        Understanding the Role of Parkinson’s Disease Nurse Specialists in Providing Personalized Care for Individuals with Parkinson’s and Their Families

        Purpose: Parkinson's Disease Nurse Specialists(PDNS) play a vital role in delivering personalized care to individuals affected by Parkinson's disease(PD) and their families. This paper explores the multifaceted responsibilities of PDNS in managing and providing comprehensive care for the unique needs of PD patients. Additionally, it delves deeper into the specialized qualifications and training opportunities available for PDNS, with a particular focus on the United Kingdom's National Health Service(NHS). Method: A systematic review of literature and publications from reputable sources, including PubMed, The Lancet, Parkinson's Europe, and Parkinson's UK, has been conducted to gather pertinent information and insights regarding the crucial role of PDNS in enhancing the quality of life for PD patients. It also includes reviewing the latest PDNS learning pathway guidelines and the EU’s PD care manifesto. Results: A growing number of PD patients and families face communication difficulties with healthcare professionals, particularly consultants. There is a lack of awareness about the disease and its symptoms, which results in delayed diagnosis and inconsistent care plans. During consultations, there is a mismatch between the information provided and the information desired. Publications emphasize the critical roles of PDNS in the delivery of personalized care plans, emphasizing the importance of effective patient-provider communication for optimal plans. Conclusion: Effective communication and understanding of PD patients' and their family members' circumstances and emotional well-being contribute to informed care plans. PDNS play diverse roles in providing comprehensive patient and family support. They serve as primary contacts, walking alongside patients in their PD journey. It is vital for PDNS to cultivate a trusted relationship and receive ongoing support to enhance their skills. Government and charities hold critical roles in raising awareness and creating learning opportunities for PDNS and healthcare professionals. Research shows low shared decision-making in European PD care plans, leading to treatment inconsistencies. The European Union recognizes the importance of funding further research, stressing knowledge sharing for well-informed and standardized care plans, benefiting PD patients and families.

        Consumer Perceptions and Intentions Towards Malaysian Mobile Marketing

        Mobile marketing is a brand new phenomenon. In Malaysia, marketeers do not seem to understand well enough the perceptions of consumers according to mobile marketing, especially to the mobile service users. This study analyses and evaluates any significant relationship between consumer perceptions and intentions with respect to mobile marketing, and seeks to determine the expectations, preferences, pattern and usage of the consumers of Mobile Marketing Product and Service (MMP&S). The data was obtained by convenience sampling in the big cities of Malaysia. A total of 500 questionnaires were distributed and a sample of 112 usable questionnaires was selected. The result of this research applying the Technology Acceptance Model (TAM) point to the fact that perceived usefulness, perceived ease of use, perceived innovative, social influence have a direct positive relationship with the intention to use mobile marketing. This, in turn, can shed light on the main factors determining consumer intentions to use mobile marketing that may control consumer adoption of mobile marketing.

        Association between Left Ventricular Systolic Dysfunction and Mortality in Patients with Septic Shock

        Background: The impact of myocardial damage on the prognosis of patients with septic shock is not clearly elucidated because complex hemodynamic changes in sepsis obscure the direct relationship. We evaluated left ventricular (LV) conditions that reflect myocardial damage independently from hemodynamic changes in septic shock and their influence on the prognosis of patients. Methods: We retrospectively enrolled 208 adult patients who were admitted to the intensive care unit and underwent echocardiography within 7 days from the diagnosis of septic shock. Patients who were previously diagnosed with structural heart disease or coronary artery disease were excluded. Left ventricular ejection fraction (LVEF) was divided into four categories: normal, ≥ 50%; mild, ≥ 40%; moderate, ≥ 30%; and severe dysfunction, < 30%. Wall motion impairment was categorized into the following patterns: normal, diffuse, ballooning, and focal. Results: There were 141 patients with normal LVEF. Among patients with impaired LV wall motion, the diffuse pattern was the most common (34 patients), followed by the ballooning pattern (26 patients). Finally, 102 patients died, and in-hospital mortality was significantly higher in patients with severe LV systolic dysfunction (hazard ratio [HR], 1.97; 95% confidence interval [CI], 1.04–3.75; P = 0.039) and in patients with diffuse pattern of LV wall motion impairment (HR, 2.28; 95% CI, 1.19–4.36; P = 0.013) than in those with a normal LV systolic function. Conclusion: Severe LV systolic dysfunction and diffuse pattern of LV wall motion impairment significantly affected in-hospital mortality in patients with septic shock. Conventional echocardiographic evaluation provides adequate information on the development of myocardial damage and accurately predicts the prognosis of patients with septic shock.

        How small is enough for the left heart decompression cannula during extracorporeal membrane oxygenation?

        Background: Left ventricular (LV) distension is a recognizable problem accompanied by sub-sequent complications during venoarterial extracorporeal membrane oxygenation (VA-EC-MO). However, no gold standard for LV decompression has been established, and no minimal flow requirement has been designated. Thus, we evaluated the efficacy of the 8-Fr Mullins sheath for left heart decompression during VA-ECMO in adult patients. Methods: Left heart decompression was performed when severe pulmonary edema was detected on chest radiography or when no generation of pulse pressure followed severe LV dysfunction in patients receiving VA-ECMO. We punctured the interatrial septum and inserted an 8-Fr Mullins sheath into the left atrium via the femoral vein. The sheath was connected to the venous catheter used for ECMO. The catheter was maintained during VA-ECMO. Results: The left heart decompression procedure was performed in seven of 35 patients who received VA-ECMO between February 2017 and June 2018. Three patients had acute myocardial infarction; three, fulminant myocarditis; and one, dilated cardiomyopathy. Four patients showed noticeable improvement of pulmonary edema within 3 days, and three patients with a pulse pressure of <10 mm Hg showed an increase in pulse pressure of >20 mm Hg within 24 hours from the left heart decompression procedure. All seven patients were successfully weaned from VA-ECMO. No complications related to the left heart decompression procedure occurred. Conclusions: An 8-Fr sheath may be a possible option for left heart decompression in adult patients with LV distension under VA-ECMO who are expecting recovery of LV function.

        Seven-day and In-hospital Mortality According to Left and Right Ventricular Dysfunction in Patients With Septic Shock

        Background and Objectives: The prognostic implications of septic cardiomyopathy have not been clearly demonstrated. We evaluated serial changes in left ventricular (LV) and right ventricular (RV) function in patients with septic shock and their prognostic value on 7-day and in-hospital mortality. Methods: Transthoracic echocardiography was performed within 48 hours of the diagnosis of septic shock and 7 days after the initial evaluation. In addition to traditional echocardiographic parameters, LV and RV function was evaluated using global longitudinal strain (GLS), and tricuspid annular plane systolic excursion (TAPSE). Results: A total of 162 patients (men, 83, 51.5%; 70.7±13.4 years; Acute Physiology and Chronic Health Evaluation [APACHE] II, 30.6±9.2) were enrolled. Initial GLS and TAPSE were −14.9±5.2% and 16.9±5.5 mm, and improved in the follow-up evaluation (GLS, −17.6±4.9%; TAPSE, 19.2±5.4 mm). Seven-day and in-hospital mortality were 24 (14.9%) and 64 (39.8%). Seven-day mortality was significantly associated with initial GLS >−16% (odds ratio [OR], 14.066, 95% confidence interval [CI], 1.178–167.969, p=0.037) and APACHE II score (OR, 1.196, 95% CI, 1.047–1.365, p=0.008). The in-hospital mortality of 7-day survivors was associated with follow-up TAPSE <16 mm (OR, 10.109, 95% CI, 1.640–62.322, p=0.013) and Sequential Organ Failure Assessment score (OR, 1.340, 95% CI, 1.078–1.667, p=0.008). GLS was not associated with in-hospital mortality of 7-day survivors. Conclusions: Fluctuation of both ventricular function was common in septic shock. Seven-day mortality of patients with septic shock was related to GLS, whereas in-hospital mortality of 7-day survivors was related to TAPSE, not to GLS.

        A 6 Gbps/pin Low-Power Half-Duplex Active Cross-Coupled LVDS Transceiver with Switched Termination

        A novel linear switched termination active cross-coupled low-voltage differential signaling (LVDS) transceiver operating at 1.5 GHz clock frequency is presented. On the transmitter side, an active cross-coupled linear output driver and a switched termination scheme are applied to achieve high speed with low current. On the receiver side, a shared preamplifier scheme is employed to reduce power consumption. The proposed LVDS transceiver implemented in an 80 nm CMOS process is successfully demonstrated to provide a data rate of 6 Gbps/pin, an output data window of 147 ps peak-to-peak, and a data swing of 196 mV. The power consumption is measured to be 4.2 mW/pin at 1.2 V.

        Vertical dimension rehabilitation in a patient with a few remaining severely worn teeth using partial removable denture prosthesis

        Tooth wear is loss of surface of teeth, and may be classified into physiological and pathologic wear. When tooth wear is accelerated excessively, it is pathologic tooth wear. Pathologic tooth wear may result in many problems including decrease of occlusal vertical dimension, as well as unacceptable damage to anterior guidance. In this study, the patient had loss of posterior teeth and anterior severely worn dentition. Vertical dimension analysis was conducted and 3 mm increase of vertical dimension was determined. After 8 weeks of stabilization period with temporary prostheses, definitive restoration with fixed dental prosthesis and partial removable denture prosthesis were accomplished and satisfactory outcomes were achieved in functional and esthetic aspects.

      • Two new species and three new records of the leafhopper subfamily Iassinae Walker (Hemiptera: Auchenorrhyncha: Cicadellidae) from Korea

        The subfamily Iassinae Walker belongs to the family Cicadellidae (Hemiptera: Auchenorrhyncha). A total of 124 genera with 1,999 species have been described in the world. To date, two genera, Batracomorphus Lewis and Iassus Fabricius, and 5 species of the subfamily have been recorded in Korea. In this talk, one new species and one new record of the genus Batracomorphus and one new species and two new records of the genus Iassus from Korea are presented. And we also provide biological information such as host plants and distributional records of the Korean Iassine species.
