search terms included LEP, lupus erythematosus profundus, lupus erythematosus panniculitis, lupus profundus, lupus panniculitis. Studies describing 3 or more patients with LEP were included). Results: The mean age was 40.8 years (range, 17-73) and the mean follow-up duration was 72.4 months (range, 0- 162). Three patients (9.3%) were diagnosed concurrently SLE with LEP and two patients (6.3%) developed into SLE after 6 to 7 years. In the review of previous studies, 189 patients were histologically confirmed as LEP including 51 patients (26.9%) accompanied by SLE. Of them, 30 patients (15.8%) had been diagnosed as SLE before LEP, and 15 patients (7.9%) were simultaneously diagnosed as SLE by LEP. Only 5 patients (2.6%) developed into SLE during the follow-up period of LEP. Conclusion: The proportion of LEP patients associated with SLE in our center (16.7%) was lower than that of literature review (26.9%). And LEP seems to rarely progresses into the SLE.

Background: Lichenoid drug eruption (LDE) is a relatively rare form of cutaneous drug eruption and that resembles lichen planus on a clinical and histological basis. Although there are some studies on histopathological findings of LDE, studies on clinical findings of LDE are limited. Objective: To investigate the clinical and histopathologic findings and prognosis of LDE. Methods: We retrospectively investigated the clinicopathologic findings of LDE patients who visited Kosin University Gospel Hospital between 1990 and 2020. Results: This study included 44 LDE patients (male:female=1.4:1). The most common causative drug was anti-tuberculous drugs (52.3%), followed by 5-fluorouracil (11.4%), and captopril (9.1%). There were pruritic erythematous scaly or lichenoid patches and plaques in all cases. The most frequently involved sites were trunk and extremities. Notably, 15 cases (34.1%) involving the scalp and 3 cases (6.8%) involving the oral mucosa. Treatment modalities included oral, topical corticosteroid, and oral antihistamines. Among 44 cases, 28 patients discontinued the causative agent, and 16 patients continued to use it after diagnosis of LDE. The mean duration of treatment for patients who discontinued or did not discontinue the causative drugs was 4, 10 weeks, respectively. The most commonly observed histopathologic findings were superficial and deep perivascular infiltration of inflammatory cells (100.0%) and eosinophil infiltration (93.2%). Conclusion: LDE can be differentiated from idiopathic lichen planus by clinicopathologic findings. LDE appears to be a mild form of drug eruption in which symptoms can be controlled with conservative treatment, even without the cessation of causative drugs for the treatment of the underlying disease.
