Acute Gastrointestinal Bleeding in COVID-19 Patients: A Systematic Review and Meta-Analysis

Background/Aims: More than 100 million people to date have been affected by the coronavirus disease-2019 (COVID-19)pandemic. Patients with COVID-19 have a higher risk of bleeding complications. We performed a systematic review and metaanalysisto evaluate the outcomes of COVID-19 patients with signs and symptoms of acute gastrointestinal bleeding (GIB). Methods: A systematic literature search was carried out for articles published until until November 11, 2020, in the Embase,MEDLINE, Web of Science, and Cochrane Library databases. We included studies on COVID-19 patients with signs and symptomsof GIB. Results: Our search yielded 49 studies, of which eight with a collective 127 patients (86 males and 41 females) met our inclusioncriteria. Conservative management alone was performed in 59% of the patients, endoscopic evaluation in 31.5%, and interventionalradiology (IR) embolization in 11%. Peptic ulcer disease was the most common endoscopic finding, diagnosed in 47.5% of thepatients. Pooled overall mortality was 19.1% (95% confidence interval [CI]; 12.7%-27.6%) and pooled mortality secondary to GIBwas 3.5% (95% CI; 1.3%–9.1%). The pooled risk of rebleeding was 11.3% (95% CI; 6.8%–18.4%). Conclusions: The majority of COVID-19 patients with GIB responded to conservative management, with a low mortalityrate associated with GIB and the risk of rebleeding. Thus, we suggest limiting endoscopic and IR interventions to those withhemodynamic instability and those for whom conservative management was unsuccessful.

Purpose: Reduction and stable fixation of the quadrilateral plate are challenging primarily due to its location in the true pelvis, limited bone stock, juxta-articular nature, and its comminution. The current study aimed to investigate the quality of reduction and functional outcomes after open reduction and internal fixation (ORIF) with infrapectineal buttress plating of the quadrilateral surface via an anterior intrapelvic approach. Materials and Methods: We conducted a retrospective review of twenty-one patients with acetabular fractures involving quadrilateral plate operated at Ghurki Trust Teaching Hospital between January 2017 and December 2018. Radiological assessment of the quality of reduction was conducted using criteria described by Matta. Functional outcomes were evaluated using a modified Postel Merle d’Aubigné score. Results: The current study included 15 males and 6 females with a mean age of 40.67±12.17 years (range, 22- 62 years). The most common fracture pattern was anterior column and posterior hemi-transverse in eight patients followed by true bicolumn and T-type fractures in seven and four patients respectively. Both transverse fractures were transtectal. The quality of reduction according to Matta criteria was anatomical in 14 patients, imperfect in five and poor in two. Functional outcomes were excellent in 47.6% cases, good in 42.9%, and fair in 9.5% cases. Both patients with fair outcomes had non-anatomical reduction, and one required total hip arthroplasty at a later time. Conclusion: Quadrilateral plate reconstruction with an infrapectineal buttress plate applied though an anterior intrapelvic approach provides high rates of anatomical reduction and yields good functional outcomes.

Efficiency of phosphorus recovery from polyaluminum chloride sludge is determined by chemicalinteractions between Al and P. Chemical structure of Al–P complex and their binding mechanisms in thePAC sludge were investigated with spectroscopy and chemical fractionation. More than 98% of the P wasbound with Al, out of which 70% was strongly bound. The ATR-FTIR analysis indicates that the majority ofthe P formed inner sphere complex with Al through ligand exchange. The availability of highly protonatedsurface for P sorption at low pH also supports the formation of Al–P outer sphere complex.
Implementing Rule-based Healthcare Edits

Automated medical claims processing and billing is a popular application domain of information technology. Managing medical related data is a tedious job for healthcare professionals, which distracts them from their main job of healthcare. The technology used in data management has a sound impact on the quality of healthcare data. Most of Information Technology (IT) organizations use conventional software development technology for the implementation of healthcare systems. The objective of this experimental study is to devise a mechanism for use of rule-based expert systems in medical related edits and compare it with the conventional software development technology. A sample of 100 medical edits is selected as a dataset to be tested for implementation using both technologies. Besides empirical analysis, paired t-test is also used to validate the statistical significance of the difference between the two techniques. The conventional software development technology took 254.5 working hours, while rule-based technology took 81 hours to process these edits. Rule-based technology outperformed the conventional systems by increasing the confidence value to 95% and reliability measure to 0.462 (which is < 0.5) which is three times more efficient than conventional software development technology.

This study aims to examine OL as a potential mediating variable in the relationship between IT and organizational performance. Organizational learning (OL) has been proposed as the mechanism to accomplish this task. Existing empirical research demonstrates that OL may indeed act as a mediator for the effect of IT on organizational outcomes. Also, existing literature discusses the use of technology in the organization, and the case for OL as the key knowledge process, and the intersection between technology and OL as a knowledgebased means for improving organizational performance. Many studies use a descriptive measure of OL despite the theory suggesting that a normative measure may be more appropriate. This study aims to address these concerns in a setting by using structural equation modelling (SEM) to compare the effectiveness of descriptive and normative measures of OL as mediating variables in knowledge-intensive organizations. Survey results support OL as a mediator between IT and organizational performance in addition to normative measures of OL outperforming descriptive measures. Implications for research and practice are discussed. To test the model, we will apply (SEM) structural equation modeling in the analysis of a moment structures (AMOS) on the empirical evidence collected from 218 Pakistani CEOs and top managers.

Polyurethane-cerium(IV) phosphate composite cation exchange membranes were prepared by solutioncasting method in different stoicheometric ratios of polyurethane and cerium(IV) phosphate. Thestructure and morphology of the prepared membranes were ascertained by FTIR, SEM, TGA and DTA. Themembrane having composition 1:2 (PU: cerium(IV) phosphate) shows best results for ion-exchangecapacity, water content, porosity, thickness and swelling. The Cu(II) selective electrode was developed byusing this membrane for the determination of Cu(II) in solutions. The accuracy of the procedure has beentested on drinking water samples spiked with known amounts of Cu(II) and results were comparable tothose generated by Atomic Absorption Spectrophotometer.
Composite Back Stepping Anti-unwinding Control of Spacecraft with Finite Time Convergence

Quaternion representation of spacecraft results in dual equilibrium points. Unwinding phenomenon occurs if both the points are not considered stable while designing control law. This research note proposes anti-unwinding attitude stabilization control of rigid body spacecraft in presence of external disturbance and inertial uncertainties. In pursuit of our goal, we formulated sliding surface using scalar component of quaternion and developed composite anti-unwinding control law using back-stepping technique (BT), in accomplice with sliding mode control (SMC). Furthermore, state transformation is performed to facilitate application of observer for estimation of lumped uncertainties. We employed extended state observer (ESO) to compensate for external disturbance and inertial uncertainties. The control law gives precise and smooth steady state performance along with faster transient response. Additionally, it has better uncertainty rejection capabilities and alleviates chattering phenomenon. Close loop stability of the system is proved using Lyapunov’s theory and Barbalat’s lemma in finite time (FT). Simulation results demonstrate effectiveness of the presented control law.
