        하악골 절제술후 재이식된 돌결자가골 재이식 : 치험 3례

        We described three cases of immediate reimplantation of a frozen-thawed autogenous mandible composed of a mixture of iliac bone, marrow, and particulate hydroxyapatite in tumors of the mandible. Acceptable outcomes were obtained in three patients who underwent immediate autogenous mandibular graft reconstruction. The conditions leading to successful outcome of the procedure are also discussed. Reimplantation of frozen autogenous lesioned mandible was performed in three patients with mandibular tumors. Two reimplanted grafts survived without complications following surgery. One case had postoperative infection that resolved with appropriate antibiotic treatment. There were no recurrences of the primary lesions. Satisfactory facial contour after surgery was achieved. These results are most promising, and we believe that, with further refinement, this technique will offer a new and acceptable modality for facial reconstruction in patients with cancer.

      • 糞尿와 都市 廢棄物과의 混合處理에 대한 硏究 (Ⅰ) : 肥料化를 中心으로한 實驗室的 考察 Experimental study for the fertilization

        우리나라에 있어서 都市廢棄物의 主인 糞尿와 固形廢棄物의 太宗인 無煙炭재를 同時에 效果的으로 處理할 수 있고 또한 堆肥化시킬 수 있는 好氣性 酸化方式의 最適條件을 얻기 위하여 生糞尿 및 煉炭재의 理化學的 및 生物學的 數値를 測定하고 이를 基礎로하여 上記 兩者를 混合處理 할수 있는 消化條件(溫度, 糞尿와 煉炭재의 配合比, 기타 固形物의 添加量, 注入되는 空氣量의 變化)에 따른 C.O.D., B.O.D.의 變化, 水分 및 電氣傳導度의 變化, 熱灼減量, N量의 變化 等을 硏究하였으며 이들 사이의 相關關係를 檢討하여 다음과 같은 結論을 얻었다. 1. 生糞尿에 固形廢棄物인 煉炭재나 紙類를 配合시켜 消化시키므로서 케이크속의 질소成分의 量을 增加 시킬수 있다. 2. 煉炭재의 配合은 糞尿의 消化速度를 增加 시킨다. 3. 糞尿의 處理를 위한 反應槽의 溫度는 50℃보다 60℃로 維持 시킬때 消化速度가 크다. 4. 反應槽의 溫度를 一定하게 했을때 空氣의 注入速度는 初期 反應時는 적게하고 後期 反應時는 크게 함이 效果的 이다. For the effective treatment and usable disposal of urban wastes in the area of Busan city, the composition of the nightsoil and anthracite bulky ash which is the main munic ipal solid wastes have been studied on the digestion of the compositive mixtures with the variable conditions that are ratio of compounding between nightsoil and ash, amount of solid additives and volume of air for the effective airation at 50, 65℃. The digestion has been detected by the physico-chemical and biological data; pH, specific gravity, content of moisture, C.O.D., B.O.D., electroconductance of the solution of the results, decrease of nition, and volume of total free nitrogen etc. The results are obtained as follows. 1) The characteristics of domestic nightsoil are different to the foreign data as table 5, but the data of anthracite ash almost are same except the composition of Fe₂O₃ and Al₂O₃. 2) Amount of total free nitrogen are increased by the digestion with the addition of solid waste, as effective absorption, to the nightsoil. 3) The digestion are accelerated by the compounding of ash, but the adding of sulfuric acid to fix the free ammonia reduced reaction. 4) The airation have to proceed slowly at initial step, and then endure enough volume of air at the later step.

      • 소아 야뇨증에 대한 임상적 고찰

        Objectives: The aim of this study was to describe the clinical characteristics of enuretic children and to evaluate the efficacy of treatments. Methods: Our study subjects enrolled were 52 children with nocturnal enuresis, who visited outpatient clinic of pediatrics in Seoul Paik Hospital and Ilsan Paik Hospital, during the period from October 1998 to August 2004. The data of gender, age, family history, symptom, clinical type of enuresis and drug therapy were collected and analyzed retrospectively. Results: The male : female ratio was 2.25 : 1. The mean age at diagnosis was 6.8 years with an age range from 4.3 years to 12.5 years. Fourty five cases(86.5%) were diagnosed as primary enuresis and 7 cases(13.5%) were diagnosed as secondary enuresis. A family history of enuresis was noted in 5 cases(9.6%) including 4 cases with parental enuresis history. The accompanying urinary symptoms were urgency(11.5%), frequency (7.7%), and dysuria(2.0%). Conclusion: We found frequent occurrences of enuresis in boys and primary type and an occasional association with a family history of enuresis. The two main treatment modalities were behavior control with an alarm system and drug therapy with imipramine, desmopressin, or oxybutynin. We recommend continuous drug medications to control nocturnal enuresis along with encouragement.

        어린이의 사시교정술시 인공호흡수와 심박수와의 상관관계

        Correlation between Respiratory Rates and the Degree of Bradycardia in Strabismus Surgery in Children Jong-Man Kang, M.D., Hee-Soo Kim, M.D., Chong0Sung Kim, M.D. and Myung-Kul Yum, M.D.^* Department of Anesthesiology, College of Medicine, Seoul National University, Seoul, and ^*Department of Pediatrics, College of Medicine, Hanyang University, Seoul, Korea Background: Strabismus surgery is frequent in pediatric ophthalmic surgery. Traction on the extra-ocular muscles can trigger an oculocardiac rdflex. We investigated the correlation between respiratoy rates and the degree of bradycardia to find another management for preventing oculocardiac reflex. Methods: NO premedications were administered. They were induced with thiopental and vecuronium and maintained with O_@-enflurane-N_2O. The tidal volume was 10 ml/kg. Respiratory rate of group 1 (n = 18) was fixed at 24 bpm, group 2 (n = 18) at 20 bpm, and group 3 (n = 19) at 16 bpm. We measured the mean heart rate from intubation to the beginning of surgery and defined this as the basal heart rate. We defined the lowest heart rate after traction of the extraocular muscles as the minimal heart rate. We calculated the correlation coefficient between the respiratory rate and basal heart rate, minimal heart rate, the change of heart rates and E_tCO_2. Results: There was a positive correlation between the respiratory rate and the basal heart rate, and minimal heart rate. There was no correlation between the respiratory rate and the change of heart rate. There was no difference in E_tCo_2 among the 3 groups. Conclusions: Can be increased both basal heart rate in anesthetized patients and minimal heart rate induced by oculocardiac reflex by making the respiratory rate more rapid if there is no significant difference of E_tCO_2. The oculocardiac reflex may be reduced by making respiratory rate more rapid if there is no significant influence on minute ventilation. (Korean J Anesthesiol 2001; 41: 450~454)

      • 의약품중 잔류유기용매 시험법 확립에 관한 연구

        Headspace-GC(딘5-GC)를 이용하여 ICH 잔류웅매 가이드라인 class 1, 2에 속하는 28종의 잔류유기웅매 분석방법을 연군한 결과 SPB-5, BB-WAX column을 동시 이용하여 각각을 확인, 정량할 수 있었다.또한 염의 참가효과, 평형시간,평형온도가 headspace분석에 미치는 영향을 검토한 결과 평형 시간 30분,평형온도 85℃,Na₂SO₄Ig을 첨가하여 시험할 때 최적 부넉조건을 나타내었다. 회수율은 1,1-achloroethene(68.3%)를 제외하고 90.9 ~ 114.5%로 패체적으로 양호하였다. 따로 HS-GCJECD, DB-524 column을 이용하여 9종의 잔류유기용매에 대하여 각각의 검랸선을 작성하고 검출한계를 구하였다. 실제 의약품 원료 10종및 제제 5종에 적용시킨 결과 모두 불검출되었으며 본 시험법은 의약품중의 잔류유기용매 분석에 적용할 수 있는 방법이라고 판단된다. The headspace-fC/FID(HS -GC/FID) method was performed for test method developnlent of residu;31 organic solvents in pfrarmaceuticals. Using SPB-S and BB-WAXcolumn, 28 kinds of solvents iri ICH residual sof)·ents guidetine class 1, 2 couBd be individuallt-identified and quantitated. The foIBowing residoal soIYents were not detected by the headspaceinjection condition : N,N-Cmethylacetamide, N,N-dimeth)rlformamide, ethyfeneglycol, formamide,2-methoryethanot, N-rnethylpyrrolidone, suBforane. The effects of the addition of salts,equilibration time, and equilibration temperature on headspace analt·sis were invesugated. Theoptimum conditions weiFe obtained with addtion of Ifa2s04 19 as a salt, simultaneously. thetime and temperature of equitibration were 30min and 8i'f, respectivety. The recovery havefound between 90.9 and 114.5% except 1,1-dichloroethene(fi8.3%). Using DB-624 column &HS-GC/:ECD method, 9 kinds of residual solvents could be individually identified andfuantitated. This HS-GC method can be applied to test the residua3 organic solvent in thepharmaceuticals.

        불소화 상수에 대한 정수기 사용시 불소농도 변화에 관한 연구

        The effectiveness of the fluoride ion in lowering the incidence of dental caries is of major significance to the entire field of dental health. Fluoride of public water supplies has been recognized for many years as an effective way to reduce dental caries. The purpose of this investigation was to determine the effect of several water conditioning systems on the fluoride concentration. A sampling apparatus was constructed to allow collection of water samples before and after conditioning. The apparatus connected the following types of water conditioning systems: a ceramic filter, a silver-carbon filter,an activated carbon filter, a reverse osmosis system and a ultra filter. These samples were tested by using colorimetric determination of fluoride concentration. The results were as follows, 1.The water fluoride concentration of primary school of Chungjcity was 0.793 ± 0.001ppm. 2.Fluoride concentrations after conditioning were 0.183 ±0.016ppm(the reverse osmosis system), 0.231 ±0.051ppm(the activated carbon filter), 0.240 ± 0.031ppm(the ceramic filter), 0.299 ±0.037ppm(the ultra filter) and 0.368 ± 0.030ppm(silvercabon filter). 3.Fluoride concentrations was reduced by the reverse osmosis system(77 ±2%), the activated carbon filter(71 ±6%), the ceramic filter(70 ±4%), the ultra filter(62 ±5%)and silver-carbon filter(54 ±4%). 4.All water conditioning systems tested caused a statistically significant reduction in fluoride(p<0.001).

        捻挫는 各關節에 生理學的인 可動영역을 초월하여 過度하게 강제 位置를 取하게 경우 或 原來 不可能한 外力이 억지로 加해질때 發生된다. 症狀은 손상된 部位에 疼痛과 腫脹이 發生하여 關節運動이 不自由하며 움직이면 痛症이 甚해진다. 그色은 紅色 또는 靑紫色을 나타낸다. 이와같은 捻挫의 治療에는 다음과 같은 選穴의 原則下에서 取穴되어야 한다. 1. 頸推捻挫에는 太陽經部이므로 手足太陽經脈을 위주로 하고 督脈經도 참고해야 한다. 2. 腰部捻挫도 太陽經部이므로 手足太陽經脈을 위주로 하고 또 腰는 腎之腑이고 督脈에 流走하므로 腎脈經과 督脈經을 참고 해야한다. 3. 四指關節의 捻挫는 陽明經部이므로 手足陽明經에서 取穴해야 한다. 4. 脇部捻挫 少陽經部이므로 手足少陽經脈에서 取穴해야 한다. 5. 以上과 같이 捻挫의 治療原則은 通血脈하여 行血去瘀 하는것 이므로 諸陽經 즉 太陽經, 少陽經, 陽明經脈과 疼痛部位를 通過하는 循經脈 및 阿是穴에서 取穴해야 한다. When a person are stressed from the external and irresistibe force essentially or have located coercive place swerving from the rang of physiological activity, a sprain breaks out at a joint. Symptoms have pains and edema in the breaking point and joint activities are not free and it has more pains of moving. It show that color is red or scarlet. In the treatment like this sprain, we must select point keeping tha following principle select point keeping the following principle. 1. Patient having the sprain of cervial vertebrae, we must select tha main point at the small Intestine channel of Hand-Taiyang and the urinary Bladder channel of Foot-Taiyang because it is the Taiyang channel, and consider the Du channel. 2. Patient having the sprain of waist region, we must select the main point at the small Intestine channel of Hand-Taiyang and the urinary Bladder Foot-Taiyang because it is the region of Taiyang channel, and consider the Du channel and the Kidney channel of Foot-shaoyin because waist is the house of kidney. 3. Patient having a limbs' joint dislocated, we must select a point at the large intestine channel of Hand-Yangming and the stomach channel of foot Yungming because limbs' sprain in the region of. Yangming channel. 4. Patient having a ribs' joint dislocated, we must select a point at the Yangming channel of hands and feet because ribs' sprain is the region of shaoyang channel. 5. Showing the above, we know the because the treatment of sprain is to promoting blood circulation by invigorating vital energe thae we must select point at the all of Yang channel, that is to say Taiyang channel and shaoyang channel and Yangming channel, and at the point selected according to ailing locality passing through teh aching part, and at the blood vessel alng the pathway of channel in the treatment principle of sprain.

For the dynamic equation of robot manipulator the linearized dynamic equation using the Taylor Series at the operating point is chosen, and the adaptive algorithm is developed by Lyapunov stability method.

      • Xylenol Orange에 依한 Chromium(Ⅲ)의 吸光度 定量法

        Microamount of chromium(Ⅲ) reacts with an excess of Xylenol orange (XO) to from Cr-XO Complex compound of red color. The absorption maxima of chelate was found at wavelengt 565mμ. The former, which was used to the spectrophotometric determination of chromium, gives the maximum absorbance in the pH range 3.5 to 4.2. Under the conditions investigated, Beer's law is obeyed up to about 35㎍ of chromium in 25㎖. The sensitivity of the reaction in 0.002㎍ Cr per ㎤. Among the anions tested chelating agents such as nitrilotriacetic acid ethylene diamine tetraacetic acid seriously with the determination. The complex has a composition of 1 to 1 in the metal-ligand ratio at pH 3. 8.
