The extracts of Scutellaria baicalensis and Acacia catechu have been shown in previous studies to alleviate joint discomfort, reduce stiffness, and improve mobility by reducing the production of proinflammatory molecules over long periods of supplementation. The acute effects of intake of these extracts have not yet been investigated. Thus, we carried out a 1 week clinical trial to examine the extent to which UP446—a natural proprietary blend of S. baicalensis and A. catechu (UP446)—decreases knee joint pain, mobility, and biomarkers of inflammation in comparison to naproxen. Seventy-nine men and women (40–90 years old) diagnosed as having mild to moderate osteoarthritis (OA) consumed either 500 mg/day of the UP446 supplement or 440mg/day of naproxen for 1 week in a double-blind randomized control trial. Pain, knee range of motion (ROM), and overall physical activity were evaluated at the start and at the end of treatment. Fasting blood was collected to determine serum interleukins 1b and 6, tumor necrosis factor-a, C-reactive protein, and hyaluronic acid. The UP446 group experienced a significant decrease in perceived pain (P = .009) time dependently. Stiffness was significantly reduced by both treatments (P= .002 UP446, P = .008 naproxen). Significant increases in mean ROM over time (P = .04) were found in the UP446 group. These findings suggest that UP446 is effective in reducing the physical symptoms associated with knee OA.
Inhibition of Aflatoxin B1 Biosynthesis by Piperlongumine Isolated from Piper Longum L.

The alkaloids, piperlongumine, piperine, pipemonaline, and piperoctadecalidine, isolated from Piper longum L., were found to inhibit the biosynthesis of aflatoxin B1, (AFB1,) in Aspergillus flavus WRRC 3-90-42-12. Piperlongumine was the most active among the compounds tested, with a 96% inhibition of AFB1, biosynthesis at 0.2% (w/v) supplement in a potato dextrose agar (PDA) medium. The three other piperidine alkaloids, piperine, pipemonaline, and piperoctadecalidine, also inhibited the biosynthesis of AFB1. Of these three alkaloids, piperoctadecalidine exhibited a potent inhibitory activity with a 100% inhibition of AFB1, production at 0.7% (w/v) supplement in a PDA medium. Therefore, piperlongumine and piperoctadecalidine could be used as antiaflatoxigenic agents in agricultural industries. To determine the antiaflatoxigenic mode of action of piperlongumine, further studies are needed.

Background and Purpose The benefit regarding co-treatment with intravenous (IV) thrombolysis before mechanical thrombectomy in acute ischemic stroke with large vessel occlusion remains unclear. To test the hypothesis that clinical outcome of ischemic stroke patients with intracranial internal carotid artery, middle cerebral artery or basilar artery occlusion treated with direct endovascular thrombectomy within 4.5 hours will be non-inferior compared with that of standard bridging IV thrombolysis followed by endovascular thrombectomy. Methods To randomize 780 patients 1:1 to direct thrombectomy or bridging IV thrombolysis with thrombectomy. An international-multicenter prospective randomized open label blinded endpoint trial (PROBE) (ClincalTrials.gov identifier: NCT03494920). Results Primary endpoint is functional independence defined as modified Rankin Scale (mRS) 0–2 or return to baseline at 90 days. Secondary end points include ordinal mRS analysis, good angiographic reperfusion (modified Thrombolysis in Cerebral Infarction score [mTICI] 2b–3), safety endpoints include symptomatic intracerebral hemorrhage and death. Conclusions DIRECT-SAFE will provide unique information regarding the impact of direct thrombectomy in patients with large vessel occlusion, including patients with basilar artery occlusion, with comparison across different ethnic groups.

Advances in acute ischemic stroke (AIS) treatment have been contingent on innovations in neuroimaging. Neuroimaging plays a pivotal role in the diagnosis and prognosis of ischemic stroke and large vessel occlusion, enabling triage decisions in the emergent care of the stroke patient. Current imaging protocols for acute stroke are dependent on the available resources and clinicians’ preferences and experiences. In addition, differential application of neuroimaging in medical decision-making, and the rapidly growing evidence to support varying paradigms have outpaced guideline-based recommendations for selecting patients to receive intravenous or endovascular treatment. In this review, we aimed to discuss the various imaging modalities and approaches used in the diagnosis and treatment of AIS.
