Background/Aims: The bedside index of severity in acute pancreatitis (BISAP) is a new, convenient, prognostic multifactorial scoring system. As more data are needed before clinical application, we compared BISAP, the serum procalcitonin PCT), and other multifactorial scoring systems simultaneously. Methods: Fifty consecutive acute pancreatitis patients were enrolled prospectively. Blood samples were obtained at admission and after 48 hours and imaging studies were performed within 48 hours of admission. The BISAP score was compared with the serum PCT, Ranson`s score, and the acute physiology and chronic health examination (APACHE)-II, Glasgow, and Balthazar computed tomography severity index (BCTSI) scores. Acute pancreatitis was graded using the Atlanta criteria. The predictive accuracy of the scoring systems was measured using the area under the receiver-operating curve (AUC). Results: The accuracy of BISAP (≥ 2) at predicting severe acute pancreatitis was 84% and was superior to the serum PCT (≥ 3.29 ng/mL, 76%) which was similar to the APACHE-II score. The best cutoff value of BISAP was 2 (AUC, 0.873; 95% con- fidence interval, 0.770 to 0.976; p < 0.001). In logistic regression analysis, BISAP had greater statistical significance than serum PCT. Conclusions: BISAP is more accurate for predicting the severity of acute pancreatitis than the serum PCT, APACHE-II, Glasgow, and BCTSI scores.

Study on Method of Sperm Aspiration and Injection into an Oocyte in Intracytoplasmic Sperm Injection(ICSI) Immobilization of spermatozoa prior to intracytoplasmic sperm iniection(ICSI) sometimes results in crooked tail and this makes it difficult to aspirate sperm into an injection pipette tail first. Head-first sperm aspiration into an injection pipette avoid this problem due to the bigger size of the sperm head. The effect of head or tail-first sperm injection into an oocyte on fertilization cleavage, percentage of grade I embryos and development to blastocyst stage in ICSI program has been studied. A single living immobilized spermatozoa from oligoasthenozoospermic patient was injected into an oocyte head-first or tail-first according to the treatment. Eighteen hours after microinjection, oocytes ware inspected for survival and fertilization Fertilized oocytes with two pronuclei were cultured in 30μl drop of mHTF supplemented with 10% heat-inactivated follicular fluid(FF) at 37℃. On day 2. embryo transfer was performed with cleaved embryos. The remaining 2-8 cell stage embryos were co-cultured with BRL cells in mHTF + 10% FF for 72 hours and the developmental stage was observed. The data were analyzed by Analysis of Variance. A total of 164 oocytes from 36 cycles were assigned to earth treatment and ICSI was performed(88 head-first, tail-first). The rates of normal fertilization were 81.8% and 76.3% for head-first and tail-first, respectively. Of the fertilized oocytes, the percentage of cleaved embryos and the percentage of grade 1 embryo among cleaved embryos were 88.9% and 68.8%, 93.1% and 74.1% for head-first and tail-first, respectively. Of the 2-8 cell embryos cultured, 44.4%(16/36) and 50.0%(10/20) for head first and tail first, respectively developed to blastocyst stage. There were no differences in fertilization, cleavage, rates of grade 1 embryos, and development to blastocyst stage. In conclusion, head-first or tail-first sperm injection into an oocyte in ICSI program does not affect fertilization and subsequent embryo development to blastocyst stage in vitro.
Retrospective study for determining recommended resection margin of neurofibroma

Background: Cutaneous neurofibroma (CNF) is a common benign tumor. Surgical intervention is currently the best treatment option for the cosmetically disfiguring appearance. Presently, there are a few guidelines for determining adequate surgical margins in literature. Objectives: This study aims to evaluate the resection margin of CNF and correlation between size of CNF and recommended margin. CNF is difficult to determine the margin visually. Therefore, study based on the size and histological findings of neurofibroma are needed to determine the resection margin. Methods: From January 2016 to July 2020 were 52 cases of CNF identified in the database of the Dept. of Dermatology at Korea University Ansan Hospital. Lesions were removed surgically with a margin of 3 mm from the clinical delineation. Tumor invasion was evaluated through histopathologic evaluation. Results: Among the 52 cases of CNF, 24 cases (46%) had a size of 1cm or more and 28 cases (54%) had a size of 1cm or less. 44 lesions (85%) were completely removed without involvement of the margin. Among the 8 cases of incompletely resected CNF, 3 cases involved both side of margin and 5 cases involved one side of margin. Interestingly, none of the cases of CNFs smaller than 1cm were incompletely resected. Conclusion: In this study, we found that CNF size should be considered when determining the resection margin. For CNFs <1cm in diameter, 3mm resection margins are mostly adequate; but for larger CNFs, greater margins are required.

Pyogenic granuloma (PG) is an benign proliferation of blood vessels presented as a rapidly growing nodule. It commonly occurs in children and young adults. It has traditionally been treated surgically, but this has the disadvantage of leaving scars, so other modalities such as laser therapy are being considered. Seven patients between 30-months and 24-years of age visited our clinic with solitary pupuric nodules and were diagnosed with PG. The lesions were present on the right cheek, left temple, right cheek, left chest, left upper eyelid, right mandible, and nasal tip, respectively. The combination of CO<sub>2</sub> laser with pulsed dye laser (V-beam Perfecta) was performed for a number of cycles on each patient. After the treatment, all lesions were cleared without any side effects. Pulsed dye laser (PDL) monotherapy, due to its non-invasiveness, is considered superior to surgical excision. However, PDL is limited in its depth of laser penetration, leading to increased treatment sessions; it is hard to cure PG with a large volume. This drawback can be ameliorated by first reducing the lesion’s volume with the CO<sub>2</sub> laser. However, CO<sub>2</sub> laser tended to cause hemorrhages, and Monsel’s (ferric subsulfate) solution was applied to control persistent bleeding. We herein reviewed 7 cases of PG successfully treated by CO<sub>2</sub> laser combined with PDL, underlining the effectiveness of combined laser therapy as a treatment modality for PG.

<P>We report an Ag nanofiber-embedded glass-fabric reinforced hybrimer (AgNF-GFRHybrimer) composite film as a reliable and high-performance flexible transparent conducting film. The continuous AgNF network provides superior optoelectronic properties of the composite film by minimizing transmission loss and junction resistance. In addition, the excellent thermal/chemical stability and mechanical durability of the GFRHybrimer matrix provides enhanced mechanical durability and reliability of the final AgNF-GFRHybrimer composite film. To demonstrate the availability of our AgNF-GFRHybrimer composite as a transparent conducting film, we fabricated a flexible organic light-emitting diode (OLED) device on the AgNF-GFRHybrimer film; the OLED showed stable operation during a flexing.</P>
